Upadacitinib for the treatment of refractory bullous pemphigoid: a case report and literature review.

Bullous pemphigoid (BP) is a common autoimmune subepidermal blistering disorder that mainly affects elderly individuals aged ≥70 years. Systemic glucocorticoids serve as first-line treatment, and immunosuppressants or biological agents such as dupilumab are added for patients with suboptimal responses. However, a proportion of patients exhibit suboptimal responses or cannot sustain therapeutic benefits under these standard therapeutic regimens, representing an important unmet clinical need. We describe a 74-year-old patient with refractory BP who did not achieve adequate disease control despit

An Active Cytomegalovirus Infection and Epstein-Barr Virus Reactivation Complicated by Cytomegalovirus Meningitis and Virus-Associated Hemophagocytic Lymphohistiocytosis during Methotrexate and Upadacitinib Therapy in a Patient with Rheumatoid Arthritis.

Methotrexate (MTX) and Janus kinase (JAK) inhibitors are effective treatments for rheumatoid arthritis (RA); however, they may increase the risk of opportunistic infections. We herein report a case of a patient with RA receiving MTX and upadacitinib who developed an active cytomegalovirus (CMV) infection and Epstein-Barr virus (EBV) reactivation, complicated by CMV meningitis and virus-associated hemophagocytic lymphohistiocytosis (HLH). This case highlights that clinicians should consider not only herpes zoster, but also infection or reactivation involving non-VZV herpesviruses, including CMV

Laryngeal Manifestations of Rheumatic Disease.

Rheumatologic diseases are systemic, immune-mediated disorders that frequently involve the larynx, producing manifestations that range from subtle dysphonia to life-threatening airway obstruction. Laryngeal involvement may precede systemic disease, progress despite adequate systemic control, or be the sole presenting feature of active disease. Diagnosis relies on flexible laryngoscopy with disease-specific serology and biopsy when indicated. Corticosteroids, immunosuppressants, and biologics such as rituximab form the foundation of medical therapy, while surgery ranges from serial endoscopic d

Calcineurin inhibitor monitoring in allogeneic hematopoietic cell transplantation: time to move beyond solid organ transplant paradigms.

Calcineurin inhibitors, such as cyclosporine and tacrolimus, have been the backbone of graft-versus-host disease prophylaxis in allogeneic hematopoietic cell transplantation for more than four decades. Yet, the pharmacological framework governing their therapeutic drug monitoring has been significantly imported from solid organ transplantation. This critical narrative review argues that this extrapolation is scientifically unsound. The unique biological environment of allogeneic hematopoietic cell transplantation characterized by toxic conditioning regimens, profound hematologic changes, syste

Prospective controlled study of the recombinant zoster vaccine in autoimmune rheumatic disease: robust seroconversion despite intravenous cyclophosphamide.

BACKGROUND: Recombinant zoster vaccine (RZV) prevents shingles in autoimmune rheumatic diseases (ARD), but intravenous cyclophosphamide (IVCYC) impact on immunogenicity is unknown. Given cell depletion/cytotoxicity, IVCYC is expected to induce profound immunogenicity impairment, we therefore evaluated its impact. METHODS: This subanalysis of prospective, randomized, placebo-controlled trial included ARD patients receiving IVCYC (ARD-IVCYC) compared with patients without IVCYC (ARD-no IVCYC) and healthy-controls. RZV was administered in two doses. Humoral response (≥4-fold), geometric mean tit

Optimising Blood Test Monitoring Frequency for Inflammatory Bowel Disease Patients: Balancing Adherence, Comfort and Risk.

BACKGROUND: Inflammatory bowel disease (IBD) therapies can cause myelosuppression and hepatitis. The European Crohn's and Colitis Organisation (ECCO) recommends 3-monthly blood test monitoring; however, these recommendations are largely based on expert consensus, and real-world evidence informing optimal long-term monitoring intervals remains limited. AIM: To determine the incidence of medication-related hepatitis and myelosuppression in a population-based IBD cohort and estimate the laboratory monitoring burden required to detect clinically significant adverse events. METHODS: A retrospecti

Use of advanced therapies in systemic lupus erythematosus: a retrospective single centre analysis based on disease phenotypes.

OBJECTIVES: To identify and analyse distinct subgroups of Systemic Lupus Erythematosus (SLE) based on serological and demographic profiles, and to assess their impact on clinical manifestations, disease activity, and use of advanced therapies. METHODS: We performed a cross-sectional analysis of a single-centre cohort of 327 SLE patients. Hierarchical cluster analysis was used to identify groups based on ten serological features: anti-dsDNA, anti-Sm, lupus anticoagulant, anti-cardiolipin, anti-B2GP1, hypocomplementemia, direct Coombs test, anti-Ro, anti-La, and anti-RNP. We further stratified

Spontaneous Postpartum Hematomas Revealing Acquired Hemophilia A with Extreme Inhibitor Interference: a Case Report and Literature Review.

BACKGROUND: Acquired hemophilia A (AHA) is a rare autoimmune bleeding disorder caused by autoantibodies against factor VIII. Extremely high-titer inhibitors may cause laboratory interference patterns mimicking com-bined factor deficiencies. METHODS: We report a 28-year-old woman with extensive postpartum hematomas. Investigations included coagulation screening, factor assays, serial dilution studies (undiluted to 1:16), and anti-factor VIII antibody quantification using the Nijmegen method. Factor VIII activity was measured by both one-stage clot-based and chromogenic assays. Immunological wo

Multiple Cavitary Lung Lesions in ANCA-Negative Small Vessel Vasculitis: A Case Report.

Multiple cavitary lung lesions present a diagnostic challenge, with a broad differential diagnosis, including infectious, malignant, and autoimmune etiologies. ANCA-associated vasculitis (AAV), particularly in its seronegative form, is a rare yet important cause that is frequently overlooked, as pulmonary involvement is uncommon and its manifestations can closely mimic infection or malignancy. We report a case of a 30-year-old male who presented with a three-week history of dyspnea and hemoptysis, and a background of membranoproliferative glomerulonephritis confirmed by renal biopsy three mont

Comparative Effectiveness of Treatments for Relapse Prevention in Myelin Oligodendrocyte Glycoprotein Antibody-Associated Disease.

BACKGROUND AND OBJECTIVES: The most effective treatment strategy to prevent relapses after a first myelin oligodendrocyte glycoprotein antibody-associated disease (MOGAD) event remains unclear. Our objective was to evaluate the comparative effectiveness of maintenance therapies in preventing relapses in pediatric-onset MOGAD. METHODS: We analyzed prospectively collected data from November 2011 to August 2023 across 13 US pediatric neuroimmunology centers. Eligible patients had demyelinating disease onset before age 18 years, confirmed MOG-IgG positivity through cell-based assay, and negative

Relapse and disability across the disease course of MOGAD: a real-world cohort study from northern China.

BACKGROUND: Relapse and long-term disability vary substantially in myelin oligodendrocyte glycoprotein antibody-associated disease (MOGAD), complicating remission-phase treatment decisions. METHODS: We retrospectively included 120 consecutive MOGAD patients from northern China. Time to first relapse was analyzed using time-dependent Cox regression, recurrent relapses using episode-level time-dependent Cox models with cluster-robust standard errors, and final Expanded Disability Status Scale (EDSS) score ≥2.0 using logistic regression. RESULTS: Fifty-three patients (44.2%) relapsed; among the

Acute autoimmune hepatitis diagnosed concurrently with human immunodeficiency virus.

We report a woman in her 50s who presented with jaundice and severe hepatocellular injury, concurrently found to have newly diagnosed advanced HIV infection (CD4 count 17, viral load 195 600 copies/mL). In this context, infectious hepatitis or opportunistic infections were the important initial consideration, but viral serology, imaging and liver biopsy excluded opportunistic and viral causes. Markedly raised immunoglobulin G, positive anti-nuclear and smooth muscle antibodies, typical histology and rapid response to corticosteroids confirmed acute autoimmune hepatitis (AIH). She improved on p

Capsulectomy at Explant: Rethinking Spinal Cord Stimulator Infected Generator Pocket Management in the Immunocompromised-A Case Series.

BACKGROUND: Surgical site infections (SSI) associated with spinal cord stimulation (SCS) implants pose risks of morbidity, mortality, and increased healthcare costs. Immunocompromised patients are especially vulnerable due to impaired immune responses, delaying diagnosis and increasing complications. Current guidelines recommend SCS explantation for deep or inadequately treated superficial SSI, though the role of capsulectomy remains underexplored despite its use in other fields like plastic surgery and cardiology. CASE PRESENTATION: We present two cases of immunocompromised patients with SSI
