BACKGROUND: Refractory autoimmune cytopenia (AIC) after allogeneic hematopoietic stem cell transplantation carries substantial morbidity and mortality in children. Conventional therapy fails in approximately half of patients, and escalating immunosuppression produces cumulative toxicity yet rarely durable remission. Autoimmune hepatitis (AIH) is an immune-mediated disease characterised by elevated transaminases, immunoglobulin G and autoantibody positivity. Treatment is based on corticosteroids and azathioprine. Up to 15% of patients will require a second line of treatment, with remission rates after this second line of about 60-75%. There is controversy about what the next step of treatment should be. We present the case of a woman diagnosed with IAH who failed several lines of treatment and had to start treatment with infliximab despite an off-label indication, but with a favorable clinical response.

The initial approach to autoimmune hepatitis is based on the use of immunosuppressants and corticosteroids. The first-line immunosuppression options include azathioprine or mycophenolate mofetil. Long-term immunosuppression predisposes patients to the development of neoplastic diseases. In this case, chronic AIH treatment with mycophenolate mofetil was associated with the onset of primary cerebral lymphoma, which was treated with methotrexate and rituximab. Rituximab is an interesting therapy for AIH, and in this case, it was able to treat both conditions.
