The authors report 2 cases of a condition often overlooked by the urologist: incrusted cystitis. It shows up clinically with severe urinary troubles and bladder calcifications. In fact the main finding is calcified plaques in the mucosa causing an inflammatory reaction and mimicking a tumor. Histologic examination of endoscopic biopsies rules out tumor and reveals a calcified necrotic mucosa. A high degree of alkalinity of the urine makes the diagnosis of incrusted cystitis after ruling out tuberculosis and bilharziasis. This study involved 27 geriatric patients with asymptomatic chronic bacteriuria; all had indwelling Foley catheters. A total of 78 strains of Haemophilus vaginalis were examined for 104 features. All strains fermented dextrin, maltose, and starch. Additionally, more than 90% of the strains fermented galactose, glucose, and ribose. Arbutin, cellobiose, melibiose, rhamnose, and salicin were not fermented by any of these strains. Borate was directly chelate-extracted from foods with 5% 2-ethyl-1,3-hexanediol (EHD) in n-hexane-n-butyl acetate mixture (8 + 2), from which borate was selectively transferred into 1% NaOH, since EHD-chelated boron did not react with curcumin to develop color. Over the past 15 years we have developed a protocol for evaluation and medical management of urinary calculi that has proved quite effective. For all patients we include plain x-ray films of the urinary tract, intravenous pyelograms, urine cultures and sensitivity studies, serum and 24-hour urinary chemical determinations, and analysis of any stones recovered. A sensitive and specific blood level method employing differential extraction was developed for the determination of clorazepate and its N-desmethyldiazepam metabolite by electron capture gas-liquid chromatography (GLC-ECD). The assay requires the initial extraction of N-desmethyldiazepam, the major metabolite, into benzene-methylene chloride (90:10) from the biological sample made alkaline with 0.1 N NaOH. The samples is then acidified with 2 N HCl to decarboxylate clorazepate to N-desmethyldiazepam, which is then extracted into benzene-methylene chloride (90:10) after adjusting the pH to 12.
