Many patients with lung cancer have disease disseminated beyond the thorax at the time of presentation. In patients with resectable disease, the first site of recurrence following surgery is frequently outside of the thorax. Therefore, the major problem in lung cancer is control of distant metastatic disease rather than failure to control local disease. For these reasons, adjuvant therapy has been directed primarily toward the microscopic systemic disease. Immunotherapy has shown some promise as a systemic agent in lung cancer, but it is clear from the early trials that the effect of immunothe

Eight investigators, using a common protocol, studied the use of intramuscular (15S)-15-methyl prostaglandin F2 alpha to abort midtrimester pregnancies which had failed treatment by other methods in 398 cases. Results of treatment of 3 subgroups, defined by reason for primary treatment failure, are presented. The combined efficacy rate was 96.2% with 79.6% of the abortions being complete. The mean time from starting intramuscular therapy to successful abortion was 7.35 hours. The incidence of gastrointestinal side-effects was lower than that previously reported for systemically administered pr

53 renal transplant recipients aged 13 to 58 years were followed up regularly clinically, microbiologically and serologically over observation periods ranging from 6 to 82 months following transplantation with a view to diagnosing mycetic complications. The only (facultative) pathogenic species found was Candida albicans. This species was isolated in every single patient, but far-reaching differences were observed with regard to the degree of form of infection in the individual person. Mainly, cases showed only a transient growth of Candida to a greater or lesser extent on the mucous membranes

Atopic dermatitis is a chronic disease marked by exacerbations and remissions. It begins in early infancy and may persist into late adulthood. Flares of dermatitis may be precipitated by emotional stess, extremes or sudden changes in humidity or temperature, and other factors. Treatment consists primarily of the use of mild topical agents to reduce inflammation and pruritus. Long-term systemic therapy with corticosteroid is not recommended. Topical preparations containing agents that increase the local levels of cyclic adenosine monophosphate (eg, caffeine) may be useful.

One hundred sixty-one patients with advanced Hodgkin's disease achieved complete remission after combination chemotherapy. Of these, 52 (32%) have subsequently relapsed and the patterns of relapse have been studied. The probability of relapse increases with increasing stage and particularly with the presence of systemic symptoms. Patients with Nodular Sclerosis histology are more likely to relapse than those with other histologies. Patients relapse primarily (92%) in sites of previous disease and particularly in nodal sites (75%). Nodal sites most frequently involved at relapse are the central

Hereditary and environmental factors are involved in the pathogenesis of essential hypertension. Obesity, salt intake and stress are predominant among the environmental influences. Autonomous nervous dysfunction, increased contractility of vascular smooth muscle cells and impaired renal handling of sodium are major abnormalities in essential hypertension. At present it cannot be decided if alterations in the activities of systemic or renal hormonal systems reflect primary defects or adaptive changes in the regulation of blood pressure. In any case the kidney is regarded to have a key position

The physiologic and pathophysiologic role of the prostaglandins in the lung is not fully understood. Prostaglandins seem to be important in the regulation of local blood flow. They might be involved in relaxation of the pulmonary vascular bed by opposing hypoxic pulmonary vasoconstriction and in constriction of the pulmonary vascular bed after microembolisation. The tonus of bronchial muscle is also probably modulated by the prostaglandins. Prostaglandin degradation in the lung might be another physiological role of lung function, which would allow the regulation of arterial blood levels. It i

Ca-antagonistic drugs increase cardiac oxygen supply by a coronary vasodilator action whilst they simultaneously lower myocardial oxygen demand. The latter drug effect is due to a decrease in cardiac contractile energy expenditure both directly (restriction of myocardial contractility) and indirectly (reduction of arterial blood pressure resulting from systemic vasodilation). In coronary disease all these factors can contribute to an improvement of the disturbed myocardial energy balance. As to the coronary vasodilator action of Ca-antagonistic drugs in particular, it is important to note that

This article describes the drugs used in helminthic infections and their therapeutic indications, mode of action, toxicity and other details of each of the recommended drugs, and discusses the nature and treatment of infection by helminths important in human medicine. Infestation due to the roundworms Enterobius vermicularis, Ascaris lumbricoides and the hookworms, Ancylostoma duodenale and Necator americanus can all be treated effectively with pyrantel pamoate. For Enterobius vermicularis, however, a newer drug, mebendazole, is equally as effective. The advantage of these drugs in the indicat

Angiostrongyliasis is an infectious disease caused by nematode parasites of the genus Angiostrongylus. The rat lung worm Angiostrongylus cantonensis, primarily a parasite of rodents, is largely responsible for human cases of eosinophilic meningitis, or meningoencerphalitis, which occurs on many Pacific islands and in Southeast Asia. The disorder, which frequently occurs in epidemic extent, is caused by invasion of the central nervous system by developing larvae of the parasite. The infection is most frequently due to ingestion of food containing the infective, third-state, larvae. Meningitic a

The in vivo mechanisms whereby systemic alpha- and beta-adrenergic stimulation exert opposing effects on renal water excretion are reviewed. An extrarenal mechanism is suggested since the effect of intravenous infusion of norepinephrine or isoproterenol on water excretion cannot be mimicked by the intrarenal administration of these agents. A ROLE OF VASOPRESSIN IS IMPLICATED SINCE NEITHER MAN NOR DOG WITHOUT A PITUITARY SOURCE OF VASOPRESSIN DEMONSTRATE THE SAME EFFECT OF CATECHOLAMINES ON WATER EXCRETION AS OBSERVED IN INTACT MAN AND DOG. Evidence also is presented that systemic alpha- and be

Although amphotericin B is the principal antibiotic for treating systemic mycoses, its clinical use is restricted, primarily because of the toxicity associated with the required prolonged therapy. Other investigators have reported results from in vitro experiments demonstrating that amphotericin B can potentiate antifungal activity of other antibiotics which are ineffective when used alone. In the present study, amphotericin B was used in combination with tetracycline for treating experimental coccidioidomycosis in mice. The results show that the combination of antibiotics is effective with a

Furosemide (4 - chloro - N - (2 - furyl - methyl) - 5 - sulfamoylanthranilic acid) caused a prompt increase in sodium, potassium, and chloride excretions in patients with chronic congestive heart failure. Doses of 50, 100, and 200 mg orally produced progressively increasing diuretic responses. When given over a period of one week to patients with essential hypertension, furosemide in doses of 100 to 200 mg orally per day caused a significant decrease in systolic and diastolic pressure. A significant lowering of blood pressure was also demonstrated in hypertensive patients treated with furosemi

The results of the experimental analysis reported in this and the two preceding papers (10, 11) indicate that in murine pneumococcal infections penicillin per se destroys the invading organisms only in those parts of the lesions where the bacteria are multiplying rapidly and are thus maximally susceptible to the bactericidal action of the drug. In areas where the bacterial growth rate is slowed, either because the pneumococci have reached a maximum population density, or because the accumulated exudate affords a relatively poor medium for rapid growth, the destructive effect of the antibiotic
