Adding artificial intelligence to the pulmonary embolism response team improves time to diagnosis and treatment of pulmonary embolism.

OBJECTIVE: There is limited data demonstrating the benefit of artificial intelligence (AI) technology for the diagnosis and triage of pulmonary embolism (PE). This study aimed to demonstrate improved time to diagnosis of PE and subsequent anticoagulation and intervention, with the goal of reducing in-hospital mortality. We hypothesized that implementation of AI-assisted computed tomography pulmonary angiogram (CTPA) detection would reduce time to diagnosis, anticoagulation, and intervention compared with the standard radiology-based workflow. METHODS: A single institution retrospective review

Personalized anticoagulant therapy in venous thromboembolism: current role of direct oral anticoagulants, unmet clinical needs, and future perspectives on Factor XI inhibition.

Venous thromboembolism (VTE), encompassing deep vein thrombosis and pulmonary embolism, remains a major cause of morbidity and mortality worldwide. The introduction of direct oral anticoagulants (DOACs) has transformed the management of VTE by providing effective and convenient alternatives to vitamin K antagonists. However, despite their favorable efficacy-safety profile, clinically relevant bleeding, treatment individualization, and the management of patients with competing thrombotic and hemorrhagic risks remain important unmet clinical needs. Contemporary VTE management increasingly requir

Chronic Anticoagulation Increases Complications in Obliterative Prolapse Surgery.

IMPORTANCE: Chronic anticoagulation has been identified as a risk factor for postoperative complications in other specialties, but there is a paucity of data on its effect on older women undergoing obliterative prolapse repair. OBJECTIVE: The objective of this study was to assess whether chronic anticoagulation is associated with increased serious adverse events (SAEs) after obliterative prolapse surgery. STUDY DESIGN: This was a retrospective cohort analysis of women 65 years or older undergoing obliterative prolapse repair at 2 academic institutions from 2016 to 2023. Patients were identif

[Livedoid vasculopathy: A case report].

Livedoid vasculopathy (LV) is a painful and thrombotic obliterative vascular disorder characterized by spontaneous thrombus formation within skin capillaries or small veins, ultimately leading to localized hypoxia and the development of skin ulcers. LV is a rare condition predominantly affecting young women, with an incidence rate of approximately 1/100 000. Clinically, it manifests as recurrent reticular green spots and painful ulcers on the distal lower extremities, which subsequently heal, leaving behind white and atrophic scars, termed "white atrophy". The skin manifestations of the diseas

Differences in treatment and outcome of patients with ST- elevation myocardial infarction (STEMI) and non-STEMI in Germany.

BACKGROUND: Although ST-segment elevation myocardial infarction (STEMI) and non-STEMI (NSTEMI) are very similar regarding pathophysiology and clinical treatments, especially NSTEMI comprises a much more heterogenic group of patients and underlying diseases. We therefore aimed to assess the treatments and outcomes of both entities in a large contemporary cohort. METHODS: Patients with STEMI and NSTEMI between 01/01/2010-31/12/2018 were identified from the largest German health insurance (AOK, ≈ 26 million members). Patient demographics, their hospital course, adherence to guideline-directed dr

Correlation of anticoagulation assays with bleeding during extracorporeal membrane oxygenation at a tertiary pediatric hospital.

IntroductionDue to anticoagulation during Extracorporeal Membrane Oxygenation (ECMO), patients face a higher risk of bleeding. This study aimed to assess the correlation between anticoagulation assays and hemorrhage in ECMO patients.MethodsWe reviewed records from patients in our critical care units, comparing anticoagulation parameters with clinical outcomes. Statistical analyses included the Wilcoxon rank sum test for continuous variables and chi-square or Fisher's exact tests for categorical variables. We used Kaplan-Meier curves to depict hemorrhage incidence and logistic regression to ide

Protocols for anticoagulation management in pediatric extracorporeal membrane oxygenation: A comparative retrospective study.

IntroductionIn children undergoing Extracorporeal Membrane Oxygenation (ECMO), anticoagulation is given to counterbalance the risk of thrombosis. Several laboratory tests are available to monitor heparin, but the ideal method still needs to be determined.MethodsThis retrospective cohort study included all patients under 18 years on ECMO support between 2010 and 2021. At our institution, the test used to monitor unfractionated heparin changed over time, dividing patients into three periods, using either activated clotting time (ACT) (2010-2014), activated partial thromboplastin time (aPTT) (201

Left Atrial Appendage Closure: Past, Present, and Future.

Left atrial appendage closure (LAAC) has emerged as an important alternative strategy for stroke prevention in patients with non-valvular atrial fibrillation (NVAF), particularly when long-term oral anticoagulation (OAC) is contraindicated or poorly tolerated. Initially developed as a surgical technique based on ligation or excision of the left atrial appendage, LAAC has progressively shifted toward refined, minimally invasive transcatheter approaches. Randomized and observational data have established non-inferiority to warfarin, and non-inferiority compared with direct oral anticoagulants ha

The efficacy of mechanical spinal manipulation in the standing position in patients with non-specific low back pain: a randomised placebo-controlled trial.

INTRODUCTION: Non-specific low back pain (NSLBP) is not attributable to a known cause and is a leading contributor to the global disease burden. The effect of specific spinal manipulation in patients with NSLBP using the vertical adjustment position standing (VAPS) technique described by Hartman remains unclear. This technique is used to address spinal restrictions and improve mobility. The main aim of the study was to examine the immediate and medium-term effects of VAPS on pain (VAS), mobility of the lumbar spine (MSt), and pain-related disability (ODI) in patients with non-specific low back

Exposure of nontarget white-footed mice (Peromyscus leucopus) to second-generation anticoagulant rodenticides in an urban context.

The pathways of nontarget exposure to anticoagulant rodenticides (ARs) are poorly understood and have yet to be examined in Ontario, Canada. We sought to determine if second-generation anticoagulant rodenticide (SGAR) baiting practices used by pest management professionals targeting commensal rodents may be causing rodenticide spillover into nontarget rodents in urban wildland areas, which could act as a vector of ARs to predators. We used snap traps to capture white-footed mice (Peromyscus leucopus) in urban wildland areas of Toronto and Vaughan, Ontario, near ongoing rodenticide baiting prog

Comprehensive precision analysis of the MIZAR 2.0 hemorheometer in EDTA-anticoagulated canine whole blood samples.

Hemorheology, the study of blood flow properties, utilizes the deformability, elasticity, and aggregability of RBCs to assess health and disease. Although promising in human medicine for diagnosing and monitoring various conditions, its clinical utility in dogs remains unexplored. Given that assay validation is paramount for establishing RIs and interpreting hemorheologic data, we characterized the precision of 13 hemorheologic RBC measurands in dogs using a novel hemorheometer (MIZAR; Alcor Scientific). We investigated the influence of precision type (within- and between-run), temperature (ro

Characterization of Patients with Atrial Fibrillation Treated Inadequately with Anticoagulants: Why Hospitalization Is Not an Opportunity to Correct Errors.

BACKGROUND: Atrial fibrillation (AF) is a common cardiac arrhythmia. Prevalence increases with age. It is a major public health concern due to high risk of thromboembolic complications such as stroke and systemic embolism. Direct oral anticoagulants (DOACs) have been shown to have improved safety and efficacy profiles and are the preferred therapy in all recent guidelines. However, patients are still discharged from internal medicine wards without appropriate DOAC therapy. OBJECTIVES: To investigate the effectiveness of hospitalization in correcting inappropriate therapy for AF in patients wh

Antithrombotic Therapy in Coronary Embolism: A Multicenter Study.

Background Coronary embolism (CE) is an uncommon cause of acute myocardial infarction, and the optimal post-discharge antithrombotic strategy remains uncertain. Methods Retrospective multicenter study including patients with definite or probable CE. Three groups of patients were identified: i) requiring with specific management, ii) with atrial fibrillation (AF), and iii) with no indication for oral anticoagulation (OAC). The latter two groups were further classified according to the antithrombotic treatment received, and long-term outcomes (composite of recurrent CE, stroke, systemic embolism

Anticoagulation-associated hemorrhage in atrial fibrillation as a marker of occult malignancy: a narrative review.

Atrial fibrillation (AF) is the most common arrhythmia in adults and significantly increases the risk of stroke. Due to common threats like systemic inflammation and treatment-related cardiotoxicity, individuals with cancer have a noticeably increased chance of developing AF. Managing AF in this population presents dual challenges: reducing thromboembolic risk while minimizing bleeding complications. Traditionally, warfarin, a vitamin K antagonist, has been the most widely used anticoagulant. However, the emergence of direct oral anticoagulants (DOACs), which offer equal or superior efficacy c

Venous thromboembolism in Cushing syndrome: current evidence and a practical thromboprophylaxis algorithm.

Cushing syndrome (CS) is a rare endocrine disorder associated with a markedly increased risk of venous thromboembolism (VTE). Chronic hypercortisolism induces a prothrombotic state through activation of coagulation pathways, impaired fibrinolysis, and endothelial dysfunction. VTE represents a major source of morbidity and mortality in this population, yet optimal strategies for prevention and treatment remain insufficiently standardized. To summarize current evidence on the epidemiology, pathophysiology, risk factors, prevention, and treatment of VTE in patients with endogenous CS and to propo

Discontinuation of oral anticoagulants is common after catheter ablation in older patients with atrial fibrillation.

AIMS: Guidelines recommend continuing oral anticoagulation (OAC) after atrial fibrillation (AF) ablation based on stroke risk, yet real-world post-procedural OAC management remains poorly characterized. We aimed to evaluate contemporary patterns of OAC use after AF ablation and identify predictors of OAC discontinuation. METHODS AND RESULTS: Using Medicare claims from 2013-2022, we established a cohort of patients undergoing a first AF ablation and assessed patterns of OAC discontinuation during the subsequent year. We defined OAC discontinuation as a dispensing gap ≥60 days. Baseline charact
