A novel risk-scoring system based on endoscopic ultrasound and clinical characteristics for the preoperative diagnosis of small gastric gastrointestinal stromal tumors.

The preoperative differentiation of small gastric gastrointestinal stromal tumors (GISTs) from other submucosal lesions remains clinically challenging due to overlapping endoscopic ultrasound (EUS) features. This study aimed to develop and validate a novel risk-scoring system integrating EUS imaging and clinical characteristics to improve preoperative diagnostic accuracy for small gastric GISTs versus non-GIST submucosal tumors. We retrospectively analyzed 1303 patients with gastric submucosal tumors (SMTs), who were divided into a training cohort (n = 670), an internal validation cohort (n =

An integrative analysis combining network pharmacology, transcriptomics, and experimental validation of the Tibetan medicine Renqing Changjue in gastric ulcer protection.

ETHNOPHARMACOLOGICAL RELEVANCE: Renqing Changjue (RQCJ), a time-honored Tibetan prescription, has long been used to treat "heat toxin"-related disorders, particularly those affecting the digestive system. Despite its extensive clinical use, the pharmacodynamic substances and molecular mechanisms underlying its anti-ulcer effects remain poorly defined. AIM OF THE STUDY: This study aimed to identify the key active constituents, core targets, and underlying mechanisms by which RQCJ protects against ethanol-induced gastric ulcer. MATERIALS AND METHODS: An integrative multi-omics framework combin

Development and Validation of an Individualized Nomogram to Identify Undifferentiated-Predominant Mixed-Type Early Gastric Cancer.

OBJECTIVES: Diagnosis of undifferentiated-type-predominant mixed-type early gastric cancer (UM-EGC) remains challenging due to its complex histological features and overlapping characteristics with other gastric cancer types. We aimed to develop a novel nomogram based on clinicopathological and endoscopic features and validate its performance in predicting UM-EGC prior to endoscopic treatment. METHODS: In this retrospective single-center study, 808 patients with early gastric cancer (EGC) who underwent curative endoscopic submucosal dissection were included. Among them, 493 were assigned to t

From common to rare: imaging spectrum of gastric malignancies.

Multidetector CT has improved the detection, diagnosis, and characterization of gastric malignancies, and as well as the evaluation of distant metastases. Gastric malignancies range from common types such as adenocarcinoma, lymphoma, and GISTs to rare types such as neuroendocrine tumors, liposarcoma, leiomyosarcoma, plasmacytoma, and metastases from other cancers. CT is often the preferred modality for their assessment. These tumors can appear as a single lesion, multifocal involvement, or diffuse wall thickening, and they often have overlapping imaging features, making diagnosis challenging.

The impact of gastric adenocarcinoma location and clinical and socioeconomic determinants on survival: a retrospective population-based cohort study using SEER data (1975-2016).

BACKGROUND: Despite the decreasing incidence of gastric cancer in the United States, cancers of the stomach remain one of the leading causes of cancer related death in globally. Most patients are asymptomatic and have advanced disease when diagnosed. Studies have shown similar prognosis for proximal and distal tumors, although most studies exclude lesions in the cardia given overlap with esophageal cancers. Distal tumors involving the pylorus often leads to symptoms, such as gastric outlet obstruction presenting with nausea and emesis, which may lead to earlier diagnosis; therefore, we compare

Diagnostic Challenge of Gastric Glomangioma Mimicking Gastrointestinal Stromal Tumor: A Case Report.

INTRODUCTION: Gastric glomus tumors (GGTs) are rare benign mesenchymal neoplasms of the stomach, often mistaken preoperatively for more common submucosal lesions such as gastrointestinal stromal tumors (GISTs). Accurate diagnosis requires a combination of endoscopic ultrasonography, histopathology, and immunohistochemistry. CASE DESCRIPTION: We report a 51-year-old female with a two-year history of epigastric burning, anorexia, and unexplained weight loss. Initial evaluation included EUS, which identified a 1.8 × 2.4 cm hypoechoic submucosal lesion arising from the muscularis propria at the g

Massive Chronic Expanding Hematoma Following EUS-guided Biopsy of a Small Gastric Plexiform Fibromyxoma Mimicking a Gastrointestinal Stromal Tumor.

BACKGROUND/AIM: Plexiform fibromyxoma (PF) is a rare benign mesenchymal tumor of the stomach that can be difficult to distinguish from gastrointestinal stromal tumor (GIST) due to overlapping clinical and imaging features. The risk of hemorrhagic complications following biopsy in PF has not been well established. CASE REPORT: A 76-year-old man was followed for a small gastric submucosal lesion that showed gradual enlargement over time. Endoscopic ultrasound (EUS)-guided biopsy was performed to obtain a definitive diagnosis, and the initial histopathological diagnosis suggested GIST. Although

Analysis of CT and MRI imaging features of ectopic pancreas.

BACKGROUND: Ectopic pancreas (EP) is a rare congenital anomaly that is often difficult to diagnose preoperatively due to its nonspecific clinical manifestations and imaging overlap with other gastrointestinal subepithelial lesions. Several characteristic imaging findings have been reported; however, the quantitative imaging features of EP remain insufficiently investigated. Therefore, this study aimed to characterize the clinical and imaging manifestations of EP, summarize its computed tomography (CT) features, quantitatively compare CT attenuation values between EP and native pancreas across

GastroMalign: Vision Transformer-Based Framework for Early Detection and Malignancy-Risk Stratification for High-Risk Gastrointestinal Lesions.

Background: Current artificial intelligence (AI) systems in gastrointestinal (GI) endoscopy primarily emphasize binary detection or static classification, providing limited support for the graded assessment of malignant potential that underpins clinical decision-making. We developed GastroMalign, a transformer-based framework designed to stratify GI lesions according to ordinal disease severity while maintaining clinical interpretability, addressing this unmet need in endoscopic risk assessment. Methods: This retrospective development and validation study used the publicly available GastroVisi

Follicular dendritic cell sarcoma associated with IgG4-related disease: a case report and literature review.

Follicular dendritic cell sarcoma (FDCS) is a rare malignant tumor involving lymph nodes and extranodal sites, classified into conventional and inflammatory pseudotumor (IPT)-like variants. The IPT-like variant predominantly involves the liver and spleen. Immunoglobulin G4-related disease (IgG4-RD) is a systemic immune-mediated disorder characterized by multiorgan involvement, whereas isolated esophageal and gastric involvement is exceedingly rare. Although FDCS may exhibit infiltration of IgG4-positive plasma cells, the coexistence of FDCS and IgG4-RD has not been previously reported. Herein,

Multimodal Diagnosis and Management of Gastric Tuberculosis: A Case Report.

Gastric tuberculosis is an exceptionally rare form of extrapulmonary tuberculosis and often presents with non-specific symptoms and endoscopic appearances, frequently mimicking submucosal tumors, peptic ulcer disease, or malignancy. These overlaps, together with the limited yield of superficial biopsies, can delay definitive diagnosis. Here, researchers report a case of gastric tuberculosis and emphasize the methodological advantage of a multimodal diagnostic strategy that integrates endoscopy, cross-sectional imaging, histopathology, and molecular testing to progressively narrow diagnostic un

Gel immersion in endoscopy: Exploring potential applications.

The challenge of effectively eliminating air during gastrointestinal endoscopy using ultrasound techniques is apparent. This difficulty arises from the intricacies of removing concealed air within the folds of the gastrointestinal tract, resulting in artifacts and compromised visualization. In addition, the overlap of folds with lesions can obscure their depth and size, presenting challenges for an accurate assessment. Conversely, in intricately folded regions of the gastrointestinal tract, such as the stomach, intestine, and colon, insufficient delivery of air or CO2 into the cavity impedes l

Vitamin C Mediates IGFBP7 to Alleviate Chronic Atrophic Gastritis via the HIF-1α/VEGF Pathway.

Chronic atrophic gastritis (CAG) is a precancerous lesion characterised by gastric mucosal atrophy and inflammation. Identifying key molecular mechanisms and potential therapeutic targets is essential to improve patient outcomes. Key modules and differentially expressed genes (DEGs) were recognised in the GSE153224 dataset using weighted gene co-expression network analysis (WGCNA) and examination of differential expression. IGFBP7 was identified as a hub gene by protein-protein interaction (PPI) network and expression validation. CAG patients' blood parameters and gastric mucosal health status

Network analysis and experimental validation to investigate chenpi against functional dyspepsia through TLR4/MyD88 by regulating the gut microbial structure.

Functional dyspepsia (FD) is a prevalent functional gastrointestinal disorder characterized by the absence of organic lesions; it affects nearly one-fifth of the global population. There is currently no specific drug for treating it. Citri reticulatae Pericarpium (CRP) has been utilized in China for millennia as a therapeutic agent for alleviating bloating and spleen-stomach disharmony. Nonetheless, the curative efficacy and precise molecular mechanisms implicated in FD warrant further investigation. This study aims to address this gap by investigating the potential mechanisms of CRP against F

Age-Related Differences in Gastric Adenocarcinoma from 2000-2020: A SEER Database Analysis.

INTRODUCTION: Gastric cancer (GC) is the 5th most common malignancy worldwide. Adenocarcinoma accounts for 95% of all GC. While most cases of gastric adenocarcinoma (GAC) arise in older age males, a significant proportion of biological disparity exists between different ages. In this study, we aim to compare the demographics and prognostic outcomes of different ages of patients with GAC. METHODS: This retrospective study utilized the GAC cases abstracted from the Surveillance, Epidemiology, and End Results Program's 17 registries dataset from 2000 to 2020. Cases were divided into different ag

Malignant gastric glomus tumor with heterochronous liver metastases: a case report and review of the literature.

BACKGROUND: Glomus tumors are mesenchymal tumors originating from the neuromuscular arterial canal or vascular lumen. While most gastric glomus tumors are benign and solitary, rare cases can be malignant and have a poor prognosis. Definitive treatment options remain limited for multifocal metastases, often necessitating salvage therapy. CASE PRESENTATION: A 36-year-old Han Chinese female patient presented with hematemesis, melena, and syncope persisting for four days. Diagnostic evaluations revealed a malignant gastric glomus tumor, prompting radical resection. During follow-up, radiological
