Atrial fibrillation (AF) is the most common arrhythmia in end-stage renal disease (ESRD) patients on dialysis. These patients face high risks of both thromboembolism and bleeding. Pivotal randomized controlled trials (RCTs) of direct oral anticoagulants (DOACs) excluded these patients, leaving the optimal anticoagulant choice uncertain. We conducted a systematic review and meta-analysis investigating the efficacy and safety of DOACs versus VKAs in AF patients undergoing dialysis. We searched PubMed, Scopus, Web of Science, and Cochrane Library for RCTs and observational studies. Primary outcom

BACKGROUND: Patients undergoing transcatheter aortic valve replacement (TAVR) often require anticoagulation therapy due to various comorbidities. However, the decision to continue anticoagulation therapy during the TAVR perioperative period and the optimal regimen for postoperative anticoagulation therapy remain debatable.
METHODS: We searched the PubMed, EMBASE, MEDLINE, and Cochrane Library databases from their date of establishment to September 2024. We evaluated the choice of anticoagulation regimen in the TAVR perioperative and postoperative period. The primary outcomes were stroke, majo

BACKGROUND: Vitamin K, through its role in the vitamin K-dependent activation of matrix-GLA-protein, has been suggested to have a lung-protective effect, though the mechanism is unknown. Chronic obstructive pulmonary disease (COPD) patients treated with vitamin K antagonists (VKAs) may lose this protection, thereby increasing their risk of an acute exacerbation of COPD (AE-COPD) and death. We examined this hypothesis in a nationwide cohort of COPD patients treated with VKA. COPD patients treated with direct oral anticoagulant (DOAC) served as controls.
OBJECTIVE: To assess the association bet

BACKGROUND: Recurrent venous thromboembolism (VTE) is a major concern after stopping anticoagulation. Thrombin generation (TG) parameters-including thrombomodulin (TM) sensitivity-have been proposed as biomarkers of recurrence, but findings remain inconsistent. The role of thrombin dynamic (TD) parameters, including prothrombin conversion and thrombin inactivation, is unclear.
OBJECTIVES: This study evaluated whether TG and TD parameters are associated with VTE recurrence.
METHODS: In 589 patients from the prospective VISTA study, TG was measured during anticoagulation and 1 month after stop

BACKGROUND: The risk-benefit profile of direct oral anticoagulants (DOACs) in kidney transplant recipients (KTRs) with atrial fibrillation (AF) remains under-investigated.
PURPOSE: To evaluate the safety and efficacy of DOACs compared with vitamin K antagonists (VKAs) in KTRs with AF.
METHODS: Retrospective analysis from TriNetX network. Adult KTRs with AF were included. Patients with mechanical valves, antiphospholipid syndrome, or an estimated eGFR <15 mL/min/1.73 m² were excluded. Anticoagulated patients were stratified into two cohorts (DOACs versus VKAs) . Cancer-associated venous thromboembolism (VTE) is one of the most frequent and life-threatening complications in oncology, representing the second leading cause of death in patients with malignancy. Its pathogenesis is multifactorial, driven by tumor-specific procoagulant activity, systemic inflammation, and the prothrombotic effects of anticancer therapies. The risk is particularly high in pancreatic, gastric, cerebral, and pulmonary cancers and is further amplified by advanced disease stage, comorbidities, and treatment-related factors. Management of cancer-associated VTE requires a careful

RATIONALE: Warfarin is a widely used vitamin K antagonist requiring strict international normalized ratio monitoring due to its narrow therapeutic window. While the interaction between warfarin and vitamin K-rich foods is well known, limited data exist regarding the anticoagulant effects of persimmons, a fruit rich in various bioactive compounds.
PATIENT CONCERNS: A 51-year-old woman with a history of double mechanical valve replacement presented for routine follow-up. She reported no symptoms or changes in medication but had consumed unripe persimmons ("Dangam") daily for 2 weeks prior to a

The impact of the choice of common data model (CDM) approach on the study results in a real-world evidence (RWE) study is unknown. We aimed to determine potential differences in the results of an RWE study when data were mapped to two different CDMs, ConcePTION and OMOP. With the same instance of CPRD GOLD, data were mapped to both CDMs. Using the same programming steps, we estimated the risk of direct oral anticoagulants (DOACs) vs. vitamin K antagonists (VKAs) on bleeding and cardiovascular (CVD) outcomes in patients with non-valvular atrial fibrillation. Baseline characteristics, incidence

BACKGROUND: We investigated population-wide changes in prescriptions of oral anticoagulants (OAC) (vitamin K antagonists [VKA]) compared to direct oral anticoagulants (DOAC) and whether these translated into a decline in incidences of OAC-related intracerebral hemorrhages (ICH).
METHODS: This observational study analyzed OAC use in Southern Finland during 2005-2009 (pre-DOAC era) and 2015-2019 (transition-to-DOAC era) based on national registries. From medical records, we identified all non-traumatic ICH patients admitted to our tertiary hospital, the only one admitting neurological emergenci

Vitamin K antagonists (VKAs) remain the standard anticoagulation therapy in patients undergoing mechanical valve replacement; despite their efficiency in this clinical context, they are associated with a very narrow therapeutic window, a factor that links them to a major risk of bleeding. Their broad involvement in the coagulation cascade and their interactions with multiple commonly used drugs can lead to supratherapeutic anticoagulation. Furthermore, prosthetic valve infective endocarditis is a serious complication that poses a diagnostic challenge, especially when echocardiographic findings

BACKGROUND: Guidance for antiphospholipid antibody (aPL) testing after unprovoked venous thromboembolism (VTE) is inconsistent. Real-world data on the prevalence and outcomes of antiphospholipid syndrome (APS) in those presenting with unprovoked VTE are limited.
OBJECTIVES: This study determined APS prevalence after unprovoked VTE, compared outcomes from universal vs selective aPL testing, and assessed thrombosis recurrence in patients with APS within a multicenter UK cohort.
METHODS: This retrospective analysis included 897 patients with unprovoked VTE from 5 UK centers. One center used uni

This report presents a case of a dialysis patient who developed severe hypoprothrombinemia associated with vitamin K deficiency during prolonged cefazolin (CEZ) therapy. A 73-year-old man undergoing maintenance hemodialysis for end-stage kidney disease due to diabetic nephropathy was hospitalized for purulent tenosynovitis of the right ankle and treated with CEZ. During hospitalization, he had persistent anorexia and experienced recurrent gastrointestinal bleeding. On a non-dialysis day, he developed a subcutaneous hematoma around the arteriovenous fistula. Despite the absence of anticoagulant

Anticoagulants are essential for preventing and treating thromboembolism across a wide range of clinical settings. Yet, they share a narrow therapeutic index that demands a precise understanding of their pharmacodynamic behavior. Thrombin generation provides an integrated measure of anticoagulant effects by capturing the timing, velocity, and total amount of thrombin formed over time. We reviewed the literature to establish a unified theoretical framework, grounded in mechanistic rationale and supported by experimentation, to better define the pharmacodynamics of anticoagulation therapy using

INTRODUCTION: Direct oral anticoagulants (DOACs) are preferred as the first line oral anticoagulation therapy for atrial fibrillation and venous thromboembolism, given their superior efficacy and safety profile over vitamin K antagonists (VKAs). However, it can be challenging to select the optimal agent, dose or strategy in many clinical situations.
AREAS COVERED: This narrative review summarizes the current recommendations for optimization of DOAC therapy and attempts to shed light from recent evidence where there is clinical uncertainty. Particular focus is given to patients with renal impa

Direct oral anticoagulants (DOACs) were historically avoided in cirrhosis due to concerns regarding impaired hepatic metabolism, cumulative toxicity, and the systematic exclusion of patients with cirrhosis from pivotal randomized trials. However, over the past decade, a growing body of retrospective cohorts, meta-analyses, and small prospective studies - including limited randomized comparisons - have evaluated DOACs against vitamin K antagonists (VKAs) for management of venous thromboembolism (VTE), splanchnic vein thrombosis (SVT), and atrial fibrillation (AF) in cirrhosis. In Child-Pugh A a

OBJECTIVE: To compare the safety profile of direct oral anticoagulants (DOACs) and vitamin K antagonists (VKAs) in the management of cerebral venous thrombosis (CVT) following initial parenteral therapy.
MATERIAL AND METHODS: The Eurasian Registry of Cerebral Venous Thrombosis (ER-CVT) is a multicenter, ambispective, comparative cohort study conducted in accordance with the DOAC-CVT protocol. The primary endpoint was the composite of symptomatic venous thromboembolism (VTE) and/or major bleeding, as defined by ISTH criteria, during a 6-month follow-up period. Secondary endpoints included comp

Inherited genetic disorders are progressive in nature and lead to organ dysfunction or death in severe cases. At present, there are no permanent treatment options for >95% of inherited disorders. Different modes of inheritance, type of gene(s) involved, and population-based variations add further complexity to finding suitable cures for approximately 400 million patients worldwide. Gene therapy is a very promising molecular technique for the treatment of rare genetic disorders. Gene therapy functions on the basis of restoration, replacement, inhibition, and, most recently, editing of gene(s) t

BACKGROUND: People undergoing major orthopaedic surgery are at increased risk of postoperative thromboembolic events. Low molecular weight heparins (LMWHs) are recommended for thromboprophylaxis in this population. New oral anticoagulants, including direct factor Xa inhibitors, are recommended as alternatives. They may have more advantages than disadvantages compared to LMWHs and vitamin K antagonists (VKAs, another type of anticoagulant).
OBJECTIVES: To assess the benefits and harms of prophylactic anticoagulation with direct factor Xa inhibitors compared with low molecular weight heparins a
