Optimal pharmacological therapy for minor acute ischemic stroke: a network meta-analysis.

BACKGROUND: Minor acute ischemic stroke (mAIS), typically defined as a National Institutes of Health Stroke Scale (NIHSS) score ≤ 5, accounts for over 50% of all ischemic strokes. However, the optimal pharmacological therapy for patients with mAIS remains controversial. Hence, this systematic review and network meta-analysis (NMA) was conducted to compare the effectiveness and safety of available pharmacological therapies for mAIS patients. METHODS: PubMed, Embase, the Cochrane Library, and Web of Science databases were systematically searched for publications up to June 1, 2025. Randomized c

Combinatorial Biosynthesis of Antithrombotic Flavonoid 6-O-Glycosides From Carthamus tinctorius.

Flavonoid 6-O-glycosides from Carthamus tinctorius (safflower) represent a unique class of potential antithrombotic agents. However, their pharmaceutical development has been hindered by limited natural resources. In this study, we characterized four key biosynthetic enzymes from C. tinctorius: Ct6OGT (6-O-glycosyltransferase), Ct7OGT (7-O-glycosyltransferase), Ct3OGT (3-O-glycosyltransferase), and CtFLS (flavonol synthase). Among them, Ct6OGT is the first identified high-regioselectivity flavonoid 6-O-glycosyltransferase. By combining these enzymes with upstream genes identified from safflowe

Small Bowel Lesions and Bleeding Risk in Hemodialysis Patients: A Narrative Review.

BACKGROUND: Gastrointestinal bleeding is very common among hemodialysis patients. This high bleeding risk is caused by uremic platelet dysfunction, vascular fragility, intradialytic hemodynamic instability, and widespread antithrombotic therapy. Small bowel lesions, especially angiodysplasias, constitute a significant but often overlooked source of recurrent or occult hemorrhage while conventional endoscopy frequently fails to identify these lesions. OBJECTIVES: Investigation of small bowel lesions in hemodialysis patients and identification of bleeding risk of this vulnerable population. DE

Biomimetic platelet membrane liposomes enable targeted shikonin delivery for thrombosis therapy.

Thrombosis is central to cardiovascular disease, yet current therapies are limited by inefficient drug delivery, poor thrombus targeting, and bleeding risk. Here we present a biomimetic platelet membrane-coated liposomal nanoplatform (SK-PLT) that integrates natural pharmacologic activity with targeted delivery. In a high-throughput screen of 171 natural products, shikonin (SK) inhibited platelet aggregation induced by ADP, thrombin, and collagen, with inhibition rates of 83.3%, 73.5%, and 81.6%, respectively. SK was then loaded into liposomes formulated with ginsenoside Rg3, which provides in

Identification of antithrombotic active components in Xuefu Zhuyu decoctionby thrombin targeted affinity ultrafiltration mass spectrometry.

OBJECTIVE: Since screening compound active ingredients is more complex than screening single ingredients, we developed a one-step high-throughput screening method using Affinity Ultrafiltration-Mass Spectrometry (AUF-UPLC-Q-TOF-MS) to rapidly identify the active anti-thrombotic components in Xuefu Zhuyu Decoction. METHOD: In this study, 50 g of Xuefu Zhuyu Decoction was subjected to reflux extraction with 90% ethanol, followed by concentration and sequential extraction using ethyl acetate, n-butanol and water. The n-butanol fraction was selected as the key research target. Using thrombin as t

Nattokinase: A multifunctional food-derived enzyme for thrombophilia management.

A thrombophilic tendency (thrombophilia) is a hypercoagulable state that significantly increases the risk of thromboembolic events. Although conventional anticoagulant therapy is effective, the inherent risk of bleeding limits its long-term use, creating an urgent need for safer and more effective alternatives. Nattokinase (NK)- a serine protease derived from the fermentation of natto by Bacillus subtilis- exerts its antithrombotic effects through multiple mechanisms and targets, including direct degradation of fibrin, inhibition of platelet aggregation, and antioxidant/anti-inflammatory actio

Clavukellines A-N: dolabellane diterpenes from the soft coral Clavulariakoellikeri.

Chemical investigation of the soft coral Clavularia koellikeri led to the isolation of fourteen new diterpenoid derivatives clavukellines A-N (1-14) featuring the dolabellane-type skeleton. Structural characterization of these compounds was accomplished through an integrated analytical strategy combining NMR spectroscopy and HR-ESI-MS. Absolute configurations were unambiguously assigned using ECD spectral simulation and DP4 + probability analysis. Preliminary pharmacological evaluation indicated that compounds 8 and 13 exhibited anti-inflammatory activity, while compounds 1-14 showed antithrom

Comparison of the Biologic Activity of Different Species of Heparin.

Heparin and related low molecular weight heparin derivatives remain critical drugs for the prevention and treatment of thrombotic conditions. Concerns relating to the stability and sufficiency of the porcine intestinal mucosa supplies from which most heparin is currently derived have spurred an interest in identifying other sources for heparin. Commercial quantities of heparin may be obtained from cow and sheep intestine, and heparin or heparin-like material has been identified in a number of other species. Being biologic in nature, these heparins exhibit unique structural and functional profi

The Management of Antithrombotic Therapy in Image-Guided Breast Biopsy: A Systematic Review With Meta-Analysis.

OBJECTIVE: Antithrombotic therapy (AT) is crucial for preventing life-threatening thromboembolic events (TEEs). However, concerns about bleeding events (BEs) often lead to AT discontinuation before image-guided breast biopsy (iBB). This systematic review and meta-analysis assess the necessity and safety of AT suspension prior to iBB. METHODS: A systematic review was conducted using Embase and PubMed/MEDLINE databases through July 2024. Studies evaluating BEs in patients who had AT and were undergoing iBB were included. Case reports, surveys, and nonretrievable full texts were excluded. Data a
