Magnetic Force Enhances Pharmacologic Thrombolysis by Unlocking the "Knob-Hole" Structure of Fibrin.

Thrombosis is a prevalent pathology underlying cardiovascular diseases, and pharmacologic thrombolysis is widely used in clinical. However, the dense structure of the fibrin network restricts the penetration and mobility of thrombolytic drugs within the thrombus, thereby reducing their efficiency. Disrupting the integrity of the thrombus requires high energy and generates small pieces of thrombus that increase the risk of pulmonary embolism. Here, it is demonstrated that applying a magnetic force at the pN level to unlock the "knob-hole" structure of fibrin disrupts the dense and stable mechan

Exosomes: the future of acellular nanotherapeutics in regenerative vascularization.

BACKGROUND: Ischemic disorders represent the world's leading cause of morbidity and mortality and can emanate from pathology in both the macrovasculature and microvasculature. Current treatment options for macrovascular disease include surgical bypass, endovascular intervention, thrombolytic drugs, and pharmacologics (vasodilators). However, when ischemia occurs at the microvascular level, conventional vascular surgical approaches are typically not feasible. In this setting, complex reconstructive surgery may be warranted, especially if concurrent open wounds are present. Thus, new pro-angioge

Thrombolytic drugs for ischemic stroke: historical perspective, state of play, and future developments.

Thrombolytic agents are serine proteases or related enzymes that facilitate blood clot breakdown via a process called thrombolysis. They work either indirectly, via activation of plasminogen into plasmin-a potent fibrin-degrading enzyme-or by direct dissolution of fibrin within clots. Numerous clinical trials investigating thrombolytic candidates for major thrombotic conditions, such as heart attack and acute ischaemic stroke (AIS), have led to the development of several potential therapies. However, while some drugs show clinical benefits, most still possess substantial limitations, especiall

Photothermal-Therapy-Based Targeting Thrombolytic Therapy.

Thrombosis, a common underlying mechanism of myocardial infarction, ischemic stroke, and venous thromboembolism, is the leading cause of death in patients. Owing to their lack of targeting ability, short half-life, low utilization rate, and high risk of bleeding side effects, the current first-line thrombolytic drugs are unable to meet the requirements for effective treatment of thrombi. Photothermal therapy (PTT) represents a promising thrombolytic modality due to its precise spatiotemporal selectivity and minimal invasiveness. However, the efficacy of PTT is constrained by the limited penetr

Research progress on the application of nanomaterials in the diagnosis and treatment of venous thromboembolism.

Venous thromboembolism (VTE), including deep vein thrombosis (DVT) and pulmonary embolism (PE), is a serious vascular disease with hidden symptoms and rapid progression. Nanomaterials provide new ideas for the diagnosis and treatment of VTE due to their high specific surface area, biocompatibility and modifiability. Due to differences in the formation mechanism and location of arterial and venous thrombosis, targeted diagnosis and treatment strategies need to be developed. This review focuses on VTE and summarizes the latest progress and limitations of nanomaterials in diagnosis and treatment.

Multi-classification Deep Learning Approach for Diagnosing Stroke Type and Severity Using Multimodal Magnetic Resonance Images.

BACKGROUND: Clinical decisions for stroke treatments, such as thrombolytic drugs for ischemic strokes or anticoagulants for hemorrhagic strokes, rely on accurate diagnosis and severity assessment. Our study uses diffusion-weighted magnetic resonance imaging and Convolutional Neural Networks (CNNs) to differentiate healthy and stroke samples, classify stroke types, and predict severity, aiding in decision-making for stroke management. METHODS: We evaluated 143 patients: 85 with ischemic stroke and 58 with hemorrhagic stroke. For stroke diagnosis, we compared multimodal (apparent diffusion coef

Ultrasound Trigger Ultrasmall Multifunction Nanobubbles for Thrombus Visual Treatment.

Intravenous injection of thrombolytic drugs is the prevailing strategy for the treatment of thrombotic disease. However, its applications are limited due to hemorrhagic risk and thrombus microenvironment. In pursuit of visualized thrombolysis and inspired by the excessive reactive oxygen species (ROS) and inflammatory factors within the thrombus site, an ultrasmall nanobubble (CeO2/MnOX@UK@PFP) for integration of imaging and therapy by low intensity pulse ultrasound (LIPUS) trigger is designed, which achieved the visual delivery of Urokinas (UK) and ROS scavenging agents to adjust thrombus mic

Branched silver-iron oxide nanoparticles enabling highly effective targeted and localised drug-free thrombolysis.

Ultrasound has been widely used as an external stimulus to trigger drug release from nanomaterials in thrombosis treatment. Here, we introduce a novel strategy leveraging nanomaterials not for drug delivery, but for enhancing US-induced thrombolysis. This innovative strategy is particularly significant, as thrombolytic drugs inherently pose a risk of systemic bleeding. We combined branched silver-iron oxide nanoparticles (AgIONPs) with low-intensity focused ultrasound to evaluate their thrombolytic potential. Binding assays in in vitro human blood clots and in a thrombosis mouse model confirme

Risk of New Indications for Anticoagulants and Thrombolytics in People With Cognitive Impairment: Implications for Anti-Amyloid Therapy.

OBJECTIVES: Anticoagulants and thrombolytics may interact with anti-amyloid monoclonal antibodies (mAbs) to increase intracranial hemorrhage risk, so expert guidance recommends against their co-prescription. We aimed to estimate how many people with mild cognitive impairment (MCI) or dementia develop a new cardiovascular indication for anticoagulant and thrombolytic drugs. METHODS: In a longitudinal cohort of adults aged 65 years or older from the Health and Retirement Study (2010-2020) with linked Medicare claims and no previous indication for anticoagulants, cognition was categorized as nor

Evolution of neutrophil extracellular traps in the pathology of stroke.

Stroke is a major cause of death and disability worldwide, and its pathogenesis is complex, involving multiple pathological processes, such as thrombosis, ischemia-reperfusion injury, inflammatory response, and blood-brain barrier disruption. In recent years, neutrophil extracellular traps have been found to be involved in the body's anti-infection defense and to play an important role in stroke. Studies have shown that neutrophil extracellular traps promote thrombus expansion and neuroinflammation in ischemic stroke, and they may be involved in disease progression and recovery in hemorrhagic

Measurement of enzyme activity of insoluble substrates based on ordered porous layer interferometry and the application in evaluation of thrombolytic drugs.

The development of innovative methods for real-time surveillance of enzymatic activity determination processes is essential, particularly for insoluble substrate enzymatic assessments. In this work, a novel method for enzymatic activity determination was devised by assembling a 190 nm silica colloidal crystal (SCC) film onto a glass slide, coupled with Ordered Porous Layer Interferometry (OPLI) technology. By fixing the substrate of the enzyme on the surface of the silica sphere, a solid-liquid interface can be formed for monitoring enzymatic activity. The enzymatic activity is gauged by the c

Nanomicrosphere sustained-release urokinase systems with antioxidant properties for deep vein thrombosis therapy.

Deep vein thrombosis (DVT) is a venous return disorder caused by abnormal clotting of blood in deep veins. After thrombosis, most of the thrombus will spread to the deep vein trunk throughout the limb. If DVT is not treated in time, most of them will develop into thrombosis sequelae and even threaten life. Intravenous thrombolytic drugs are the most promising strategy for treating DVT, but current drugs used for thrombolysis suffer from short half-lives and narrow therapeutic indexes. To effectively manage DVT, it is necessary to develop a novel multifunctional drug-loading system to effective

Diverse origins of fibrinolytic enzymes: A comprehensive review.

Fibrinolytic enzymes cleave fibrin which plays a crucial role in thrombus formation which otherwise leads to cardiovascular diseases. While different fibrinolytic enzymes have been purified, only a few have been utilized as clinical and therapeutic agents; hence, the search continues for a fibrinolytic enzyme with high specificity, fewer side effects, and one that can be mass-produced at a lower cost with a higher yield. In this context, this review discusses the physiological mechanism of thrombus formation and fibrinolysis, and current thrombolytic drugs in use. Additionally, an overview of
