Preoperative guidance and an adolescent's surgical recovery experience: a qualitative study.

BACKGROUND: Adolescents are at increased risk of persistent postsurgical opioid use. Qualitative data may provide insight on protective experiences and guidance. STUDY DESIGN: Adolescents aged 11-21 years who were enrolled in a larger longitudinal survey study participated in semi-structured interviews one and 3 months after surgery between September 2022 and March 2023. Experiences were examined among adolescents who used opioids in postoperative weeks 3 and 4, after severe pain resolved. RESULTS: Fifty interviews with 46 adolescents undergoing common procedures associated with severe pain

Clinical Instruments Assessing Harms and Benefits of Long-Term Opioid Therapy for Chronic Pain: A Scoping Review.

BACKGROUND: While rates of opioid prescribing for chronic non-cancer pain have declined, long-term opioid therapy (LTOT) remains common. Current clinical guidelines emphasize the importance of continuous reassessment and weighing of harms and benefits associated with LTOT to inform treatment decisions. However, specific guidance on which harms and benefits to assess and how to weigh these factors is generally lacking. This scoping review aimed to identify and characterize clinical instruments used to assess opioid-related harms and benefits in adults receiving LTOT for chronic non-cancer pain

Opioid Risk Tool Scores are Not Associated With Opioid Misuse After Elective General Surgery: A Prospective Study.

INTRODUCTION: Assessment for opioid misuse is not widely utilized preoperatively. Yet, it can offer valuable clinical insight for patient counseling. The opioid risk tool (ORT) and its opioid use disorder-specific variant (ORT-OUD) predict opioid misuse in chronic pain patients. Although recommended for surgical patients, neither has been validated. Herein, we sought to validate the ORT and ORT-OUD in patients undergoing elective general surgery procedures. METHODS: This was a prospective observational study. Consented patients filled out the ORT. Demographics, prescribed opioids, and adjunct

An Emerging Synthetic Opioid in the Drug Supply: A Case of Opioid Overdose with Biological Confirmation of Cychlorphine.

INTRODUCTION: Cychlorphine (also referred to as N-propionitrile chlorphine) is a non-fentanyl synthetic opioid that has been linked to the illicit drug supply and fatal overdoses in the United States since 2024. CASE REPORT: A female in her 20s presented to the emergency department (ED) following ingestion of what the patient reported was alprazolam. Unresponsiveness and respiratory depression prompted 6 mg of intranasal naloxone administration by emergency medical services prior to arrival to the ED. Comprehensive toxicological testing of serum revealed the presence of cychlorphine. Addition

Trends and Patterns of Postoperative Opioid Exposure Among Surgical Patients After the 2016 Center for Disease Control Guideline for Prescribing Opioids.

INTRODUCTION: Perioperative opioid exposure remains a public health concern because short-term postoperative prescribing may be associated with persistent opioid use and downstream substance-related harms. We examined postoperative opioid exposure, persistent opioid use, and substance use disorder (SUD) diagnoses among opioid-naive surgical patients before and after the 2016 Center for Disease Control and Prevention (CDC) opioid-prescribing guideline. METHODS: Electronic health record data from the All of Us Research Program were used to identify opioid-naive adults undergoing qualifying surg

Effects of opioids and stimulants on the respiratory system.

Drug overdose remains an urgent public health concern as it is a leading cause of fatal injury in the United States, with approximately 90% being attributed to opioid-induced respiratory depression from synthetic opioids. This statistic is primarily driven by the rise in the use of fentanyl, a potent synthetic opioid and its increased use in combination with stimulant drugs. The rise in the combined use of opioids and stimulants raises questions about pharmacological and physiological interactions, and its implication for the treatment of their addictive consequences and of overdoses. Though o

From Innovation to Impact: The CREATE Framework as a Blueprint for Large Language Model Adoption in Opioid Treatment Programs.

Large language model (LLM)-based systems have tremendous potential to improve patient-centered health care, especially for medically underserved populations. However, realizing this potential requires careful consideration of the socio-technical contexts in which LLMs are used. The design of such systems should consider the vulnerabilities of any medically underserved group that the system intends to support, and provide trustworthy evidence for its use. This viewpoint reports the lessons learned from our experience and research with the CREATE (Culture, Respect, Education, Advancement, Trust,

Opioid use and naloxone-identified suspected opioid-related adverse events in a tertiary pediatric hospital: a retrospective study.

OBJECTIVE: Opioids are essential for analgesia and sedation in pediatric inpatient care, particularly in perioperative and intensive care settings. However, their use requires careful monitoring because children, especially infants and critically ill patients, may be vulnerable to respiratory, cardiovascular, and neurological adverse effects. METHODS: This retrospective observational study evaluated opioid administration records and naloxone-identified suspected opioid-related adverse-event episodes at a tertiary pediatric hospital in Brazil from January 2019 to September 2024. Opioid adminis

Insomnia is associated with altered arachidonic acid-induced platelet aggregation and lower cholesterol independent of age in aspirin-treated unstable angina: a machine learning analysis.

BACKGROUND: In aspirin-treated unstable angina (UA), heterogeneity in on-treatment arachidonic acid (AA)-induced platelet aggregation and cholesterol reduction contributes to residual ischemic risk. Insomnia is prevalent in coronary disease, yet its association with specific aspirin-sensitive platelet aggregation kinetics and lipid parameters remains undefined. METHODS: In a cross-sectional cohort of 198 aspirin-treated UA patients, 38 routine hematologic or biochemical variables including AA-induced platelet aggregation indices were analyzed using principal component analysis (PCA) and unsup

Chronic pain in the aging adult with sickle cell disease.

Improved survival in sickle cell disease (SCD) has led to a growing population of older adults who experience a high burden of chronic pain. With aging, pain often shifts from episodic vaso-occlusive crises to persistent, multifactorial syndromes driven by cumulative tissue injury, central sensitization, and progressive organ damage. This evolution presents significant clinical challenges, particularly in the context of comorbidities, polypharmacy, and age-related physiological changes. This narrative review examines the epidemiology, pathophysiology, and management of chronic pain in aging ad

Inhibition of adipocyte-macrophage interaction via CSF1-CSF1R by curcumin.

Adipocytes (Ad), macrophages (Mac), and lipopolysaccharide (LPS) in the adipose tissue (AT) of individuals with obesity may jointly contribute to obesity-induced pathogenesis. This study aims to examine the roles of adipocyte-macrophage interaction (AMI) and LPS in obesity-induced pathogenesis, as well as the effects of curcumin (Cur) on these interactions. An indirect co-culture model using conditioned media (CM) from 3T3-L1 Ad and J774A.1 Mac, with LPS, was used. AMI in AT and its association with systemic insulin resistance (IR) were further evaluated in a high-fat diet (HFD)-induced obese

Daily sedation interruption versus no daily sedation interruption for critically ill adults and children requiring invasive mechanical ventilation.

RATIONALE: Daily sedation interruption (DSI) aims to limit drug accumulation and promote a more awake state, thereby reducing the duration of mechanical ventilation and its associated complications. This is an update of a Cochrane review first published in 2014. OBJECTIVES: To assess the effects of DSI versus sedation management without DSI on the duration of invasive mechanical ventilation, mortality, intensive care unit (ICU) length of stay, adverse events related to under- or oversedation (including accidental removal of endotracheal tube and tracheostomy), total doses of sedative and anal

Efficacy and safety of adjunct methadone with opioid therapy for cancer pain: a systematic review.

CONTEXT: The use of opioid analgesia for cancer-related pain is challenging. Adequate pain relief is not always achieved where prolonged prescription of opioids carry risks including adverse effects, tolerance and hyperalgesia. Recent studies suggest that methadone as an adjunct to ongoing opioid therapy is effective in reducing pain severity without increases in adverse effects. OBJECTIVES: To systematically evaluate current evidence of the efficacy, safety and tolerability of adjunct methadone in combination with ongoing opioid regimens for cancer-related pain. METHODS: A systematic search

Health-related quality of life after ketamine or esketamine treatment for chronic pain: a review of randomized controlled trials.

Chronic pain, lasting over three months, significantly impairs patients' health-related quality of life (HRQoL) due to its impact on physical, psychological, and social well being. Given the limitations and risks associated with opioid use, there is growing interest in alternative analgesics. Ketamine and its S-enantiomer, esketamine, as NMDA receptor antagonists, are becoming important treatments for central sensitization. However, evidence regarding their direct positive effect on HRQoL in chronic pain patients remains sparse. This systematic review was conducted following Cochrane standards

Palliative and end-of-life care on the battlefield: a scoping review.

BACKGROUND: Modern conflicts have brought about a paradigm shift in military medicine. In Prolonged Field Care, recommendations have so far focused on damage control and care aimed at ensuring survival. But, particularly in large scale combat operation, not all patients will survive, and despite the development of palliative care in the civilian sector, no extension of this approach has yet been envisaged for military medicine. This review aims to identify knowledge gaps in palliative and supportive care in combat situations. METHODS: We conducted a literature review in PubMed, CAIRN, Scopus

Discovery of novel dual μOR/σ1R ligands as potent analgesics with reduced tolerance liability and improved safety profiles.

Opioid tolerance remains a widespread challenge in pain management, as prolonged μOR activation progressively diminishes analgesic efficacy, necessitating dose escalation and thereby increasing the risk of adverse effects. σ1R antagonism has been shown to potentiate opioid antinociception and mitigate the development of opioid tolerance. However, whether these complementary pharmacological actions can be effectively integrated into a single dual-target ligand remains underexplored. Our previous work yielded 4x, a promising dual μOR agonist/σ1R antagonist with potent in vitro and in vivo activi
