Acetaminophen toxicity: A comprehensive revie.

Poisoning by acetaminophen is a serious public health concern because of its accessibility and the ease with which overdoses can occur accidentally or on purpose. The liver metabolizes acetaminophen, and when taken in excess, it is transformed into a poisonous metabolite that seriously damages the liver and may result in hepatic failure and death. The cornerstone of treatment is the early administration of N-acetylcysteine, which is most effective within 8 hours of ingestion. Co-ingestants, therapy timing, and pre-existing liver diseases are some of the factors that affect toxicity. Both unint

Opioid Pain Medication Use in Athletes.

The use of opioids for pain management has become increasingly accepted over the years. There are known adverse effects of opioids that have implications for athletes when used for pain relief in the context of sports. Our objective was to conduct a review of the available literature to evaluate the following: (1) opioid use in athletes; (2) how to prevent initial use; (3) tapering opioids; and (4) nonopioid alternatives. A literature review was conducted on the prevalence of opioid use in athletes, strategies for preventing initial use and tapering opioids, and nonopioid alternatives for pain

Risk of Overdose and Adverse Events With Concomitant Opioids and Gabapentinoid Use After Total Joint Arthroplasty.

PURPOSE: We evaluated the safety outcomes of opioids alone versus opioids plus low-dose gabapentinoids in patients naïve to both opioids and gabapentinoids, after total joint arthroplasty (TJA) of the knee, hip, or shoulder. METHODS: This was a multicenter, population-based, retrospective cohort study based in a large, integrated health system. It included opioid-naïve and gabapentinoid-naïve patients who underwent TJA between 2018 and 2024. Patients were categorized into two groups: those dispensed opioids and low-dose gabapentinoids or those dispensed opioids alone postoperatively. The prim

Drug discrimination distinguishes pharmacologically distinct κ-opioid receptor agonists in rats.

Ligand-dependent signaling has emerged as a central concept in κ-opioid receptor (KOR) pharmacology, yet whether differences among KOR agonists are reflected in their discriminative stimulus effects remains unclear. The present study compared the discriminative stimulus effects of pharmacologically distinct KOR agonists and examined the contribution of intracellular signaling-related mechanisms to the discriminative stimulus produced by the prototypical KOR agonist U50,488H. Male Fischer 344 rats were trained to discriminate (±)U50,488H (5.0 mg/kg, s.c.) from saline under a two-lever fixed-rat

Duration of Medication Use for Opioid Use Disorder and Risk of Postpartum Overdose and Death.

IMPORTANCE: Overdose is the leading cause of maternal mortality in the US, yet evidence on how medications for opioid use disorder (MOUD) affect postpartum overdose and mortality risk is limited. It is unclear whether cumulative MOUD exposure during this period reduces overdose or death beyond 90 days post partum. OBJECTIVE: To examine whether longer duration of MOUD dispensed during pregnancy and early post partum is associated with reduced overdose or death through 365 days post partum. DESIGN, SETTING, AND PARTICIPANTS: This is a retrospective cohort study of individuals delivering betwee

Scoping evidence review on the prevention and treatment of opioid overdose.

BACKGROUND AND AIM: Opioid overdose is a public health crisis. Globally, ~ 600,000 deaths each year are attributable to drug use, and roughly three-quarters are linked to opioids. The rising prevalence of high-potency synthetic opioids like fentanyl necessitates comprehensive prevention and treatment strategies. This scoping review aims to map the published literature on the early detection and community management of opioid overdose and post-overdose care and identify research gaps. The aim was also to inform the potential research questions for updating the WHO and other international guidel

A quantitative weight-of-evidence review of preclinical studies examining the potential carcinogenicity of acetaminophen.

Introduced in the 1950s, acetaminophen is one of the most widely used antipyretics and analgesics worldwide. In 1999, the International Agency for Research on Cancer (IARC) judged the available data inadequate to determine carcinogenicity, and in 2024 an IARC Advisory Group recommended high-priority re-evaluation. Given this context, we conducted a quantitative weight-of-evidence (QWoE) analysis of in vivo rodent carcinogenicity studies using a previously developed framework for biological relevance and methodological quality (Kougias et al., 2025a; Kougias et al., 2025b). A total of 192 QWoE

CRISPR/Cas9-mediated genome editing reveals the involvement of a polyphenol oxidase in the shikonin-specific biosynthesis in Lithospermum erythrorhizon.

Shikonin, a 1,4-naphthoquinone derivative produced by some limited Boraginaceae species, exhibits unique pharmacological properties and is also used as a natural dye. The regulatory factors of shikonin production have been demonstrated using a cell culture system of Lithospermum erythrorhizon. Among these factors, copper is known to be the strongest enhancer of shikonin production, and the copper-requiring biosynthetic step is shikonin-specific, unlike the byproduct benzoquinones. Despite the long history of shikonin biosynthesis studies for over 40 years, the copper-involved reaction has been

Symptomatic injection granuloma of buttock and management with ultrasound-guided barbotage procedure.

Injection granulomas (IGs) are recognized as a consequence of the injection of injectates into adipose tissue after routine drug or vaccine injections without reaching the muscular tissue. A series of reactions of the injectates in adipose tissue results in necrotic tissue and deposition of calcium crystals. IGs are usually asymptomatic but may rarely be symptomatic. In the past, symptomatic cases were mostly managed conservatively or surgically. However, to the best of our knowledge, no procedure other than surgical or conservative treatment has been reported. A 57-year-old patient with a his

Effects of salicylate on organ-of-Corti vibrations in the second turn of the gerbil cochlea.

Sound-evoked responses of the organ of Corti (OoC) in the living cochlea are enhanced by active processes driven by outer hair cells (OHCs). To assess how suppression of OHC function affects intracochlear mechanics, we applied salicylate-a blocker of OHC electromotility-to the round window of anesthetized Mongolian gerbils and measured both distortion product otoacoustic emissions (DPOAEs) and sound-evoked vibrations of the OoC in the second cochlear turn using optical coherence tomography (OCT). Salicylate reduced DPOAE amplitudes in a time- and frequency-dependent manner and decreased intrac

Dermatomal spread in lateral quadratus lumborum blocks versus transversus abdominus plane blocks after laparoscopic colorectal surgery: a randomized clinical trial.

INTRODUCTION: Postoperative pain after laparoscopic abdominal surgery remains complex. While lateral quadratus lumborum (QL) block and transversus abdominus plane (TAP) block are similar techniques, no studies have examined if one technique has greater dermatomal spread. METHODS: Laparoscopic colorectal surgical patients were randomized to lateral QL or TAP blocks. Dermatomal anesthetic spread, as measured by loss of sensation to temperature in the postanesthesia care unit, was the primary outcome. If a clear level of dermatomal anesthesia could not be defined, the block was categorized as "i
