[Opioid usage in Denmark].

The debate on an opioid crisis in Denmark points in two directions. While prescriptions for opioids are declining, there is a marked increase in illegal use, predominantly among young people. There is still a lack of knowledge in the healthcare system regarding optimal dosing, treatment duration, and the risk of dependence. In addition, evidence is limited on the sharing of leftover prescription medications between citizens and on systematic monitoring of illegal opioid use. This review concludes that a comprehensive national strategy is needed to prevent an opioid crisis in Denmark.

[Opioid substitution and pain management in a case of severe PTSD].

This case report describes a highly complicated case with combined chronic pain, substance use disorder (SUD) and social challenges following war participation in Ukraine. The patient had developed PTSD and SUD because of pain management and was enrolled in dual diagnosis treatment in out-patient psychiatry. The patient successfully transitioned to long-acting buprenorphine leading to better pain management and remission in SUD, improving both treatment relation and clinical treatment of PTSD by use of integrated dual diagnosis treatment.

Electroacupuncture analgesia for cancer-related pain.

PURPOSE OF REVIEW: Cancer-related pain remains a major clinical problem arising from both malignancy and its treatment. Current pharmacotherapies rely on opioids and adjuvant analgesics, which still provide unsatisfactory relief for the majority of patients. The use of complementary therapies for the management of cancer-related pain is growing, and acupuncture is increasingly incorporated into integrative cancer care. We explore the clinical and preclinical evidence base for the use of electroacupuncture in tumour-associated pain and treatment-associated pain, with an emphasis on experimental

Peripartum Esketamine: A Narrative Review of Analgesia, Postpartum Depressive Symptoms, Sleep-Related Outcomes, and Safety.

Pain, depressive symptoms, and sleep disruption frequently overlap during the peripartum period, although their relationships are associative rather than established as causal. Esketamine, an N-methyl-D-aspartate receptor antagonist, has attracted interest as a peripartum intervention with potential effects on pain, mood, and sleep. This narrative review evaluates the clinical and mechanistic evidence for peripartum esketamine in analgesia, postpartum depressive symptoms, sleep-related outcomes, and maternal-infant safety, with particular attention to the directness and interpretability of the

Economic Value and Caregiver Burden of Continuous Fascia iliaca Compartment Block in Elderly Patients with Intertrochanteric or Subtrochanteric Fracture: A Cost-Consequence Analysis Based on a Propensity Score-Matched Cohort.

BACKGROUND: Hip fracture imposes substantial clinical and economic burden on older adults in China. Continuous fascia iliaca compartment block (CFICB) may improve perioperative recovery in elderly patients with intertrochanteric or subtrochanteric fracture, but its economic consequences remain uncertain. We compared direct medical costs, caregiver-related indirect costs, and clinical outcomes of CFICB with routine systemic analgesia (RA; a non-block regimen, distinct from regional anaesthesia, of which CFICB is itself a technique). METHODS: We performed a cost-consequence analysis alongside a

Jail-based opioid treatment implementation, outcomes, and costs: lessons learned from the Massachusetts Justice Community Opioid Innovation Network (MassJCOIN).

Limited access to medications for opioid use disorder (MOUD) is a major driver of the opioid overdose crisis. Jails have been slow to implement MOUD treatment, especially agonist medications (methadone, buprenorphine). Massachusetts mandated certain county jails pilot-test delivery of all FDA-approved MOUD treatments, stipulating jails continue MOUD, or initiate it prior to release, and facilitate MOUD continuation in the community post-release. The Massachusetts Justice Community Opioid Innovation Network (MassJCOIN) partnered with seven pilot jails, the Massachusetts Department of Public Hea

Patterns of complementary and alternative medicine and health care utilization among long-term opioid users with chronic non-cancer pain: a repeated cross-sectional study.

BACKGROUND: High opioid use among individuals with chronic non-cancer pain (CNCP) raises concerns about safe pain management, leading some patients to seek complementary and alternative medicine (CAM). However, treatment-seeking patterns, including how CAM and conventional care are combined, remain unclear among those receiving long-term opioid treatment. The aim was to investigate health-seeking behaviours among individuals with CNCP receiving long-term opioid treatment. METHODS: Repeated cross-sectional data from the Danish Health and Morbidity Surveys (2000-2021) were linked to national ad

NaV1.8 Inhibition and the Future of Nonopioid Analgesia in Otolaryngology: A Narrative Review.

IMPORTANCE: Postoperative pain management in otolaryngology requires balancing effective analgesia while minimizing adverse effects, such as opioid-related complications. Suzetrigine is a first-in-class selective voltage-gated sodium channel (NaV1.8) inhibitor that has been recently approved by the US Food and Drug Administration. Suzetrigine selectively inhibits NaV1.8 channels expressed in peripheral nociceptive neurons, thereby reducing pain signaling without the respiratory depression, sedation, or misuse potential associated with opioids. This review summarizes the pharmacology, efficacy,

Ketamine in Modern Medicine: Current Evidence in Acute Pain, Opioid Use Disorder, and Depression.

Ketamine has continued to be a valuable adjunct in perioperative care, supported by recent meta-analyses demonstrating opioid-sparing effects and improved analgesia, particularly in opioid-tolerant patients and painful surgeries. Its use should be targeted to patients most likely to benefit from its unique properties. Beyond acute pain, ketamine shows promise in patients with opioid use disorder by mitigating tolerance and withdrawal through N-methyl-d-aspartate receptor modulation and downstream neuroadaptive effects. Additionally, its established antidepressant properties position ketamine a

Suzetrigine (JOURNAVX) for Acute Postoperative Pain: Evidence, Perioperative Integration, and Clinical Guidance.

Suzetrigine (JOURNAVX) is an oral, first-in-class selective NaV1.8 inhibitor approved in the United States for adults with moderate-to-severe acute pain. In 4 randomized controlled trials (n = 2768) using bunionectomy and abdominoplasty models, suzetrigine improved SPID48 versus placebo and showed similar efficacy to hydrocodone/acetaminophen 5/325 mg. Its short-term safety profile differs from opioids, with less nausea and dizziness but more pruritus and muscle spasms. Clinicians must screen for CYP3A interactions and counsel patients using certain hormonal contraceptives. This review summari

Postoperative pain outcomes following minor oral surgery in opioid tolerant patients: a retrospective cohort study.

BACKGROUND: Evidence guiding postoperative pain management in patients taking prescription or illicit opioids undergoing minor oral surgery is limited. It remains unclear whether standard analgesic protocols for opioid naïve patients provide adequate pain relief in opioid tolerant individuals. This study evaluated whether standard postoperative analgesia achieves adequate pain control in opioid tolerant patients undergoing minor oral surgery with expected mild to moderate pain. METHODS: A single centre retrospective study was conducted of patients undergoing minor oral surgical procedures at

Management of Patients on Medications for Opioid Use Disorder in the Perioperative or Peripartum Setting.

There is an increasing proportion of surgical and obstetric patients on medications for the treatment of opioid use disorder (medications for opioid use disorder [MOUD]), formerly known as medication-assisted treatment, in the United States. Due to lack of institutional protocols and limited evidence-based guidelines, perioperative management of patients on MOUD has largely been varied and often provider-dependent. This article explores the current evidence and recommendations for perioperative and peripartum management of patients on buprenorphine, methadone, and naltrexone; as well as key pr

Intraoperative intercostal nerve cryoablation vs. patient-controlled analgesia after thoracotomy: a single-center retrospective cohort study demonstrating comparable early pain and opioid use.

BACKGROUND AND AIMS: Thoracotomy is among the most painful surgical procedures and is associated with a high risk of chronic postoperative pain. Optimizing perioperative analgesia is essential for enhancing recovery and reducing opioid-related side effects. This study evaluated whether adding intraoperative intercostal nerve cryoablation to standard postoperative analgesia improves pain control and reduces opioid use following thoracotomy. MATERIALS AND METHODS: This retrospective cohort study included patients who underwent thoracotomy at a tertiary cancer center in 2024. Patients receiving

Ketamine Beyond Anaesthesia: Analgesic, Psychiatric, Antitumour, and Immunomodulatory Actions and Their Signalling Mechanisms.

PURPOSE OF REVIEW: To synthesize current evidence on ketamine for cancer-related pain and its intersecting psychiatric, anti-tumor, and immunomodulatory actions, emphasizing mechanisms, clinical relevance, and uncertainties relevant to oncology. RECENT FINDINGS: Ketamine provides analgesia primarily through non-competitive antagonism of N-methyl-D-aspartate receptors and additional effects on excitatory, inhibitory, and opioid-related signaling. In patients with refractory cancer pain, it may be a useful adjunct to opioid-based multimodal analgesia and may reduce opioid requirements; however,

Prescribing to protect case series: naltrexone as a novel intervention to prevent fentanyl overdose in people who use cocaine.

BACKGROUND: The rise in fentanyl contamination of cocaine has contributed to a surge in opioid-involved overdose deaths among people who use cocaine (PWUC). Existing harm reduction strategies, including naloxone distribution, drug checking, and counseling people to use with others, are inadequate in fully addressing this risk. Despite robust evidence supporting contingency management (CM) as an effective treatment for cocaine use disorder (CUD), its implementation remains limited. Naltrexone, a potent opioid antagonist, has been evaluated for treatment of CUD and has not been found to be effec

Bolus-only versus bolus plus continuous infusion hydromorphone for refractory cancer pain: a prospective, multicenter, randomized noninferiority trial.

INTRODUCTION: Refractory cancer pain (RCP) remains difficult to manage despite guideline-based therapy. Patient-controlled intravenous analgesia (PCIA) is increasingly used, yet the optimal administration mode remains unclear. METHODS: In this prospective, multicenter, randomized, non-inferiority trial, patients with refractory cancer pain were enrolled between December 1, 2018, and August 31, 2022, and randomly assigned to either a bolus-only PCIA group (Group B) or a bolus plus continuous infusion group (Group BC). Pain intensity was assessed using the Numerical Rating Scale (NRS). The prim

A Carfentanil-Targeted Vaccine Adjuvanted with a Covalent TLR7/8 Agonist and a T-Helper Epitope Confers Protective Immunity against Carfentanil-Induced Effects.

Carfentanil, an ultrapotent synthetic opioid, poses a severe public health threat because of its rapid brain penetration and potent activation of μ-opioid receptors. Conventional anti-drug vaccines are limited by the poor hapten immunogenicity and dominant anti-carrier immunity. Here, we report a molecularly encoded synthetic carfentanil vaccine (MECVPLA-PEI) integrating a carfentanil hapten, TLR7/8 agonist, and T-helper epitope within a polymeric scaffold to coordinate B and T cell activation. MECVPLA-PEI exhibited markedly enhanced lymph node targeting, with 21.2-fold higher accumulation at
