Integrating Clinical Modeling and Machine Learning for Risk Assessment of Paracetamol and Other Nonsteroidal Anti-Inflammatory Drug Hypersensitivity in Children.

BACKGROUND: Nonsteroidal anti-inflammatory drug (NSAID) hypersensitivity is a common cause of drug-related reactions in children. Pretest risk stratification may improve the safety and efficiency of drug provocation testing. OBJECTIVE: To develop a clinically interpretable risk stratification tool (nomogram + simplified score) for pediatric paracetamol and/or other NSAID hypersensitivity and to validate its performance against machine learning (ML) models. METHODS: We conducted a retrospective cohort study (2014-2025) of children evaluated for suspected paracetamol and/or other NSAID hyperse

MOTS-c Protects Against Acetaminophen-induced Liver Injury through the MAPK Signaling Pathway.

OBJECTIVE: Acetaminophen (APAP)-induced liver injury (AILI) is a leading cause of acute liver failure worldwide, but effective therapeutic strategies are still lacking. MOTS-c, a mitochondrial-derived peptide, has demonstrated hepatoprotective properties in models of nonalcoholic steatohepatitis (NASH) and hepatitis B virus (HBV) infection. This study aims to explore the role and underlying mechanisms of MOTS-c in AILI. METHODS: An AILI model was established in male C57BL/6 mice via intraperitoneal (i.p.) injection of APAP (300 mg/kg). The therapeutic potential of MOTS-c and its mechanisms we

Multi-omics reveals that 4'-O-β-D-glucosyl-5-O-methylvisamminol ameliorates acute liver injury by modulating the miR-30e-3p/Tfrc axis and a multi-dimensional network of ferroptosis, inflammation, metabolism, and gut microbiota.

BACKGROUND: Overuse of acetaminophen (APAP) can induce acute liver injury (ALI), and its mechanism is closely related to ferroptosis. 4'-O-β-d-glucosyl-5-O-methylvisamminol (5-O) is a natural coumarin glycoside with anti-inflammatory and antioxidant activities, but whether it can alleviate liver injury through intervention in ferroptosis and multi-dimensional mechanisms remains unclear. PURPOSE: To explore whether 5-O can alleviate APAP-induced ALI through multiple mechanisms such as inhibiting ferroptosis, regulating inflammation, promoting lipid metabolism, and reshaping the intestinal micr

Circulating Liver-Enriched miR-122 in COVID-19 Patients: A Longitudinal Real-Life Study.

Abnormal liver function tests are frequently reported in patients with COVID-19. This study aimed to identify potential treatment-associated hepatocellular injury in COVID-19 patients by dynamically assessing circulating miR-122, a biomarker with high hepatic specificity and sensitivity. An exploratory approach was additionally used, given the limited evidence regarding factors influencing miR-122 expression in this setting. We performed a prospective cohort study including 96 adult participants enrolled at a tertiary hospital in Bucharest, Romania, between March 2022 and July 2023: 78 COVID-1

Injury-Retention Strategy for Discriminative Magnetic Resonance Imaging and Enhanced Antioxidant Therapy of Drug-Induced Liver Injury.

Drug-induced liver injury (DILI) is a prevalent cause of acute liver failure worldwide, characterized by impaired hepatic clearance which creates an opportunity for targeted nanomedicine. We present an injury-retention strategy for DILI management using a gadolinium hexacyanoferrate nanozyme (GdHF). This nanozyme enables simultaneous high-fidelity diagnosis and sustained treatment. It intrinsically combines high longitudinal relaxivity (r1 = 13.23 mM-1·s-1, 3.9-fold greater than Gd-DTPA) with multienzyme mimetic activities. In an acetaminophen-induced DILI model, GdHF specifically accumulated

Early aspirin withdrawal versus dual antiplatelet therapy in high-risk patients after percutaneous coronary intervention: Meta-analysis of randomized trials.

BACKGROUND: Patients at high ischemic or bleeding risk after percutaneous coronary intervention (PCI) require protection against thrombotic events with dual antiplatelet therapy (DAPT) while avoiding bleeding. Although guidelines recommend 12-month DAPT after acute coronary syndrome (ACS), recent trials have tested the safety of early aspirin withdrawal with potent P2Y12-inhibitor monotherapy. METHODS AND FINDINGS: We performed a meta-analysis of randomized trials (from inception through August 2025) comparing early aspirin withdrawal (≤3 months) with transition to ticagrelor- or prasugrel-mo

Familial confounding and causal inference in child and adolescent neurodevelopment and mental health.

Recent announcements by the US Government linking paracetamol use during pregnancy to autism in offspring highlight the risks of misinterpreting observational research to inform policy; this is a clear example of the principle that association does not equal causation. Unmeasured familial confounding is a common bias in epidemiological studies, whereby shared genetic or environmental factors within families produce spurious associations between risk factors and outcomes. In this Viewpoint, aimed at clinicians from a range of disciplines working with children and young people with neurodevelopm

Efficacy and Safety of Aspirin-free versus Aspirin-based Strategies in Patients With Acute Coronary Syndrome Undergoing Percutaneous Coronary Intervention: A Systematic Review and Meta-Analysis.

Aspirin (ASA) is the cornerstone of the acute coronary syndrome primary and secondary prevention. Still, its role is debated in some high bleeding risk patients or cases that underwent second-generation drug-eluting stents with a better scaffold. This study compared the efficacy and safety of aspirin-free versus aspirin-based strategies in patients with ACS undergoing PCI. We systematically searched PubMed, Embase, Scopus, and ScienceDirect for studies comparing aspirin-based versus aspirin-free strategies in patients with ACS undergoing PCI. Pooled relative risk (RR) with 95% CI was calculate

Low-dose ticagrelor combined with aspirin in the prevention of early neurological deterioration of high-risk non-disabling ischemic cerebrovascular events: a PROBE clinical study.

BACKGROUND: Early neurological deterioration (END) seriously endangers the clinical prognosis of high-risk non-disabling cerebrovascular events (HR-NICE), and strong antiplatelet therapy is an effective means of preventing the occurrence of END. Studies have confirmed that ticagrelor can avoid the low efficacy of clopidogrel in antiplatelet therapy caused by CYP2C19 gene dysfunction and that its combination with aspirin can improve the clinical prognosis of patients with HR-NICE. However, the overall risk of bleeding events is relatively high. In this protocol, we compared the prevention and s

Electrochemical evaluation of plant extracts as potential antidotes for acetaminophen poisoning.

Acetaminophen, a widely used analgesic, causes severe liver damage in overdose situations due to N-acetyl-p-benzoquinoneimine (NAPQI), a toxic metabolic intermediate. Current antidotes, like N-acetylcysteine (NAC), are effective but costly and not universally accessible. There is a critical need for novel, cost-effective antidotes to mitigate acetaminophen-induced hepatotoxicity. This study investigates the electrochemical oxidation of acetaminophen in the presence of plant extracts, including pomegranate peel, pomegranate, bell pepper, black radish, and turnip, to evaluate their potential as

Impact of first-trimester preeclampsia screening on perinatal and maternal morbidity: the RANSPRE open multicenter randomized trial.

BACKGROUND: Preeclampsia (PE) affects 2–8% of pregnant women and is a leading cause of maternal and perinatal morbidity and mortality. In high-risk pregnant women, low-dose aspirin (LDA 100—160 mg/day) started before 16 WG reduces the incidence of PE, prematurity, perinatal mortality and low birth weight. First trimester screening of PE allows the identification of a population of pregnant women at high risk of early onset PE and perinatal mortality. It remains unclear whether, on a population scale, the systematic implementation of first trimester screening of PE with treatment of high-risk p
