Daily sedation interruption versus no daily sedation interruption for critically ill adults and children requiring invasive mechanical ventilation.

RATIONALE: Daily sedation interruption (DSI) aims to limit drug accumulation and promote a more awake state, thereby reducing the duration of mechanical ventilation and its associated complications. This is an update of a Cochrane review first published in 2014. OBJECTIVES: To assess the effects of DSI versus sedation management without DSI on the duration of invasive mechanical ventilation, mortality, intensive care unit (ICU) length of stay, adverse events related to under- or oversedation (including accidental removal of endotracheal tube and tracheostomy), total doses of sedative and anal

Benzodiazepine Prescription and Use in the Older Adults in Catalonia, Spain from 2018 to 2022: A Drug Utilization Study.

BACKGROUND AND OBJECTIVE: Benzodiazepines (BZDs) remain among the most frequently prescribed psychotropic drugs in older adults, despite well-documented risks and guideline recommendations for short-term use. This study analyzed BZD prescribing trends among individuals aged ≥ 65 years in Catalonia, Spain, between 2018 and 2022. METHODS: A retrospective drug utilization study was conducted using anonymized electronic health records from the SIDIAP (Information System for Research in Primary Care) database, covering approximately 80% of the Catalan population. All patients aged ≥ 65 years with

Development of a Women's Empowerment Nurse-Led Group Intervention for Benzodiazepine and Z-Hypnotic Deprescription in Primary Healthcare: Results of a Co-Creation Process.

AIM: The aim of this study was to co-create a nurse-led women's empowerment group intervention to facilitate the deprescription of benzodiazepines and Z-hypnotics in primary care. DESIGN: User-centred design based on co-creation. METHODS: A literature review was conducted to identify female empowerment programmes that served as an initial prototype. The subsequent user-centred design applied a co-creation methodology in two structured phases: (1) Exploratory, to identify contextual needs and barriers, and (2) Development, to collaboratively define the intervention's final structure and conte

Hospitalist perspectives and practices in treating hospitalized patients with alcohol withdrawal syndrome in the era of phenobarbital re-emergence.

Medication treatment for moderate to severe alcohol withdrawal syndrome (AWS) is evolving, particularly with renewed phenobarbital use. However, little research has examined how hospitalists manage AWS. We conducted a multiple-methods study involving hospitalists from a national US Hospital Medicine research network. Thirty-three hospitalists from 27 tertiary hospitals completed surveys and participated in seven focus groups on August 8, 2025. Benzodiazepines remained the most common treatment, but phenobarbital use was reported in 67% of emergency departments and 44% of inpatient medical unit

Effects of Benzodiazepines on Heart Failure Outcomes and Adverse Events: A Systematic Review and Meta-Analysis.

The effects of benzodiazepines (BZDs) for insomnia on heart failure (HF) remain unclear, particularly among high-risk populations. Relevant studies were retrieved by searching seven databases from inception to August 7, 2025. Hazard ratios (HRs) or risk ratios estimated for related HF outcomes were extracted. Meta-analyses were conducted using a random-effects model, and study quality was assessed using the Newcastle-Ottawa Scale (NOS). Eleven studies met the inclusion criteria, with follow-up durations ranging from 180 days to 15 years. The meta-analysis demonstrated that BZD use was associat

Repeated remimazolam sedation for interstitial brachytherapy in cervical cancer: a randomized controlled trial.

BACKGROUND: Evidence regarding repeated remimazolam administration remains limited. We hypothesized that remimazolam would be associated with fewer adverse events than propofol while maintaining stable sedative requirements across three repeated sedation sessions. METHODS: Patients with cervical cancer undergoing three fractions of interstitial brachytherapy were randomized to propofol or remimazolam. The primary outcome was the proportion of sedation episodes with at least one prespecified intraoperative adverse event across three sessions. Secondary outcomes included sedative dose, hemodyna

Evaluating seltorexant as a treatment option for major depressive disorder.

INTRODUCTION: Major depressive disorder is a leading cause of disability, with a significant rate of patients responding inadequately to antidepressants, and residual symptoms, including insomnia, worsening overall outcomes. Current adjunctive options display partial efficacy and tolerability, motivating the search for different strategies. Beyond regulating the sleep-wake cycle, orexin systems are implicated in reward and cognition, and have emerged as new targets for depressive disorders. AREAS COVERED: This Drug Profile summarizes the published peer-reviewed evidence on seltorexant (JNJ-42

Protective effects of central leptin on whole-body energy homeostasis following acute olanzapine exposure.

Antipsychotics represent mainstay treatment of schizophrenia and are also prescribed on- and off-label in other severe mental illnesses. Newer antipsychotic use is associated with severe metabolic side effects such as obesity and type 2 diabetes. Leptin is a hormone secreted by adipose tissue, and it acts on the brain to decrease body weight by reducing food intake and stimulating energy expenditure. Leptin also improves glucose and lipid metabolism. In this study, we examined the short-term impact of olanzapine, a commonly used second-generation antipsychotic, on the central leptin-mediated r

Genes Do Not Read DSM: Tuberous Sclerosis Complex May Dominate with Multi-Neuropsychiatric Symptoms.

Tuberous sclerosis complex (TSC) is a multisystem genetic disorder frequently accompanied by epilepsy and TSC-associated neuropsychiatric disorders. Psychiatric-predominant seizure presentations may, however, delay recognition. This clinical challenge describes a 26-year-old woman with recurrent panic-like episodes characterized by dyspnea, choking sensations, palpitations, and derealization, followed by depressive symptoms. The episodes persisted despite treatment with selective serotonin reuptake inhibitors and benzodiazepines. Further examination revealed hypomelanotic macules, prompting ne

Cerebral Hemodynamic and Metabolic Responses to Anesthesia and Vasopressors in Adult Shoulder Surgery: A 2 × 2 Factorial Design Randomized Controlled Trial with Light-based Neuromonitoring (CHEM-FACT Study).

BACKGROUND: Ephedrine has been suggested to improve cerebral microcirculation and perfusion compared with phenylephrine. However, prospective intraoperative data are lacking, and possible interactions with anesthetic choice remain untested. METHODS: Forty adults undergoing shoulder surgery in the beach chair position were recruited between February and November 2024 at St. Joseph's Hospital (London, Canada). Patients were randomized in a 2 × 2 factorial design to anesthetic maintenance (propofol vs . sevoflurane) and vasopressor strategy (phenylephrine infusion vs . ephedrine boluses). Relati

Prescribing Trends, Adherence, and Tolerability of Antianxiety Medications in an Indian Tertiary Care Psychiatry Outpatient Department: A Cross-Sectional Study.

Background Pharmacological therapy, particularly the use of selective serotonin reuptake inhibitors (SSRIs), plays a central role in managing anxiety spectrum disorders. However, real-world clinical practice can deviate from evidence-based recommendations, leading to challenges such as prolonged benzodiazepine use, polypharmacy, and low rates of generic prescribing. Evaluating these real-world prescribing patterns, alongside patient adherence and adverse drug reactions (ADRs), is crucial for optimizing treatment. This study aimed to describe the prescribing patterns of anti-anxiety medications

Exploratory study of electroencephalogram markers of responsiveness to surgical noxious stimuli under propofol anaesthesia.

BACKGROUND: While general anaesthesia typically induces unconsciousness, some patients retain the capacity to respond behaviourally to noxious stimulation. Reduced frontal alpha (8-12 Hz) power has been proposed as a marker of arousal, but its clinical reliability remains inconsistent. This exploratory study aimed to identify multichannel EEG spectral and connectivity markers associated with intraoperative behavioural responsiveness around noxious stimulation. METHODS: Sixty-four-channel EEG was recorded intraoperatively from seven patients undergoing microlaryngoscopy under propofol anaesthe

Brewing Anxiety: Caffeine-Induced Thigmotaxis in Zebrafish Larvae.

BACKGROUND: Thigmotaxis, or edge preference, is widely used as a measure of anxiety in vertebrate model systems. However, variability in this behavior limits its utility as a scalable measure for anxiolytic drug discovery. In the current study, we examine caffeine-induced thigmotaxis, using 5-day-old zebrafish larvae as a model system. METHOD: We modified our imaging system and AI-assisted analysis for measurements of behavior in six-well plates. The imaging system was used to measure thigmotaxis and several other behaviors, including overall activity, scoot and burst swimming, and responses

The FUSION Study: compatibility of antibiotics with commonly used medication infusions in a paediatric intensive care unit.

OBJECTIVES: Beta-lactams and vancomycin often require extended or continuous infusion strategies for antibiotic optimisation in the paediatric intensive care unit (PICU). Simultaneous administration of multiple drugs through a single lumen via a Y-site connector is utilised with uncertainty due to limited intravenous access and the common need for sedative-analgesic infusions in critical illness. The compatibility data supporting antibiotics and sedative-analgesics co-administration is lacking. This study describes the physicochemical compatibility between antibiotics and sedative-analgesics c
