Prediction of Alcohol Withdrawal Severity Scale (PAWSS) for patients cared for in an emergency department/Level I Trauma Center: An evidence-based practice change.

BACKGROUND: Patients with alcohol use disorder (AUD) often come to the Emergency Department (ED) for unrelated complaints but can still experience withdrawal. The purpose of this Evidence-Based Practice (EBP) project was to detect the potential for alcohol withdrawal in the ED using the Prediction of Alcohol Withdrawal Severity Scale (PAWSS) and then provide necessary treatment to prevent negative patient outcomes (i.e., alcohol withdrawal-related seizures, admissions to the intensive care unit (ICU), and prolonged stays in the ED). METHODS: This EBP project was implemented at a Level I Traum

Evaluation of risk factors associated with phenobarbital dose escalation for the treatment of alcohol withdrawal in the critically ill.

AIMS: While there are risk stratification tools for alcohol withdrawal syndrome (AWS), there are no well-established risk factors associated with phenobarbital escalation; the primary aim was to identify risk factors associated with needing phenobarbital dose escalation for AWS management. METHODS: This single-center, retrospective, cohort study evaluated adult patients with diagnosed AUD admitted to a surgical or medical intensive care unit of a large academic medical center within 24 hours of the initial phenobarbital dose for AWS management if admitted between 1 December 2021 and 1 Decembe

A Systematic Review of Clinically Significant Drug-Drug Interactions With Phenobarbital and Primidone.

Phenobarbital (PB) is historically used for management of seizure disorders but more recently has emerged as a prominent option for the management of alcohol withdrawal syndrome given data suggesting its benefit over benzodiazepines. An important consideration of PB therapy is its induction of cytochrome-P 450 enzymes (CYP450), which may lead to underrecognized drug-drug interactions (DDIs). Knowledge of these PB DDIs, including their onset and offset, is important to better elucidate how to manage these DDIs. To identify published literature evaluating the relevance of DDIs between PB and sel

Sedative effects of physcion and its modulatory interaction with diazepam: in vivo and in silico evaluation.

Physcion (PHY), an anthraquinone derivative, has demonstrated various biological properties, including antioxidant and anti-inflammatory effects. This study aimed to evaluate the sedative activity of PHY in Swiss mice using in vivo and in silico approaches. The thiopental sodium (TS)-induced sleep model, fireplace test, and hole cross test were employed to assess sedation along with molecular docking studies. PHY significantly (p < 0.05) reduced sleep latency (17.29 ± 2.33 min) and increased sleep duration (162.85 ± 8.38 min) compared to controls at higher doses. It also significantly increase

[A case where phenobarbital was remarkably effective against drug-induced liver injury presenting with prolonged intrahepatic cholestasis caused by Ashwagandha].

This case concerns a 39-year-old male with a history of adjustment disorder, and he had been using Ashwagandha-self-prescribed and self-imported online-for stress relief and vitality enhancement for the past 3 months. He visited the emergency department with symptoms of hyperventilation, and blood tests showed total bilirubin of 2.37mg/dL, ALT of 384U/L, and ALP of 92U/L, indicating hepatocellular-type liver dysfunction, thereby raising suspicion for drug-induced liver injury (DILI). Although ursodeoxycholic acid was initiated after referring to our hospital, his condition showed minimal impro

Let-7c-5p Drives Collagen Gene Repression Linking Phenobarbital Teratogenicity to Cleft Palate.

The treatment of seizures during pregnancy remains challenging due to the teratogenic effects of many anticonvulsants. Phenobarbital (PB), despite its known risks, is widely used and associated with congenital malformations such as cleft palate (CP). CP is a common craniofacial defect resulting from disrupted palatogenesis, influenced by genetic and environmental factors, including maternal medication. miRNAs are key post-transcriptional regulators implicated in palate development and possibly modulated by teratogenic agents. This study investigated whether miRNA dysregulation observed in CP p

Role of apocynin in alleviation of oxidative stress and neuronal autophagy after traumatic brain injury by activating the PI3K/Akt/Nrf2 pathway in rats.

INTRODUCTION: The study aimed to determine whether apocynin (APO) can protect neurons in rats with traumatic brain injury (TBI) by activating the PI3K/Akt/Nrf2 pathway, acting as an antioxidant, and suppressing autophagy. MATERIAL AND METHODS: Ninety male Sprague-Dawley rats (220-260 g) were randomly assigned to three groups: a control group with a sham operation, a group with TBI, and a group that received APO following TBI. Saline and APO were intraperitoneally administered for three consecutive days based on the aforementioned groups. Ten rats from each group underwent behavioral assessmen

Prognostic Factors Associated with Pneumonia in Patients with Traumatic Brain Injury: A Systematic Review and Meta-analysis.

BACKGROUND: Traumatic brain injury (TBI) is a leading cause of morbidity and mortality worldwide, often complicated by hospital-acquired pneumonia (HAP) or ventilator-associated pneumonia (VAP). These infections may contribute to prolonged hospitalizations and increased morbidity. We conducted a systematic review and meta-analysis to identify predicting factors associated with the development of pneumonia (HAP or VAP) in patients hospitalized following acute TBI. METHODS: We conducted a comprehensive search of Medline and Embase from inception to 29 September 2025. We included studies that in

Development of Indole-3-yl-methylene-thiobarbital Derivatives as Inhibitors of HDAC8 Enzyme Activity.

Histone deacetylase 8 (HDAC8), a zinc-dependent class I enzyme, plays key roles in regulating gene expression and is implicated in various malignancies. Although hydroxamic acid-based inhibitors, like PCI-34051, have demonstrated potent HDAC8 inhibition, limitations including poor selectivity, instability, and off-target effects underscore the need for novel scaffolds. Here, we designed and synthesized a series of indole-based (thio)barbiturate derivatives to identify selective HDAC8 inhibitors. Compound 3d showed promising potency (IC50 = 2.9 μM) and selectivity for HDAC8 over other isoforms.

Gyrase and Topoisomerase IV as Antibacterial Targets for Gepotidacin and Zoliflodacin: Teaching Old Enzymes New Tricks.

The essential bacterial type II topoisomerases gyrase and topoisomerase IV have been exploited as the therapeutic targets of fluoroquinolone antibacterials for over four decades. Despite their broad utility, the effectiveness of fluoroquinolones has been increasingly undermined by the widespread emergence of target-mediated resistance, highlighting the need for alternative therapeutic strategies. Recent advances have produced two mechanistically distinct classes of gyrase/topoisomerase IV-targeted antibacterials: the triazaacenaphthylenes and the spiropyrimidinetriones. The first-in-class agen

Lorazepam-Assisted Interview in Clinical Psychiatry: A Narrative Review of Current Evidence and Research Gaps.

INTRODUCTION: Drug-assisted interviews have been used in psychiatry for about a century. The use of barbiturates for this purpose has declined, and the use of lorazepam has gained popularity over time for conducting drug-assisted interviews, given a better safety profile. However, the evidence for the use of lorazepam for drug-assisted interviews is fragmented and anecdotal, and mostly in the form of case reports. The current review aims to synthesize available evidence on the use of lorazepam-assisted interviews in psychiatry. METHODS: For this narrative review, a literature search was condu

Outpacing Emerging Drug Threats: Validation of ToxBox Kits That Automate LC-MS/MS Analyses.

The Centers for Disease Control and Prevention (CDC) recently began addressing critical informational gaps associated with the emergence of novel psychoactive substances (NPSs) through the formation of the Overdose Data to Action (OD2A) biosurveillance program. This program uses public health laboratories (PHLs) to report drug trends in nonfatal and fatal overdose cases, but meeting the OD2A program's goal of reporting use trends in near real-time is a challenge for resource-limited PHLs. New technology that automates NPS testing will help PHLs meet this goal and sustain OD2A testing. This stu

Dibenzofuran Based Donor-Pi-Acceptor Molecules: Effect of Azo and Aminomethylene Linkages on Photophysical and NLO Properties.

We report here dibenzofuran-based D-π-A molecules containing azo and aminomethylene linkages. Both linkages are used to attach donor dibenzofuran to various active methylene compounds, such as un/substituted barbituric acid, Meldrum's acid, etc. Compounds 8a-c and 9a-f are studied for their photophysical properties, aggregation-induced emission in polar aprotic solvents and mercury sensing done with compound 9f. The DFT and TD-DFT calculations are performed to correlate the observed photophysical properties with calculated properties, which are further used to confirm the intramolecular charge

Benzodiazepines at the crossroads: navigating therapeutic promise and perils of misuse.

Benzodiazepines (BZDs) are a widely prescribed class of psychoactive drugs known for their rapid anxiolytic, anticonvulsant, and sedative effects. Introduced as safer alternatives to barbiturates, BZDs quickly gained popularity across clinical settings but have since become a subject of public health concern due to rising rates of misuse, dependence, and overdose, particularly when co-administered with opioids. This narrative review explores the historical development, pharmacologic mechanisms, clinical indications, and adverse outcomes associated with BZD use, while highlighting emerging area

False positive urine drug screens.

Immunoassay-based urine drug screens are widely employed in clinical toxicology due to their speed, low cost, and ease of automation. However, these assays are inherently limited by antibody cross-reactivity, which can result in false-positive findings and incorrect interpretations with major implications for patient care, employment, and legal outcomes. This review updates prior literature by analyzing reported false-positive interferences published between 2013 and 2024 across commonly screened drug classes, including opioids, amphetamines, benzodiazepines, cannabinoids, barbiturates, phency

Increased intracranial pressure in severe traumatic axonal injury patients - A retrospective single-center study.

INTRODUCTION: Traumatic axonal injury (TAI), often caused by rapid rotational forces and high-energy accidents, is common in severe traumatic brain injury (sTBI). The intracranial pressure (ICP) dynamics are often unpredictable, and the need for ICP monitoring remains debated. RESEARCH QUESTION: What is the incidence of ICP elevation in patients with TAI, and how often is escalated ICP-lowering treatment required? MATERIAL AND METHODS: Retrospectively, sTBI patients treated between 2007 and 2022 with TAI lesions at the grey-white matter interface, corpus callosum, deep central structures, an

The Effect of Optimizing Urine Drug Screening Panels at Two Academic Medical Centers.

BACKGROUND: Drugs included in the urine drug screening (UDS) panel may not reflect current drug use patterns and therefore their clinical utility, particularly for drugs with low positivity rates, may be minimal. Laboratories should frequently review the components of their UDS panel. METHODS: Based on clinical need and local epidemiology, the updated UDS panel included immunoassay screens for amphetamines, benzodiazepines, cocaine metabolite, fentanyl, opiates, and oxycodone. At Site 1 (June 2022), barbiturates and cannabinoids (THC) were removed, while fentanyl and oxycodone were added. At
