Concomitant medications and outcomes of bispecific T-cell engager therapy: A propensity-matched study of tarlatamab in ES-SCLC.

BACKGROUND: Tarlatamab, a bispecific T-cell engager (BiTE), has shown efficacy in extensive-stage small cell lung cancer (ES-SCLC). However, the impact of concomitant medications on treatment outcomes remains unknown. We evaluated the association between commonly prescribed medications and survival and toxicities in patients receiving tarlatamab. METHODS: We conducted a retrospective cohort study using TriNetX between April 2024 and May 2026. Lung cancer patients on tarlatamab were stratified according to exposure to concomitant medications within 3 months before or after treatment initiation

Melatonin to improve sleep in patients undergoing major surgery: protocol for a double-blind, randomised placebo-controlled trial (MELROSE).

INTRODUCTION: Sleep disturbance is common after major surgery and may impair recovery, immune function and postoperative outcomes. Environmental disruptions, pain and routine hospital care often contribute to poor sleep during hospitalisation. Sedative medications such as benzodiazepines are frequently used but may cause adverse effects. Melatonin, a hormone regulating circadian rhythm, has been shown to improve sleep disturbance with a favourable safety profile; however, evidence for its perioperative use remains limited. METHODS AND ANALYSIS: The MELROSE trial is a prospective, double-blind

Temperament traits based on TEMPS-A scale and the use of CBD and other psychoactive substances and medications - an original study.

BACKGROUND: Cardiac injury is common and associated with poor clinical outcomes in COVID-19. Data are lacking whether high-dose intravenous vitamin C (HIVC) could help to ameliorate myocardial injury in the pandemic. METHODS: The retrospective cohort study included consecutive severe and critically ill COVID-19 patients with cardiac injury receiving symptomatic supportive treatments alone or together with HIVC. Troponin I and inflammatory markers were collected at admission and day 21 during hospitalization from the electronic medical records. RESULTS: The patients (n = 113) were categorized

Comparative Efficacy and Acceptability of Treatment Strategies for Antipsychotic-Induced Akathisia: A Systematic Review and Network Meta-analysis.

BACKGROUND: Antipsychotics are the treatment of choice for schizophrenia, but they often induce akathisia. However, comparative efficacy of treatment strategies for akathisia remains unclear. DESIGN: We performed a systematic review and network meta-analyses (PROSPERO CRD42023450720). We searched multiple databases on July 24, 2023. We included randomized clinical trials comparing 1 or more treatment strategies for antipsychotic-induced akathisia against each other or control conditions. We included adults with schizophrenia or other psychiatric disorders treated with antipsychotics. The prim

Increased risk of venous thromboembolism in children and teenagers with inflammatory bowel disease: a systematic review and meta-analysis.

BACKGROUND: The increased risk of venous thromboembolism (VTE) in adults with inflammatory bowel disease (IBD) is well known, but we know less about the VTE risk of IBD in children and teenagers. We evaluated the risk of VTE in children and teenagers through systematic review and meta-analysis. METHODS: A systematic search was conducted in PubMed, Embase, Cochrane Library, Web of Science for studies from the establishment of these databases to February 10, 2026, to find relevant research on the risk of VTE in children and teenagers with IBD (PROSPERO, ID: CRD420251081653). Drug Interactions in People on Cannabidiol: Is There Cause for Concern?

INTRODUCTION: Cannabidiol (CBD) exhibits multiple therapeutic properties, but its use in advanced cancer patients raises concerns about potential drug-drug interactions (DDIs) due to polypharmacy. This study aims to look for evidence of DDIs between concomitant medications and CBD oil in a randomized placebo-controlled trial of CBD oil for symptom control (MedCan-1 parent study). MATERIALS AND METHODS: Surrogate measures were used to identify possible drug interactions: (1) the maximum mL of oil self-selected by patients in CBD or placebo groups in relation to opioids, specific drug groups, o

Detailed Review of Melatonin and its Role in Managing the Symptoms of Depression.

Depression, which is emerging as one of the most widely prevalent neuropsychiatric disorders worldwide, has affected people across all age groups. However, it is currently primarily affecting adults aged 18 to 25. The condition is characterized by disrupted sleep cycles, diurnal variation, and disturbed core body temperature rhythms. Currently, the anti-depressant medications that are prescribed and authorized, such as tricyclic antidepressants (TCAs), selective serotonin reuptake inhibitors (SSRIs), serotonin-norepinephrine reuptake inhibitors (SNRIs), monoamine oxidase inhibitors (MAOIs), be

Association of volume and prehospital paediatric care quality in emergency medical services: retrospective analysis of a national sample.

BACKGROUND: Children represent fewer than 10% of emergency medical services (EMS) encounters in the USA. We evaluated whether agency-level paediatric volume is associated with the quality of prehospital care provided. METHODS: We conducted a retrospective analysis of 7104 agencies that contributed data consistently to the 2022-2023 National Emergency Medical Services Information System database, including children (<18 years) from an out-of-hospital EMS encounter. We assessed outcomes based on adherence to paediatric-specific quality benchmarks using mixed-effects models. RESULTS: We identif

Mediation of chronic pain and disability on opioid use disorder risk by pain management practices among adult Medicaid patients, 2016-2019.

We estimated the extent to which different pain management practices, considered together as well as individually, mediated the relationship between chronic pain or physical disability and new-onset opioid use disorder (OUD) in a large cohort of adult Medicaid patients. Considering the plausibility of the assumptions required to identify different mediational estimands, we estimated natural indirect effects when considering mediation through the group of mediators together and estimated interventional indirect effects when considering mediation through each pain management practice individuall

Sedative-hypnotics are associated with additional risk of suicide in older adults: a population-based case-control study.

STUDY OBJECTIVES: To examine the pattern of sedative-hypnotic prescriptions associated with the risk of suicidal behavior among older adults with and without insomnia. METHODS: The study included 8319 suicide cases and age- and sex-matched controls from the population-wide National Health Insurance Research Database. Suicide methods and insomnia were defined using diagnostic codes. The sedative-hypnotic use pattern was categorized by half-life, prescription volume measured in defined daily doses (DDDs), and its temporal relationship with suicide. Odds ratios were employed to assess the risk o

Impact of adjuvant analgesics on outcomes of spinal cord and peripheral nerve stimulation: a scoping review.

INTRODUCTION: Spinal cord stimulation (SCS) and peripheral nerve stimulation (PNS) are used to treat refractory pain, but even well-selected patients can fail to have analgesic benefit following implantation. The analgesia afforded by SCS/PNS may be enhanced or attenuated by the ongoing use of analgesic medications that are often consumed by patients who receive SCS and PNS implants. We undertook a scoping review to scan and summarize the evidence for impact of adjuvant pharmacotherapy on SCS and PNS therapy in animal and human settings. MATERIALS AND METHODS: A comprehensive medical literatu

Pre- and Postoperative Prescription of Opioids and Benzodiazepines in the US.

BACKGROUND: Postoperative opioid and benzodiazepine use often extends beyond recovery, yet research on surgical prescribing tends to focus on opioid-naive patients. Less is known about those already using these medications preoperatively, who face different risks and care needs. STUDY DESIGN: Using a national US claims database (PearlDiver, 2010 to 2022), we identified insured adults receiving 1 of 18 common surgeries. We examined patterns of filled prescriptions for opioids and benzodiazepines before and after surgery, including (1) opioids without benzodiazepines (opioid monotherapy), (2) b

Benzodiazepine exposure and 6-month mortality in older adults post-hospitalization: time to consider deprescribing?

BACKGROUND: Benzodiazepines (BZDs) are commonly used in older populations despite their association with multiple adverse outcomes. However, the impact of continuing versus deprescribing BZDs during hospitalization on short-term mortality after discharge remains uncertain. This study aims to evaluate the association of BZD-use at hospital discharge with 6-month mortality and explore whether in-hospital deprescribing modifies mortality risk. METHODS: This is a retrospective analysis of prospectively collected data from patients (aged 66-101 years) consecutively admitted to an Acute Geriatric U

The Number Needed to Treat for Music as a Medicine against Perioperative Anxiety: A Systematic Review and Meta-Analysis.

BACKGROUND: Music intervention is effective in reducing perioperative anxiety, which occurs in a majority of hospitalized surgical patients. A calculated Number Needed to Treat (NNT) provides an intuitive means of conveying the effectiveness of an intervention that can help clinicians decide whether or not to implement said intervention. This study aimed to calculate an NNT to provide extra context to help clinicians consider the implementation of music intervention. METHODS: To calculate the NNT of music intervention for perioperative anxiety, a systematic review and meta-analysis were perfo
