Independent Factors Associated With Opioid Refills After Inpatient Otolaryngology-Head and Neck Surgery.

IMPORTANCE: Persistent opioid use after otolaryngology-head and neck surgery (OHNS) is concerning. However, evidence-based guidelines for managing opioid refill prescriptions within 90 days after discharge, a critical period for the transition from acute to persistent opioid use, are lacking. OBJECTIVE: To identify perioperative risk factors associated with opioid refills at 1 to 30, 31 to 60, and 61 to 90 days after OHNS procedures. DESIGN, SETTING, PARTICIPANTS: This cohort study used opioid prescription data from a large academic medical center. The study included adult patients, regardle

A Scoping Review of Benzodiazepine Dosing Regimens for Adults Experiencing Moderate-to-Severe Alcohol Withdrawal Syndrome in the Acute Hospital Setting.

BACKGROUND/AIMS: Benzodiazepines have been established as a safe and efficacious treatment option for Alcohol Withdrawal Syndrome (AWS). The applicability of current evidence for the management of AWS in the acute hospital setting with benzodiazepines is not clear. This review sought to capture efficacy and safety outcomes for benzodiazepine use within acute hospital settings in those experiencing moderate-severe AWS symptoms. METHODS: Cochrane Registry, Medline, Embase and APA PsycInfo were searched for studies published between 2000 and 2025. Adult patients experiencing at least moderate se

Therapeutic Use of Cannabis and Cannabinoids: A Review.

IMPORTANCE: Approximately 27% of adults in the US and Canada report having ever used cannabis for medical purposes. An estimated 10.5% of the US population reports using cannabidiol (CBD), a chemical compound extracted from cannabis that does not have psychoactive effects, for therapeutic purposes. OBSERVATIONS: Conditions for which cannabinoids have approval from the US Food and Drug Administration include HIV/AIDS-related anorexia, chemotherapy-induced nausea and vomiting, and certain pediatric seizure disorders. A meta-analysis of randomized clinical trials reported a small but significant

Anxiolytic Medication Use in Low- Middle- and High-Income Countries: A World Mental Health Surveys Report.

BACKGROUND: Anxiolytic medications, particularly benzodiazepines, are widely prescribed, giving impetus to long-standing debates about how often these agents should be employed in clinical practice. There are, however, few cross-country studies of the pharmacoepidemiology of these agents. We report on the frequency of anxiolytic medication use, reasons for use, and perceived effectiveness of use in general population surveys across 20 countries. METHODS: Face-to-face interviews with community samples totaling n = 49,919 respondents in the World Health Organization World Mental Health (WMH) Su

Exploration of Extended-Release Lorazepam: Implications for Safe Nurse Practitioner Practice.

In 2021, the U.S. Food and Drug Administration approved Loreev XR®, a long-acting formulation of lorazepam, raising important clinical considerations for nurse practitioners (NPs) who routinely manage anxiety, insomnia, and stress-related conditions. Despite well-established risks, including dependence, withdrawal, cognitive impairment, and heightened danger when combined with central nervous system depressants, benzodiazepines (BZDs) remain widely prescribed in the United States. The current article examines the rationale for introducing a new extended-release BZD, the pharmacokinetic and pha

Olanzapine and the Spectrum of Drug-induced Diabetes: Unveiling Dual Risk Factors for Type 2 Diabetes and Acute-onset Ketoacidosis.

Olanzapine, a widely prescribed atypical antipsychotic, is associated with significant metabolic side effects including hyperglycemia and new-onset diabetes mellitus. While it often contributes to or mimic conventional type 2 diabetes through weight gain and insulin resistance, olanzapine can also induce a distinct, acute, ketosis-prone form of diabetes characterized by acute, severe hyperglycemia, and a high risk of diabetic ketoacidosis (DKA). This review summarizes the current understanding of olanzapine's multifaceted diabetogenic mechanisms, distinguishing its contributions to both type 2

Prehospital rescue treatment of cluster epileptic seizures and status epilepticus.

Benzodiazepines play a crucial role in the management of cluster epileptic seizures and status epilepticus (SE), particularly in prehospital settings where timely intervention is essential to prevent prolonged seizures and minimize neurological damage. This review focuses on the efficacy, safety, and practical considerations of benzodiazepines use in these critical situations, with an emphasis on prehospital administration. Benzodiazepines, such as diazepam, lorazepam, and midazolam, are first-line treatments for SE due to their rapid onset of action, which facilitates seizure control. Midazol

Sedation, Sleep, and Sudden Death: How ICU Sedation Alters Cardiac Rhythmicity and Autonomic Tone.

Sedation is essential for modern intensive care management, yet its effects on autonomic regulation, circadian biology, and cardiac electrophysiology remain incompletely understood. This review synthesizes current evidence on how commonly used sedative agents influence autonomic tone, sleep architecture, and electrophysiologic stability in critically ill patients. Mechanistic data indicate that propofol, dexmedetomidine, benzodiazepines, and ketamine exert distinct autonomic signatures that alter heart rate variability, baroreflex sensitivity, and ventricular repolarization. Clinical studies s

Sedation management in mechanically ventilated intensive care unit patients: Meta-analysis review.

Sedation is crucial for managing mechanically ventilated intensive care unit (ICU) patients, but agents differ in their effects. Propofol, benzodiazepines and α 2-agonists are commonly used, yet their comparative impact remains unclear. This review searched OVID MEDLINE and Embase from January 2006 to June 2025 for randomised controlled trials in adult ICU patients. The primary outcome was duration of mechanical ventilation; secondary outcomes were ICU length of stay, delirium and mortality. Twenty-six trials (N = 4,993) were included. Dexmedetomidine significantly shortened mechanical ventila

Deprescribing benzodiazepines across different healthcare settings: a quality improvement initiative.

Due to growing awareness of risks associated with benzodiazepine (BZD) use, these medications are now considered potentially inappropriate for older adults. Despite this, older adults are more likely to receive these medications than younger adults, with those in long-term care (LTC) being particularly vulnerable, emphasising the importance of reducing BZD prescriptions. Other studies have shown success in reducing BZD prescriptions using a patient-focused multicomponent intervention: however, this approach has not been well studied in LTC.During a seventeen-month quality improvement (QI) init

Inclusion of "palliative care pharmacology" as a new chapter in the pharmacology textbook for medical undergraduate students: A strength, weakness, opportunity, and threat analysis.

Palliative care is a multidisciplinary approach aimed at improving the quality of life (QoL) and alleviating sufferings of people with serious and terminal illnesses. The present health system understands that palliative and rehabilitative services offer care to patients when cure is not possible. The concept is relatively new for most Indian graduates due to little emphasis on palliative care in the medical curriculum in India. Although palliative care has been incorporated in Universal Health Coverage in India and now also in the new competency-based medical curriculum in various subjects, i

A systematic review of pharmacological effects on human aversive memory.

A large body of work has investigated the effect of various pharmacological compounds on aversive memory formation, retrieval, and modification in humans. A broad overview across signalling pathways and memory models is currently lacking. Here, we systematically review publications that tested the impact of acute pharmacological interventions on aversive memory in healthy humans, following PRISMA-2020. We identified 215 candidate compounds from 17 systems and searched PubMed, Web of Science and Scopus, until 14 June 2024. We identified 100 publications with 36 compounds targeting 13 systems. T

Perceived Pain Following Hypnotic Deprescribing in Older Adults.

BACKGROUND: Older adults with chronic insomnia often use benzodiazepine receptor agonists (BZRAs) despite known associated risks and non-pharmacological alternatives such as cognitive behavioral therapy for insomnia (CBTI). CBTI reduces insomnia severity and could potentially improve other outcomes such as the impact of pain on daily activities, even when BZRAs are deprescribed. Yet concerns that deprescribing may worsen pain (which is often comorbid with insomnia) can be a barrier to engagement in BZRA deprescribing. This study examined changes in pain outcomes associated with deprescribing B
