Analysis of factors driving variations in neonatal parenteral nutrition over a decade: A retrospective study.

BACKGROUND: Parenteral nutrition (PN) is essential for supporting preterm and critically ill neonates who are unable to tolerate adequate enteral feeds. However, PN composition remains highly variable due to differences in guidelines, institutional practices, prescriber discretion, and more. This lack of standardization contributes to inconsistent nutrient delivery and may affect clinical outcomes. A deeper understanding of the sources of variation is needed to inform efforts toward more consistent and evidence-aligned prescribing. METHODS: This retrospective study analyzed 54,464 neonatal PN

NRS-2002-defined nutritional risk and associated factors in intensive care unit patients: a cross-sectional study.

BACKGROUND: Malnutrition is common in intensive care units (ICUs) and is associated with adverse clinical outcomes. This study aimed to evaluate NRS-2002-defined nutritional risk in adult ICU patients and to examine factors associated with high nutritional risk. METHODS: This cross-sectional study was conducted in a tertiary-level ICU of a training and research hospital in Türkiye between April 2024 and May 2025. A total of 169 adult patients were included. Nutritional risk was assessed using the Nutritional Risk Screening-2002 (NRS-2002), and patients were classified as low (< 3) or high (≥

Pre-transition nutrition dose and mortality using a CRP-Free operational metabolic transition framework: A MIMIC-IV transportability analysis.

BACKGROUND & AIMS: A physiology-based framework to identify transition from early catabolism toward recovery was previously derived using a steroid-corrected insulin-resistance index (IRI) and concordant recovery criteria. For external testing in MIMIC-IV, we prespecified a C-reactive protein-free operational rule requiring a ≥40% post-peak IRI decline and ≥3 concordant non-CRP criteria, and asked whether the pre-transition feeding pattern seen in derivation could be reproduced. METHODS: We performed a retrospective MIMIC-IV re-implementation. After adult/eligible stay filtering, ICU length o

Enhanced Energy Metabolism and Developmental Signaling in Neonatal Brains of Female Piglets Parenterally Fed with 18-Carbon n-3 Fatty Acid-Based Vegaven Compared with Fish Oil-Containing Lipid Emulsion.

BACKGROUND: Vegaven, an intravenous lipid emulsion based on 18-carbon n-3 (ω-3) fatty acids, reduces the accumulation of lipopolysaccharide (LPS) in peripheral organs including the brain of parenterally fed piglets compared with fish oil-containing SMOFlipid. Inflammation may impair brain energy metabolism and growth signaling. OBJECTIVES: This study aimed to compare key brain metabolic and developmental signaling pathways with the use of different lipid emulsions for parenteral nutrition (PN). METHODS: In this study, 3-4-d-old female piglets were randomly assigned to isocaloric isonitrogeno

Neonatal Total Parenteral Nutrition and Long-Term Metabolic Outcomes: An 18-Year Nationwide Cohort Study of 2.3 Million Infants.

Background/Objectives: Total parenteral nutrition (TPN) is essential for neonates unable to receive adequate enteral feeding, but its long-term metabolic effects remain uncertain. This study evaluated associations between neonatal TPN exposure, exposure duration, and later endocrinologic or metabolic disorders. Methods: This nationwide cohort included 2,424,716 infants born in South Korea between 2005 and 2010. TPN exposure was identified during the first 28 days of life. The primary outcome was the first occurrence of type 2 diabetes mellitus (T2DM), essential hypertension, dyslipidemia, or o

Safety of Early Mobilisation in Critically Ill Patients Under Unusual High-Risk Conditions: A Latin American Case Series.

BACKGROUND: Early mobilisation (EM) in the intensive care unit (ICU) is associated with a reduced incidence of intensive care unit-acquired weakness, shorter hospital length of stay and improved functional outcomes. However, most of the available evidence derives from patient populations at low risk of safety-related adverse events. This limits the generalisability of its benefits to more complex clinical scenarios, such as the concurrent administration of blood products, continuous parenteral nutrition, antiarrhythmic infusions or multiple haemodynamic pharmacological support. Consequently, e

Association between dietary counselling and nutrition provision during transition from ICU to post-ICU wards. Three-centre retrospective analysis.

BACKGROUND: Adequate nutrition is crucial in critical illness and remains important after discharge from the intensive care unit. The transition from enteral to oral feeding increases the risk of protein and energy deficiency. This study evaluated whether structured dietary counselling affects nutritional provision and reduces deviation from calculated nutritional goals during hospitalization and the transition from the intensive care unit to the post-intensive care unit. METHODS: This retrospective study included adult patients discharged from the intensive care unit to in-hospital wards aft

Drug-Induced Anaphylaxis in Children: A Comparative Analysis of Inpatient versus Outpatient Settings.

INTRODUCTION: Studies specifically focusing on drug-induced anaphylaxis (DIA) in children are few. There is no study comparing inpatient (IP) and outpatient (OP) settings in DIA, either in children or in adults. We aimed to compare the characteristics of pediatric DIAs observed in IP and OP settings. METHODS: Patients aged ≤18 years who were diagnosed with DIA at our institution over the past 15 years were included in the study. The diagnosis of DIA was made clinically, and diagnostic testing was performed in eligible patients. Patients were categorized as IP and OP groups based on the settin

Long-term risk of incident dementia following documented COVID-19 among older adults with type 2 diabetes: a propensity score-matched cohort study.

BACKGROUND: Type 2 diabetes mellitus (T2DM) is a well-established risk factor for dementia, and COVID-19 has been associated with persistent neurological and cognitive sequelae. However, whether this association extends beyond the early post-infection period among older adults with T2DM-a population with compounded vascular and metabolic vulnerability-remains unclear. METHODS: Using the TriNetX Global Collaborative Network, we conducted a retrospective cohort study of patients aged ≥65 years with T2DM, comparing those with documented COVID-19 to a matched unexposed cohort (1:1 propensity scor

ESPEN practical guideline on ethical aspects of medical nutrition therapy.

The provision of medical nutrition therapy (MNT) is a medical intervention that requires an indication, a therapeutic goal, and the consent of the patient. The decision-making process surrounding MNT often involves complex ethical considerations. The aim of this guideline is to provide practical recommendations on an ethical approach to the decision-making process in clinical nutrition. This practical guideline was developed by an international multidisciplinary working group. The practical, solely consensus-based guideline is based on and updates the 2016 ESPEN Guideline on Ethical Aspects of

Arterial and Venous Thrombotic Complications Following Recreational Nitrous Oxide Misuse: A Comprehensive Review.

Recreational nitrous oxide (N2O) misuse has increased markedly over the past decade, particularly among adolescents and young adults. Although neurological toxicity is well established, venous and arterial thrombotic events remain underrecognized in cardiovascular practice. A comprehensive review of the literature was performed by searching PubMed/MEDLINE, Embase, and Google Scholar from inception to January 2026, using combinations of "nitrous oxide," "thrombosis," "pulmonary embolism," "deep vein thrombosis," "stroke," "myocardial infarction," "aortic thrombus," and "homocysteine." Association of preoperative corticosteroids with persistent opioid requirements following total knee arthroplasty: an exploratory population-based cohort study.

BACKGROUND: Used to suppress inflammation, preoperative corticosteroid exposure is common among patients undergoing total knee arthroplasty (TKA). New preclinical evidence links corticosteroid exposure to developing chronic pain, which may lead to long-term opioid treatment. We aimed to explore whether there is an association between preoperative corticosteroid exposure and persistent opioid requirements in previously opioid-naïve patients undergoing TKA. METHODS: In an exploratory retrospective cohort study, we searched a nationwide USA claims database for patients who were opioid-naïve and

Comparative Outcome of Extraoral versus Intraoral Approaches in the Management of Isolated Zygomatic Arch Fractures: A Systematic Review.

INTRODUCTION: Meckel's diverticulum is a common congenital abnormality of gastrointestinal tract in children. Planar scintigraphy using Technetium-99m pertechnetate is widely used in the diagnosis of Meckel's diverticulum. Single photon emission computed tomography/computed tomography (SPECT/CT) fusion imaging may help to locate the Meckel's diverticulum lesion. We now present a Meckel's diverticulum case which tends to be missed. CASE DESCRIPTION: The patient was diagnosed with Mecke's diverticulum by planar scintigraphy in 2007. After seven years, a recurrence of hematochezia made the patie

Intraperitoneal Local Anesthetic Instillation for Post-Operative Pain Control After Bariatric Surgery: A Systematic Review and Meta-Analysis of Randomized Controlled Trials.

BACKGROUND: Postoperative pain after bariatric surgery is a significant clinical challenge, with approximately 61.4% of patients reporting persistent pain. Intraperitoneal local anesthetic instillation (IPLA) has been proposed as a strategy to reduce pain and opioid-related adverse effects. However, as its effectiveness in bariatric procedures remains controversial, this meta-analysis was conducted to clarify the role of IPLA in this setting. METHODS: This systematic review and meta-analysis was conducted following PRISMA guidelines and was prospectively registered in PROSPERO. We performed a
