Intramuscular droperidol, olanzapine, midazolam, or lorazepam to treat methamphetamine intoxication in the emergency department.

INTRODUCTION: Emergency department (ED) visits for acute methamphetamine-associated agitation are increasing. Many cases require parenteral medications. Benzodiazepines are often recommended first-line, however randomized trials of intravenous medications suggest antipsychotics are also effective. High-quality data on intramuscular medications are lacking. OBJECTIVE: To compare the effectiveness of intramuscular droperidol, olanzapine, midazolam, and lorazepam to treat methamphetamine-associated agitation in the ED. METHODS: This was a secondary analysis of previously published data from 201

Computerized gravimetric control of parenteral analgesic admixtures: A retrospective quality improvement study.

OBJECTIVE: Postoperative pain and labor pain are among the most prevalent and intense types of acute pain. Their effective management often requires the administration of parenteral analgesic mixtures combining opioids with local anesthetics. Since no ready-to-use commercial formulations are available and given the complexity and risk of error in their preparation, the Spanish Guide to Good Medication Preparation Practices in Hospital Pharmacy Services recommends their centralized compounding in clean rooms within the pharmacy department. The aim of this study is to describe the implementation

Post-intensive care unit (ICU) nutrition and rehabilitation: A multidisciplinary survey of healthcare professionals involved in ICU care.

AIMS: Recovery after critical illness frequently extends beyond intensive care unit (ICU) discharge, with many patients experiencing persistent physical, cognitive impairments encompassed within post-intensive care syndrome, alongside ongoing nutritional deficits. Despite increasing awareness of the role of nutrition and rehabilitation in this phase, post-ICU care remains heterogeneous, and standardized guidelines are lacking. This survey aimed to evaluate current practices, challenges, and perceptions regarding nutritional assessment, therapy and rehabilitation in ICU survivors. METHODS: A 3

Feeding the injured brain: nutrients, dose, timing, and monitoring.

Nutrition is a key component of the management of neurocritically ill patients. The healthy brain accounts for up to 25% of total resting energy expenditure (REE), and after injury the brain faces inflammation, altered glucose metabolism, and catabolic stress. This review outlines the evolving science and current practice of nutrition in neurocritical illness, emphasizing pathophysiology, metabolic phases, and individualized strategies. Enteral nutrition remains the preferred route, with early, low-dose initiation within 24-48 h generally recommended. Feeding intolerance, aspiration risk, and

Sublingual Administration for Pain Control: Pharmacology, Clinical Applications and Future Directions.

Sublingual (SL) drug delivery represents an important alternative route of analgesic administration, particularly in settings where rapid pain control is desired or oral and parenteral routes are limited. By enabling direct absorption through the oral mucosa, SL administration partially bypasses first-pass hepatic metabolism, potentially improving bioavailability and accelerating the onset of action for select medications. Historically, SL analgesia has been most closely associated with opioid formulations such as fentanyl, buprenorphine, and sufentanil. SL fentanyl has demonstrated efficacy i

Glucose Deprivation of Tumor Cells via Selective Nutrient Delivery: A Potential Therapy for Metastatic Breast Cancer.

BACKGROUND/AIM: Metastatic breast cancer remains a major clinical challenge despite the availability of various chemotherapeutic agents. Current metabolic inhibitors have limitations, prompting the need for innovative strategies that selectively target tumor cells while sparing normal tissues. This study aimed to propose a novel approach focused on depriving tumor cells of glucose while ensuring nutrient delivery to normal cells. MATERIALS AND METHODS: A comprehensive literature review was conducted using PubMed and Google Scholar. The proposed strategy involved focusing on studies that deple

A Multispecies Systematic and Critical Review of Intranasal Administration in Veterinary Anaesthesia and Emergency Care: Promising Evidence and Overlooked Challenges.

Intranasal (IN) drug delivery has increasingly considered as an easy, practical and non-invasive alternative to parenteral administration in veterinary medicine, offering rapid systemic and potential nose-to-brain effects. The first part of this review systematically collected and synthesized published evidence on IN administration across animal species, while the second part critically analysed the anatomical, pharmacological and technical factors that determine its success and limitations. Part I consisted a total of 110 eligible studies published between 1991 and 2025, encompassing dogs, ca

2025 expert consensus on the use of parenteral prostanoids in incident and prevalent PAH patients: The Italian perspective.

BACKGROUND: The demographic shift of pulmonary arterial hypertension (PAH) patients, the expanding therapeutic armamentarium with new emerging drugs and the heterogenous experience in handling more complex therapies are making the management of PAH increasingly challenging. This project was developed to standardize PAH management across Italian centres and ensure timely access to the optimal therapeutic strategies. METHODS: A consensus document containing 10 statements, mainly focused on the use of parenteral prostanoids, was developed by eight experts in PAH during in-person and web-based me

Boosting Bupivacaine: The Role of Adjuvant Perineural Dexmedetomidine in Femoral Nerve Blocks.

Background Effective pain management in patients with femoral fractures, such as intertrochanteric fractures, remains a major challenge in emergency medicine. Ultrasound-guided femoral nerve blocks administered on arrival have emerged as a promising alternative to conventional parenteral analgesics due to their ease of administration, rapid onset of analgesia, prolonged sensory blockade, and reduced risk of systemic adverse effects. Dexmedetomidine, a selective α2-adrenergic agonist, has been increasingly studied as an adjuvant to local anesthetics because of its ability to hasten the onset an

Real-Time Continuous Glucose Monitoring Improves Glycemic Control in the Early Postoperative Period Following Liver Transplantation: A Randomized Controlled Trial.

BACKGROUND: Early postoperative glycemic control after orthotopic liver transplantation (OLTx) is challenging because of surgical stress, corticosteroid and calcineurin-inhibitor immunosuppression, and parenteral nutrition. We evaluated whether adding real-time continuous glucose monitoring (rtCGM) to standard care increases time in the target glucose range. METHODS: In this single-center randomized controlled trial, OLTx recipients were randomized after surgery to open (Active) or blinded (Control) rtCGM (Dexcom G6); in the Active group, sensor data guided insulin titration. Effects of Combining Short- and Long-Acting Local Anaesthetics in Peripheral Nerve Blocks-A Systematic Review With Meta-Analysis and Trial Sequential Analysis.

BACKGROUND: This systematic review aims to clarify how combining short- and long-acting local anaesthetics (LA) affects onset time and duration in peripheral nerve blocks. METHODS: We included randomised controlled trials comparing adults undergoing peripheral nerve blocks with combined short- and long-acting LA with those receiving blocks performed with either a short- or long-acting LA. The primary outcome was the duration of analgesia. Secondary outcomes included the duration of onset time. We conducted meta-analyses, trial sequential analyses (TSA), risk of bias-2 and Grading Recommendati

Ultralow concentration levobupivacaine 0.0625% for continuous wound infusion after open abdominal surgery: a prospective observational study.

Continuous wound infusion with local anaesthetics after open abdominal surgery may provide opioid-sparing analgesia, but evidence on very low-concentration regimens is limited. We conducted a prospective observational study including 50 patients receiving continuous infusion of levobupivacaine 0.0625% via pre-peritoneal or subfascial catheters at a flow rate of 12 ml.h-1 per catheter. Six patients (12%) required intravenous morphine rescue in the first 48 h, with a median dose of 3 mg. Median pain scores remained consistently low and 14 patients (28%) received additional local anaesthetic bolu

Incidence of hemi-diaphragmatic paresis with different volumes of local anaesthetics in interscalene brachial plexus block.

BACKGROUND AND AIMS: The incidence of diaphragmatic-palsy following interscalene brachial plexus block (IBPB) is almost 100% where the drug volume plays a significant role. We compared the incidence of hemidiaphragmatic paresis and the success rate following IBPB using three different volumes of local anaesthetics. METHODS: Ninety patients undergoing shoulder and arm surgeries under ultrasound-guided IBPB were randomly allocated into three groups: Group A (10 ml), Group B (15 ml), and Group C (20 ml). The drug administered was 0.75% ropivacaine with 50 mcg dexmedetomidine. The diaphragm excur

Comparative effectiveness of minimally invasive therapies for plantar fasciitis: a systematic review and network meta-analysis.

Plantar fasciitis is a common and often disabling condition that affects both general and athletic populations, with chronic cases significantly impairing function and quality of life. Although a wide range of conservative and non-invasive, and minimally invasive treatments are available, the absence of direct comparative evidence has limited clarity about which interventions offer the most durable benefits. We conducted a systematic review and network meta-analysis of 63 randomized controlled trials including 4170 participants to evaluate extracorporeal shock wave therapy, prolotherapy, and i

Effect of topical dexamethasone and bupivacaine application on postoperative pain following total extraperitoneal hernioplasty: a randomized controlled prospective study.

BACKGROUND: Although pain after laparoscopic total extraperitoneal (TEP) hernioplasty is lower compared with open techniques, effective pain control in the early postoperative period still remains clinically important. Although local anaesthetics are widely used, their limited duration of action often necessitates additional analgesics. This study was conducted to evaluate the effectiveness of the combination of dexamethasone and bupivacaine, applied locally to the surgical site, in reducing early postoperative pain and the need for analgesics. METHODS: In this prospective, randomized controll

Current controversies in paediatric regional anaesthesia.

PURPOSE OF REVIEW: Regional anaesthesia has well established benefits in paediatric perioperative care and acute pain interventions. This article explores current controversies and emerging evidence in paediatric regional anaesthesia. RECENT FINDINGS: Despite rates of local anaesthetic systemic toxicity (LAST) remaining low, dosing of local anaesthetics in children remains controversial, particularly in neonates, in whom maximum safe doses can easily be exceeded. Local anaesthetic additives, such as dexamethasone, dexmedetomidine, and clonidine, are commonly used to prolong regional anaesthes

Spinal anaesthesia using chloroprocaine 1% for caesarean birth.

Spinal anaesthesia with hyperbaric bupivacaine is the standard technique for caesarean birth. However, when amide local anaesthetics are contraindicated, alternative strategies are required. We report the management of a patient undergoing emergency caesarean birth for pre-eclampsia in whom a history suggestive of allergy to amide local anaesthetics precluded the use of bupivacaine. A spinal anaesthetic was performed using intrathecal chloroprocaine 1%, an ester local anaesthetic, in combination with diamorphine. An adequate sensory block to the T4 dermatome was achieved promptly. The density
