Flecainide binding dynamics in voltage-gated sodium channels: A comparative isoform analysis of the state-dependent block.

BACKGROUND AND PURPOSE: Local anaesthetics and class I anti-arrhythmic drugs block voltage-gated Na+ channels, but the molecular basis for the isoform differences in drug sensitivity remains unclear. Understanding these mechanisms is essential for optimizing therapeutic efficacy and safety. Flecainide, a class Ic anti-arrhythmic agent, is also used for non-dystrophic myotonias but lacks use in epilepsy. This study aimed to elucidate the mechanisms underlying the isoform differences in the state-dependent block of flecainide. EXPERIMENTAL APPROACH: We employed site-directed mutagenesis, electr

Ultrasound-Guided Sciatic and Saphenous Nerve Blocks Enhance Perioperative Analgesia in Sheep Undergoing Experimental Orthopaedic Hindlimb Surgery.

The demand for refined anaesthetic protocols in ovine experimental surgery has increased due to ethical considerations and the need to improve perioperative stability. This study evaluated the analgesic efficacy of ultrasound-guided combined sciatic and saphenous nerve blocks using two different local anaesthetics in Hair Canarian Sheep undergoing invasive orthopaedic hindlimb surgery. Fifteen clinically healthy sheep were randomly assigned to one of three groups: lidocaine (2%), bupivacaine (0.5%), or control (general anaesthesia alone). Intraoperative physiological parameters, including hear

Perineural Dexamethasone Versus Intravenous Dexamethasone as an Adjunct to Local Anaesthetic Mixture in Supraclavicular Brachial Plexus Block: A Prospective Double-Blind Randomized Controlled Study.

Introduction Supraclavicular brachial plexus block is most preferred for upper limb surgeries since it produces conduction blockade of the trunks of the brachial plexus. Additives are used with local anaesthetics to enhance the effect of analgesia in patients. Therefore, this study is aimed at comparing the efficacy, onset, and duration of block, intraoperative hemodynamics, and postoperative complications, if any, when dexamethasone is used as an adjuvant in supraclavicular block perineurally and intravenously. Methods Ninety adult patients scheduled for elective forearm and hand surgery were

The LAST word on safety: an iterative quality improvement study of local anaesthetic documentation.

The use of local anaesthetics is widespread in peri-operative care and guidance recommends calculating and documenting the maximum safe dose and recording drug concentration in mg.ml-1. We conducted a single-centre quality improvement project to improve documentation of local anaesthetic dosing in 357 adult surgical patients. Six plan-do-study-act cycles were implemented over 10 months, including staff education, a dose-calculation nomogram, a modified WHO sign-in pause and a standardised local anaesthetic documentation sticker. At baseline, no record documented the maximum dose or recorded co

Ultrasound-guided rectus sheath blocks after midline laparotomy: an exploratory observational comparative implementation study comparing the analgesic effect between liposomal bupivacaine and continuous ropivacaine.

BACKGROUND: Open abdominal surgery can cause significant postoperative pain, requiring optimal management to facilitate recovery. The use of epidural analgesia has declined, resulting in the increased use of locoregional analgesic techniques as part of a multimodal regime. METHODS: This exploratory observational comparative implementation study aimed to evaluate the postoperative opioid consumption and analgesic outcome following the clinical implementation of liposomal bupivacaine in ultrasound-guided rectus sheath blocks, compared to a historical cohort receiving continuous ropivacaine afte

Clinical efficacy of intra-articular lidocaine with or without adrenaline during shoulder arthroscopy in dogs.

INTRODUCTION: Intra-articular (IA) administration of local anaesthetic drugs has been widely reported in shoulder, elbow, ankle and knee arthroscopy in human medicine. The use of IA adrenaline to control bleeding in arthroscopy is relatively common, but its combined effect with local anaesthetics on perioperative pain has not been studied in dogs. This study aims at evaluating the efficacy of IA shoulder administration of lidocaine or lidocaine plus adrenaline in dogs (Canis lupus familiaris) as pre-emptive analgesia during arthroscopic surgery under general anaesthesia. METHODS: Twenty-four

Comparative evaluation of segmental spinal anaesthesia with conventional spinal anaesthesia in endoscopic transurethral surgery.

BACKGROUND AND AIMS: Lumbar spinal anaesthesia (LSA) for endoscopic urological procedures often causes extensive sympathetic blockade, leading to hypotension, bradycardia, and delayed ambulation. Thoracic segmental spinal anaesthesia (TSSA) offers targeted sensory blockades with limited motor involvement and improved haemodynamic stability. The primary objective of the study was to compare motor block onset between TSSA and LSA. Secondary objectives included sensory block characteristics, two-segment regression time, haemodynamics, perioperative complications, and patient and surgeon satisfact

Specific neurotoxicity of local anaesthetics revisited: An evidence-based narrative review.

Local anaesthetics are essential to regional anaesthesia but may exert neurotoxic effects on peripheral nerves under specific conditions. Although perioperative nerve injury is increasingly recognised, the contribution of local-anaesthetic neurotoxicity, the relative toxicity of different agents and concentrations, and the influence of patient-related risk factors remain incompletely defined. This evidence-based narrative review synthesises experimental and clinical data on peripheral nerve neurotoxicity associated with local anaesthetics and discusses implications for clinical practice and fu

Safe Use of Adrenaline in Cryosurgery for Eyelid Basal Cell Carcinoma Evaluated With Infrared Thermography.

The efficacy of cryosurgery for skin cancers is believed to depend on thaw time, which may be influenced by the presence of adrenaline in local anaesthetics due to its vasoconstrictive effects. This is the first study to perform detailed, non-invasive temperature measurements using infrared (IR) thermography during cryosurgery for basal cell carcinomas (BCCs), evaluating the impact of adrenaline on thaw time. A total of 15 patients with eyelid BCCs underwent cryosurgery with local anaesthetics both with and without adrenaline. Thawing was continuously monitored using a high-resolution IR camer

Therapeutic gases as emerging treatments for oral diseases.

Oral delivery of gas-based therapies provides a targeted, minimally invasive approach to treating oral diseases. Conventional strategies, such as mechanical debridement, antibiotics, and surgical intervention, are limited by the inaccessibility of oral biofilms, the development of antimicrobial resistance, and challenges in promoting tissue regeneration. Therapeutic gases, including oxygen (O2), ozone (O3), nitrous oxide (N2O), nitric oxide (NO), carbon monoxide (CO), carbon dioxide (CO2), hydrogen (H2), hydrogen sulfide (H2S), and argon-based plasma, have emerged as promising options to addre
