Exploring clinician and patient perspectives on choice of general anaesthetic. A report of two U.S. surveys: an anesthesia clinician Multicenter Perioperative Outcomes Group survey and a patient Amazon Mechanical Turk survey.

BACKGROUND: Inhalation anaesthesia or propofol-based TIVA are the two primary options for administering general anaesthesia. Shared decision-making between clinicians and patients is critical. This study explores the perspectives of both anesthesia clinicians and patients on general anaesthesia techniques, aiming to identify gaps in communication and decision-making. METHODS: Two surveys were conducted in December 2020: an 18-item clinician survey disseminated through the Multicenter Perioperative Outcomes Group to clinicians who administer general anesthesia and a 33-item patient survey dist

Gas-propelled microneedles for precision transdermal therapeutics.

Transdermal drug delivery is an appealing approach because it is easy to administer, bypasses first-pass metabolism, and utilizes an extensive surface area of the skin. Microneedles, as an emerging transdermal drug delivery system, offer minimal invasiveness, painlessness, and targeted in-situ treatment. However, the existing microneedle technologies rely on passive diffusion, which can result in unpredictable drug penetration profiles. To enhance the active deep penetration and diffusion of cargoes of the microneedles, gas-propelled microneedles have gradually gained attention from researcher

A summary guide for detecting and reducing nitrous oxide infrastructure leaks in healthcare facilities.

Leaks from nitrous oxide (N2O) infrastructure have been identified as a significant contributor to the greenhouse gas emissions of healthcare facilities. Recent studies in Australia and in the United Kingdom have found at least half (and often more than 70%) of the N2O supplied to many healthcare facilities leaks from medical pipelines and associated infrastructure before clinical administration. To assist in addressing this issue, the University of Melbourne, in collaboration with the Interim Australian Centre for Disease Control and the Australian Government's Department of Health and Aged C

PICC migration and perforation of internal jugular vein after neck dissection.

Peripherally inserted central catheters (PICCs) were introduced as an alternative to centrally inserted central catheters (CICCs) inserted into the jugular vein to provide central venous access. In head and neck reconstructive surgery, PICCs are preferred over CICCs because they do not obstruct surgical access, such as during neck dissection. Additionally, PICCs offer a safer option than peripheral access for the infusion of vesicants, irritants, hyperosmolar solutions, antibiotics, prolonged parenteral nutrition, and chemotherapy agents. We present a case of perforation of the internal jugula

Therapeutic gases and the Nrf2/Keap1 pathway: direct thiol modification vs . indirect modulation.

The nuclear factor erythroid 2-related factor 2/Kelch-like Ech-associated protein 1 (Nrf2/Keap1) pathway is a central homeostatic module that integrates oxidant, electrophilic, inflammatory, and metabolic signals into adaptive transcriptional programs. In parallel, nitric oxide (NO), hydrogen sulfide (H 2 S), and molecular hydrogen (H 2 ) have emerged as therapeutically relevant gaseous mediators with distinct chemical identities, tissue distribution profiles, and pharmacological constraints. This review re-examines these gases through a single organizing question: how can highly dissimilar sm

Impact of single vs. dual local anaesthetic regimen on the efficacy and safety of percutaneous stellate ganglion block for electrical storm: new insights from the STAR study.

AIMS: Percutaneous stellate ganglion block (PSGB) is a promising treatment for refractory electrical storm (ES). However, there is a lack of solid indications regarding the optimal pharmacological regimens and combination of local anaesthetics (LAs). This study, from the multicentre STAR registry, aimed to evaluate the impact of single vs. dual LA regimen on the efficacy and safety of PSGB. METHODS AND RESULTS: We analysed 422 PSGB procedures from 298 patients (30% single LA, 70% dual LAs). Both regimens significantly reduced the number of treated ventricular arrhythmias (VAs) in the 12 h pos

Intranasally delivered fucoidan nanozyme-decorated photosynthetic Chlamydomonas regulates vascular pathology and hypoxia in diabetes-associated retinal microvascular dysfunction.

Diabetic retinopathy (DR) is a leading cause of vision loss and is strongly associated with microvascular pathology and retinal hypoxia. Here, we report an intranasally delivered microalgae drug delivery system, Chlamydomonas@Pro-Fu-PEDOT nanozymes (CHL@Pro-Fu-PEDOT-NZs), engineered to target retinal injury. Under 660-nm red-light irradiation, the platform exploits the photosynthetic activity of Chlamydomonas to generate bioelectric cues and therapeutic gases (O2), thereby alleviating hypoxia and reducing vascularization. Fucoidan functionalization further confers P-selectin-mediated vascular

Use of Intraperitoneal Lidocaine in Horses Undergoing Laparotomy for Colic.

Perioperative lidocaine is widely used in horses with acute abdomen for its analgesic, prokinetic, anti-inflammatory, and anti-endotoxic effects, although evidence of its visceral analgesic efficacy remains inconclusive. Given the potential of alternative routes of administration for local anaesthetics, this prospective, randomised, unblinded clinical trial evaluated whether intraperitoneal (IP) lidocaine improves early postoperative recovery, including recovery from anaesthesia and the first 24 h after laparotomy for colic. A Multifactorial Numerical Rating Composite Pain Scale for equines wa

Efficacy and safety of intrathecal adjuvants in lower limb orthopaedic surgery: a systematic review and network meta-analysis of randomised controlled trials.

BACKGROUND: Intrathecal adjuvants are frequently used in spinal anaesthesia for lower-limb surgery to enhance block quality and prolong postoperative analgesia. However, evidence remains fragmented across individual agents. This systematic review with frequentist network meta-analysis aimed to compare the efficacy and safety of intrathecal adjuvants in patients undergoing lower limb orthopaedic surgery to evaluate the effectiveness and safety of intrathecal adjuvants combined with long-acting local anaesthetics for lower limb orthopaedic surgery. METHODS: A systematic search of PubMed, CENTRA

Secondary Analysis of the CAN-Study, a Randomised Controlled Trial - Local Anaesthesia and Overall Survival.

INTRODUCTION: This study investigates long-term survival in the Cancer and Anaesthesia (CAN) study, focusing on the potential impact of local anaesthetic infiltration during breast cancer surgery. A positive effect from peritumoral infiltration has recently been suggested. We conducted an exploratory data analysis to examine a possible association between uncontrolled exposure to local anaesthetics during breast cancer surgery and long-term survival. METHODS: Breast cancer patients were recruited to compare long-term survival according to whether anaesthesia was maintained with inhaled sevofl

Acute Abdomen Requiring 14 Relooks and Five Months in ICU With a Favourable Outcome: A Report and Review of Literature.

An open abdomen or laparostomy is a lifesaving strategy used in critical situations to protect vital organs and stabilize patients before definitive closure of the abdominal incision. The indications for maintaining an open abdomen are broadly classified into two categories: the first is where it is impossible to close the abdomen due to bowel oedema and abdominal compartment syndrome (ACS), and the second is where there is a need for damage-controlled laparotomy or repeated intra-abdominal interventions. These open abdomens are frequently reported to require up to four relook laparotomies, an

Molecular Mechanisms of Intestinal Adaptation in Short Bowel Syndrome: A Comprehensive Review.

Short bowel syndrome (SBS) develops when the remaining intestine is unable to sustain adequate nutrient and electrolyte absorption following extensive bowel resection. The condition is characterized by malabsorption and significant fluid losses which lead to dehydration and progressive weight loss, thus promoting patient dependence on parenteral fluids or nutrition. After an initial acute phase marked by accelerated intestinal transit and gastric hypersecretion, long-term clinical outcomes are largely determined by the capacity of the remaining bowel for intestinal adaptation-a sustained proce

Uncommon intraoral source of generalized tetanus: Case report and review of intensive care strategies.

INTRODUCTION: Tetanus is an acute disease caused by the tetanospasmin toxin produced by Clostridium tetani, with a high mortality particularly in developing countries. Although preventable through primary immunization and booster vaccination, cases in adults with uncertain immunization status remain common. Intraoral wounds are uncommonly reported as portals of entry. CASE PRESENTATION: A 54-year-old male developed generalized tetanus after manipulating a tooth with a toothpick, causing bleeding. Initial dental pain and oral numbness progressed to trismus, opisthotonus, and risus sardonicus.
