AI-guided precision parenteral nutrition for neonatal intensive care units.

One in ten neonates are admitted to neonatal intensive care units, highlighting the need for precise interventions. However, the application of artificial intelligence (AI) in guiding neonatal care remains underexplored. Total parenteral nutrition (TPN) is a life-saving treatment for preterm neonates; however, implementation of the therapy in its current form is subjective, error-prone and resource-consuming. Here, we developed TPN2.0-a data-driven approach that optimizes and standardizes TPN using information collected routinely in electronic health records. We assembled a decade of TPN compo

External Validation of Risk Prediction Score for Candidemia in Critically Ill Patients: A Retrospective Observational Study.

Candidemia is associated with high mortality in critically ill patients. Early diagnosis of candidemia is imperative for starting treatment. Therefore, this study was designed to externally validate the candidemia risk prediction scores. This single-center, retrospective observational study included all critically ill patients admitted to the intensive care unit at a tertiary referral center from January 2018 to December 2023. The risks and patient outcomes were analyzed using bivariate and multivariate regression analysis. A total of 500 patients were analyzed with 2 dropouts due to incomplet

Effect of Postconditioning With Intralipid in Patients Undergoing Off-Pump Coronary Artery Bypass Surgery: A Randomized Controlled Trial.

BACKGROUND: Off-pump coronary artery bypass (OPCAB) surgery is an alternative to conventional on-pump coronary artery bypass surgery for cardiac revascularization. It offers several advantages, including reduced mortality, decreased morbidity, and faster recovery. However, maintaining optimal myocardial function during OPCAB remains a challenge due to ischemia-reperfusion injury. Intralipid (IL), a fat emulsion used for parenteral nutrition, has shown potential cardioprotective effects. While the literature has elucidated IL's role as a preconditioning agent in OPCAB and a postconditioning age

Pharmacokinetics of Intraperitoneal Lidocaine for Sustained Postoperative Analgesia in Adults.

BACKGROUND AND OBJECTIVE: Intraperitoneal lidocaine is an emerging strategy for analgesia following abdominal surgery but its pharmacokinetics are poorly quantified. We aimed to develop a pharmacokinetic model for unbound and total lidocaine by intraperitoneal and intravenous routes. METHODS: Unbound and total lidocaine concentrations, and pain scores (visual analogue score 0-10) were from a published randomized control trial of adults (n = 56) undergoing laparoscopic colon resection. Participants received intravenous or intraperitoneal lidocaine (2 mg/kg bolus then 1.5 mg/kg/h infusion) for

A New Perspective on Overfeeding in the Intensive Care Unit (ICU): Challenges, Dangers and Prevention Methods.

Overfeeding, currently defined as providing excessive energy and nutrients beyond metabolic requirements, is a common yet often overlooked issue in the intensive care unit (ICU) setting. Understanding the factors contributing to overfeeding and implementing strategies to prevent it is essential for optimizing patient care in the ICU. Several factors contribute to overfeeding in the ICU, including inaccurate estimation of energy requirements, formulaic feeding protocols, and failure to adjust nutritional support based on individual patient needs. Prolonged overfeeding can lead to insulin resist

Roles, responsibilities, and opportunities for the nutrition support physician working with adults: A critical review.

The dual burden of undernutrition and overnutrition requires a greater understanding and commitment to physician training and specialization in nutrition support. Despite the significant healthcare need and existence of organizations focused on nutrition support such as the American Society for Parenteral and Enteral Nutrition (ASPEN), physician involvement in the nutrition care of high-risk patients remains low. In an ASPEN survey of members, inadequate training among physicians was reported along with significant barriers to involvement in nutrition support, such as insufficient protected ti

Low-methoxy pectin-containing enteral nutrition in critical care for intestinal tolerance (LOME-PECT): Study protocol for a randomized controlled trial.

BACKGROUND: Enteral nutrition is preferable over parenteral nutrition for critically ill patients, but is often discontinued due to enteral feeding intolerance. Diarrhea is one of the most common causes of the discontinuation of enteral nutrition and may be attributed to the composition of enteral formulas. Dietary fiber attenuates diarrhea by normalizing the intestinal microbiota, providing energy for colonic epithelial cells, and exerting a thickening effect on intestinal contents. We herein conducted a randomized controlled trial (RCT) to test the hypothesis that the administration of an en

In Vitro Proof-of-Concept Study: Lidocaine and Epinephrine Co-Loaded in a Mucoadhesive Liquid Crystal Precursor System for Topical Oral Anesthesia.

Background: Local anesthesia is essential for most dental procedures, but its parenteral administration is often painful. Topical anesthetics are commonly used to minimize local anesthesia pain; however, commercial formulations fail to fully prevent the discomfort of local anesthetic injection. Methods: We developed and characterized a novel lidocaine and epinephrine co-loaded liquid crystalline precursor system (LCPS) for topical anesthesia. The formulation was structurally characterized using polarized light microscopy (PLM) and small-angle X-ray scattering (SAXS). Rheological behavior was a

Ketamine-Occasioned Mystical Experience in Veterans with Treatment-Resistant Depression: A Retrospective Exploratory Analysis.

INTRODUCTIONS: Mystical experiences are powerful experiences that may have therapeutic value. Ketamine, a dissociative anesthetic, has shown potential to induce mystical experiences. However, little research has explored this phenomenon, particularly in treatment-resistant depression. METHODS: We analyzed clinical data from 60 veterans with treatment-resistant depression receiving a total of 189 ketamine treatments. Veterans either received intranasal esketamine or racemic parenteral ketamine (intravenous or intramuscular). The Revised Mystical Experience Questionnaire (MEQ-30) was administer

The Effectiveness of Intranasal Ketamine on Intramuscular Ketamine Injection Pain Among Children in the Emergency Department, a Randomized, Controlled Trial.

OBJECTIVE: Children in the emergency department (ED) experience distress due to many factors, such as pain. Although ketamine has analgesic and sedative effects, its parenteral administration causes pain and anxiety. It is notable that ketamine can be administered intranasally for analgesia. We hypothesized that the administration of intranasal (IN) ketamine for analgesia before intramuscular ketamine injection would diminish the injection pain without any adverse effect. METHODS: This study was a randomized, double-blind, placebo-controlled clinical trial conducted in the ED. The IN ketamine

Injectable In Situ Thermoreversible Gel Depot System of Lidocaine Nanoemulsion for Prolonged Anesthetic Activity in Dental and Operative Procedures.

Background/Objectives: Lidocaine hydrochloride (LD-HCl) is the most commonly used local anesthetic in dentistry, often administered with epinephrine to extend its duration and reduce systemic absorption. However, its relatively short duration of action, the need for repeated injections, and the unpleasant taste may limit patient compliance and procedural efficiency. This study aimed to develop and evaluate a novel injectable nanoemulsion-based in situ gel depot system of LD to provide prolonged anesthetic activity. Methods: LD-loaded nanoemulsions were formulated by high-shear homogenization f

Routes of postoperative analgesia administration in surgical wards: practice vs. guidelines and economic implications.

INTRODUCTION: Multimodal analgesia, critical to postoperative recovery, typically involves oral (PO), intravenous (IV), or regional analgesia. The PO route is preferred for its non-invasive nature, cost-effectiveness, and comparable efficacy to parenteral routes. This study assessed whether analgesic practices in surgical wards align with current guidelines and evaluated the economic impact of deviations. MATERIALS AND METHODS: Data from 2,354 adult patients, collected using the PAIN OUT methodology across 10 Serbian hospitals, were analyzed. Patients undergoing various surgeries were observe

Does parenteral Omega-3 fatty acid administration increase the risk of atrial fibrillation? An analysis of the current evidence.

BACKGROUND & AIMS: In 2023, the Pharmacovigilance Risk Assessment Committee (PRAC) of the European Medicines Agency (EMA) issued new safety information on oral fish oil (FO) pharmacotherapy. This information indicated a dose-related increased risk of atrial fibrillation (AF) with FO in patients with established cardiovascular disease (CVD). The aim of this study is to analyse the existing evidence on this risk and establish whether it can be extrapolated to FO-enriched intravenous lipid emulsions (FO-ILEs) or to other patient groups, such as critically ill patients with organ dysfunction.
