How I Do It: Vascular Reconstruction of Ileo-Jejunal Venous Tributaries in Setting of Caudal Involvement of the Superior Mesenteric Vein by Pancreatic Tumor.

This article presents a technical description of a vascular reconstruction involving two distinct second order SMV tributaries, developed to manage complex caudal SMV involvement during pancreatic surgery. The approach includes the use of a temporary meso-caval shunt and an interposition graft to enable safe resection. High-quality anatomical illustrations and a literature-based discussion support the rationale and reproducibility of the technique.

Effects of lyophilized Lactiplantibacillus plantarum DC2 probiotic powder on gastrointestinal morphology and microbiota in mice.

Probiotics have potential benefits in modulating gastrointestinal function. However, their efficacy and application depend largely on specific bacterial strains and environmental contexts. In our previous study, a strain of Lactiplantibacillus plantarum (L. plantarum) DC2 exhibited superior functionality in pork fermentation processes and was granted a Chinese patent (ZL202110743486.8). However, its storage and application characteristics remain unclear. To evaluate its direct application potential for gastrointestinal regulation, in this study, the formula and process of lyophilized L. planta

Proximal jejunal perforation by an ingested toothpick: a rare case report.

INTRODUCTION: Toothpick ingestion is a rare event that may cause gastrointestinal perforation, abscess, or peritonitis. Jejunal involvement is exceptionally uncommon. CASE PRESENTATION: A 45-year-old man, previously operated on for appendicular peritonitis, presented with generalized abdominal pain and vomiting. A CT scan revealed pneumoperitoneum with a linear foreign body perforating the proximal jejunum. Laparoscopy showed diffuse peritonitis and a toothpick perforating the antimesenteric border 70 cm from the duodenojejunal flexure. Conversion to laparotomy was required because of adhesio

Self-enhanced PepT1-mediated transport of egg white-derived tripeptide FGR in apical-out mouse jejunal organoids.

Our previous study has verified efficient PepT1-mediated transport of the egg white-derived tripeptide FGR (Phe-Gly-Arg), but its transport mechanism under physiological intestinal conditions remains elusive and is difficult to fully recapitulate using conventional culture models. This study established an apical-out mouse jejunal organoid model combined with Astral-DIA quantitative proteomics to investigate the molecular mechanisms and regulatory factors underlying egg white-derived tripeptide FGR transport via PepT1. Immunofluorescence confirmed that the model achieved successful polarity re

Pelvic splenosis and endometriosis in adolescent with hereditary spherocytosis.

The patient is a 16-year-old with a history of hereditary spherocytosis. At age 12, she underwent a laparoscopic partial converted to total splenectomy for intraoperative bleeding. The spleen was placed in a retrieval bag and removed via morcellation. She first reported abdominal pain at age 8, with dysmenorrhea after menarche at age 13. Her dysmenorrhea was managed with a progesterone IUD. Due to worsening pain, she underwent MRI which showed multiple lobulated structures throughout the pelvis and ventral abdomen, suggestive of endometriomas versus splenules. Laparoscopy revealed dozens of bl

Dietary phosphorus source reprograms the kidney-intestine axis to dictate calcium-phosphorus efficiency and egg production in aging hens.

BACKGROUND: The decline in egg production in aging hens poses a significant challenge to sustainable poultry production. Given that evidence suggests the phosphorus source affects late‑lay performance, we tested the hypothesis that the dietary phosphorus source is a decisive regulator sustaining performance by reprogramming systemic mineral homeostasis. Using a systems biology approach, we analyzed the coordinated expression of eleven key mineral transporter genes across the duodenum, jejunum, ileum, and kidney of hens fed dicalcium phosphate (DCP) or tricalcium phosphate (TCP). RESULTS: Duri

A tributyrin-monolaurin complex protects piglets against PEDV infection via viral inhibition, lipid metabolism restoration, and microbiota modulation.

Porcine epidemic diarrhea virus (PEDV) is a major pathogen causing severe diarrhea and intestinal damage in piglets. Monolaurin (GML) and tributyrin (TB) are well-documented functional lipids with intestinal health benefits, but their combined efficacy against PEDV infection remains unclear. The present study first investigated the anti-PEDV effect of GML, TB, and their mixtures using PEDV-infected Vero cells. Then, the effects of a microencapsulated TB-GML complex (PE) were evaluated in PEDV-infected piglets, focusing on its protective mechanisms. Vero cells were simultaneously treated with P

Genetic resistance confers protection against clinical severity, intestinal colonization, and mortality in weaned pigs experimentally challenged with pathogenic F18 and F4 InPEC Escherichia coli.

Post-weaning colibacillosis (PWC) is a leading cause of morbidity and mortality in nursery pigs, affecting a large proportion of individuals on affected farms, caused by intestinal pathogenic Escherichia coli (E. coli) (InPEC). Its control requires comprehensive control strategies, including measures to address multidrug-resistant E. coli strains and reduced susceptibility to disinfectants. This study assessed the clinical outcome in two pig lines with distinct genetic susceptibility to enterotoxigenic E. coli: a resistant line (Genotype A) and a susceptible line (Genotype B), following experi

Unicentric hyaline-vascular Castleman disease of the jejunal mesentery mimicking a gastrointestinal stromal tumor or neuroendocrine tumor: a case report.

BACKGROUND: Castleman disease (CD) is a rare lymphoproliferative disorder classified as unicentric (UCD) or multicentric (MCD). The hyaline vascular variant of UCD most commonly arises in the mediastinum, whereas mesenteric involvement is exceedingly rare. As it can mimic hypervascular mesenteric tumors like neuroendocrine tumors (NETs) and gastrointestinal stromal tumors (GISTs), preoperative diagnosis remains challenging. CASE PRESENTATION: A 36-year-old woman presented with a 3-month history of vague abdominal discomfort, early satiety, and an unintentional 9-kg weight loss. Contrast-enhan

Duodenal atresia associated with apple-peel configuration of distal small bowel in a late preterm neonate: a rare case report and literature review.

INTRODUCTION: The association of duodenal atresia with apple-peel (Christmas tree) configuration of the distal small bowel is exceptionally rare, with only a few cases reported in the literature. This unusual combination is of particular interest because it may provide evidence supporting a vascular etiology for selected cases of duodenal atresia. We report a neonate with type III duodenal atresia associated with apple-peel bowel and absent proximal superior mesenteric artery anatomy. CASE PRESENTATION: A late-preterm (35 + 5 weeks) female neonate with antenatal suspicion of jejunal atresia p

Strategic wound and nutritional management for short bowel syndrome and wound dehiscence following non-occlusive mesenteric ischemia: a case report.

INTRODUCTION AND IMPORTANCE: Non-occlusive mesenteric ischemia (NOMI) is life-threatening and may require extensive bowel resection, resulting in short bowel syndrome (SBS), nutritional deficiencies, and impaired wound healing. Severe wound dehiscence further complicates postoperative management, and reports describing this sequence are limited. This report highlights coordinated, phase-oriented management in an immunocompromised patient. PRESENTATION OF CASE: An 81-year-old female receiving immunosuppressive therapy underwent emergency Hartmann's procedure for perforated sigmoid diverticulit

Jejunal mesenteric cyst presenting as acute midgut volvulus in a child: a rare cause of surgical abdomen.

INTRODUCTION: Mesenteric cysts are defined as benign intra-abdominal tumors located in the mesentery. Mesenteric cysts are rare intra-abdominal lesions in children and are often asymptomatic. Acute midgut volvulus secondary to a mesenteric cyst is an exceptionally uncommon presentation that requires prompt surgical intervention to prevent bowel ischemia. CASE PRESENTATION: An 8-year-old boy was referred from the pediatric department with acute abdominal pain and recurrent bilious vomiting. Clinical examination revealed around 20 × 20 cm size cystic lump involving whole abdomen. Contrast-enhan

Ultra-short bowel syndrome after near-total small bowel resection for gangrenous gut due to superior mesenteric artery thrombosis: a case report.

Ultra-short bowel syndrome (USBS) is a rare and life-threatening condition defined by the retention of ˂30 cm of small intestine following resection. We report the case of a 53-year-old male who presented with gangrenous gut secondary to superior mesenteric artery thrombosis, necessitating near-total small bowel resection. The patient was left with ~15 cm of proximal jejunum and 10 cm of distal ileum, with a jejuno-ileal end-to-end anastomosis. He developed severe postoperative watery diarrhoea and required Total Parenteral Nutrition (TPN) via a peripherally inserted central catheter line. Wit

Glutamate metabolism reprogramming is involved in the inhibitory effect of ellagic acid on rotavirus replication.

INTRODUCTION: Rotavirus (RV) is an important zoonotic pathogen that causes acute diarrhea in infants and young animals. There are several limitations in the prevention and control strategies currently, which result in an urgent need to develop innovative natural active substances against RV. This study aims to explore the RV infection preventive mechanism of ellagic acid (EA) and determine whether it exerts its effect by regulating the glutamine-glutamate metabolic axis. METHODS: RV-infected Balb/c mice and IEC-6 cell models were established to conduct mechanistic research by employing untarg

Jejunal Leiomyoma Arising in a Diverticulum Mimicking a Hollow Viscus Perforation: A Diagnostic Challenge.

Small intestinal leiomyomas are rare benign smooth muscle tumors that are now infrequently encountered following the reclassification of many mesenchymal tumors as gastrointestinal stromal tumors (GISTs). They are usually asymptomatic or present with nonspecific symptoms such as abdominal pain or gastrointestinal bleeding. Acute presentation with perforation peritonitis is exceedingly rare and poses a diagnostic challenge. We report the case of a 50-year-old female who presented with fever, vomiting, and acute abdominal pain with features of localized peritonitis. Contrast-enhanced computed to

A Bidirectional Barbed Suture Technique for Secure Entry-hole Closure in Robotic Overlap Esophagojejunostomy: Initial Experience.

BACKGROUND: Although minimally invasive total and proximal gastrectomy have been increasingly adopted for gastric cancer, anastomotic complications, particularly at the esophagojejunal anastomosis, remain more frequent than after distal gastrectomy. Anastomotic leakage or stenosis can delay postoperative recovery and the initiation of adjuvant chemotherapy, thereby potentially affecting oncologic outcomes. Reliable entry-hole closure during overlap esophagojejunostomy remains technically challenging, especially when luminal diameter mismatch exists between the esophagus and jejunum. METHODS:

Lengthening Bowel Endoscopically: Hydrogel-Enabled Endoscopic Spring-Mediated Distraction Enterogenesis.

We evaluated the feasibility to lengthen small bowel endoscopically utilizing a spring device constrained by an engineered, slow biodegrading, hyaluronan and elastin-like protein (HELP) hydrogel as a potential treatment for short bowel syndrome. While we previously demonstrated that the HELP hydrogel could provide device confinement and enable non-invasive device removal, here we demonstrate that the HELP hydrogel can also be designed to be catheter-injectable while still being stiff enough to achieve device confinement. Six pigs underwent endoscopic spring-mediated distraction enterogenesis (

miR-206 regulates copper-induced pyroptosis in porcine jejunal epithelial cells by targeting Caspase-1.

Copper (Cu) is an essential trace element in animal organisms, participating in numerous vital processes. However, excessive intake can cause poisonous damage to organs such as the intestines. This study aims to elucidate the role of miR-206 in copper-induced pyroptosis in porcine jejunal epithelial cells and its molecular mechanism. In this study, we found that high-dose copper exposure induces pyroptosis in jejunal epithelial cells, while miR-206 expression was significantly reduced to 0.60 and 0.64 times that of the control group in vivo (Fed a diet containing 250 mg/kg of copper) and in vi

Combined glutamine, dietary fiber, and oligosaccharide supplementation attenuates fasting-associated intestinal dysbiosis and stress responses in rats.

BACKGROUND: Prolonged preoperative fasting is associated with intestinal dysbiosis, impaired mucosal homeostasis, and delayed postoperative recovery. Combined nutritional supplementation with glutamine, dietary fiber, and oligosaccharides has the potential to protect intestinal function during nutritional stress. In this study, we investigated the effects of this nutritional combination in a rat model of prolonged fasting. METHODS: Male Sprague-Dawley rats received oral supplementation with a combination of glutamine, dietary fiber, and oligosaccharides (GFO) or a 15% glucose solution twice d

Desmoid-Type Fibromatosis of the Proximal Jejunum Mimicking Gastric Gastrointestinal Stromal Tumor: Diagnostic Pitfalls in Determining the Tumor Origin.

INTRODUCTION: Desmoid-type fibromatosis (DTF) of the small intestine is rare and often mimics gastrointestinal stromal tumor (GIST). Its infiltrative growth can also make identification of the true organ of origin difficult. CASE PRESENTATION: A 56-year-old woman was referred with a suspected gastric GIST based on imaging and endoscopic US-guided fine-needle aspiration. Before the planned surgery, the patient developed anorexia and vomiting. Emergency CT revealed rapid tumor enlargement with duodenal obstruction. Urgent multi-organ resection was performed, including partial gastrectomy, jejun
