Tunnel Anastomosis vs Double-Tract Jejunal Interposition Reconstruction After Proximal Gastrectomy: Protocol for a Multicenter Prospective Randomized Controlled Trial.

BACKGROUND: Tunnel anastomosis is a novel anastomotic technique for digestive tract reconstruction following proximal gastrectomy. A previous retrospective study by our team demonstrated its favorable antireflux effect; therefore, we hypothesize that tunnel anastomosis is noninferior to double-tract jejunal interposition reconstruction in preventing postoperative reflux esophagitis, and we will conduct this prospective study to further validate this assumption. OBJECTIVE: In this study, we will prospectively compare tunnel anastomosis with the currently more prevalent double-tract jejunal int

Composite Probiotic Fermented Feed Enhances Growth Performance and Intestinal Health in Weaned Piglets by Modulating the Gut Microbiome and Metabolome.

This study investigated the effects of compound microbial fermented feed on the growth performance, intestinal architecture, microbiota composition, and metabolic profiles of weaned piglets. Fifty-four weaned piglets were randomly allocated to three dietary treatment groups: a control group (basal diet), a 50% fermented feed group (T1), and a 100% fermented feed group (T2), for a 33-day feeding period. The results indicated that both T1 and T2 diets significantly improved final body weight and average daily gain (ADG), while decreasing the feed-to-gain ratio (F/G) compared with the control (p

Effects of conventional and raised without antibiotic feeding systems and exposure to infectious bursal disease virus on microbial diversity of the jejunal microbiota in broiler chickens.

Preventative use of antimicrobials in the feed in broiler chicken production is decreasing due to consumer demand. Hence broiler chickens raised without antibiotics (RWA) receive increased attention. The objective of this study was to compare jejunal microbiota in RWA and conventional feeding systems in commercial broiler chickens. A total of 6 broiler chicken farms were selected for this study, in each farm raising both conventional and RWA chicken flocks. Jejunal contents were collected from Ross 308 (n=8/flock) at 25 d of age for metagenomics analysis for 16SrRNA amplicon sequencing. Serum

Unusual Gastrointestinal Bleeding due to an Aberrant Artery Neighboring a Jejunal Gastrointestinal Stromal Tumor.

Tumor rupture and/or ulceration of the intestinal mucosa are well-known causes of gastrointestinal stromal tumor (GIST) hemorrhage; however, bleeding from an aberrant artery is exceedingly rare. An elderly female presented with profound anemia (nadir Hgb 4.8 g/dL) and ongoing melena over the past week. Esophagogastoduodenoscopy, colonoscopy, and conventional angiography were negative. Tagged red blood cell scintigraphy localized the bleed to the proximal jejunum prompting diagnostic bedside push enteroscopy. Intraoperative examination revealed diverticular bleeding from a large-caliber aberran

Dietary energy alters jejunal microbial function without changing its structure in small-tailed Han sheep.

Dietary energy levels typically influence the structure and functional profile of the gastrointestinal microbial community. In this study, thirty 6-month-old Small-tailed Han (STH) sheep were randomly divided into three groups and fed corn-based diets with different energy levels for 150 days. Jejunal contents were then collected and analyzed using metagenomic sequencing to assess microbial alpha diversity and taxonomic composition. Functional annotation and enrichment analysis were performed using the KEGG database. Principal coordinate analysis (PCoA) and alpha diversity indices (Chao1, Shan

Combined supplementation of lysophospholipids, synthetic emulsifier and monoglycerides alleviates the adverse effects of energy- and amino acid-deficient diets on growth performance, nutrient utilization, and intestinal health in broilers.

Improving nutrient utilization in nutrient-restricted diets is an important strategy to reduce feed costs and maintain productivity in broiler production. Dietary emulsifiers are commonly used to improve lipid digestion and overall nutrient utilization in poultry diets. The objective of this study was to investigate the effects of a combination of lysophospholipids, synthetic emulsifier and monoglycerides (LEX) in energy- and amino acid-deficient broiler diets on growth performance, carcass traits, nutrient digestibility, and intestinal morphology from d 11 to 35. Gut microbiota-driven IL-17/PPAR axis mediates epigallocatechin-induced intestinal repair in weaned lambs.

BACKGROUND: Early weaning is a key strategy to improve lamb production efficiency; however, it inevitably compromises intestinal barrier integrity and function. This study aimed to investigate the effects of epigallocatechin (EGC) on growth performance and intestinal barrier function in weaned lambs, using metagenomics, metabolomics, and intestinal transcriptomics to elucidate the underlying mechanisms. RESULTS: Weaning induced oxidative stress, inflammation, and metabolic disruptions in the jejunum. Supplementation with 12.5 mg/kg EGC (LE) significantly improved growth performance, reduced d

Jejunal adenocarcinoma with isolated mesorectal metastasis: a case report of a rare entity.

Small bowel (SB) neoplasms are rare and often diagnosed at advanced stages, commonly presenting with obstruction or obscure gastrointestinal bleeding (OGB). An 83-year-old man presented with anemia and hematochezia. Imaging revealed an 8 cm SB mass and a 5 cm rectal lesion initially suggestive of GIST. Biopsy confirmed adenocarcinoma. Due to recurrent bleeding, the patient underwent segmental enterectomy and Hartmann's procedure. Histology showed jejunal mucinous adenocarcinoma with mesorectal metastasis (pT4N0M1). Despite adjuvant chemotherapy and hemostatic radiotherapy for pelvic recurrence

Enterotoxigenic Escherichia coli-induced intestinal epithelial necroptosis drives lamina propria immune cell pyroptosis and mucosal injury in piglets.

Necroptosis is an inflammatory programmed cell death pathway linked to diverse physiological and pathological disorders, yet its role in Enterotoxigenic Escherichia coli (ETEC)-induced intestinal inflammation and mucosal injury remains poorly understood. This study aimed to elucidate the contribution of intestinal epithelial cell necroptosis to the development of intestinal inflammation and injury induced by ETEC infection in piglets. Following ETEC challenge in piglets, key proteins involved in necroptosis, including phosphorylated receptor-interacting protein kinase 3 (p-RIP3) and high-mobil

JAK inhibitors alleviate EGFR-inhibitor-induced diarrhea by protecting intestinal stem cells from adaptive-immune-exacerbated injury.

While intestinal stem cells (ISC) are essential for epithelial homeostasis, their dynamic regulation during immune-mediated injury remains undefined. Here we show that suppression of jejunal ISC proliferation contributes to pathology arising from oral EGFR (epidermal growth factor receptor) tyrosine kinase inhibitor (TKI) treatment. Suppression of adaptive immunity via genetic intervention reverses ISC suppression and accelerates mucosal repair via inhibiting the TKI-induced, chemokine-directed migration of T and B lymphocytes from Peyer's patches. Spatial transcriptomics reveals enhanced cros

Surgical approaches to esophageal cancer gastrointestinal reconstruction: a systematic review of jejunum, colon, and ileocolonic interposition with comparative outcome analysis.

Esophageal cancer is a highly aggressive malignancy of the digestive tract, with surgical resection remaining the cornerstone of curative treatment. For patients with an intact stomach, reconstruction using a gastric conduit via gastric pull-up is the preferred approach following esophagectomy. However, in cases where the stomach is unavailable—due to prior gastric surgery, polyps, or concurrent tumors—alternative conduits must be considered. By reviewing relevant domestic and international research literature, this review evaluates the indications, benefits, limitations, and common complicati

Spatially and temporally comparative proteomics provide insights into dynamic response patterns to PEDV infection.

Characterizing host response patterns to porcine epidemic diarrhea virus (PEDV) infection at the protein level is essential for capturing rapid-response events and elucidating the pathogenesis mechanisms of porcine epidemic diarrhea (PED). However, the specificity and dynamicity of the tissue-based proteome in the localized region of infected intestines remain elusive. In this study, we conducted a spatiotemporal comparative proteomics to quantify cellular proteins in jejunal and colon tissues throughout the stages of PEDV infection. By integrating multidimensional host proteomics, we obtained

Perforated Jejunal Pseudodiverticulum Managed With Limited Segmental Resection: A Case Report.

Jejunal diverticulitis (JD) is a rare disease with significant clinical relevance, as it often causes intense pain in the epigastrium that radiates to the left flank, accompanied by hemodynamic instability and leukocytosis. Diagnosis must be complemented with clinical and imaging studies.  A 72-year-old woman presented with abdominal pain of colic type in the epigastrium and left flank for 12 hours. The computed tomography scan showed two diverticula in the jejunum, the first with localized pneumoperitoneum and signs of perforation, the second with no signs of an inflammatory process. An explo

BMAL1-mediated transcriptional regulation of CYP3A13 drives circadian rhythms in intestinal first-pass metabolism.

Intestinal first-pass metabolism serves as the primary metabolic barrier during the absorption of orally administered drugs, in which cytochrome P450 (CYP) enzymes (particularly the CYP3A subfamily) play a critical role. This study demonstrates that the CYP3A family is highly expressed in the jejunum, with CYP3A13 as the predominant isoform. Its expression and metabolic activity exhibit a robust circadian rhythm with significantly higher expression during the night than during the day, which directly influences the exposure rhythm of the orally administered drug verapamil. Furthermore, through

The relevance of starch and protein digestive dynamics on broiler performance: A Meta-analysis.

Although nutrient digestibility (dig) is traditionally assessed in the distal ileum, the majority of starch and protein digestion and absorption occurs in the jejunum. Increasing attention has been directed toward the synchronization of starch and protein and their impact on growth performance and feed efficiency. The objective of this meta-analysis is to validate the relevance of starch and protein digestive dynamics on growth performance and to evaluate the validity of jejunal dig of nutrients as the indicator for digestion rate. A total of 260 data points extracted from 32 published papers

Gut site and sex-specific enrichment of bacterial taxa and predicted metabolic pathways in wild American black bear (Ursus americanus).

American black bears' (Ursus americanus) omnivorous feeding strategy, simple gut morphology, and rapid transit time prevent regulation of the gut microbiome (GMB). We analyzed stable isotopes and 16S rRNA sequences from 48 wild bears to assess the impacts of diet, age, gut site, and sex on GMB composition and PICRUSt2-predicted functional pathways. While alpha and beta diversity did not differ, we identified bacterial taxa and predicted pathways enriched based on gut site and sex. Enterococcus, Incertae, Papillibacter, and Shuttleworthia were enriched in jejunum samples (linear discriminant an
