Impact of antidementia medicines on the dispensing of antidepressants, anxiolytics and antipsychotics.

Behavioural and psychological disturbances are common among individuals with dementia. It is unclear if the use of antidementia medicines reduces their presence. This study aimed to clarify if the introduction of antidementia medicines leads to a decline in the dispensing of antidepressants, anxiolytics and antipsychotics. We used time-series analyses to examine the association between the supply of cholinesterase inhibitors or memantine and psychotropics. Participants were males and females aged 60 years or over included in the Australian 10 % Pharmaceutical Benefits Scheme database between 2

"Dream Insertion": A Case Report of a Novel Psychotic Symptom in a Male Inpatient With Treatment Resistant Schizophrenia.

BACKGROUND: Individuals with schizophrenia appear to experience differences in dreaming compared to control populations-the small amounts of research that does exist demonstrates abnormalities such as simpler dreams that are more bizarre and with greater negative content than those of controls. Dream-related psychopathology remains insufficiently described however-particularly experiences in which patients believe their dreams are externally generated or controlled. Such phenomena extend classically described first rank symptoms-such as thought insertion and passivity phenomena-into the domain

First-generation antipsychotic shortages in the United States: Utilization and switching patterns.

BACKGROUND: The United States Food and Drug Administration (FDA) has declared an increasing number of psychotropic shortages, which may adversely affect health outcomes by forcing psychotropic switches or rationing. There is a paucity of literature describing the impact of psychotropic shortages. OBJECTIVE: To describe antipsychotic utilization patterns following FDA-declared antipsychotic shortages or discontinuations. METHODS: This was a descriptive, pre-post analysis of the Komodo Healthcare Map® between January 2016 and September 2023. Individuals were included if they had a psychotic sp

Quetiapine modestly improves sleep and breathing but impairs next-day performance in people with obstructive sleep apnea and difficulty maintaining sleep: a randomized controlled trial.

RATIONALE: Quetiapine is commonly prescribed "off-label" to people with insomnia symptoms. People with undiagnosed obstructive sleep apnea (OSA) frequently report insomnia symptoms, particularly difficulty maintaining sleep. In 2023, 10.7 million prescriptions were dispensed for quetiapine in the United States. Yet, there is limited information regarding the effects of quetiapine on sleep, breathing, and next-day performance in people with OSA. OBJECTIVES: To determine the effects of 50 mg of quetiapine versus placebo on OSA severity and other polysomnographic parameters plus next-day vigilan

Clozapine-induced Myocarditis in Korea: Analysis of a Nationwide Database and Comparison to Other Countries.

OBJECTIVE: Clozapine, though the mainstay treatment for treatment-resistant schizophrenia, is associated with potentially fatal adverse reactions, including myocarditis. The incidence of clozapine-induced myocarditis (CIM) demonstrates geographic and ethnic variability; however, reports from Asia remain scarce. This study aimed to provide a comprehensive overview of CIM in Korea and compare findings with reports from other countries. METHODS: CIM cases were identified from Korean National Health Insurance Service (NHIS) database based on diagnostic codes, prescription timelines, and clinical

Adherence to ECG Monitoring Before Initiating Antipsychotics in Psychiatric Inpatients at Sultan Qaboos University Hospital: A Clinical Audit.

BACKGROUND: Antipsychotic medications are associated with QT interval prolongation and an increased risk of serious arrhythmias. Clinical guidelines, therefore, recommend baseline electrocardiogram (ECG) monitoring prior to initiation. This audit evaluated the compliance of baseline ECG monitoring before the initiation of antipsychotic medications in psychiatric inpatients at Sultan Qaboos University Hospital (SQUH) and explored the association of demographic and clinical factors. METHODS: A retrospective review was conducted of all psychiatric inpatients who were started on antipsychotics fr

Exploring Combination Therapy for the Management of Psychotic Disorders: A Comprehensive Review.

Schizophrenia is a severe brain disorder impacting approximately 1% of the overall world population. A large number of FDA-approved antipsychotics are available in the market for the management of schizophrenia and other related disorders. However, 20-30% of patients exhibit treatment resistance, known as pharmacoresistant or refractory schizophrenia, where monotherapy proves to be ineffective. This review describes the various prevalent psychotic disorders, the antipsychotic mechanism of action, and drug resistance in monotherapy. The review highlights the significance of combination drug the

Glucagonlike Peptide-1 (GLP-1) Receptor Agonists for Cardiometabolic Risk in Severe Mental Illness: A Narrative Review.

People with severe mental illness face a substantially increased cardiometabolic risk and premature mortality, largely driven by antipsychotic-associated metabolic disturbances, including significant weight gain, insulin resistance, dyslipidemia, impaired glucose homeostasis, and increased rates of metabolic syndrome. Despite existing recommendations for monitoring and early intervention, managing this risk remains a significant clinical challenge. Glucagonlike peptide-1 receptor agonists have emerged as a potential adjunctive therapeutic strategy in this context. This narrative review evaluat

Breastfeeding and Neurodevelopmental Outcomes in Children of Women Taking Antiseizure Medications.

IMPORTANCE: Concerns regarding infant exposure to antiseizure medications (ASMs) via breast milk frequently discourage breastfeeding in women with epilepsy (WWE). Longitudinal neurodevelopmental data for breastfeeding while taking contemporary ASM regimens are essential for evidence-based management. OBJECTIVE: To evaluate the association between breastfeeding duration and neurodevelopmental outcomes at age 6 years in children exposed to contemporary ASM regimens. DESIGN, SETTING, AND PARTICIPANTS: The Maternal Outcomes and Neurodevelopmental Effects of Antiepileptic Drugs study is a prospec

Trifluoperazine reduces infarct size, restores neurovascular coupling and improves early outcomes in experimental acute ischemic stroke.

Cerebral edema and neurovascular dysfunction are reliable predictors of outcome after acute ischemic stroke (AIS), yet effective targeted therapies remain limited. We investigated the therapeutic potential of trifluoperazine (TFP), an FDA-approved antipsychotic and calmodulin inhibitor that modulates astrocytic aquaporin-4 expression. TFP administered after recanalization in a mouse model of AIS reduced infarct volume and improved early neurological recovery. Importantly, TFP restored neurovascular coupling and enhanced cerebral blood flow responses to spreading depolarizations, indicating imp

Projected mortality impact of increasing second-generation long-acting injectable antipsychotic exposure in Italy: A scenario-based population model.

BACKGROUND: In Italy, 165,846 adults receive specialist mental-health care for schizophrenia and other functional psychoses. People with schizophrenia have excess mortality, and long-acting injectable antipsychotics (LAIs) may reduce treatment discontinuity; however, the population impact of increasing second-generation LAI (SGA-LAI) exposure is unknown. METHODS: We developed a scenario-based closed-cohort model using the 2024 Italian Mental Health Information System denominator, SISM age-specific treated rates, and ISTAT 2024 age- and sex-specific mortality rates. Schizophrenia excess mortal

Neuropsychiatric Effects of Glucagon-Like Peptide-1 Receptor Agonists in Schizophrenia-Spectrum Disorders: A Systematic Review.

People with schizophrenia-spectrum disorders die 15-20 years prematurely, predominantly from cardiovascular disease, and antipsychotic-induced weight gain is a major, partly iatrogenic contributor to this excess mortality. Glucagon-like peptide-1 receptor agonists (GLP-1RAs) are potent weight-lowering agents that are increasingly used as metabolic adjuncts in this population, and preclinical evidence has suggested that they may also exert central, potentially pro-cognitive effects. However, their effects on psychiatric symptoms, cognition, and quality of life have not previously been systemati

Brewing Anxiety: Caffeine-Induced Thigmotaxis in Zebrafish Larvae.

BACKGROUND: Thigmotaxis, or edge preference, is widely used as a measure of anxiety in vertebrate model systems. However, variability in this behavior limits its utility as a scalable measure for anxiolytic drug discovery. In the current study, we examine caffeine-induced thigmotaxis, using 5-day-old zebrafish larvae as a model system. METHOD: We modified our imaging system and AI-assisted analysis for measurements of behavior in six-well plates. The imaging system was used to measure thigmotaxis and several other behaviors, including overall activity, scoot and burst swimming, and responses

Patients With Atypical Symptoms of Depression Respond Better to Lithium Augmentation Than Patients Without Atypical Symptoms.

INTRODUCTION: Understanding the differences in treatment response between subtypes of major depressive disorder (MDD) may help to prevent lengthy trial-and-error processes by identifying effective treatment options. The aim of the study was to investigate the impact of atypical features of MDD (MDD-A) on the response to lithium augmentation (LA). METHODS: In a multicenter prospective cohort study, response to LA was compared between patients with atypical symptoms of MDD (MDD-A, n = 24) versus patients without atypical symptoms (MDD-nA; n = 94) using a Cox regression analysis. RESULTS: A tot

Sex differences in the efficacy, acceptability, and tolerability of antipsychotics in people with acute schizophrenia: individual participant data pairwise, dose-response, and network meta-analyses.

BACKGROUND: Antipsychotics are the mainstay of schizophrenia treatment, but their efficacy and tolerability might differ between females and males, and previous evidence on sex differences has been inconsistent and methodologically limited. We aimed to evaluate sex differences in the efficacy, acceptability, and tolerability of antipsychotics in adults with acute schizophrenia. METHODS: We performed individual participant data pairwise, dose-response, and network meta-analyses of antipsychotics versus placebo or another antipsychotic, using data from the Vivli and YODA trial data-sharing plat
