Why did antidepressant deprescribing fail? A scoping review of contributing factors.

BACKGROUND: Antidepressant use continues to rise in primary care, often beyond recommended durations. Previous reviews have found insufficient evidence to support long-term antidepressant discontinuation compared with maintenance treatment. OBJECTIVE: To describe how antidepressant deprescribing interventions have been designed in primary care and identify features that explore potential reasons for limited effectiveness. DESIGN: Scoping review following Preferred Reporting Items for Systematic Reviews and Meta-Analyses extension for scoping reviews (PRISMA-ScR) guidance. DATA SOURCES: PubM

Targeting neurovascular dysfunction in diabetic neuropathic pain: a review of mechanism-based pharmacotherapy beyond gabapentinoids.

BACKGROUND: Diabetic neuropathic pain (DNP) is a condition that affects about 20-30% of people with diabetes, significantly reducing health-related quality of life and having a significant economic impact on healthcare economics. Although conventional "gold standard" treatments such as gabapentinoids, serotonin-norepinephrine reuptake inhibitors (SNRIs), and tricyclic antidepressants (TCAs) are widely used in the clinic, a substantial number of patients have poor results with these medications in terms of the quality of their analgesia or their systemic side effects. OBJECTIVE: In order to ov

Kynurenine pathway profiles as markers of ketamine response in treatment-resistant depression.

Treatment-resistant depression (TRD) remains a major clinical challenge, with highly variable responses to ketamine. Immune activation characterizes a subset of patients with TRD. The kynurenine pathway (KP), which integrates immunometabolic signals with glutamatergic neurotransmission, may provide candidate biomarkers to inform personalized treatment. We investigated whether KP metabolites and ratios are associated with ketamine response and clinical outcomes over time. Thirty adults with TRD received six intravenous ketamine infusions over three weeks. At baseline (D1), after the first infus

The need for another tool: Australian healthcare professionals on the use of psilocybin for existential distress in people with cancer.

BACKGROUND: Existential distress is commonly experienced by people with cancer, yet there are limited treatment options. The therapeutic potential for psychedelic-assisted therapy (PAT) utilising psilocybin for this cohort is underexplored, with emerging literature showing clinical improvements in wellbeing. In Australia there is limited knowledge on healthcare professionals' (HCPs') attitudes, beliefs, and perceptions of the use of PAT for existential distress in people with cancer, and their opinions on components of delivery. METHODS: Qualitative semi-structured interviews were used to inv

Tryptophan metabolism regulates immunosenescence and inflammaging: The macrophage immunometabolic axis and therapeutic targets.

Aging features two linked immune traits: immunosenescence and chronic low-grade inflammaging. Other than senescent lymphocytes, pathologically remodelled macrophages are key drivers of age-related immune dysfunction, mainly via tryptophan (Trp) metabolism through the kynurenine pathway (KP) and microbiota-related indole pathway. This review suggests a bidirectional amplification loop model, in which dysbiosis of the microbiota in the aged gut causes decreased production of microbial indoles and subsequent deficit in aryl hydrocarbon receptor (AhR) signalling in intestinal mucosal macrophages.

In vivo and computational investigation of the antidepressant-like activity of boldine from Peumus boldus in mice subjected to chronic unpredictable mild stress.

Boldine, an aporphine alkaloid mainly isolated from the bark and leaves of Peumus boldus, is reported to possess neuroprotective, antioxidant, and nootropic activities. Due to its antioxidant and neuroprotective potential, this study explored the antidepressant-like effects of boldine. Boldine (1.5, 3.0, and 6.0 mg/kg, p.o.) was evaluated for antidepressant-like activity in unstressed and stressed Swiss young male albino mice, while molecular docking (Autodock Vina) and pharmacokinetic predictions (SwissADME) provided mechanistic insights. In stressed mice, boldine significantly decreased the

The Neuroimmune Axis in Gastric Cancer: Bridging Neural Regulation, Tumor Microenvironment, and Immunotherapy.

Some theories of human cultural evolution posit that humans have social-specific learning mechanisms that are adaptive specialisations moulded by natural selection to cope with the pressures of group living. However, the existence of neurochemical pathways that are specialised for learning from social information and individual experience is widely debated. Cognitive neuroscientific studies present mixed evidence for social-specific learning mechanisms: some studies find dissociable neural correlates for social and individual learning, whereas others find the same brain areas and, dopamine-med

Microfluidic synthesis of alumina particle-laden beads via capillary number tuning for varying particle concentrations.

The mechanisms underlying the antifertility effects of hyperprolactinemia have yet to be established in an appropriate experimental model. Hyperprolactinemia is a known side effect of fluphenazine, a broad spectrum, long-acting phenothiazine known to be dopamine type-D2 receptor antagonist. In our earlier study in adult male rats, we reported that fluphenazine at a dose of 3 mg/kg/day suppressed serum FSH but not testosterone (T) through increasing dopamine (DA) metabolism in the pituitary gland, within 60 days. Fluphenazine treatment affected sperm quality and male rats treated with fluphenaz

Knowing when to stop: Interoceptive prediction of aversive outcomes guides intake regulation.

Adaptive behavior requires knowing when to stop. Internal bodily signals are known to influence intake, yet the mechanisms through which interoceptive signals help terminate intake before harm occurs remain poorly defined. Here, we developed a model of interoceptive prediction to test whether internal signals predicting that continued intake will produce an aversive outcome can regulate consumption below that threshold. Male and female Sprague-Dawley (n = 110) self-administered saccharin, and intake exceeding an individualized threshold, defined as 50% of the rat's baseline consumption, was fo

Triplex electrochemical sensing of dopamine and antipsychotic drugs enabled by palladium and phosphorus co-doped graphitic carbon nitride.

Chlorpromazine (CPZ) and clozapine (CLZ) are widely used antipsychotics that exert therapeutic effects primarily through dopamine (DA) receptor antagonism. Simultaneous monitoring of these drugs alongside DA in biological fluids is crucial to avoid adverse effects in clinical practice. In this work, we developed an electrochemical sensor based on Pd and P co-doped g-C3N4 nanosheets for the synchronous detection of CPZ, CLZ, and DA. The synergistic incorporation of P and Pd effectively modulates the electronic structure by narrowing the bandgap and reducing charge transfer resistance, thereby s

Pharmacological Properties of Psychedelics with a Special Focus on Potential Harms.

Psychedelics are a group of substances within the heterogeneous class of hallucinogenic drugs. Via binding to the serotonin (5-HT) 2A receptor, psychedelics exert profound alterations in various mental domains, including sensation, cognition, emotions, and self-perception. Psychedelics comprise phenethylamines (e.g., mescaline), tryptamines (e.g., psilocybin), and ergolines (e.g., LSD). These drugs have been used recreationally for decades but have also regained attention as potential treatments for various psychiatric as well as neurological illnesses. While psychedelics are generally conside

Set and Setting for Psychedelic Harm Reduction.

Set and setting is a fundamental concept in the field of psychedelic studies, denoting the crucial importance of context to determining the character and quality of experiences with psychedelics. This paper suggests a framework for thinking of set and setting and its relevance for psychedelic harm reduction. It discusses how "set," the diverse factors related to an individual's mindset, and "setting," the diverse environmental dimensions, influence the psychedelic experience and its outcomes, and the relevance to the field of harm reduction. It additionally provides an overview of four basic m

A Delphi-method-based consensus for clinical domains and assessment of the inflammatory subtype of major depressive disorder from the ASPIRE study.

BACKGROUND: Elevated inflammatory biomarkers have been consistently associated with a subset of depressive symptoms, particularly atypical and energy-related features, as well as antidepressant treatment resistance. This evidence has supported the definition of an inflammatory subtype of major depressive disorder (ISMDD), characterized by immune-inflammatory dysregulation and reduced response to first- and second-line antidepressants. However, a lack of consensus on its definition and assessment tools limits research and treatment development. METHODS: Using a Delphi approach, international e

Comparative Efficacy and Acceptability of Treatment Strategies for Antipsychotic-Induced Akathisia: A Systematic Review and Network Meta-analysis.

BACKGROUND: Antipsychotics are the treatment of choice for schizophrenia, but they often induce akathisia. However, comparative efficacy of treatment strategies for akathisia remains unclear. DESIGN: We performed a systematic review and network meta-analyses (PROSPERO CRD42023450720). We searched multiple databases on July 24, 2023. We included randomized clinical trials comparing 1 or more treatment strategies for antipsychotic-induced akathisia against each other or control conditions. We included adults with schizophrenia or other psychiatric disorders treated with antipsychotics. The prim

Epistemic Risk Reduction in Psychedelic-Assisted Therapy.

Belief change is crucial to therapeutic benefit in psychedelic-assisted therapy as well as in more traditional forms of therapy. However, the use of psychedelics comes with a few unique challenges that require extra caution. First, drastic belief changes may occur faster than in regular therapy. Facing radical and transformative insights all at once rather than through a gradual process of discovery and integration can lead patients to a volatile, confusing or disorienting epistemic state. Additionally, we know psychedelic substances generate hallucinatory experiences that come with a high deg

Relationship Between Clozapine-Induced Inflammation and Eosinophilia: A Retrospective Cohort Study.

BACKGROUND AND HYPOTHESIS: Eosinophilia has not been highlighted in clozapine-induced adverse inflammatory events, as it is often asymptomatic and self-limiting, while drug reaction with eosinophilia and systemic symptoms (DRESS) syndrome occurs rarely. This study aimed to reveal the temporal relationships between eosinophilia and other inflammatory events during clozapine initiation. STUDY DESIGN: The temporal relationships between eosinophilia and other inflammatory events were evaluated among 241 patients with schizophrenia treated with clozapine for the first time at 7 hospitals. Risk fac
