Dopamine supersensitivity psychosis: a critical evaluation of diagnostic validators.

Dopamine antagonists are the cornerstone of treatment for psychotic disorders, but long-term exposure may induce dopamine supersensitivity psychosis (DSP)-a syndrome characterized by psychotic relapse upon dose reduction or discontinuation, tolerance to antipsychotic effects, and co-occurrence of tardive dyskinesia. Despite decades of clinical observation and supporting preclinical evidence, the diagnostic validity of DSP and its relationship to treatment resistance remain incompletely understood. This systematized review evaluates clinical evidence supporting DSP as a distinct diagnostic enti

Treatment-Resistant Psychosis in an Adolescent at Risk of Metabolic Syndrome: A Case Report.

Management of treatment-resistant psychosis is inherently complex, presenting unique clinical challenges in pediatric populations when metabolic syndrome risk limits pharmacologic options. We report a case of an adolescent with rapid decline and persistent psychotic symptoms despite trials of first-line antipsychotic agents. His refractory clinical course required careful consideration of the relative benefits of other antipsychotic therapies against their potential for significant metabolic adverse effects. Clinical decision-making was further complicated by the need to engage the patient's m

Association between clozapine-induced hypertriglyceridemia and defenestration of liver sinusoidal endothelial cells.

Clozapine is the most effective antipsychotic drug, while also causing the most severe metabolic side effects. The underlying mechanisms of these side effects appear multifactorial and are not entirely understood. We hypothesized that liver sinusoidal endothelial cells (LSEC) dysfunction is linked to the metabolic side effects of clozapine, based on known association between reduced LSEC porosity, hypertriglyceridemia and insulin resistance. Accordingly, we conducted a translational study by assessing the porosity of murine LSECs following clozapine exposure in vitro and correlating serum cloz

Antipsychotic-Induced Parkinsonism With Concurrent Benztropine-Associated Anticholinergic Effects in a Community Emergency Department Setting: A Case Report.

Antipsychotic-induced parkinsonism (AIP) is a neurologic side effect of first- and second-generation antipsychotics. We present a case of AIP associated with paliperidone with findings concerning for concurrent anticholinergic side effects as a consequence of AIP treatment in a young patient presenting to a community emergency department. Temporarily withdrawing the offending dopamine receptor-blocking antipsychotic medication as well as implementing an adjunct therapy such as benztropine may alleviate the acute presentation of AIP. However, concurrent anticholinergic adverse effects or toxici

Structure based discovery of antipsychotic-like TAAR1 agonists.

Schizophrenia is a severe mental illness whose current treatments primarily target dopamine and serotonin receptors. These drugs often cause side effects and vary in effectiveness across patients. The trace amine-associated receptor 1 (TAAR1), which modulates monoamine signaling, has emerged as a promising alternative target. To discover previously unexplored TAAR1 ligands, we computationally dock 65 million molecules against the active state of TAAR1 and experimentally test 55 of those highly ranked. Fourteen TAAR1 agonists emerge with potencies ranging from mid-nanomolar to micromolar. This

Potential Role of Zn Supplementation in Ameliorating Reserpine-Induced Fibromyalgia Manifestations Through Central NLRP3-Autophagy-Redox Network Modulation in Rats.

Fibromyalgia is a chronic disorder of hyperalgesia and fatigue among other debilitating manifestations. Current antidepressant-based therapy is hampered by suboptimal tolerability. The main pathomechanism is central sensitization that is largely precipitated by glial neuroinflammation involving NLRP3 inflammasome pathway. Administration of Zn, a relatively tolerable and accessible mineral with established antioxidant and autophagy-promoting properties, was shown effective against fatigue syndromes and NLRP3 inflammasome activation, yet its effect on fibromyalgia remains elusive. The current st

Access to antipsychotic in Brazil: using risk management to assess changes in pharmaceutical services.

The treatment of schizophrenia is essential for patients' quality of life and relies on the continuous use of antipsychotics provided by the Brazilian Unified Health System (SUS). This study aimed to compare the processes that affect access to antipsychotics across different components of pharmaceutical services (PS) using risk management. The risk management methodology applied establishes risk levels, from very low to very high, based on the combination of probability and impact scales, following consecutive stages from context understanding to risk treatment. A total of 20 risk events were

Antipsychotic-Induced Remodeling of the Gut Microbiota: Drug-Specific Patterns and Molecular Links to Metabolic Adverse Effects.

Antipsychotic drugs are essential for the treatment of schizophrenia-spectrum disorders and other severe psychiatric conditions, but several agents produce clinically important metabolic and gastrointestinal adverse effects. Increasing evidence suggests that these outcomes may involve bidirectional interactions between antipsychotics and the gut microbiota. This structured narrative review integrates human, animal, gnotobiotic, in vitro, pharmacokinetic, and multi-omic evidence to examine drug-specific microbiome effects and microbiota-dependent modulation of host responses. Olanzapine has the

On-demand design of controlled-release systems using an expert-mimic AI framework.

Navigating a vast formulation-preparation design space that is highly sensitive to perturbations makes trial-and-error approaches inefficient and costly for diverse clinical needs. Here, we introduce an expert-mimic AI framework called E-MAF that integrates a process-aware surrogate with an optimized genetic algorithm to emulate coarse-to-fine reasoning, autonomously generating lab-ready designs tailored to desired release profiles. We show that E-MAF delivers on-demand release and shortens design cycles from 6-12 months to approximately 1 hour. We demonstrate that in vitro release, in vivo ph

Change in HbA1c and weight after initiation of antipsychotic treatment in people with severe mental illness in England: an individual-level interrupted time-series analysis.

BACKGROUND: Second-generation antipsychotics contribute to excess cardiometabolic morbidity in people with severe mental illness, but the timing of early glycaemic change and whether it parallels weight gain remain unclear. We aimed to evaluate glycated haemoglobin A1c (HbA1c) and weight trajectories before and after initiation of antipsychotic treatment. METHODS: We conducted an individual-level interrupted time-series analysis in England using electronic health records (EHR) from the Clinical Practice Research Datalink Aurum database between Jan 1, 2000, and June 7, 2023. Parkinsonism and psychopathology severity in real-world outpatients with schizophrenia: A cross-sectional PRISM study.

Extrapyramidal symptoms (EPS) have been described as intrinsic to schizophrenia spectrum disorders (SSD), beyond medication-induced effects. Among EPS, parkinsonism is the most prevalent subtype and has been linked to poorer clinical and functional outcomes. We aimed to examine the relationships between parkinsonism and psychopathology and social functioning, as well as its demographic and pharmacological correlates, in a real-world sample of outpatients with schizophrenia receiving stable antipsychotic medication. Data were drawn from the PRISM project, an EU-funded, multicentre, cross-sectio

Intramuscular pharmacological treatment and mechanical restraint duration in acute psychiatric settings: Implications for nursing practice - A retrospective observational study.

BACKGROUND: Mechanical restraint is used in psychiatric settings despite international efforts to minimize restrictive practices. Intramuscular medication is routinely administered during the restraint episode as rapid tranquillization with the aim of achieving behavioral control and earlier removal, but evidence regarding its effect on restraint duration in psychiatric inpatient settings is lacking. AIM: To examine whether intramuscular pharmacological interventions are associated with restraint duration in acute psychiatric wards using standardized dosing. METHODS: Retrospective observatio

Changes in Weight and Metabolic Indices after Switching to Aripiprazole Once-Monthly in Patients with Schizophrenia According to Prior Oral Antipsychotics.

INTRODUCTION: Antipsychotic-induced weight gain and metabolic disturbance are common in schizophrenia, but liability varies across agents. Aripiprazole has a relatively favorable metabolic profile, yet evidence on aripiprazole once-monthly, its long-acting injectable formulation, remains limited. We hypothesized that switching from a higher liability antipsychotic would yield a greater weight reduction than switching from a lower liability agent. METHODS: This 16-week, prospective, open-label, multicenter study enrolled patients with stable schizophrenia. Participants were stratified by the m

Olanzapine for the Prevention and Treatment of ICU Delirium: Protocol for a Systematic Review With Meta-Analysis.

BACKGROUND: Intensive care unit (ICU) delirium is a multifactorial acute brain dysfunction associated with prolonged hospitalization, increased morbidity and mortality, and higher healthcare costs. Despite these substantial negative impacts of delirium, the optimal prevention and treatment strategies are unclear. Olanzapine, an atypical antipsychotic drug, is commonly used for delirium management. We will perform an updated systematic review and meta-analysis to assess the benefits and harms of olanzapine for the prevention and treatment of ICU delirium. METHODS: This protocol is developed in

Association of Neutrophil-to-Lymphocyte Ratio and Monocyte-to-Lymphocyte Ratio With Symptom Severity and Subjective Well-Being in Schizophrenia: A Cross-Sectional Study From Central India.

BACKGROUND: Systemic inflammation has increasingly been implicated in schizophrenia. The neutrophil-to-lymphocyte ratio (NLR) and monocyte-to-lymphocyte ratio (MLR), derived from routine complete blood count (CBC) investigations, are inexpensive inflammatory indices, but Indian data relating them to symptom severity and subjective well-being remain limited. METHODS: This hospital-based cross-sectional study included 100 adults with schizophrenia (59 receiving antipsychotic treatment and 41 not currently receiving antipsychotic treatment, comprising newly diagnosed antipsychotic-naïve particip

Protective effects of central leptin on whole-body energy homeostasis following acute olanzapine exposure.

Antipsychotics represent mainstay treatment of schizophrenia and are also prescribed on- and off-label in other severe mental illnesses. Newer antipsychotic use is associated with severe metabolic side effects such as obesity and type 2 diabetes. Leptin is a hormone secreted by adipose tissue, and it acts on the brain to decrease body weight by reducing food intake and stimulating energy expenditure. Leptin also improves glucose and lipid metabolism. In this study, we examined the short-term impact of olanzapine, a commonly used second-generation antipsychotic, on the central leptin-mediated r

Endocannabinoid plasma concentrations and their relationship with clinical outcome and cannabis use in antipsychotic-naïve first episode psychosis.

Disturbances in the endocannabinoid system (ECS) have been reported in psychosis, with evidence suggesting consistent alterations in N-arachidonoylethanolamine (anandamide; AEA) and inconclusive findings for 2-arachidonoylglycerol (2-AG). Lower AEA concentrations have been associated with greater symptom severity, while the relationship between ECS and cannabis use remains unclear. However, the prognostic value of peripheral endocannabinoid concentrations for clinical outcomes and cannabis use in first-episode psychosis (FEP) remains largely unknown. This study examined whether baseline plasma
