Novel 5-Phenylmorphans as Opioid Receptor Antagonists: Synthesis, Structure-Activity Relationship, Biological Evaluation and Computational Modeling.

The widespread emergence of ultra-potent synthetic opioids in illicit drug supplies and the limited effectiveness of naloxone in reversing certain synthetic opioid overdoses highlight the urgent need for potent high-affinity opioid antagonists. This study describes an improved synthetic approach and structure-activity relationship (SAR) study of N-substituted analogs of a potent and active MOR antagonist, 3-((1S,5R,9R)-9-methyl-2-phenethyl-2-azabicyclo[3.3.1]nonan-5-yl)phenol ((1S,5R,9R)-1). Pharmacological activity of the synthesized compounds at mu (MOR), delta (DOR), and kappa (KOR) recepto

Peripherally acting μ-opioid receptor antagonists for recovery of bowel function after radical cystectomy: a systematic review and meta-analysis.

BACKGROUND: Postoperative ileus (POI) is common after radical cystectomy (RC) and contributes to prolonged hospital stay, increased healthcare costs, and reduced patient quality of life. Peripherally acting μ-opioid receptor antagonists (PAMORAs), including alvimopan, naloxegol, and methylnaltrexone, have been investigated as strategies to reduce POI while preserving opioid-mediated analgesia. Previous systematic reviews have shown benefit of PAMORAs, particularly alvimopan, in abdominal procedures such as bowel resection and hysterectomy, but evidence specific to RC remains heterogeneous and

Zebrafish behavioral phenomics reveals functional subtypes of autism risk genes.

Autism spectrum disorder (ASD) is a highly heterogeneous neurodevelopmental condition caused by many genetic mutations, each conferring modest individual risk. Despite its genetic complexity and clinical burden, no effective treatments targeting core symptoms are available. Although ASD is diagnosed as a single disorder, accumulating evidence suggests that it comprises biologically distinct subtypes, yet how genetic risk maps onto phenotypic substructure remains poorly defined. Here, we apply zebrafish behavioral phenomics to systematically link ASD risk genes to functional behavioral subtypes

Efficacy and safety of opioid-receptor antagonists for opioid-induced constipation: a systematic review and meta-analysis.

BACKGROUND: Opioid-induced constipation (OIC) is a common and serious side effect of long-term opioid analgesic therapy. As traditional laxatives often show limited efficacy, it is crucial to explore treatment strategies that effectively relieve constipation without compromising analgesic effects. In response to this clinical need, Opioid-receptor antagonists have been approved for OIC. Although new evidence has emerged in recent years, a comprehensive analysis of efficacy outcomes (such as constipation symptoms, quality of life, and satisfaction) is still lacking. OBJECTIVE: To summarise and

Oxytocin treats respiratory depression and reduces mortality from fentanyl and the combination of xylazine-fentanyl.

UNLABELLED: Opioid addiction and misuse are a serious national crisis that affects public health, as well as social and economic welfare. Mortality due to opioid misuse is further exasperated by the combination of opioids with non-opioid respiratory depressants such as xylazine that are resistant to mu opioid receptor antagonists such as naloxone. This study tested the hypothesis that oxytocin can mitigate the severe opioid induced respiratory depression (OIRD) and mortality induced by high doses of fentanyl or the combination of fentanyl with xylazine. Our results show OXT can improve surviva

Acute buprenorphine exposure depresses neonatal respiratory chemoreflexes in the presence or absence of naloxone.

Perinatal opioid exposure is a prevalent clinical concern linked to respiratory instability and adverse infant outcomes. The opioid buprenorphine is prescribed as a medication for opioid use disorder during pregnancy and used to treat neonatal opioid withdrawal syndrome, yet its direct effects on neonatal control of breathing have not been examined. Here, we asked how acute buprenorphine exposure affects breathing at rest, and during chemoreceptor stimulation. Using dual-chamber head-out plethysmography, we measured pulmonary ventilation rate ( V ˙ I ) and metabolic rate in awake male and fe

Opioid antagonist treatment in alcohol use disorder: extinction, reward memory, and the possible importance of timing.

Opioid receptor antagonists, particularly naltrexone and nalmefene, are among the best-supported pharmacotherapies for alcohol use disorder (AUD), yet their clinical efficacy remains modest and highly heterogeneous. The mechanisms through which transient opioid receptor blockade produces sustained therapeutic benefit remain incompletely understood. Classical explanations emphasize attenuation of alcohol reward and suppression of craving, whereas extinction-based models propose that therapeutic effects emerge when alcohol consumption occurs under opioid receptor blockade, leading to weakening o

Opioid induced constipation: mechanisms and management.

Opioid-induced constipation (OIC), a prevalent form of opioid-induced bowel dysfunction, significantly affects patients with chronic pain, both with and without cancer, who rely on opioid analgesics. OIC reduces opioid effectiveness, impairs quality of life and is frequently underdiagnosed and inadequately managed. A comprehensive literature search of MEDLINE and EMBASE (2000-April 2025) was conducted using terms such as opioid-induced constipation, epidemiology, pathophysiology, treatment, and guidelines. Relevant studies, meta-analyses, and consensus statements were analysed to synthesize me

Can Naldemedine Allow Higher Oxycodone Dosing in Patients with Cancer? A Single-Center Retrospective Study.

BACKGROUND: Opioid-induced constipation (OIC) limits opioid titration in cancer care. Before the use of peripherally acting μ-opioid receptor antagonists (PAMORAs), OIC often required switching from oxycodone to lower-risk opioids, such as transdermal fentanyl. Whether naldemedine allows oxycodone escalation without switching remains unclear. We evaluated the maximum scheduled daily oxycodone dose as a surrogate for maintaining therapy without constipation-driven switching. METHODS: We retrospectively reviewed adults with cancer pain who initiated oral oxycodone at Toyama University Hospital

Opioids in breast cancer: Between analgesia and modulation of tumour progression.

Preclinical investigations consistently demonstrate that activation of μ-opioid receptors and δ-opioid receptors promote proliferation, migration, angiogenesis, epithelial-mesenchymal transition, acquisition of cancer stem cell phenotypes, and chemoresistance. Conversely, selected opioids with atypical pharmacological profiles, including tramadol, nalbuphine, and dezocine, as well as antagonists of opioid receptors such as naloxone, naltrexone, and low-dose naltrexone, exhibit antineoplastic and immunomodulatory properties, frequently mediated through noncanonical pathways such as the opioid g

German Expert Consensus on Opioid-Induced Constipation (OIC): Recommendations and a Treatment Algorithm for Clinical Practice.

Background/Objectives: Opioid-induced constipation (OIC) is a frequent adverse effect of opioid therapy. In contrast to other opioid-related side effects, OIC usually does not improve over time and significantly impairs the quality of life of affected patients. Despite its high prevalence, OIC remains underdiagnosed and undertreated in clinical practice, which has been demonstrated in several European countries. Healthcare data indicates that approximately 2.3 million people in Germany received potentially OIC-inducing opioids in 2023, the majority being patients with chronic non-cancer pain.

Current Strategies and Future Perspectives in the Management of Opioid-Induced Constipation: A Review of International and Japanese Guidelines.

BACKGROUND: Opioids are indicated for pain that occurs with cancer and other disorders that become more prevalent with aging. The most prevalent and persistent adverse effect of opioid therapy is opioid-induced constipation (OIC). OIC impairs patient quality of life (QoL) and leads to suboptimal pain management due to medication non-adherence. Unlike other adverse effects, OIC persists and is not associated with pharmacological tolerance, necessitating long-term management. OBJECTIVES: The objective of this study is to evaluate current clinical strategies for OIC by comparing recommendations

The protective role of µ- and δ-opioid receptor blockage on memory deficits after mild traumatic brain injury is dependent on preexisting high endogenous opioid tone.

Mild traumatic brain injury (mTBI) causes deficits in memory formation and retrieval by disrupting the balance in synaptic excitability and inhibition. The pre-trauma endogenous opioid tone could influence the severity of post-trauma cognitive deficits. Thus, the current investigation aimed to evaluate how post-mTBI adaptations in opioid receptor subtype signaling affect the severity of memory dysfunction depending on the endogenous opioid tone. For this purpose, we utilized adult male mice divergently bred for high (HA) and low (LA) endogenous opioid system activity. Mild traumatic brain inju

Opioid receptors regulate the acquisition and extinction of second-order fear in both sequential and simultaneous conditioning protocols.

The endogenous opioid system is thought to regulate Pavlovian fear conditioning in rats by instantiating some component of the error correction signal that underlies this learning. Consistent with this view, opioid receptor antagonists enhance the acquisition of first-order conditioned fear when a novel stimulus (S1, e.g., a light) is paired with a brief but painful foot shock; and impair the extinction of this fear when an already-conditioned S1 is repeatedly presented in the absence of the shock. The present study examined whether opioid receptors also regulate the acquisition and extinction

Ocular effects of novel synthetic opioids: A systematic review and narrative synthesis of current evidence.

INTRODUCTION: The recent emergence of novel synthetic opioids (NSOs), including fentanyl analogs-eg, carfentanil, acetylfentanyl-and nonfentanyl drugs-eg, U-47700, nitazenes-on the drug market has caused high levels of morbidity and mortality. Even though their systemic toxicities have been well documented, their ocular effects have not been well explored. This review thus engages to systematically review the currently available clinical and experimental data on NSO-related ocular complications. METHODS: According to Preferred Reporting Items for Systematic reviews and Meta-Analyses 2020 guid

Prevalence of opioid-induced constipation in the emergency department: a retrospective study.

Opioids are widely prescribed for the treatment of moderate to severe pain. Gastro-intestinal side effects are common with opioid use, of which opioid-induced constipation (OIC) is the most prevalent. OIC can be difficult to manage, and may lead to increased healthcare utilization, including emergency department (ED) visits. However, data on the prevalence of OIC in the ED are limited. We conducted a retrospective, single-center study of 2017 adult long-acting opioid users presenting to the ED between 2016 and 2022. OIC was diagnosed in 129 patients (6.4%; 95% CI 5.4-7.6%). Treatment included

Is the antidepressant efficacy of ketamine and esketamine mediated via opioid mechanisms?

BACKGROUND: Ketamine and esketamine produce rapid and sustained antidepressant effects in persons with treatment-resistant depression (TRD). Although it is posited that these effects are largely attributed to N-methyl-D-aspartate receptor antagonism, the potential involvement of the opioid system remains unclear. This systematic review investigates whether ketamine and esketamine antidepressant efficacy is mediated through the opioid system. METHODS: We conducted a systematic search of preclinical and clinical studies investigating the potential involvement of the opioid system in the antidep
