The effect of neuromuscular blocking reversal agents on perioperative neurocognitive function after general anaesthesia: a systematic review and meta-analysis.

BACKGROUND: Perioperative neurocognitive dysfunction (PND) is influenced by various perioperative factors. Recent studies suggest that neuromuscular blocking reversal agents (NMBRs) may impact on PND. However, the results have been inconsistent. Therefore, we aimed to compare the effects of perioperative NMBRs on PND through this systematic review and meta-analysis. METHODS: We searched PubMed, CENTRAL, Embase, Web of Science, Scopus, and China Biology Medicine from their inception until May 2024. Two reviewers independently identified randomized controlled trials (RCTs) that compared the per

Systematic review for VNS vs. pharmaceutical modulations for multifaceted neurological disorder management through cross-case, network meta-analysis.

BACKGROUND: As an adjunct or alternative to conventional pharmacotherapy, vagus nerve stimulation (VNS) which is FDA-approved has arisen as a novel means for various neurological disorders. METHOD: We searched multiple databases (through 2024) for randomised trials and observational studies of VNS (invasive and transcutaneous) and pharmacological treatments (e.g. cholinergic agents, antiepileptics, antidepressants) across several neurological disorders. Prior to comparing between VNS and pharmacological treatments, subgroup analyses of VNS studies were performed for disorder type, patient dem

Comparison of the efficacy of unilateral amblyopia treatments: A systematic review and network meta-analysis.

Amblyopia, a disorder that impairs visual acuity, encompasses a spectrum of clinical interventions. The true efficacy of these treatments, however, remains elusive. Consequently, we elucidate the effectiveness of each intervention, providing critical insights into optimal therapeutic strategies. We systematically searched PubMed, Embase, Web of Science, and the Cochrane Library for randomized controlled trials up to August 12, 2024, including amblyopic patients without residual amblyopia. Interventions included spectacles alone, 2-hour patching, 6-hour patching, full-time patching (FTP), atrop

Coclaurine N-methyltransferase-like enzymes drive the final biosynthetic reaction of the anti-Alzheimer's drug galanthamine in Amaryllidaceae.

Galanthamine, an isoquinoline alkaloid used to treat symptoms of Alzheimer's disease, is predominantly extracted from Amaryllidaceae plants, yet its supply remains limited. In this study, we identified, isolated, and characterized N-methyltransferases (NMTs) from three galanthamine-producing species: Leucojum aestivum, Lycoris radiata, and Hippeastrum papilio. The transcriptomic analysis identified five unique NMT isoforms, among which LaLrHpNMT1, an isoform highly conserved across all three species, exhibited the highest catalytic activity. Phylogenetic and structural analyses revealed that t

Development of benztropine-based DAT blockers for Parkinson's disease interventions: A biologic and synthetic overview.

Parkinson's disease is considered a neurodegenerative brain affection where the substantia nigra, a group of dopamine producer neurons are eliminated and hence, dopamine levels go to dangerous lower levels. With this molecule limited, an imbalance in neurotransmitters is set, a decreased neurological activity is set affecting the motion and several communication pathways. The most recognized outcome lies in the motor cortex, which is characterized by different motor symptoms such as bradykinesia, rigidity, uncontrollable tremors, imbalance, stiffness, and sudden slowness for several corporal m

The association between neostigmine and postoperative atelectasis in elderly patients undergoing laparoscopic pancreaticoduodenectomy: a retrospective single-center propensity score matched study.

BACKGROUND: It is uncertain whether neostigmine reversal improves postoperative pulmonary outcomes. This study aimed to evaluate the association between neostigmine and postoperative atelectasis, and other complications in elderly patients undergoing laparoscopic pancreaticoduodenectomy (LPD). METHODS: This single-center retrospective cohort study included elderly patients who underwent LPD between 2019 and 2022, using cis-atracurium as the sole neuromuscular blocking agent. Participants were divided into two groups based on exposure to neostigmine: the neostigmine reversal group (exposed) an

Antidote administration in mice exposed to sarin resulted in brainwave stabilization without cognitive protection.

Resurgence of sarin use in the Syrian conflict demonstrates that chemical weapons are a vivid threat. Like other organophosphate compounds, sarin inhibits acetylcholinesterase resulting in cholinergic crisis. The current antidote therapy includes atropine (a muscarinic cholinergic antagonist) + an oxime (e.g., pralidoxime) to reactivate the inhibited cholinesterases. Long-term sequelae reported in victims of the 1995 Tokyo subway sarin attack emphasize the need to improve neuroprotection and to delineate more precise biomarkers of intoxication severity. Previously, we proposed that brain oscil

Acetylcholine and muscarinic receptor targeting in bipolar disorder: does xanomeline-trospium chloride and other investigational muscarinic agonists hold promise as mechanistically informed treatments for manic episodes, mixed features and cognitive deficits in bipolar disorder?

INTRODUCTION: Xanomeline-trospium chloride (Cobenfy, KarXT) received FDA approval on September 26, 2024, for the treatment of adults with schizophrenia. Xanomeline-trospium chloride is the first muscarinic M1, M4 acetylcholine receptor partial agonist approved to treat schizophrenia. Preliminary evidence also indicates that xanomeline-trospium chloride improves measures of cognition in Alzheimer's disease and schizophrenia. AREAS COVERED: Acetylcholine's physiology as well as evidence implicating disturbance in acetylcholine availability and/or its canonical receptors in mania and cognitive i

Uniformity in air trapping distribution is associated with the severity of airway hyper-responsiveness in asthma.

OBJECTIVES: Severe airway hyper-responsiveness is associated with poor outcomes in asthma. Spatial heterogeneities in airway closures and subsequent air trapping (AT) during bronchoconstriction have been poorly assessed in asthma. MATERIALS AND METHODS: We assessed the spatial distribution of AT by acquiring expiratory lung CT at each dose of a methacholine challenge in 31 women with asthma. A statistical method based on k-nearest-neighbors (knn) informed about between AT-area distance toward uniformity at the peak of bronchoconstriction (PC20). Fractal dimensions of binarized low attenuation

The Tacrine-Induced Endoplasmic Reticulum Stress in AChE-Expressed Cells Leads to Improper Assembly and Transport of the Oligomeric Enzyme: Reversal by Trehalose.

Alzheimer's disease (AD) is the most common dementia with progressive loss of cognitive functions. Acetylcholinesterase (AChE) inhibitors have been approved as conventional pharmacotherapies for AD. Tacrine was the first AChE inhibitor introduced into clinics for AD; however, it was withdrawn from use in 2013 because of safety concerns. In cultured neurons, as well as in mice, tacrine was found to induce endoplasmic reticulum (ER) stress and finally lead to cell apoptosis: the event was triggered by binding the inhibitor to the intracellular enzyme serving as a pharmacological chaperone. Treha

SAHRANG: Subarachnoid Hemorrhage Recovery and Galantamine: A Pilot Multicenter Randomized Placebo-Controlled Trial.

BACKGROUND: Subarachnoid hemorrhage (SAH) causes life-long neurologic dysfunction. Peripheral inflammatory processes as a reaction to brain injury have been shown to worsen outcomes after SAH. Galantamine has been shown to reduce proinflammatory microglial activities and improve synaptic connections. We hypothesize that galantamine treatment after SAH mitigates inflammation-mediated neuronal injury and improve outcomes. We conducted a pilot clinical trial to examine the tolerability and safety of galantamine in patients with SAH. METHODS: This prospective, multicenter, double-masked, randomiz

Occurrence and potential ecological risks of pharmaceuticals and illicit drugs (PhIDs) in the Santos-São Vicente Estuarine System (SSVES), Brazil.

Despite the substantial amount of research on the presence of pharmaceuticals and illicit drugs (PhIDs) in freshwater environments, there is a paucity of data on these contaminants in estuarine systems, particularly in South America. Industries, the biggest port in the Southern Hemisphere, and substantial urban areas surround the Santos-São Vicente Estuarine System (SSVES), located in a Brazilian subtropical area. This study constitutes the first assessment of contamination by PhIDs in this estuarine area. Samples of superficial water, sediments, and oysters were analyzed through liquid chroma

The effect of sugammadex on postoperative urinary retention and recovery characteristics: a systematic review and meta-analysis.

BACKGROUND: Postoperative urinary retention (POUR) is a frequent complication following general anesthesia. There is growing interest in whether sugammadex, a neuromuscular blockade (NMB) reversal agent that does not require anticholinergic coadministration, might reduce its incidence. This systematic review and meta-analysis aimed to evaluate the effects of sugammadex on POUR and other postoperative recovery outcomes, such as 30-day readmission rates and postoperative pain scores (POPS). METHODS: PubMed, Cochrane, Embase, and Web of Science were comprehensively searched for studies published

Comparative effects of glycopyrrolate vs. atropine combined with neostigmine on heart rate dynamics and safety outcomes in pediatric patients: a prospective randomized controlled trial.

BACKGROUND: Neostigmine is a widely used muscle relaxant antagonist that requires combination with anticholinergic agents like atropine to mitigate its muscarinic effects. Glycopyrrolate exerts less influence on cardiac conduction than atropine, making it a preferable option. However, clinical research on its pediatric use is limited. This study aims to compare the effects of glycopyrrolate and atropine, in combination with neostigmine, on heart rate and adverse events in pediatric patients. METHODS: Children undergoing penile flap surgery under general anesthesia combined with a caudal block

Preclinical efficacy of the muscarinic agonist ML-007 in psychosis models depends on both M1 and M4 receptors.

Muscarinic agonists represent a new class of treatments for psychosis with a mechanism distinct from typical and atypical antipsychotics. The muscarinic subtype M4 has been proposed as the primary mediator of efficacy but results from recent clinical trials with M4-selective compounds have drawn this hypothesis into question. Instead, activation of both M1 and M4 receptor subtypes may be required for robust treatment effects. Here, we characterize the clinical-stage muscarinic agonist ML-007 in preclinical models and explore its therapeutic potential for treating psychosis in schizophrenia and

Expert Opinion on the Efficacy and Safety of Antispasmodics with a Focus on Irritable Bowel Syndrome.

Antispasmodics are commonly used to treat irritable bowel syndrome (IBS) symptoms. They are generally regarded as safe. However, some have been found to be ineffective or not cost-effective. Marked discrepancies in the availability and formulation of antispasmodic agents across countries—ranging from the limitation to only fundamental compounds in regions such as the United States to the marketing of diverse combination products elsewhere—have generated considerable ambiguity and debate within the field. Meta-analyses show varying results on the efficacy of antispasmodics, indicating the need f
