Peripheral Neuromodulation for Central Pain: A Case of Thalamic Pain Syndrome Managed With Percutaneous Nerve Stimulation.

BACKGROUND: Thalamic pain syndrome is a debilitating neuropathic condition that may develop after a thalamic stroke. Patients experience diffuse whole-body pain, contralateral to their stroke, that is often refractory to standard therapies. CASE REPORT: A 53-year-old woman with a history of right basal ganglia and thalamic hemorrhagic stroke presented with 10 years of severe, left-sided pain unresponsive to antispasmodics, anticonvulsants, and nonsteroidal anti-inflammatory drugs. A diagnostic sciatic nerve block provided temporary relief, prompting placement of a percutaneous peripheral nerv

Association of a stellate ganglion block-based care pathway with hypnotic medication reduction in chronic insomnia with anxiety symptoms: a real-world retrospective cohort study.

BACKGROUND: Chronic insomnia with anxiety symptoms is frequently accompanied by sustained hypnotic or sedative medication use. Evidence regarding procedure-based adjuncts to structured medication reduction remains limited. OBJECTIVE: To evaluate whether participation in a stellate ganglion block (SGB)-based care pathway, compared with contemporaneous routine care without SGB, was associated with hypnotic or sedative medication reduction at 1 month. METHODS: This single-center retrospective cohort study screened 420 records from 1 January 2022 to 31 December 2025 and included 160 adults with

Photosensitive ropivacaine- and chlorin e6-loaded hydrogel targeting the substance P-tachykinin receptor 1-toll-like receptor 7 axis enhances photodynamic and photothermal therapy against oral cancer.

This study examines the important role of neuro-tumoral crosstalk in oral squamous cell carcinoma (OSCC) progression and therapy. We identified that dorsal root ganglia (DRG)-secreted substance P (SP) activates a pro-metastatic Tacr1-Tlr7 neuroinflammatory axis. To therapeutically disrupt this axis, a multifunctional photosensitive hydrogel patch (PDA/Rop-Ce6@DA-HA) was engineered. This patch co-delivers the local anesthetic ropivacaine (Rop) to block SP release from neurons and the photosensitizer chlorin e6 (Ce6) for combined photothermal (PTT) and photodynamic therapy (PDT). Under near-infr

Biochemical and Pharmacological Studies on Kynurenic Acid Metabolism in the Helix pomatia-Snail Model of Learning and Memory.

Kynurenic acid (KYNA), a metabolite of the L-kynurenine pathway of L-tryptophan degradation, is an endogenous blocker of glutamate ionotropic excitatory amino acid (EAA) receptors and nicotinic acetylcholine receptors (nAChRs). KYNA plays a significant role in various neuropsychiatric disorders and the aging process. Some researchers have suggested that KYNA may contribute to memory impairment. In this study, we examined the impact of L-kynurenine (a KYNA substrate) and the anti-dementia drugs D-cycloserine and Cerebrolysin on kynurenine aminotransferase (KAT) activity, an enzyme forming KYNA,

Chemogenetic activation of cholinergic intrinsic cardiac ganglia improves border zone oxygenation and reduces arrhythmias during acute local ischemia.

BACKGROUND: Current therapeutic strategies for acute myocardial infarction rely on reperfusion and pharmacological management, which are typically administered hours after an event. Activation of the cardiac cholinergic efferents superior to the heart soon after a coronary occlusion has shown promise as a potential therapy to reduce arrhythmias and improve ventricular function. We tested whether selective activation of cholinergic neurons within the intrinsic cardiac ganglia (ICG) would also reduce arrhythmias and improve oxygenation of ischemic border zone tissue after an acute coronary occlu

[Neuroleptic Malignant Syndrome].

Neuroleptic malignant syndrome (NMS) is a rare but potentially life-threatening neurological emergency, most commonly associated with antipsychotic drugs and other dopamine receptor-blocking agents. Although its incidence has declined over recent decades, NMS remains a diagnostic challenge due to its heterogeneous clinical presentation and overlap with other drug-induced syndromes. The classic clinical picture is defined by a tetrad of hyperthermia, generalized muscle rigidity, altered mental status, and autonomic dysfunction, frequently accompanied by marked creatine kinase elevation secondar

Antimuscarinic drugs exert β-arrestin-biased agonism at the muscarinic acetylcholine type 1 receptor to promote DRG neuritogenesis.

Pirenzepine (PZ) and muscarinic toxin 7 (MT7) are muscarinic acetylcholine type 1 receptor (M1R) antagonists that promote neuritogenesis in primary adult rodent dorsal root ganglion (DRG) sensory neurons, in part through β-arrestin-dependent activation of ERK1/2. Here, we found that PZ and MT7 exhibited β-arrestin-biased agonism at M1R. PZ and MT7 recruited β-arrestin 2 to M1R and increased ERK phosphorylation in both HEK293 cells and DRG neurons in a concentration-dependent manner. Moreover, ERK activation by MT7 occurred only in M1R-positive DRG neurons and did not require G protein signalin

Effective ropivacaine delivery using lipid nanoparticles enables simultaneous cancer therapy and pain control.

Cancer and its associated pain pose a serious threat to human health. Cancer therapy and pain management are two distinct medical disciplines and recently proposed cancer neuroscience offers a possibility to simultaneous cancer therapy and pain control. Ropivacaine (Rop), a representative local anesthetic possessing both analgesic and anti-tumor properties, emerges as an ideal agent to achieve this dual objective; however, its therapeutic potency is limited under conventional administration. Herein, a lipid nanoparticles (LNPs)-based Rop delivery system is developed to enhance its in vivo perf

Inducing Daytime Circadian Phase Shifts Using Chemogenetic and Spectral Approaches.

In mammals, circadian phase shifting during the daytime is limited by reduced photic responsiveness of the suprachiasmatic nucleus (SCN), restricting the ability to experimentally manipulate the circadian clock during this phase. Reliable methods to induce daytime phase shifts are therefore essential for investigating mechanisms of circadian plasticity and photic entrainment. Complementary genetic and spectral strategies are described to enable robust, temporally precise manipulation of the circadian clock during the day. The first approach, currently limited to mice, employs a chemogenetic st

Preoperative Ultrasound-Guided Stellate Ganglion Block for Prevention of Postoperative Sore Throat After Thoracic Surgery: Study Protocol for a Randomized Controlled Trial.

PURPOSE: Postoperative sore throat (POST) is one of the most common complaints after thoracic surgery with double-lumen endobronchial tubes (DLTs), significantly impacting postoperative recovery and patient satisfaction, yet it is often overlooked. Stellate ganglion block (SGB), a minimally invasive technique, may alleviate airway inflammation and modulate sympathetic tone. It has significant potential in reducing the incidence of POST, but evidence in DLTs patients is lacking. This trial aims to test whether preoperative ultrasound-guided SGB can ameliorate the incidence and severity of POST

Injectable thermosensitive hydrogel embedding lignin-modified PLGA microspheres enables sustained lidocaine release and prolonged neuromodulation.

Malignant ventricular arrhythmias (MVA) post myocardial infarction (MI) is a leading cause of sudden death in affected patients. Hyperactivation of the sympathetic nervous system plays a crucial role in the onset and persistence of MVA after MI. While percutaneous stellate ganglion block with local anesthetics is an effective minimally invasive approach for transient MVA attenuation, achieving prolonged sympathetic blockade remains challenging. This study developed a novel injectable thermosensitive hydrogel-lignin-microsphere system for the sustained release of lidocaine (Gel-LMS Lido), aimin

Obstructive sleep apnea-hypopnea syndrome induced by dexmedetomidine in a patient with long-term sedatives: A case report.

RATIONALE: Dexmedetomidine is extensively recognized as a sedative featuring a relatively mild respiratory depressant effect. Nevertheless, for patients who are on long-term sedative use and potentially suffer from co-morbid insomnia and sleep apnea (COMISA), the safety of this drug remains ambiguous. This case report uncovers a rare yet severe adverse reaction associated with this drug. PATIENT CONCERNS: A 62-year-old male with a 2-year history of insomnia has been on long-term use of multiple sedative agents. He reported persistent symptoms including poor sleep quality, daytime fatigue, and

Interventional Treatment of Arterial Hypertension in The Modern Era: An Updated Review from the Young Investigator Group of the European Society of Hypertension.

Achievement of blood pressure targets in hypertensive patients is a challenge for clinicians despite the different classes of drugs currently available. For this reason, taking advantage of the technological progress, new interventional strategies have been proposed. Each of these approaches acts on autonomic and vascular regulation, critical components of hypertension pathophysiology with the aim to recalibrate aberrant sympathetic activation and vascular dysfunction. Renal artery denervation is the only modality currently supported by guideline recommendations in selected patients. Barorefle

Determination of the ED90 and ED99 of Liposomal versus Conventional Bupivacaine for Ultrasound-Guided Stellate Ganglion Block: A Biased-Coin Design Study.

BACKGROUND: Local anesthetics commonly used for ultrasound-guided stellate ganglion block (SGB) have a relatively short duration of action (6-12 hours). Liposomal bupivacaine, utilizing a multivesicular liposome sustained-release technology, can extend the block duration to over 72 hours, offering a potential option for prolonged sympathetic blockade. Determining its optimal effective dose relative to conventional bupivacaine is crucial for balancing prolonged efficacy with safety. METHODS: All patients received ultrasound-guided SGB, successful blockade was explicitly defined as the onset of

Tonabersat Blocks Gap Junctions and Alleviates Thermal Pain Behavior in Mice.

Gap junctions (GJs) are channels that enable exchange of ions and small molecules between cells, and have been implicated in the development and maintenance of neuropathic pain. Injury-induced glial activation in sensory ganglia is associated with increased coupling by GJs, which in turn enhances neuronal excitability, contributing to pain signaling. Tonabersat (TON) was suggested to act as a GJ blocker with analgesic actions, but these claims have been disputed. Here we examined whether TON blocks GJs and whether it influences pain behavior in a mouse pain model. Gap junctional coupling was a

Cardiac Sympathetic Neuromodulation in the Management of Refractory Electrical Storm: A Narrative Review.

Electrical storm (ES) is a life-threatening clinical condition characterized by recurrent ventricular arrhythmias within a 24 h period, carrying a high mortality rate. Despite conventional therapies, including hemodynamic optimization, antiarrhythmic drugs, implantable cardioverter-defibrillator (ICD) reprogramming, and catheter ablation, a subset of patients develop refractory ventricular arrhythmias. In this setting, cardiac sympathetic neuromodulation can interrupt arrhythmic circuits by reducing efferent sympathetic outflow to the myocardium. Stellate ganglion block (SGB) with local anaest

[Adjunctive agents for nerve blocks/local injections in the treatment of zoster-associated pain: A systematic review based on levels of evidence].

OBJECTIVES: Zoster-associated pain (ZAP) is an acute and chronic neuropathic pain condition caused by reactivation of varicella-zoster virus in sensory ganglia, which can severely impair patients' quality of life. Nerve block is one of the most commonly used interventional techniques in clinical practice and is often combined with various adjunctive agents to enhance therapeutic efficacy. However, these agents differ in mechanisms of action and levels of evidence, and due to differences in the pathological mechanisms at different stages of ZAP, there is currently a lack of systematic evaluatio

[Mechanism of SGCs-HMGB1 axis-mediated peripheral sensitization in migraine based on neuroimmune microenvironment of trigeminal ganglion and traditional Chinese medicine intervention].

Migraine is a common neurovascular disorder, and its chronification and recurrent attacks are closely associated with peripheral sensitization. Trigeminal ganglion(TG) is an important initial site of peripheral sensitization in migraine, in which neurons and satellite glial cells(SGCs) are major contributors. The neuroimmune microenvironment composed of the two plays a key role in pain signal amplification. Upon external stimulation, neurons and SGCs can release alarmin high mobility group protein B1(HMGB1), which binds to specific receptors to activate inflammasomes, thereby further amplifyin

Radiological and Clinical Effects of Exercise Intervention Therapy on Adolescent Idiopathic Scoliosis: A Meta-analysis.

Cutaneous allodynia, pain resulting from application of a non-noxious stimulus to normal skin, is a recently described symptom of migraine, with a potential role in directing optimal treatment for migraine attacks. Manifestations of cutaneous allodynia include discomfort when combing the hair, shaving, and wearing glasses, contact lenses, earrings or tight clothing. The exact mechanism by which a migraine attack is triggered is not known, but it has been theorised that, in some patients, once the attack has begun, central neurons can propagate information about the pain process without the nee
