Angiotensin Receptor Blockers Versus Calcium Channel Blockers for First-Line Antihypertensive Therapy and Survival in Adults Aged 75 Years or Older.

BACKGROUND: Evidence guiding first-line antihypertensive drug choice in adults aged 75 years or older is limited, despite widespread use of angiotensin receptor blockers (ARBs) and calcium channel blockers (CCBs) in late older age. METHODS: We conducted a target trial emulation using a new-user design in a nationwide linked healthcare claims database in Japan. Adults aged ≥ 75 years who had no prescription of either drug class in the preceding 12 months and initiated an ARB (n = 10,037) or a CCB (n = 19,785) were followed from treatment initiation. The primary outcome was all-cause mortality;

Apparent treatment-resistant hypertension in ambulatory hypertensive patients in Northeast Ethiopia: prevalence, treatment patterns, and predictors.

BACKGROUND: Apparent treatment-resistant hypertension (aTRH) significantly contributes to the rising prevalence of uncontrolled hypertension in Africa, leading to serious cardiovascular and cerebrovascular risks. Despite its critical importance, comprehensive, evidence-based data on aTRH in sub-Saharan Africa, including Ethiopia, remains scarce. The Northeast Amhara region has been particularly underexplored in this regard. This study aimed to assess the prevalence, treatment pattern, and predictors of aTRH among ambulatory adult hypertensive patients attending hypertension clinics in Dessie C

Revisiting the Association Between Calcium Channel Blockers and Parkinson's Disease in the E3N Cohort.

BACKGROUND: Previous studies on calcium channel blockers (CCBs) and Parkinson's disease (PD) risk reached conflicting conclusions. OBJECTIVES: We examined the relationship between CCBs and PD in the E3N cohort of French women followed for 15 years (2004-2018), while taking into account the potential for reverse causation. METHODS: New users of CCBs were identified using drug reimbursement databases. We validated incident PD using several data sources. We described trajectories of CCBs use using mixed logistic models within a nested case-control study. We used Cox proportional hazards models

Consensus on the management of hypertension in individuals with diabetes by Asian-Pacific Society of hypertension (APSH) & Diabetes Asia Study Group (DASG).

Hypertension and diabetes commonly coexist, sharing mechanisms such as insulin resistance, obesity, and endothelial dysfunction, which together amplify cardiovascular, renal, and cerebrovascular risk. This dual burden is particularly pronounced in the Asia-Pacific region, where rapid demographic and lifestyle transitions have led to a surge in non-communicable diseases. The Asian-Pacific Society of Hypertension (APSH) and the Diabetes Asia Study Group (DASG) have jointly developed this consensus to provide region-specific, evidence-based recommendations for managing hypertension in individuals

Role of natural inter-individual variability in the different penetrance of congenital long QT syndromes (LQTS) types 1 and 2: an in silico approach.

Congenital Long QT Syndrome types 1 and 2 (LQT1 and LQT2) are caused bymutations in the KCNQ1 and KCNH2 genes, responsible for the IKs and IKr currents, respectively. However, the penetrance of these mutations is highly variable, since some carriers remain asymptomatic, while others exhibit severe clinical manifestations. To elucidate how physiological variability in other cardiac ionic currents may influence the arrhythmic phenotype, human ventricular action potentials were simulated in silico using the O'Hara-Rudy dynamic model implemented in the Virtual Assay software. Conductances of nine

The Effect of Rate-Controlling Medication on the Performance and Outcome of Dobutamine Stress Echocardiography in the Assessment of Patients with Suspected Angina: A Retrospective Cohort Study.

Background/Objectives: Stress echocardiography (SE) had been recommended by professional societies for assessing patients with suspected angina. SE protocols are variable across hospitals and countries in the recommendation of the cessation of rate-controlling medication (RCMx) prior to SE. Some expert opinion papers recommend the cessation of beta receptor blockers (BBs) and rate-controlling calcium channel blockers 48 h prior to SE to improve the diagnostic accuracy of the test. There is no evidence that the continuation of RCMx can affect the outcome of SE and short-term major adverse cardi

Targeting the US21 viroporin of human cytomegalovirus by calcium channel blockers as a new antiviral strategy.

The Human cytomegalovirus (HCMV) US21 protein is a calcium-conducting viroporin that modulates intracellular Ca2+ homeostasis, safeguards cells from apoptosis, stimulates cell migration, and supports efficient HCMV replication. To validate pUS21 as a novel target for the identification of antiviral agents, in silico structure-based virtual screening was performed using its predicted structure to identify small molecules capable of engaging the inner part of the pore. Four dihydropyridine compounds (azelnidipine, efonidipine, lercanidipine, and niguldipine) were selected from 249 Calcium Channe

CACNA1C as a Prognostic Biomarker and Therapeutic Target in High-Grade Serous Ovarian Cancer: Clinical Validation and Molecular Dynamics of Nifedipine Blockade.

High-Grade Serous Ovarian Cancer (HGSOC) is the most lethal gynecological malignancy due to aggressive growth, widespread metastases, and high intra-tumoral heterogeneity. Poor prognosis is largely due to late diagnosis, hence there is an urgent need to identify novel biomarkers for screening, diagnosis, and monitoring. Here, we propose the voltage-dependent calcium channel hCaV1.2 encoded by CACNA1C as a potential biomarker and therapeutic target in HGSOC. Using IHC analysis for ten ovarian cancer patients, cytotoxicity assay, TCGA gene expression and survival analyses, homology modeling, mol

Suppression of Calcium Influx Reduces Coxsackievirus B3 Lethality With Elevated Interferon Beta Responses in Mice.

The disease course of severe enterovirus (EV) infection with hepatic necrosis is usually fulminant and fatal. Coxsackievirus (CVB3) is the emerging and important serotype of newborn EV inducing hepatic necrosis. Calcium (Ca2+) regulates host immunity, type I interferon (IFN-I), and viral infections. The interaction of Ca2+, IFN-I, and CVB3-induced hepatitis remains to be investigated. To address these issues, we used the Ca2+ blocker manidipine, which reduces Ca2+ influx into cells and is used in the clinic, the human hepatoma cell line (HuH7) for in vitro studies, and a murine infection model

Hypertensive retinopathy as a masquerade neuroretinitis in a child: a case report.

BACKGROUND: High blood pressure in children is rare and often underdiagnosed, but it can cause irreversible damage to target organs. Ocular signs of hypertensive retinopathy are nonspecific and may resemble those of neuroretinitis or papilledema, which can delay diagnosis. CASE PRESENTATION: A 9-year-old girl presenting with bilateral visual loss was referred to our emergency department. Best-corrected visual acuity was 20/200 in the right eye and counting fingers in the left. Fundus examination revealed bilateral optic disc swelling with a macular star and flame-shaped hemorrhages. Blood Pressure Variability (BPV) as a Novel Digital Biomarker of Multisystem Risk and Diagnostic Insight: Measurement, Mechanisms, and Emerging Artificial Intelligence Methods.

Hypertension has been traditionally known to be highlighted by mean blood pressure; however, emerging evidence exhibits that blood pressure variability (BPV), including short-term, day-to-day, and visit-to-visit fluctuations can have an implication across multiple body systems. Elevated BPV reflects repetitive hemodynamic stress, affecting the physiologic hemostasis contributing to vascular injury and end organ damage. This narrative review is a compilation of recent evidence on the prognostic value of BPV, explained by pathophysiology, various devices with its measurement approaches, and, ess

24-hour diastolic blood pressure average real variability predicts renal progression in diabetic kidney disease: a comprehensive cohort study.

OBJECTIVE: Diabetic kidney disease (DKD) is the leading cause of end-stage renal disease (ESRD) worldwide. Blood pressure variability (BPV), especially diastolic BPV (DBPV), is closely linked to renal microcirculation but remains understudied in DKD. This study aimed to evaluate the association of DBPV with renal progression, identify an optimal risk threshold, and explore antihypertensive drug implications. METHODS: We conducted a retrospective cohort study of 2,143 DKD patients who underwent 24-hour ambulatory BP monitoring (ABPM) between 2018 and 2022, with a median follow-up of 4.8 years.

Shear Stress Promotes Metastasis of Triple-negative Breast Cancer Cells Through Calcium Channel-ROS-FOS Axis.

During metastasis, circulating tumor cells (CTCs) are subjected to fluidic shear stress (SS), which eliminates many of them but paradoxically enhances malignancy and metastatic success. Meanwhile, given the critical roles of reactive oxygen species (ROS) in stress response and cancer, we engineer a circulation-mimicking microfluidic system which generates pulsatile SS to investigate the interplay among SS, ROS and metastasis. A 3-hour SS treatment rapidly elevates ROS levels, boosting metastatic abilities in triple-negative breast cancer (TNBC) cells in vitro and in vivo. RNA-sequencing and su

Altered molecular signaling pathways in the hippocampus of rhesus monkeys following chronic alcohol use.

Context-induced relapse is a significant factor limiting recovery from alcohol use disorder (AUD). However, the molecular processes in the hippocampus, critical for contextual memory, impacted by chronic alcohol use, remain poorly understood. We used a non-human primate model to test the hypothesis that chronic alcohol use impacts hippocampal molecular pathways that may serve as therapeutic targets for memory processing in chronic alcohol use. We conducted RNAseq profiling on hippocampal samples from adult male rhesus monkeys with chronic alcohol use (n = 7) and controls (n = 5). We identified

Genetics of bipolar disorder: Translating discoveries to biological insights and clinical decision support.

Bipolar disorder (BD) is a clinically heterogenous mental disorder with a diversity in clinical trajectories and treatment response. Twin studies have shown that BD is highly heritable, with estimates ranging from 60% to 90%. However, our understanding of this genetic component has yet to be translated into clinical interventions or prediction tools. In this review, we summarize findings from large-scale international collaborations, with the latest and largest study finding 298 genome-wide significant loci and 36 credible mapped genes associated with BD, alongside the first genes with an incr

Crotoxin B from the South American Rattlesnake Crotalus vegrandis Blocks Voltage-Gated Calcium Channels Independent of Its Intrinsic Catalytic Activity.

Neurotoxicity following South American Crotalus rattlesnake bite is primarily caused by crotoxin, the most abundant component in their venom. Despite the central role of voltage-gated calcium channels (CaV) in neurotransmission, direct targetability by crotoxin has been poorly explored. Crotoxin is a non-covalent heterodimer formed by an acidic subunit (CA) and a basic toxic phospholipase A2 subunit (CB). Here, we chromatographically isolated the CB subunit from Crotalus vegrandis and studied its effect on CaV heterologously expressed in tsA201 cells using the whole-cell patch-clamp technique.

Laser-Assisted Management of Amlodipine-Induced Gingival Overgrowth in a Medically Complex Patient.

BACKGROUND: Drug-induced gingival overgrowth (DIGO) is a well-recognized adverse effect associated with several systemic medications, including calcium channel blockers such as amlodipine. Management becomes particularly challenging in medically complex patients requiring continuous anticoagulant and antiplatelet therapy, in which conventional surgical approaches may increase the risk of perioperative bleeding. CASE PRESENTATION: We report the case of a 66-year-old male patient with severe, multifocal amlodipine-induced gingival overgrowth associated with generalized severe periodontitis (Sta
