Comparison of non-dihydropyridine calcium channel blockers with beta blockers as treatment for atrial fibrillation: protocol for a systematic review with meta-analysis.

BACKGROUND: Heart rate control is one of the cornerstones of atrial fibrillation (AF) management and is recommended in all AF patients, even as background therapy for rhythm control. Both non-dihydropyridine calcium channel blockers and beta blockers are recommended as first choice rate control drugs, but no preference is given. Even though there are important differences in pharmacological mechanisms and side effects between these drugs, large randomized controlled trials are lacking. We aim to critically evaluate and synthesize the scientific evidence comparing calcium channel blockers and b

Ethosuximide and Irritable Bowel Syndrome-Related Abdominal Pain: A Randomized Clinical Trial.

IMPORTANCE: T-type calcium channels, particularly the Cav3.2 subtype, are involved in pain transduction. Experimental studies and human data suggested that increased T-type channels activity or expression contributes to visceral hypersensitivity in irritable bowel syndrome (IBS), and that their inhibition alleviates pain. OBJECTIVE: To evaluate the therapeutic potential of ethosuximide, a T-type calcium channel blocker, for IBS-related abdominal pain. DESIGN, SETTING, AND PARTICIPANTS: This proof-of-concept, multicenter, double-blinded, placebo-controlled randomized clinical trial started in

Inhibition of store-operated calcium entry mitigates lipopolysaccharide-induced hepatotoxicity via modulating inflammatory and oxidative stress responses.

IntroductionHepatotoxicity can arise secondary to several medical conditions, including inflammation, sepsis, and therapy. Lipopolysaccharide (LPS) can activate diverse inflammatory pathways and has been implicated in hepatotoxicity. The store-operated calcium entry (SOCE), a key process for maintaining cellular calcium homeostasis, was shown to modulate inflammatory signaling. Reactive oxygen species, which serve a significant role in maintaining cellular function and homeostasis, are often elevated during inflammation, contributing to tissue injury. Therefore, we hypothesized that blocking t

The effects of sub-chronic calcium treatment on ethanol-induced dopamine elevation and the alcohol deprivation effect in the rat.

Alcohol use disorder (AUD) is a serious mental health condition and a risk factor for morbidity and preterm death. The drug acamprosate (Campral® - calcium-bis[N-acetylhomotaurinate]) is one of few pharmacological treatments available for AUD. Recent research suggests that the properties of acamprosate may be attributed to calcium, but the acute and long-term effects by calcium supplementation on ethanol-induced dopamine release and relapse-like drinking is not fully known. We used in vivo microdialysis and the alcohol deprivation model, to further define the interaction of local or systemic c

Coronary Artery Vasospasm Presenting as Non-ST Segment Elevation Myocardial Infarction in a Nigerian Female: A Case Report.

Coronary artery spasm (CAV) is characterized by transient vasoconstriction of normal or diseased epicardial coronary arteries, with a resultant reduction in myocardial perfusion, ischemia, infarction, or sudden cardiac death. Although the precise pathophysiologic mechanism of CAV remains unclear, vascular smooth cell contraction constitutes a core underlying feature. A 46-year-old Nigerian woman with pre-existing hypertension was admitted following an episode of chest pain; subsequent elevation of cardiac biomarkers and electrocardiogram showing T-wave inversions in the anterolateral and infer

S100A4 triggeres the pyroptosis of vsmcs: association with mitochondrial damage, impaired mitophagy, and Ca2+ dysregulation.

BACKGROUND: Atherosclerotic occlusive disease of the lower extremities (ASO-LE) is associated with high rates of disability and mortality, and even after surgery, the rate of long-term lumen patency remains low. Vascular smooth muscle cells (VSMCs) are key players in atherosclerosis. The aim of this study was to investigate the action of S100 calcium-binding protein A4 (S100A4) in regulating VSMCs in ASO-LE. RESULTS: The overexpression of S100A4 in the vessel wall of ASO-LE is detected, and the expression of markers of pyroptosis is high. S100A4 causes mitochondrial damage in VSMCs, increases

Targeting Vascular Calcification: Novel Insights into Molecular Pathways and Clinical Interventions.

Vascular calcification (VC), a pathological hallmark of advanced atherosclerosis, exerts a profound impact on arterial stiffness and the incidence of cardiovascular disease. Its development involves not only actively regulated biological processes but also passive mineral deposition and is particularly prevalent in patients with diabetes, chronic kidney disease, and endocrine disorders. Although conventional therapies-such as phosphate binders, calcium channel blockers, bisphosphonates, and endovascular interventions-form the current clinical foundation, they remain inadequate for preventing e

Social media data and its potential for pharmacovigilance: a comparative analysis of reported prevalences regarding drug-induced gingival overgrowth (DIGO).

Drug-induced gingival overgrowth (DIGO) is an increasingly relevant topic among dentists, physicians and patients as well. Due to high prescription rates of calcium channel blockers, anticonvulsants and immunosuppressants, the prevalences of this adverse effect should not be underestimated. To identify differences in the prevalence rates presented, but also possible similarities in the available data, we compared information from package leaflets, social media platforms, current literature and summaries of product characteristics. To obtain a comprehensive overview of the current situation reg

Elevated plasma aldosterone-to-renin ratio as a potential risk marker of adverse left ventricular remodeling: a cross-sectional study.

BACKGROUND: Recent evidence has suggested that primary aldosteronism (PA) is the predominant cause of secondary hypertension and is linked to adverse left ventricular (LV) remodeling. However, few studies have investigated the potential associations of aldosterone-to-renin ratio (ARR), an important parameter for PA screening, with the risk of adverse LV remodeling in the Chinese population. This study aimed to investigate the associations of ARR, plasma aldosterone concentration (PAC), and plasma renin concentration (PRC) with adverse LV remodeling in a population from Guangxi, China. METHODS

Inhibition of Calcium-Dependent Lipid Droplets Relocation of ACSL4-PKCβ-ALOX15 Complex Alleviates Ferroptosis and Acute Pancreatitis.

Ferroptosis, an iron-dependent form of programmed cell death driven by toxic lipid peroxide accumulation, plays a critical role in various diseases, making its modulation a promising therapeutic strategy. In this study, we identified several L-type calcium channel blockers as novel inhibitors of ferroptosis. We further elucidated that calcium-dependent activation of PKCβ drives ferroptosis by phosphorylating two key enzymes, ACSL4 and ALOX15, at multiple sites. We generated phosphorylation-specific antibodies targeting these sites and confirmed their specificity in the context of ferroptosis.

Selective RyR2 inhibition reduces arrhythmia susceptibility in human cardiac slices.

Ryanodine receptor 2 (RyR2) hyperactivity is frequently observed in structural heart disease (SHD), commonly caused by ischemic heart disease. This aberrant Ca2+ release promotes irregular electrical activity and life-threatening arrhythmias. Our previous work demonstrated that the pan-ryanodine receptor (RyR) inhibitor dantrolene can reverse arrhythmogenic substrates, but its lack of RyR2 selectivity limits its therapeutic potential. The unnatural verticilide enantiomer (ent-verticilide) was identified as a selective RyR2 inhibitor, showing promising specificity without activating skeletal mu

Design and Characterization of a Dual COX-2/CaV2.2 Inhibitor with Potent Analgesic Activity.

Cyclooxygenase-2 (COX-2) and N-type voltage-gated calcium channels (CaV2.2) play pivotal roles in mediating inflammatory responses and regulating neuronal excitability in chronic pain. Concurrent modulation of these targets can achieve synergistic analgesic effects. We designed and synthesized a series of diarylpyrazole-based dual COX-2/CaV2.2 inhibitors. Structure-activity relationship analysis identified compound 5d as the lead candidate, exhibiting balanced inhibitory potency and favorable selectivity against COX-2 and CaV2.2, with IC50 values of 0.26 ± 0.17 μM and 0.29 ± 0.07 μM, respectiv

Amlodipine Suppresses Lung Metastasis by Increasing Cell Stiffness.

BACKGROUND/AIM: Osteosarcoma is the most common primary malignant bone tumor in children and adolescents, and lung metastasis is the major cause of death. Highly metastatic osteosarcoma cells are mechanically softer than low-metastatic cells, and calcium ions are known to regulate cytoskeletal organization and cell stiffness. We tested whether clinically available ion channel blockers could increase the stiffness of LM8 osteosarcoma cells and prevent lung metastasis in vivo. MATERIALS AND METHODS: We evaluated the actin cytoskeletal structure and polymerization in ion channel blocker-treated

Calcium Channel Blockers Inhibit Pancreatic Neuroendocrine Neoplasms Progression via Cav1.2-Epigenetic Circuit.

Pancreatic neuroendocrine neoplasms (pNEN) are rarely encountered, accounting for about 2% of all pancreatic neoplasms. Disease progression is frequently observed as recurrence or distal metastasis. Mechanisms underlying pNEN progression are still poorly investigated, and treatments against pNEN are challenging due to the pronounced neoplastic heterogeneity. Here, by performing clinicomolecular analysis, we report a novel mechanism of positive regulatory circuit between Cav1.2-mediated calcium signaling and epigenetic control by H3K27 acetylation (H3K27ac). Tumor-cell-specific expression of Ca
