Targeting Myocardial Energetics in Nonobstructive Hypertrophic Cardiomyopathy: The Role of Ninerafaxstat.

Nonobstructive hypertrophic cardiomyopathy (nHCM) is a condition that has significant therapeutic challenges due to its complicated pathophysiology. Beta-blockers and calcium channel blockers are the existing treatment modalities that offer limited symptom relief and fail to address underlying metabolic deficits. This review evaluates emerging targeted therapies, focusing on ninerafaxstat, a cardiac metabolic modulator that inhibits 3-ketoacyl-CoA thiolase to shift myocardial substrate utilization from fatty acids to glucose, thereby improving energy efficiency. Clinical trials of novel agents

TREAT: A Multicentre Cross-sectional TReatment Evaluation of Apparent Resistant hyperTension in Belgium.

The goal of the TREAT study is to estimate the prevalence of true resistant hypertension in a Belgian setting of patients with apparent treatment resistant hypertension (aTRH) and to evaluate which determining factor of pseudo-resistant hypertension contributes the most to this prevalence. TREAT is a single-visit, multicentric, non-interventional, cross-sectional survey. Inclusion criteria included: patients over 18 years receiving 3 or more antihypertensive molecules and referred to specialized hospital centers for aTRH. A total of 201 eligible patients were included. Only 56.7% of the patien

A multi-centre study of prescribing patterns for the management of resistant hypertension amongst cardiologists.

Resistant hypertension is conventionally defined as failure to achieve blood pressure control despite treatment with optimal or maximally tolerated doses of three antihypertensive drugs, typically including a renin-angiotensin system blocker, a calcium channel blocker, and a diuretic. In real-world practice, however, many patients receive four or more antihypertensive agents without necessarily fulfilling classical resistant hypertension criteria, and data describing such prescribing patterns among cardiologists are limited. We conducted a prospective, multicentre, cross-sectional observationa

Uncontrolled hypertension in siblings: an unsuspected diagnosis.

Hypertension in the young is defined as the occurrence of elevated blood pressure in patients aged <40 years. It is unusual for hypertension to be present before the second decade of life and more so in a pair of siblings. This study aimed to report a case of a sibling pair (18- and 16-year-olds), reared as females, who presented with primary amenorrhea and resistant hypertension. The initial presentation was hypertension. The siblings were treated with escalating doses of antihypertensive agents, including calcium channel blockers, beta-blockers, and diuretics, with suboptimal control. Primar

Incidence and risk factors of calcium channel blocker-related edema in hypertensive patients: A Multicenter retrospective cohort study.

BACKGROUND: Hypertension is a major risk factor for cardiovascular disease and remains the leading cause of mortality worldwide. Calcium channel blockers (CCBs) are commonly used to lower blood pressure because they are effective and affordable. However, CCBs can cause vasodilatory adverse effects, including peripheral edema, which may lead to additional therapy and affect adherence. This study assessed the incidence and risk factors of CCB-related edema among hypertensive patients in Ethiopia. METHODS: This retrospective multicenter cohort study involved interviews and reviews of medical rec

High-dose insulin euglycaemic therapy for a severe calcium channel blocker overdose.

Calcium channel blockers are a common cause of poisoning and are associated with high mortality in affected patients. Treatment is complex. Although poorly researched, high-dose insulin euglycaemic therapy (HIET) appears to be an effective treatment. In this case report, we review a patient with a severe calcium channel blocker overdose who was successfully treated with HIET.

Incidental Myocardial Bridging Coinciding With Myocardial Ischemia and Takotsubo Cardiomyopathy in an Asymptomatic Patient.

Although most cases of myocardial bridging (MB) are clinically benign, it can sometimes be a risk factor for myocardial infarction (MI) and life-threatening arrhythmia. In this study, we present a case of MB-induced ST-segment elevation MI in a patient with stress cardiomyopathy. A 71-year-old man presented to the emergency department for evaluation of hepatic encephalopathy. The initial ECG was consistent with generalized ST elevation between leads 1, 2, 3, and V1-V6. Coronary angiography showed a segment of severe myocardial bridging in the mid-distal left anterior descending (LAD). Bridging

Mechanistic insights into nicardipine's anti-colitic action: inhibition of pro-inflammatory cytokines, attenuation of oxidative stress, and protection of the mucosal barrier.

BACKGROUND: Ulcerative colitis (UC) is a chronic inflammatory bowel disease characterized by excessive immune activation and oxidative stress. UC is characterized by elevated inflammatory cytokine levels. Nicardipine is primarily used for the management of hypertension and angina. Beyond its cardiovascular applications, nicardipine has demonstrated anti-inflammatory properties in various experimental disease models. This study investigates the therapeutic role of nicardipine in a murine model of dextran sulfate sodium (DSS)-induced colitis. METHOD: Fifty mice were divided into control, DSS-in

L-type calcium channel-mediated lipid metabolic reprogramming in gastric adenocarcinoma progression.

BACKGROUND: l-type calcium channels (LTCCs) play an important role in tumorigenesis, but their expression profile, functional significance, and therapeutic potential in gastric adenocarcinoma (GAC) remain unclear. Concurrently, aberrant lipid metabolism is widely acknowledged as a key driver of GAC progression, but its upstream regulatory mechanisms have yet to be fully elucidated. This study was designed to explore the association between LTCCs and lipid metabolic reprogramming in GAC and to investigate its influence on GAC progression and clinical outcomes. METHODS: The expression profiles

Pharmacological and non-pharmacological management of long COVID.

Coronavirus disease 2019 (COVID-19), caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), has caused a major global health burden in terms of acute infection and long-term consequences. Approximately 10% of infected experience autonomic dysfunction, cardiovascular complications, and neurological impairments. While immune dysregulation, persistent viral reservoirs, chronic inflammation, gut dysbiosis, and vascular dysfunction are implicated, the exact pathophysiological mechanisms of Long COVID remain unclear. Additionally, treatment options are limited and challenging to pre

Brain penetrant calcium channel blockers do not reduce alcohol consumption: Converging results from two large independent cohort studies using electronic health records.

Alcohol use disorder (AUD) remains a major public health problem, with few effective medications and suboptimal adherence. L-type calcium channel blockers (LTCCBs) have genetic and preclinical support as potential treatments for AUD. We evaluated whether brain penetrant (BP)-LTCCBs are associated with reduced alcohol consumption by conducting two preregistered (https://osf.io/huawv) observational cohort studies using electronic health records (EHRs) from the US Department of Veterans Affairs (VA) and Kaiser Permanente Northern California (KPNC). New users of BP-LTCCBs (nifedipine or felodipine

Preoperative first-line amlodipine in phaeochromocytoma/paraganglioma: perioperative haemodynamic instability and its predictors.

OBJECTIVES: Preoperative alpha blockade is recommended for phaeochromocytomas and paragangliomas (PPGLs), whereas evidence for calcium channel blockers remains limited. This study evaluated perioperative haemodynamic outcomes with first-line amlodipine for preoperative blockade and identified predictors of intraoperative haemodynamic instability (iHDI). METHODS/DESIGN: In this monocentric retrospective study, 35 operated PPGL patients who received preoperative first-line amlodipine (July 2021-March 2024) were analysed. Continuous intraoperative arterial pressure recordings were analysed for e

Renin-angiotensin-aldosterone system inhibitors and risk of pancreatic cancer in older adults: A target trial emulation.

Pancreatic ductal adenocarcinoma (PDAC) is a major cause of cancer-related mortality worldwide. Preclinical studies suggest that the renin-angiotensin-aldosterone system (RAAS) inhibitors may help prevent PDAC by promoting acinar ductal metaplasia reversal, as the RAAS pathway is most inhibited during this process. Our study aims to examine the association between RAAS inhibitor use and the risk of PDAC compared with calcium channel blockers (CCB). Leveraging the target trial emulation framework, we used US Medicare data to emulate a new user, active comparator trial examining the effects of i

Repurposing T-type calcium channel blocker lomerizine as a therapeutic strategy for glioblastoma.

Glioblastoma (GBM) is the most malignant primary brain tumor. The presence of glioma stem/initiating cells (GICs) is known to cause strong treatment resistance; therefore, GICs are a major target for GBM therapy, although there are no therapies targeting GICs clinically. To identify novel treatments for GBMs, we performed drug repurposing screening using GICs and identified the T-type calcium channel blocker lomerizine - a migraine prophylactic drug. Lomerizine inhibited proliferation, migration, invasion, and cell cycle progression and induced apoptosis in GICs and differentiated glioma cells

Beta-blocker use and outcome after allogeneic hematopoietic stem cell transplantation in acute myeloid leukemia.

Allogeneic hematopoietic stem cell transplantation (HSCT) is curative for patients with acute myeloid leukemia (AML) through an immune-mediated graft-versus-leukemia effect, but relapse is common. Studies in solid tumors have shown that adrenergic stress impairs anti-tumor immunity, and that beta-blockade can prolong survival in mice and patients receiving immunotherapy. To this end, we investigated whether beta-blocker use impacts outcomes in AML patients undergoing HSCT. We analyzed the cumulative incidences of relapse (CIR), non-relapse mortality (NRM) and overall survival (OS) in 413 patie

Dual Case Report: Symptomatic Myocardial Bridging Managed with Calcium Channel Blockers.

BACKGROUND: Myocardial bridging (MB) is a congenital coronary artery anomaly in which a segment of an epicardial coronary artery, most commonly the left anterior descending (LAD) artery, courses intramyocardially, and undergoes systolic compression. Although historically regarded as a benign anatomical variant, myocardial bridging has increasingly been recognized as a potential cause of myocardial ischemia, exertional chest pain, and other cardiac symptoms, particularly in patients without obstructive coronary artery disease. Awareness of this condition is important, as its diagnosis and manag
