Recommendations from the Clinical Toxicology Recommendations Collaborative on the administration of activated charcoal in acute oral overdose.

INTRODUCTION: The Clinical Toxicology Recommendations Collaborative was established by three international clinical toxicology societies and tasked to produce recommendations on the management of poisonings. The Activated Charcoal in Clinical Toxicology Workgroup (the Workgroup) was formed to provide recommendations on the administration of activated charcoal for gastrointestinal decontamination and enhanced elimination in poisoning. METHODS: Based on a systematic review of the literature, 43 poisons or poison categories were selected for appraisal. Voting statements were drafted using a pred

Outcomes of extra-dose exposures of Vaughan Williams Class I or III antidysrhythmic medications.

INTRODUCTION: Antidysrhythmic medications are commonly prescribed, but there is little information on the incidence of adverse effects following an inadvertent extra dose of these medications. This poses a challenge to poison centers when trying to decide which patients need to be sent to a healthcare facility for monitoring. METHODS: A retrospective data review of cases reported to a regional poison center between January 1, 2012 and December 31, 2022 was performed. Cases were included if the exposure was a double dose or less of the patient's own antidysrhythmic medication, the exposure was

Comprehensive review of cardiovascular toxicity of drugs and related agents.

Cardiovascular diseases are a leading cause of morbidity and mortality in most developed countries of the world. Pharmaceuticals, illicit drugs, and toxins can significantly contribute to the overall cardiovascular burden and thus deserve attention. The present article is a systematic overview of drugs that may induce distinct cardiovascular toxicity. The compounds are classified into agents that have significant effects on the heart, blood vessels, or both. The mechanism(s) of toxic action are discussed and treatment modalities are briefly mentioned in relevant cases. Due to the large number

Brugada Pattern Caused by a Flecainide Overdose.

BACKGROUND: Brugada pattern can be found on the electrocardiogram (ECG) of patients with altered mental status, usually with fever or drug intoxication. Diagnosis remains challenging, because the ECG changes are dynamic and variable. In addition, triggers are not always clearly identified. In patients with atrial fibrillation (AF), the use of class IC antidysrhythmic drugs can unmask a Brugada pattern on the ECG, especially if combined with other medications acting on sodium channels. CASE REPORT: A 62-year-old man with a medical history of AF was admitted to our emergency department for alte

Successful treatment of flecainide-induced cardiac arrest with extracorporeal membrane oxygenation in the ED.

Flecainide is a class Ic antidysrhythmic agent used to prevent and treat tachydysrhythmias. Flecainide toxicity primarily causes cardiovascular and neurologic effects through sodium-channel blockade. There is scant evidence to support specific management, and recommended therapies have been extrapolated from management of other sodium-channel blocking drugs. Traditionally, these therapies have consisted of intravenous fluids, sodium bicarbonate, vasopressors, and cardiac pacing. Novel therapies include intravenous fat emulsion and venoarterial extracorporeal membrane oxygenation (VA-ECMO). A 2

Cardiotoxicity associated with intentional ziprasidone and bupropion overdose.

BACKGROUND: Ziprasidone and bupropion are medications prescribed for mood and behavior disorders. They have apparently safe cardiac safety profiles in both therapeutic and supratherapeutic doses, but recently the Federal Drug Administration has issued a caution regarding ziprasidone use in combination with other drugs that are known to prolong the QTc interval. CASE REPORT: A 17-year-old male developed a widened QRS and a prolonged QTc interval following an overdose of ziprasidone and bupropion. He required hospital admission for aggressive cardiac monitoring and antidysrhythmic therapy, stab

Cocaine-induced wide complex dysrhythmia.

Cocaine is a local anesthetic with the potential to induce dysrhythmia due to direct myocardial sodium channel antagonism similar to class I antidysrhythmic drugs. The hallmark of myocardial sodium channel poisoning is wide complex dysrhythmia, and the current accepted treatment is intravenous bicarbonate. Wide complex dysrhythmio due to cocaine in the absence of myocardial infarction is rare, and optimum management is undefined. We report three cases of acute cocaine intoxicating during which patients developed wide complex dysrhythmia consistent with sodium channel poisoning. In one case, wi

Significance of metabolism in the disposition and action of the antidysrhythmic drug, dofetilide. In vitro studies and correlation with in vivo data.

Dofetilide, a class III antidysrhythmic agent, undergoes both renal and metabolic clearance. Characterization of the metabolism in vitro allows explanation of species differences, whereas identification of the human enzymes involved permits assessment of potential drug interaction. In liver microsomes, the rate of oxidative metabolism of dofetilide is in the order: male rat > female rat > dog > humans, which correlates with the metabolic clearance seen in vivo. In vitro products of oxidative metabolism, formed by N-dealkylation, are the same as those formed in vivo, with the N-desmethyl being

[Cardiotoxicity of a selected group of drugs].

Drugs and other chemical substances can impair the heart in any of its functions--electrical activity, mechanical activity and metabolism, most frequently by a graded pharmacological action or a direct toxic effect, or immunological mechanisms (hypersensitive reaction). The drugs with the potentially greatest cardiotoxic effect on the function of the heart include beta 1-sympathomimetics, beta 1-blockers, cardiac glycosides, antidysrhythmics, inhalational general anaesthetics, methylxanthine, and anthracycline cytostatics. Other drugs, which are mainly of toxicological importance and which can

Measurement of the class III antidysrhythmic drug, UK-68,798, in plasma by radioimmunoassay.

A sensitive radioimmunoassay (RIA) for the specific determination of 1-(4-methanesulphonamidophenoxy)-2-[N-(4-methanesulphonamido -phenethyl)-N- methylamino]ethane (UK-68,798), a novel class III antidysrhythmic agent, in human plasma is described. Specific antisera were raised in sheep using desmesyl-UK-68,798-succinate-ovalbumin conjugate as the antigenic hapten carrier protein. The antisera produced exhibited high specificity for UK-68,798 compared with known metabolites from animals, other antidysrhythmic agents and co-administered drugs. Good correlation was found in a comparison of the RI

First degree atrioventricular block.

Prolongation of the PR interval is a nonspecific condition. Etiologies include normal physiologic variance, idiopathic bundle branch fibrosis, calcific valvular heart disease, ischemic heart disease, cardiomyopathies, infections resulting in myocarditis, drugs (especially type I antidysrhythmic medications), collagen vascular diseases, tumors, trauma, electrolyte imbalances, and many other miscellaneous causes. Isolated first degrees atrioventricular block is generally not associated with an increased risk of sudden death, syncope, or progression to higher degrees of atrioventricular block. Ex

Antiarrhythmic activity of cetiedil and its analogues.

Cetiedil), originally a molecule used for treatment of some vascular diseases, exhibits several pharmacological effects. The authors studied cetiedil's stabilizing effect on the membrane and the antagonism against calcium of Cetiedil and its analogues (amino 3-thienylacetic esters). After completion of numerous pharmacological tests giving evidence of antidysrhythmic effects of the molecule and more especially in the digitalic arrhythmias and ventricular fibrillation induced by calcium (where it is more active than other products tested), the authors conclude that Cetiedil might enter in group

Emergency management of acute myocardial infarction by the general practitioner.

In the Cape GP Emergency Coronary Care Project, 129 general practitioners (GPs) collaborated over a 14-month period. The objectives were to reach all patients with symptoms of acute myocardial infarction as promptly as possible, and to institute therapy according to a simple protocol, where the emphasis was on the prophylactic administration of antidysrhythmic drugs. The results showed that 38% of a total of 333 patients received treatment within 1 hour of the onset of their symptoms, and 75% within 4 hours. The mortality at the end of 1 month was 15% in 333 patients of all ages, and 10,7% for
