Skeletal muscle fibrosis is a pathological process characterized by excessive deposition of extracellular matrix (ECM). It commonly occurs in various diseases such as muscular dystrophy, aging, cancer cachexia, and muscle injury. This condition leads to destruction of muscle structure, loss of function, and impaired regeneration, significantly affecting patients' quality of life. This review systematically summarizes the molecular mechanisms underlying skeletal muscle fibrosis. Key signaling pathways include transforming growth factor-beta (TGF-β)/Smad, yes-associated protein/transcriptional c

Treating hypertension in diabetic patients with antihypertensive medications should be done with careful consideration because these medications may interfere with macro- and microvascular complications of diabetes, such as myocardial infarction, stroke, and diabetic nephropathy. Some medications also come with cardio- and reno-protective effects along with lowering blood pressure. This narrative review focuses on major antihypertensive drug classes and their efficacy, safety, and metabolic impacts in type 1 and type 2 diabetes mellitus. We found that most classes of drugs, such as ACEIs (Angi

Claims-based analyses can suffer from residual and unmeasured confounding due to factors that are poorly captured in claims. Some of these factors may be measured in other data sources, such as in structured fields of electronic health records (EHR), for example, laboratory test results, or in free-text physician notes. We conducted a proof-of-principle study to demonstrate a process for evaluating the potential risk of confounding-factors poorly captured in claims data but measurable in the EHR as part of drug safety surveillance activities. In future practical applications, this approach cou

BACKGROUND: Inflammation-driven mechanisms play a central role in adverse outcomes after non-ST-elevation myocardial infarction (NSTEMI), yet simple, widely available biomarkers for early risk stratification remain insufficiently defined. Hemogram-derived indices and iron-related inflammatory markers may provide complementary prognostic information.
OBJECTIVE: To evaluate the prognostic significance of the mean platelet volume-to-monocyte ratio (MMR) and serum ferritin in predicting major adverse cardiovascular events (MACE) in patients with NSTEMI, and to assess the association of angiotensi

This study aimed to investigate the effects of ACE inhibitors (ACEIs) and angiotensin II receptor blockers (ARBs) on pathological scar formation, following median sternotomy, focusing on the role of the renin-angiotensin system (RAS) in tissue remodelling and wound healing. Designed as a cross-sectional observational study, it included 100 patients who had undergone median sternotomy at least 6 months prior and were receiving continuous antihypertensive treatment. Patients were categorised into three groups based on their medications: ACEIs (n = 33), ARBs (n = 35), and other antihypertensives

High-altitude hypoxia and cold stress challenge broiler production by inducing oxidative stress and vasoactive imbalances, primarily through activation of the renin-angiotensin system (RAS). This study investigated whether dietary captopril, an ACE inhibitor, could mitigate these adverse effects. 240 one-day-old broilers were randomly assigned to four diets: a basal control diet or the basal diet supplemented with 10 (CAP10), 20 (CAP20), or 30 (CAP30) mg captopril/kg feed. Birds were reared at 2100 m altitude under cold stress. Growth performance, carcass characteristics, ascites-related morta

Chronic kidney disease (CKD) is a major global public health issue. The primary causes include diabetes mellitus, hypertension, and glomerular diseases, which involve different pathophysiological and molecular mechanisms, but lead to similar outcomes. In this review, the authors examine the role of pharmacological agents that modify the renin-angiotensin system (RAS) in patients with renal diseases and significant proteinuria. The use of these agents can lead to a reduction in albuminuria and proteinuria, stabilize renal function during follow-up, and slow the progression of the disease. Clini

The rapidly expanding body of biomedical literature encompasses a wealth of information concerning the pharmacological effects, mechanisms of action, adverse reactions, and repurposing potential of small-molecule therapeutics. Nevertheless, the systematic extraction and integration of this knowledge continue to pose substantial challenges. In this study, we propose an integrated text-mining framework for the automated extraction and structured representation of information on the biological activities of low-molecular-weight compounds, exemplified by angiotensin-converting enzyme (ACE) inhibit

Background and Objectives: Renin-angiotensin system inhibitors (RASIs) are recommended to preserve residual kidney function (RKF) in patients on peritoneal dialysis (PD); however, evidence of benefit is inconsistent. This study evaluated the effect of RASI on RKF decline among patients undergoing PD. Materials and Methods: We conducted a retrospective cohort study among PD patients at a large metropolitan dialysis centre in Australia. RKF was assessed using residual Kt/V and urine volume from PD adequacy tests. Time zero was PD initiation. RASI exposure was modelled as a time-dependent variabl

It is still unclear whether hypertensive patients using angiotensin II receptor blockers (ARBs) have a different risk of developing dementia compared with those taking angiotensin-converting enzyme inhibitors (ACEIs). We conducted a prospective cohort study using the 45 and Up Study Cohort in patients aged ≥ 45 years with hypertension who initiated ARB or ACEI therapy between 2004 and June 30, 2022. ARB/ACEI exposure was defined as having a supply that covered at least 80% of their follow-up period. Propensity score matching was used to balance baseline characteristics. We used the Cox proport

Royal jelly proteins (RJPs) are rich sources of bioactive peptides with health-promoting functions. This study aimed to identify and characterize angiotensin-I-converting enzyme (ACE) inhibitory peptides derived from the gastrointestinal digest of RJPs using an integrated peptidomics and bioinformatics strategy. A total of 1983 peptides were identified by LC-MS/MS, of which 237 exhibited high predicted bioactivity (PeptideRanker score ≥ 0.8). After in silico screening, 49 soluble bioactive peptides were selected, showing favorable molecular weights, charge properties, and oral bioavailability.

Chlamydomonas reinhardtii, a nutrient-rich microalga and emerging food resource, remains largely unexplored as a source of bioactive peptides. In this study, eight protease hydrolysates were prepared from C. reinhardtii. Among these, the alkaline protease hydrolysate (CRPA) demonstrated the most potent activity, exhibiting strong angiotensin-I converting enzyme (ACE) inhibitory activity in vitro and significant in vivo antihypertensive activity in spontaneously hypertensive rats (SHRs), with the effective dose corresponding to an estimated human-equivalent dose of 16 mg/kg/day. Dairy white wastewater (WW), a by-product of industrial cleaning processes, contains residual milk proteins that can be enzymatically converted into bioactive peptides. In this study, WW proteins were hydrolyzed using four enzymes, pepsin, trypsin, thermolysin, and pronase E, for up to 240 min, and the resulting hydrolysates were evaluated for ACE inhibition, DPP-IV inhibition, and antioxidant capacity. Thermolysin hydrolysates exhibited the strongest ACE inhibition, with IC50 values as low as 21.0 μg protein/mL, whereas pepsin and pronase E hydrolysates showed DPP-IV inhibitory activities wit

BACKGROUND: Ischemic stroke is the leading cause of epilepsy in the elderly. Various antihypertensive drugs have shown potential anti-epileptic effects and are widely used in cerebrovascular disease for the comorbidity of hypertension. However, it remains unknown whether they can reduce the occurrence of seizures and epilepsy after ischemic stroke. OBJECTIVE: To examine the effect of antihypertensive drugs on reducing the risk of post-stroke early-onset seizures (ES) and post-stroke epilepsy (PSE). DESIGN: A retrospective cohort study. METHODS: In this retrospective cohort study, we enrolled p

Exercise training (ET) based cardiac rehabilitation (ExCR) is considered as a ‘pillar’ of heart failure (HF) management, alongside drug and medical device treatment improving health related quality of life and reducing hospitalizations. However, it is reported that the large beneficial effects of optimal medical therapy (OMT) and device therapy may have reduced the potency of ExCR in patients with HF and reduced ejection fraction (HFrEF), associated with a non-clear benefit in mortality, as well as, with modest changes in aerobic capacity, at least compared to the pre-OMT era.
