Ambrisentan as adjunctive therapy for IgA nephropathy: a retrospective single-center analysis.

BACKGROUND: Proteinuria and hematuria are both significant predictors of impaired kidney function in IgA nephropathy (IgAN). We evaluated the efficacy and safety of ambrisentan, a selective endothelin A receptor antagonist, as adjunctive therapy in a real-world cohort of high-risk IgAN patients. METHODS: In this retrospective, single-center study, we analyzed 169 patients with biopsy-proven IgAN who received adjunctive ambrisentan for ≥6 months. A propensity score-matched historical control group (138 pairs) was generated from patients receiving standard care prior to the ambrisentan era. Lab

Kidney Protection Options in 2025: Are Renin-Angiotensin System Inhibitors Still Needed?

The therapeutic advantage of angiotensin-converting enzyme inhibitors was first described more than 40 years ago by Brenner and colleagues. Since then, a number of clinical trials have demonstrated the utility of drugs that modify the renin-angiotensin system (RAS) to slow the rate of progression of kidney disease in patients with and without diabetes. However, despite the well-known benefits of these drugs in reducing cardiorenal events, most clinicians are not using them consistently in their practice. The lack of use is related to concerns about increases in serum creatinine and the develop

Perindopril: A comprehensive profile.

Perindopril (PER) is classified as angiotensin-converting enzyme (ACE) inhibitors and is used primarily for the management of hypertension and cardiovascular conditions, including heart failure and stable coronary artery disease. Two primary salts of PER have been developed, perindopril arginine (PER-Arg) and perindopril erbumine (PER-E). This chapter presents a detailed overview on the physicochemical attributes of PER-Arg, molecular structure, synthetic pathway, crystallographic behavior, thermal stability, and UV absorbance profile of PER-Arg. Moreover, the chromatographic, spectrophotometr

Systematic Analysis of Peptide Mapping and Structural and Functional Identification of Protein Hydrolysates Derived from Six Kinds of Livestock/Poultry Meats Based on Peptidomics.

The purpose of this study was to investigate the effects of cooking treatment on the structural properties and potential bioactive peptide release of six meat protein hydrolysates using a cooking-assisted enzymatic hydrolysis model. FTIR showed that cooking reduced the contents of α-helix and β-sheet structures and promoted the formation of β-turn and random helix structures. The enzymatic hydrolysates demonstrated evident antioxidant activity in vitro, both before and after cooking, which was affected by species differences. LC-MS/MS combined with bioinformatics analysis indicated that cookin

Empagliflozin in combination with trandolapril but not with triple therapy improves glucose and lipid metabolism in non-diabetic hypertensive model.

Our previous study demonstrated antihypertensive, anti-inflammatory and metabolic effects of SGLT-2 inhibition in non-diabetic, Ren-2 transgenic hypertensive rats (TGR). The current study aimed to compare the combination of empagliflozin with angiotensin converting enzyme inhibition (trandolapril) versus triple therapy (hydralazine, hydrochlorothiazide, reserpine). Adult male hypertensive Ren-2 transgenic rats were pretreated with these drugs for four weeks. For the following two months, the rats were concomitantly given empagliflozin. Renal function and echocardiography were analyzed througho

The Association of Guideline-Directed Medical Therapy Scores and Patient Outcomes in Patients With Heart Failure Across the Spectrum of Left Ventricular Ejection Fraction.

BACKGROUND: Guideline-directed medical therapy (GDMT) improves outcomes in heart failure with reduced ejection fraction (HFrEF), but its impact in patients with mildly reduced (HFmrEF) or preserved ejection fraction (HFpEF) is less defined. METHODS: We analyzed data from 25,333 adults with ambulatory encounters for heart failure (HF) from February 2022 to February 2024 across >300 clinics in 2 states. Patients were categorized by left ventricular ejection fraction (LVEF) into HFrEF (≤40%), HFmrEF (41-49%), and HFpEF (≥50%), excluding those with improved EF. An electronic health record-embedde

Decreased potential for lymphatic vessel generation is a hallmark of early diagnosed arterial hypertension and can be reversed by treatment with angiotensin converting enzyme inhibitors.

Arterial hypertension is the main risk factor for cardiovascular diseases. It is characterized by inflammation and impaired generation of blood microvasculature leading to organ damage. While the immune system is an important actor involved in both phenomena, we hypothesized that hypertensive patients show alterations in numbers of regulatory T cells (Tregs) and proangiogenic CD31 + CXCR4+ T cell subsets, which lead to alterations in serum cytokine pattern and decreased generation of blood and lymphatic vessels. Sixteen recently diagnosed hypertensive patients before treatment implementation w

Causal relationship between renin-angiotensin system inhibitors and cancer: a two-sample, two-step Mendelian randomization study.

BACKGROUND: The renin-angiotensin system (RAS), particularly the angiotensin II (AT2)/angiotensin I receptor axis, is implicated in promoting tumorigenesis. However, the causal relationship between angiotensin-converting enzyme inhibitor (ACEI) or angiotensin receptor blocker (ARB) use and cancer remains controversial. This study employed a two-sample, two-step Mendelian randomization (MR) approach to investigate whether there is a causal relationship between ACEI/ARB use and various cancers, and if so, the direction and potential mechanism of this association. METHODS: Genome-wide associatio

Angiotensin-Converting Enzyme Inhibitors Versus Angiotensin Receptor Blockers for Cardiovascular and Renal Protection in Type 2 Diabetes: A Systematic Review and Meta-Analysis.

Type 2 diabetes mellitus (T2DM) increases the risk of cardiovascular morbidity and end-stage renal disease. Renin-angiotensin-aldosterone system (RAAS) inhibitors, specifically angiotensin-converting enzyme inhibitors (ACEIs) and angiotensin receptor blockers (ARBs), are standard therapies for organ protection. However, uncertainty remains regarding their comparative efficacy and the safety of combination therapy. This systematic review and meta-analysis aimed to compare the efficacy of ACEIs versus ARBs in reducing cardiovascular and renal events in patients with T2DM and to assess the safety

Methyl Palmitate, a Potent Angiotensin Converting Enzyme Inhibitor, Attenuates Vascular Remodeling in L-NG-Nitro Arginine Methyl Ester-Induced Hypertensive Wistar Rat Models.

UNLABELLED: <p>Introduction: Angiotensin converting enzyme (ACE) regulates blood pressure via the renin-angiotensin and bradykinin systems. Though synthetic ACE inhibitors are more effective, they pose several side effects. Methyl palmitate (MP), a natural fatty acid methyl ester with cytoprotective, antioxidant, anti-inflammatory, and vasodilatory properties, is not explored for its ACE inhibitory or antihypertensive potential. This study aimed to investigate the in vitro ACE inhibition of MP and its effect on L-NG-Nitro Arginine Methyl Ester (L-NAME)-induced hypertensive male Wistar

Early administration of renin-angiotensin system inhibitors improves survival and cardiac remodeling in heart failure with preserved ejection fraction.

Heart failure with preserved ejection fraction (HFpEF) is a major cardiovascular disease that accounts for 50% of all cases of heart failure. Patients with HFpEF have limited therapeutic options because of the complex pathogenesis of this disease. Decreased nitric oxide (NO) levels and increased renin-angiotensin system (RAS) activity may be associated with HFpEF pathogenesis. However, whether soluble guanylate cyclase (sGC) stimulators and RAS inhibitors protect against HFpEF remains unclear. This study aimed to evaluate the preventive effects of RAS inhibitors captopril (Cap) and/or sacubitr
