Antihypertensive effect and mechanism of Mytilus edulis-derived peptide in spontaneously hypertensive rats.

This study aimed to investigate the stability, hypotensive effects, and underlying mechanisms of Mytilus edulis-derived angiotensin-converting enzyme inhibitory peptides (MEPs-F) in spontaneously hypertensive rats (SHRs). The results demonstrated that MEPs-F exhibited remarkable stability under thermal treatment, various pH conditions, high-salt exposure, and simulated gastrointestinal digestion. The MEPs-F(H) group produced maximum short-term reductions of 49 mmHg in systolic blood pressure (SBP) and 71 mmHg in diastolic blood pressure (DBP), and the maximum long-term reductions reached 85 an

Discontinuation of Heart Failure Pharmacotherapy in Older Adults With Frailty.

BACKGROUND: Guideline-directed medical therapy (GDMT) improves outcomes in heart failure irrespective of frailty status. Nevertheless, the magnitude of frailty-associated discontinuation of GDMT remains insufficiently studied. We aimed to quantify 12-month discontinuation rates of GDMT across frailty strata. METHODS: This retrospective cohort study used a nationwide insurer-based database. We identified patients aged ≥65 years who were hospitalized for heart failure between 2014 and 2022 and newly initiated on at least 1 GDMT at discharge, including angiotensin-converting enzyme inhibitors/an

Stopping renin-angiotensin-aldosterone system inhibitors in kidney disease.

PURPOSE: The KDIGO guidelines recommend the use of renin-angiotensin system inhibitors (RASi) in chronic kidney disease (CKD) given their kidney and cardiovascular benefits. Despite this recommendation, real-word data suggest low rates of the use of these medications, and RASi are frequently discontinued. The objective of this review is to summarize the outcomes associated with the discontinuation of RASi in individuals with CKD, hyperkalemia, and acute kidney injury (AKI). RECENT FINDINGS: Trial-grade data suggest that there is no benefit to the discontinuation of RASi in patients with low e

Challenges and Perspectives in Optimising the Treatment of Arterial Hypertension: Role of the Ramipril-Amlodipine-Hydrochlorothiazide Single-Pill Combination.

Globally, hypertension remains inadequately controlled, despite the availability of effective therapies and guideline recommendations. This narrative review synthesises current evidence on the clinical rationale, efficacy and implementation of single-pill combination (SPC) therapy to control blood pressure (BP), with a focus on an SPC containing ramipril, amlodipine and hydrochlorothiazide. These agents offer complementary mechanisms of action and a favourable tolerability profile, supporting their use in dual and triple SPCs to overcome therapeutic inertia (failure to intensify therapy when B

Harms of Continued Antihypertensives: An Overview of Reviews.

BACKGROUND: Antihypertensives are effective drugs for treatment of hypertension, which is one of the most important risk factors for cardiovascular morbidity. Intensive hypertension treatment significantly reduces cardiovascular risk among older people. Based on these findings, guidelines recommend stricter blood pressure management nowadays. Reasoning on benefit and harms of antihypertensive treatment can be challenging in daily practice because little evidence is available on specific harm outcomes. OBJECTIVE: With this overview of reviews, we aim to summarize evidence from systematic revie

Identification and mechanistic characterization of novel ACE-inhibitory peptides derived from Nattokinase.

Hypertension remains a major global health challenge and is associated with increased cardiovascular risk. Angiotensin-converting enzyme (ACE) plays a pivotal role in blood pressure regulation; however, conventional synthetic ACE inhibitors are often accompanied by undesirable side effects. Nattokinase (NK), a serine protease derived from natto, has shown antihypertensive potential via bioactive peptides released during digestion. In this study, we identified two novel ACE-inhibitory peptides (LGG and TW) from simulated digestion of NK. LGG exhibited potent competitive inhibition (IC50 = 10.78

Isoindolines and Isoindoline-1,3-diones as Nonpeptide ACE Inhibitors: An In Silico and In Vitro Modeling Approach.

Hypertension, a major cardiovascular risk factor, is often treated with peptide-derived angiotensin-converting enzyme inhibitors (ACEi), which can have several side effects. This study examined a new alternative: isoindoline and isoindoline-1,3-dione derivatives as nonpeptide ACE inhibitors. The synthesis and testing of these compounds involved both in silico molecular docking studies and optimized in vitro inhibitory kinetic assays, along with acute toxicity tests in mice. isoindoline-1,3-dione, D-05, demonstrated the strongest ACE inhibition in vitro (IC50 = 416.4 μM) and effectively bound t

[Medication review in a geriatric patient with inappropriate polypharmacy].

Polypharmacy is common among the elderly. This case report details the experience of an 85-year-old man whose notable polypharmacy and additional over-the-counter medicines led to anemia and acute kidney injury. These issues were traced to an unintended »triple-whammy« effect due to the interaction of diuretics, ACE inhibitors, and NSAIDs, and concurrent treatment with an anticoagulant. Highlighting the role of systematic medication reviews, this case illustrates how targeted interventions can prevent severe health outcomes and underscores the importance of medication monitoring in elderly pat
