Calcium signaling pathway implicates a shared genetic basis between psychiatric and cardiovascular diseases.

Psychiatric and cardiovascular diseases (CVDs) are frequently comorbid and are interconnected through the brain-heart axis. However, the underlying shared genetic etiology remains unknown in East Asians. To address this critical gap, we conducted a genome-wide pairwise trait pleiotropy study by leveraging genome-wide association studies of three major psychiatric disorders (schizophrenia [SCZ], bipolar disorder [BIP], major depressive disorder [MDD]) and ten cardiovascular traits (including eight CVDs) in East Asians. We identified genetic overlaps across seven disease pairs, such as SCZ with

Optimized cortical EEG modeling for Parkinson disease diagnosis with snow Shepherd Stride tuning mechanism.

Parkinson's disease (PD) is a neurodegenerative disease that causes extensive impacts on cognitive and motor function, hence making correct and early diagnosis essential for efficient clinical management and enhancement of the quality of life. Analysis using EEG has now become a safe and possible method for the identification of neural abnormalities in PD. Nevertheless, current models must struggle with several constraints: high false detection rates, poor generalizability across subjects, sensitivity to EEG noise pollution, and the inability to extract deep cortical representations, which hav

An Unusual Presentation of Nicolau Syndrome in the Upper Limb: A Case Report from Northern Ecuadorian Amazonia.

Nicolau syndrome, also known as embolia cutis medicamentosa, is a rare iatrogenic reaction that may occur following parenteral drug administration, including inadvertent intra-arterial or periarterial injection. Its pathophysiology remains poorly understood; however, several mechanisms have been proposed, including vasospasm, embolization, cytotoxic inflammation, and secondary tissue necrosis. We report the case of a 22-year-old transgender woman who received intravenous benzathine penicillin in the left arm without a medical prescription following a reactive syphilis screening performed outsi

Fast and sustainable active pharmaceutical ingredient (API) screening in over-the-counter and prescription drug products by surface-assisted plasma-based desorption/ionization high-resolution mass spectrometry.

Fast chemical analysis of pharmaceutical preparations is important for quality assurance, counterfeit drug detection, and consumer health. While quantitative methods such as high-performance liquid chromatography mass spectrometry (HPLC-MS) are powerful, there is a need for sustainable and environmentally friendly methods, which require less chemicals and produce less waste. Here, solvent-free ambient desorption/ionization (ADI) MS methods are attractive because they do not require time-consuming chromatography, produce little to no chemical waste, and, thus, contribute to green chemistry prac

The Safety and Outcome of Erector Spinae Plane Block in Patients Using Antiplatelets, Anticoagulants, or Peripheral Vasodilators.

BACKGROUND: The erector spinae plane block (ESPB) is used in various clinical situations with good to excellent analgesic effectiveness. The ESPB can be performed safely even in patients with altered hemostasis. However, the safety of ESPB in patients being treated with antiplatelets and anticoagulants is based on limited clinical data, mostly from single case series. OBJECTIVES: The primary endpoint of this study was to identify any complications related to a thoracic or lumbar ESPB in patients using an antiplatelet, an anticoagulant, or a peripheral vasodilator without any preprocedure or p

Medical Management of Peripheral Artery Disease.

Peripheral artery disease (PAD) is a common type of atherosclerotic disease of the lower extremities associated with reduced quality of life and ambulatory capacity. Major adverse cardiovascular events and limb amputations are the leading cause of morbidity and mortality in this population. Optimal medical therapy is therefore critical in these patients to prevent adverse events. Risk factor modifications, including blood pressure control and smoking cessation, in addition to antithrombotic agents, peripheral vasodilators, and supervised exercise therapy are key pillars of medical therapy. Medical optimization of the peripheral artery disease patient.

Peripheral artery disease (PAD) impacts an estimated 230 million adults worldwide, including more than 9.5 million adults older than 40 years in the United States. PAD remains more underdiagnosed and undertreated than manifestations of atherosclerosis elsewhere in the body, such as coronary artery disease and cerebrovascular disease. Medical therapies benefit all patients with PAD, including those who are asymptomatic, as well as those with symptoms and advanced disease requiring intervention. Comprehensive medical management of PAD is based on tempering atherosclerotic disease processes and s

Acute limb ischemia in an adolescent with COVID-19 and systemic scleroderma: a case report.

BACKGROUND: Juvenile Scleroderma is a rare autoimmune disease of the connective tissue. Its concurrence with COVID-19 can lead to limb ischemia as both disease entities are pro-inflammatory and pro-thrombotic. To date, there is no case report describing the symptomatology and course of disease in patients with juvenile Scleroderma and COVID-19. CASE PRESENTATION: An adolescent with acute limb ischemia presented with a history of generalized hypo-and-hyperpigmented skin lesions and mild, non-productive cough. She tested positive for SARS-CoV-2 on nasopharyngeal swab RT-PCR. Further work-up rev

Evidence-Based Medical Management of Peripheral Artery Disease.

Peripheral artery disease is an atherosclerotic disease of the lower extremities associated with high cardiovascular mortality. Management of this condition may include lifestyle modifications, medical management, endovascular repair, or surgery. The medical approach to peripheral artery disease is multifaceted and includes cholesterol reduction, antiplatelet therapy, anticoagulation, peripheral vasodilators, blood pressure management, exercise therapy, and smoking cessation. Adherence to this regimen can reduce limb-related complications like critical limb ischemia and amputation, as well as

Decades-old renin inhibitors are still struggling to find a niche in antihypertensive therapy. A fleeting look at the old and the promising new molecules.

Hypertension is a diverse illness interlinked with cerebral, cardiovascular (CVS) and renal abnormalities. Presently, the malady is being treated by focusing on Renin- angiotensin system (RAS), voltage-gated calcium channels, peripheral vasodilators, renal and sympathetic nervous systems. Cardiovascular and renal abnormalities are associated with the overactivation of RAS, which can be constrained by angiotensin- converting enzyme inhibitors (ACEIs), angiotensin II (Ang-II) -AT1 receptor blockers (ARBs) and renin inhibitors. The latter is a new player in the old system. The renin catalyzes the

Alleviation of mechanical stress-induced allodynia by improving blood flow in chronic constriction injury mice.

Reduced blood flow in the skin is observed in patients with neuropathic pain and in animal models. The aim of the present study was to elucidate the relationship between reduced skin blood flow and neuropathic pain in mice with a chronic constriction injury (CCI). Noradrenaline-induced contraction was enhanced in isolated plantar arteries ipsilateral to the CCI surgery compared to the contralateral arteries. Ten μM hydralazine, a peripheral vasodilator, at improved the enhanced contractile response in the ipsilateral arteries. The plantar blood flow in vivo was lower on the ipsilateral side of

Preferred Fourth-Line Pharmacotherapy for Resistant Hypertension: Are We There Yet?

Resistant hypertension (RH) is defined as blood pressure (BP) that remains above target levels despite adherence to at least three different antihypertensive medications, typically including a diuretic. Epidemiological studies estimate that RH is increasing in prevalence, and is associated with detrimental health outcomes. The pathophysiology underlying RH is complex, involving multiple, overlapping contributors including activation of the renin-angiotensin aldosterone system and the sympathetic nervous system, volume overload, endothelial dysfunction, behavioural and lifestyle factors. Cardiovascular medication and intraocular pressure: results from the Gutenberg Health Study.

BACKGROUND: Intraocular pressure (IOP) is well known to be associated with blood pressure and other cardiovascular risk factors. The influence of systemic cardiovascular, in particular antihypertensive, medication on IOP is still controversial. This study analyses the association between the use of cardiovascular medications and IOP in a large European cohort. METHODS: The Gutenberg Health Study is a population-based, prospective, observational cohort study in mid-western Germany. IOP was measured using a non-contact tonometer. The medication classes examined were peripheral vasodilators, diur

Acute acoustic trauma: how to manage and how to prevent?

INTRODUCTION: The consequence of an exposure to intense sounds can be a temporary or permanent hearing loss and even with a rapid therapeutic management, severe sensorineural sequelae may persist. METHODS: the authors report a retrospective study about 64 patients followed for an acute acoustic trauma during a period of 8 years (2006 to 2013). For all the cases, a clinical examination associated to a pure-tone audiometry was conducted. Hearing levels were measured at the frequencies 500, 1000, 2000 and 3000 Hertz. Auditory evoked potentials were performed in 17 cases. The therapeutic and evol

Histopathological and clinical traps in lichen sclerosus: a case report.

Lichen sclerosus et atrophicus and limited systemic scleroderma (acrosclerosis) are inflammatory skin diseases that ultimately evolve into two distinct modes of atrophic scar formation, but which can easily be confused clinically. They are very rarely associated. The literature has reported cases in which lichen sclerosus was associated with various forms of scleroderma, but often with localized morphea. The characteristic histopathological picture of lichen sclerosus includes a thin epidermis, with orthohyperkeratosis and vascular degeneration in the basal layer, loss of elastic fibers, and b

Effects of watermelon supplementation on arterial stiffness and wave reflection amplitude in postmenopausal women.

OBJECTIVE: Postmenopausal women have increased arterial stiffness (brachial-ankle pulse wave velocity [baPWV]) and wave reflection. L-Citrulline supplementation reduces baPWV but not brachial blood pressure. Peripheral vasodilators decrease wave reflection amplitude or second systolic peak (SBP2) in radial artery and aorta, which are related to aortic systolic blood pressure (SBP). We examined the effects of L-citrulline-rich watermelon supplementation on baPWV, wave reflection characteristics, and aortic SBP in postmenopausal women. METHODS: In a randomized cross-over study, 12 postmenopausa

Vasomodulation influences on the transdermal delivery of Ibuprofen.

The purpose of the study was to evaluate the effect of adding peripheral vasodilators, tolazoline, or papaverine, to transdermal drug delivery vehicles with the goal of improving the tissue bioavailability of transdermally delivered ibuprofen. Ibuprofen (150 mg) formulations with several concentrations of two different vasodilators and/or a penetration enhancer (PE) complex were topically applied to rabbits. Plasma levels of ibuprofen were determined by a validated high-performance liquid chromatography method and evaluated at 0, 0.5, 1, 2, and 3 h. The PE complex enhanced the plasma ibuprofen

Benzopyran derivatives as cardio-selective ATP-sensitive potassium channel openers: a review.

Benzopyran derivatives are the potassium channel openers (KCOs) having antihypertensive, cardio-protective, myocardial protectors, powerful peripheral vasodilators and anti-ischemic activity. Their usage as anti-ischemic including angina, hypertension and diabetes is thought to be due to the stimulation of KATP channels which are contemplated to produce vasorelaxation and myocardial protection. It is observed that potassium channels are involved in mediating the cardio-protective effects of pre-conditioning in animal models and man. KCOs protect heart from an ischemic insult without contributi
