[Molluscum contagiosum-like papules within the scope of disseminated infection with Mycobacterium avium complex in an AIDS patient].

A 38-year-old HIV-positive man had several attacks of high fever associated with extensive perspiration over a 10-week period. Simultaneously, he developed molluscum contagiosum-like papules and an erythematous plaque on the face, ulcerated papules on both shoulders and buttocks and subcutaneous nodules on the arms. Histological examination of biopsy specimens revealed a diffuse, histiocytic infiltrate with abundant rod-shaped bacteria. Mycobacterium avium complex was cultured from the tissue and Mycobacterium avium complex DNA was detected by the polymerase chain reaction. The diagnosis of di

Chromoblastomycosis. A fungal infection primarily observed in the lower extremity.

Chromoblastomycosis is a cutaneous-subcutaneous fungal infection that is being seen more frequently in patients living in the US. The disease normally occurs in patients living in tropical and subtropical regions, but as the number of immigrants into the US increases, podiatrists must be able to recognize the manifestations of chromoblastomycosis. The most common sight involved is the lower extremity where it easily can be confused with other diseases such as tertiary syphilis, phaeohyphomycosis, and cutaneous tuberculosis, among others. Small lesions should be excised, while antifungal drugs,

[Patients with calcinosis cutis in National Leprosarium Matsuoka Hoyo-En].

As of the year 1991 there were 358 leprosy patients in National Leprosarium Matsuoka Hoyo-En, including 223 patients (62.3%) who had received the injections of chaulmoogra oil before. Calcinosis cutis caused probably from the injections was noted on 73 patients (32.7%): 67 lepromatous and 6 tuberculosis cases. It has never been reported before that the T type patient suffering from calcinosis cutis was observed in the cases of the chaulmoogra oil injection in Japan. The detectable positions of calcinosis cutis were mostly at the injected sites, that is, outside the right brachium followed by b

[Weber-Christian disease].

A fourteen month old infant was admitted for evaluation because of continuous high fever and an indurated nodular lesion at the left thigh of one month course. After admittance painful inflammatory subcutaneous nodules appeared in the face and trunk, these were accompanied by enlarged cervical lymph nodes and hepatomegaly. Histological evaluation of the skin biopsy showed destruction of subcutaneous tissue, foamy cells, vasculitis and polymorphonuclear leukocyte infiltration; histiocytic proliferation in the lymph nodes and steatosis in the liver biopsy. Osteoarticular infection, cellulitis, s

Pulmonary sporotrichosis: review of treatment and outcome.

Four culture-documented cases of pulmonary sporotrichosis, three primary infections and one with multisystem involvement, are presented. Two of these patients are the first reported cases of primary lung disease treated with ketoconazole. This antifungal agent appears to be ineffective in eradicating this infection. The four cases, as well as a review of the literature, illustrate several important aspects of this rare disease. Pulmonary sporotrichosis is most commonly found in males with a history of alcohol abuse who are between the ages of 30 and 60. The infection is usually confined to the

Mortality among white and nonwhite farmers in North Carolina, 1976-1978.

Death certificate information identified 9,245 white and 3,508 nonwhite men who died in North Carolina during 1976-1978 and who had been farmers. The distribution of deaths from various causes among these men was compared to that of other male decedents in the state. For both white and nonwhite farmers, proportional mortality ratios (PMRs) were elevated for tuberculosis (whites, 1.6; nonwhites, 1.7), diseases of the skin and subcutaneous tissue (whites, 2.5; nonwhites, 1.5), and external causes (whites, 1.2; nonwhites, 1.1) and were decreased for cancers of the esophagus (whites and nonwhites,

Subcutaneous calcifications masquerading as pulmonary lesions in long-term hemodialysis. Review of nodular pulmonary opacities in the population undergoing hemodialysis.

Calcification of soft tissue frequently occurs in patients undergoing hemodialysis for chronic renal failure and involves the arteries, eyes, periarticular regions, lungs, heart, abdominal viscera, subcutaneous tissues, and skin. In this report, two such patients are described who presented with apparent pulmonary nodules on their chest roentgenograms obtained on admission. By the use of tomograms and apical lordotic views, both proved to have extrapulmonary calcific deposits in the subcutaneous tissues overlying the upper pulmonary zones. The tendency for these collections to undergo progress

Biodistribution of 64Cu in inflamed rats following administration of two anti-inflammatory copper complexes.

64Cu was administered in two anti-inflammatory formulations to normal rats and to rats with 2 forms of local inflammation, namely (a) an acute paw oedema (elicited with carrageenan) or (b) a chronic granulomatous response to an implanted irritant (Mycobacterium tuberculosis in a polyurethane sponge). The copper formulations used were (i) a slow release one consisting of Cu(II) salicylate applied dermally with ethanol/DMSO and (ii) short acting hydrophilic complex (Cu(I)Cu(II)-penicillamine)5- given subcutaneously. Three types of changes in copper biodistribution with these forms of inflammatio

Mycobacterial infections in renal transplant recipients. Seven cases and a review of the literature.

During an 11-year period, 1,069 patients received renal allografts at the University of Minnesota Hospital, Minneapolis, and infections developed in seven (0.65%) due to mycobacteria (Mycobacterium tuberculosis and M kansasii). The primary infection was in joint or subcutaneous tissue in six patients and pulmonary (miliary) in one. Infections in joint or skin shared common features regardless of the species of Mycobacterium and usually mimicked acute pyogenic bacterial infection; all responded to antimycobacterial drugs. The clinical manifestations in our patient in miliary tuberculosis were c

[Non-venolymphatic cellulitis].

Nonlymphangiophlebitic hypodermatitis includes acute, subacute and chronic afflictions. The first are primarily expressed by erythema nodosum, which can be triggered by many factors and is characterized microscopically by the presence in the interlobular septa of lymphocytes, histiocytes and polynuclears. Subacute and chronic hypodermic inflammation have different microscopic substrata. Erythema induratum is a form of cutaneous tuberculosis; its nodules result from the presence of tuberculous infiltrates in the fat lobules. The lesions of nodular vasculitis are the clinical expression of an af

Lymphoid follicles in subcutaneous inflammatory disease.

A review of inflammatory subcutaneous disease revealed that lymphoid follicles are observed frequently in lupus panniculitis. They are found rarely in other lymphocytic inflammatory diseases involving the panniculus, such as morphea, erythema nodosum, and erythema induratum. The lymphoid follicle may be an indication of immunoreactive and connective tissue disease, and cases should be evaluated with this in mind.

Dumbbell granulomatous abscess of the chest wall following needle biopsy of the pleura.

A 38-year-old woman who had a Cope needle biopsy of the pleura was treated for plural tuberculosis on the basis of a positive PPD-S skin test and presence of caseating granulomas in the pleural biopsy. Ten months later she developed a tender, subcutaneous nodule in the area of the previous needle biopsy. Surgical exploration revealed a dumbbell abscess through the chest wall communicating with an area of consolidation in the right middle lobe. En bloc surgical resection of the abscess and peripheral portion of the right middle lobe was curative, although all pathologic and cultural studies of

[A special form of lupus disease. Deep cutaneous lupus. Apropos of 2 cases].

Deep cutaneous lupus is a clinical form of lupus disease for which a very old description has recently been brought up to date. Two new cases are reported here and a review of the literature makes it possible to analyse the clinical, biochemical, histological, and immunological features of this panniculitis. Different from the Weber-Christian syndrome, it is characterized by repeated eruptions of nodules and/or subcutaneous plaques, and histologically by vasculitis, lymphocyte infiltration, and sometimes te presence of immunoglobulins on the basal dermal membrane and around the vessels.

Aspermatogenesis in the guinea pig induced by testicular tissue and adjuvants.

The injection into the dorsal skin of a suspension of guinea pig testis or spermia incorporated in a water-in-oil emulsion containing killed mycobacteria induces aspermatogenesis in guinea pigs. The injury begins with the inhibition of the maturation of spermia and proceeds through the degeneration and exfoliation of spermatids, spermatocytes, and finally spermatogonia. These germinal cells pass from the seminiferous tubules into the epididymis. The process is not associated with inflammation. No significant changes occur in the intertubular spaces and the Leydig cells do not seem to be affect
