Self-Management of Reflux-Like Symptoms: A Patient-Centered Decision-Making Approach.

<p>Background: Guidelines for the management of reflux-like symptoms (primarily heartburn and regurgitation) offer limited guidance to support decision-making in the nonprescription setting. This review aimed to summarize and compare nonprescription treatment options within the context of reflux-like symptom pathophysiology and provide a practical evidence-based tool to help guide tailored treatment selection. Summary: A patient-centered approach should consider the requirement for reactive versus preventative therapies and each product's unique characteristics. Antacids neutralize refluxed ac

Metabolomics combined with machine learning reveals candidate metabolites and a putative working network associated with cereulide production in emetic Bacillus cereus.

Cereulide, a heat-stable toxin produced by Bacillus cereus, is recognized as a major foodborne risk. However, the metabolic features associated with cereulide production and their biological relevance remain insufficiently characterized. This study aimed to prioritize candidate metabolites associated with cereulide-related phenotypes and to explore their potential pathway-level relationships. An integrated analytical pipeline combining RF-CNN-assisted feature prioritization, pathway enrichment, time-series profiling, and temporal association analysis was developed to support both classificatio

Senna Laxative-Associated Severe Ischemic Bowel Disease in an 87-Year-Old Patient: A Clinical Warning.

Senna laxative, one of the major stimulant laxatives, has received less attention regarding its potential risk of intestinal ischemia in the elderly population. An 87-year-old female with a past medical history of chronic constipation developed abdominal pain and fever after oral ingestion of senna laxative for constipation and was subsequently diagnosed with ischemic bowel disease upon hospitalization. Symptomatic and supportive treatment, such as antibiotics, acid inhibition and stomach protection, fluid replenishment, and intravenous nutrition, was given. After treatment, the patient's cond

Fatal emetic Bacillus cereus outbreak associated with consumption of pre-cooked tortellini, Germany, August to September 2024.

In September 2024, the competent food control authority was informed that two people had fallen ill after visiting a restaurant and one of them died shortly after. As other people had already reported serious symptoms and hospitalisation after visiting the venue in question, a food-related outbreak had to be assumed. The investigations clarified that the outbreak was most likely triggered by pre-cooked tortellini with meat filling. High bacterial levels of emetic toxin-producing bacteria of the Bacillus cereus group were detected in food and environmental samples from the restaurant as well as

Irinotecan-Loaded Vaterite Microspheres for Drug Delivery: Drug Release and Dissolution Kinetics and Mechanism in an Aqueous Solution and Human Serum.

The coprecipitation synthesis of calcium carbonate vaterite microspheres loaded with chemotherapeutic drug irinotecan is reported. High drug loading comparable to the best reported for other drugs in porous vaterite is achieved, and characterization confirms that the characteristic spherical morphology of vaterite is not disrupted by the presence of irinotecan. UV-vis spectroscopy demonstrates the release of the drug over time, and the latter, upon comparison with optical microscopy dissolution data, shows that drug release correlates with the time scale of dissolution of the vaterite host. The pH factor: unraveling drug-drug interactions in protein kinase inhibitors through the regulatory lens.

Drug-drug interactions (DDIs) are a significant concern in clinical practice, DDIs might be relevant when drugs with pH-dependent solubility are co-administered with gastric acid-reducing agents (ARAs) such as histamine, H2-receptor antagonists and proton pump inhibitors. One class prone to such DDI at the absorption phase are the weakly basic protein kinase inhibitors (PKIs). The aim of this work is to review recent Food & Drug Administration (FDA) and European Medicines Agency (EMA) submissions for PKIs and evaluate the various approaches by drug developers to characterize pH-dependent DDI l

Micro- and nanoscale CaCO3 carriers for localized melanoma therapy: impact of 'cold' labeling on drug loading, release, and in vivo performance.

The calcium carbonate drug delivery systems (DDSs) provide a very promising strategy for various types of treatments, including chemo- and radiotherapies. Effective design of this platforms including surface engineering and employing organic/inorganic additives has a significant influence on drug loading, stability and drug release. In this study, calcium carbonate microparticles (mCa) and nanoparticles (nCa) were synthesized for drug loading of novel therapeutic molecules and subjected to the so-called 'cold' labeling procedure, i.e. efficient non-radioactive synthetic routes that can be appl

Dependence-related screening positivity among regular stimulant-laxative users with chronic constipation: a multicenter prospective observational study.

BACKGROUND: Chronic constipation is common, and the appropriate use of stimulant laxatives remains challenging. Although dependence due to habitual stimulant-laxative use is frequently observed, the long-term effects of regular use on dependence and tolerance have not been fully elucidated. This study aimed to characterize the clinical features and substance use-related symptoms associated with stimulant-laxative use in patients with chronic constipation. METHODS: This multicenter prospective observational study was conducted at three centers and enrolled patients with chronic constipation, i

Non-Opioid Pharmaceutical Alternatives for Acute Pain Management in the Emergency Department: A Scoping Review.

OBJECTIVES: In light of the ongoing opioid epidemic, emergency clinicians have faced the difficult challenge of managing acute pain while reducing opioid prescriptions. Improved use of non-opioid analgesics could decrease the need for opioid medications in the management of acute pain. Limited work has been done to systematically produce a comprehensive list of non-opioid pharmaceuticals targeted to specific pain-associated conditions. METHODS: We conducted a scoping review of recent literature for non-opioid pharmaceuticals that could help manage five painful conditions commonly treated in o

Antiulcer Activity of Herbal Medicines: A Review of Current Research.

Peptic ulcers, a common digestive disorder, are characterized by an inflammatory response with necrotic lesions of the stomach mucosa. Peptic ulcers severely damage the mucosal lining of the stomach and duodenum. Numerous factors, such as smoking, Nonsteroidal Anti-Inflammatory Drugs (NSAIDs), acid and pepsin, genetics, and Helicobacter pylori, might contribute to it. Proton Pump Inhibitors (PPIs), Histamine (H2) receptor antagonists, antacids, potassium-competitive acid blockers, and antibiotics are among the common drugs shown to have antiulcer properties. However, many studies have shown th

Toll-like receptor 4 mediates lipopolysaccharide-induced emesis.

BACKGROUND AND PURPOSE: Lipopolysaccharide (LPS) induces inflammation and sepsis through Toll-like receptor 4 (TLR4) activation. Common laboratory animals do not exhibit emesis, but administration of LPS in piglets, cats, and dogs induces emesis via peripheral mechanisms. However, no studies have confirmed whether TLR4 is involved or whether TLR4 antagonists have clinical utility as anti-emetics. EXPERIMENTAL APPROACH: Male Suncus murinus (house musk shrew) was implanted with radiotelemetry transmitters to examine whether LPS-induced emesis and physiological changes indicative of 'nausea' (PC
