Metastatic hypercalcemia from laxative misuse: a rare and serious complication of anorexia nervosa.

A 33-year-old woman with anorexia nervosa (AN) presented with lethargy, diarrhea, and abdominal pain and developed progressive hypercalcemia during hospitalization. Biochemistry demonstrated an inappropriately normal parathyroid hormone (PTH) level in the setting of chronic kidney disease, hyperphosphatemia, and severe vitamin D deficiency. Despite initial denial, chronic misuse of calcium-containing stimulant laxatives was identified, with ingestion of up to 200 tablets daily, including covert use during admission. Imaging revealed extensive metastatic calcification involving the kidneys, lun

A glycan-based adjuvant expands the breadth and duration of protection of mRNA-based vaccines.

The activation of pattern recognition receptors (PRRs) orchestrates inflammation and regulates adaptive immunity. To test whether tuning inflammation through PRR stimulation enhanced the efficacy of mRNA vaccines, we combined an mRNA-based vaccine generated against the ancestral spike protein of SARS-CoV-2 with mannadjuvant, a formulation of fungal mannan and aluminum hydroxide targeting the PRR dectin-2. In mice and non-human primates, mannadjuvant increased the magnitude and durability of the response elicited by the mRNA-based vaccine, and it also led to the induction of neutralizing antibo

Unravelling Voriconazole's Hepatic Metabolism: A Narrative Review on Factors Impacting Voriconazole-N-Oxide Exposure.

BACKGROUND: Voriconazole (VRC), a triazole antifungal agent, is recommended as primary treatment for invasive aspergillosis. Due to its unpredictable dose-exposure relationship, therapeutic drug monitoring (TDM) is recommended. Despite TDM, the probability of achieving therapeutic targets is only approximately 50%. This highlights the need for new dosing strategies. Calculating the metabolic ratio (MR) of NOX, the principal N-oxide metabolite of voriconazole primarily formed by the genetically polymorphic CYP2C19 enzyme, to VRC concentrations could serve as a surrogate for phenotype informatio

Calcium Carbonate-Stabilized Nano-Caffeine Emulsion Attenuates Diabetic Cardiomyopathy via Antioxidant, Anti-Inflammatory, and Anti-Fibrotic Pathways in Type 2 Diabetic Rats with HPLC-Quantified Cardiac Caffeine Levels.

INTRODUCTION: Type 2 diabetes mellitus (T2DM) is one of the most commonly diagnosed metabolic diseases. Notably, two-thirds of diabetic patients may develop diabetic cardiomyopathy (DCM), a life-threatening condition for which no curative treatment currently exists. METHODS: This study aimed to investigate the potential ameliorative effects of caffeine against DCM development, utilizing a novel oral sustained-release caffeine-loaded Pickering emulsion formula stabilized by calcium carbonate nanoparticles to enhance its pharmaceutical and pharmacological properties. Eighty-four rats were divid

Safety of treatments for children with functional constipation.

INTRODUCTION: Functional constipation (FC) in children is a common diagnosis that can be challenging to treat effectively. Internet and social media are shaping parental knowledge and beliefs has become of great importance. AREAS COVERED: A literature review was conducted by searching electronic databases (inception-April 2026) for studies on safety of current management options for FC in children. Our main goal was to describe safety and patients' and parents' satisfaction regarding pharmacological, neuromodulation and surgical therapies. We also addressed myths commonly shared by the lay pu

Acute tree nut-triggered food protein-induced enterocolitis syndrome in two infants: a case report.

INTRODUCTION: Food protein-induced enterocolitis syndrome (FPIES) is a non-IgE-mediated food allergy characterised by delayed repetitive vomiting and can be difficult to distinguish from infectious illness or even an IgE-mediated allergy. Tree nuts are uncommon but increasingly recognised triggers, and evidence regarding atypical FPIES with concomitant IgE sensitisation remains limited. CASE PRESENTATION: We describe two infants with severe acute reactions to tree nuts who were managed at a paediatric allergy centre. The first, a boy with eczema and cow's milk allergy, developed reproducible

Impact of oliceridine versus sufentanil on postoperative nausea and vomiting in patients undergoing thyroid surgery: a prospective, double-blind, randomized controlled trial.

PURPOSE: Postoperative nausea and vomiting (PONV) is a common complication following thyroid surgery, often exacerbated by opioid use. Oliceridine, a novel G protein-biased μ-opioid receptor agonist, may reduce opioid-related adverse events. This study aimed to compare the impact of oliceridine versus sufentanil on the incidence and severity of PONV in patients undergoing thyroid surgery. PATIENTS AND METHODS: In this prospective, double-blind, randomised controlled trial conducted between May 2025 and February 2026, 232 patients scheduled for thyroid surgery were randomly assigned to receive

Euglycaemic ketoacidosis associated with dual-acting GLP-1/GIP receptor agonist treatment following bariatric surgery.

SUMMARY: Euglycaemic ketoacidosis is characterised by high anion gap metabolic acidosis with elevated serum ketone bodies in the absence of significant hyperglycaemia. Although traditionally associated with sodium-glucose cotransporter 2 (SGLT-2) inhibitors, incretin-based therapies have recently been implicated. Tirzepatide is a dual-acting glucagon-like peptide 1 (GLP-1)/glucose-dependent insulinotropic polypeptide (GIP) receptor agonist. It is an approved treatment for obesity in patients without diabetes mellitus. We present the case of a 51-year-old woman without diabetes, 2 years after s

Acute Functional Colonic Dysmotility Mimicking Mechanical Bowel Obstruction Following Self-Limited Diarrheal Illness: A Case Report.

Acute constipation following diarrheal illness is commonly encountered and is often benign. However, when absolute constipation persists despite adequate hydration, preserved mobility, and failure of multiple laxative classes, it can closely mimic mechanical bowel obstruction and create significant diagnostic uncertainty. We report a case of a 56-year-old female with a previously normal daily bowel habit who developed acute obstipation following a self-limited diarrheal episode, unresponsive to bulk-forming, osmotic, and stimulant laxatives. Despite adequate oral intake and activity, symptoms

Wrist Ganglion Cyst Localization in Relation to Cyst Size and Associated Injuries: An MRI-Based Retrospective Study.

BACKGROUND: Three siblings with goitre and latent to mild hypothyroidism were suspected of having thyroid peroxidase (TPO) abnormality. Direct sequencing of their genomic DNAs showed two novel mutations of the TPO gene, one of which was G1687T (Gly533Cys; exon 9) and the other 1808-13del (Asp574/Leu575del; exon 10). The two mutations were compound heterozygous, as the former was found in their father's DNA as heterozygous, and the latter was found in DNA from their mother, also as heterozygous. As Gly533 and Asp574/Leu575 were well-conserved amino acids in the peroxidase superfamily, Gly533Cys

Diagnosis and management of irritable bowel syndrome.

Irritable bowel syndrome (IBS) is a disorder of gut-brain interaction characterised by recurrent abdominal pain or discomfort that is often related to defaecation or associated with a change in stool frequency or form. IBS is common in Australia and affects women more than men. The exact pathophysiology remains unclear, though it can be multifactorial and relate to gastrointestinal dysmotility, post-infection and microbial changes, and a patient's psychosocial background. A positive clinical diagnosis can be made when the Rome V criteria are met, alarm features are absent and simple screening

Kaposi Sarcoma as a Cause of Anemia in Kidney Transplant Recipients: Two Case Reports.

INTRODUCTION: Kaposi Sarcoma (KS), caused by human herpesvirus 8 (HHV-8), may be observed in transplant recipients in the form of immunosuppression-related KS. While it primarily affects the skin, it can also involve internal organs, particularly the gastrointestinal (GI) system. GI involvement is often asymptomatic, but bleeding episodes may occur. Post-transplant anemia (PTA) is a common complication associated with increased mortality and decreased renal function. Early diagnosis and management are crucial. This report discusses 2 cases of KS in kidney transplant recipients presenting with

Beyond the Snapshot: Overcoming GES Limitations to Characterize Gastric Phenotypes and Therapeutic Responses Using a 6-Day Ambulatory Wearable Monitor.

BACKGROUND: Gastric emptying scintigraphy (GES) is considered the standard for differentiating gastroparesis from functional dyspepsia and other foregut disorders characterized by nausea and vomiting but provides only a brief physiologic snapshot that may not accurately reflect daily function. We utilized a non-invasive wireless patch system (WPS) to characterize multiday gastric myoelectrical activity, evaluate its relationship with GES, and assess changes following a therapeutic intervention. METHODS: Adults referred for GES for symptoms thought to represent gastroparesis were enrolled in a

Sustained proton-pump inhibitor exposure and incident age-related macular degeneration: a new-user, population-based cohort study.

OBJECTIVE: Proton-pump inhibitors (PPIs) may disrupt lysosomal acidification, a pathway implicated in age-related macular degeneration (AMD) pathogenesis. We evaluated whether sustained PPI exposure increased the risk of incident AMD in older adults aged ≥60 years. METHODS AND ANALYSIS: This retrospective observational cohort study included adults aged ≥60 years with sustained PPI exposure, matched 1:1 to unexposed controls on demographics, overall health, healthcare utilisation, cardiometabolic factors. Participants were identified from electronic health records. Sustained PPI exposure was d

Induction of regulatory T cells by an aluminum-CpG combination adjuvant for therapeutic and preventive vaccination against pollen allergy.

Pollen allergy is a prevalent allergic disorder affecting a substantial proportion of the global population. Allergen-specific immunotherapy (ASIT) is the only disease-modifying treatment, acting by inducing regulatory T cells (Tregs) and Th1-based immunity. However, current ASIT approaches often elicit insufficient allergen-specific tolerance and fail to correct underlying immune dysregulation, thereby limiting clinical efficacy. Here, a combination adjuvant composed of aluminum oxyhydroxide and CpG oligodeoxynucleotides (Al-CpG) is developed and formulated with the T-cell epitope derived fro

Slow Transit Constipation: Pathophysiological Perspectives and Management Updates.

Slow transit constipation (STC) is a chronic colonic motility disorder characterized by markedly delayed transit, leading to reduced bowel movements, abdominal discomfort, and significant quality-of-life impairment. It predominantly affects women and is associated with abnormalities in enteric neuronal signaling, smooth muscle contractility, interstitial cells of Cajal, gut peptides, bile acid homeostasis, and autonomic regulation. Secondary causes of constipation and structural lesions must be excluded before the diagnosis of STC, with colonic transit studies serving as the gold standard. Com

Reteplase (Retavase) is a plasminogen activator, mimicking endogenous tissue plasminogen activator (t-PA), a serine protease, converting plasminogen to plasmin and thereby precipitating thrombolysis. It is a third-generation recombinant form of t-PA that operates in the presence of fibrin (i.e. it is fibrin specific). Reteplase can be administered as a bolus dose (nonweight-based) rather than an infusion, which promotes rapid and safe administration. The ease of administration of this reteplase dosage regimen (two 10U bolus doses, each over 2 minutes, 30 minutes apart) is conducive to prehospi
