BACKGROUND: Chronic constipation is a common and costly health problem occurring in approximately 4.5 million Americans. Current management of constipation is suboptimal and requires a stepwise approach using a combination of laxatives to decrease symptoms.
OBJECTIVE: The objective of this review was to describe the efficacy and safety of a new therapeutic entity, lubiprostone, recently approved by the US Food and Drug Administration for the treatment of chronic idiopathic constipation.

We evaluated the pharmacological properties and spasmogenic activities of Yumijangquebotrade mark, a Korean herbal laxative formulation. Doses in the range 12-50 microg/ml induced a large spasmogenic effect in isolated guinea pig ileum, similar to that induced by acetylcholine. Pre-treating the tissue with atropine (0.2 microM) completely abolished the contractile effect of Yumijangquebo. The spasmogenic effect of Yumijangquebo and the inhibition of this effect by atropine suggest that a cholinergic mechanism is responsible for its effects. Constipation and the use of laxatives were investigated in patients with chronic cancer pain treated with oral morphine and transdermal fentanyl in an open sequential trial. Forty-six patients were treated with slow-release morphine 30-1000 mg/day for 6 days and 39 of these patients were switched to transdermal fentanyl 0.6-9.6 mg/day with a conversion ratio of 100:1. Using a pH-sensitive radiotelemetering device the effect of lactulose on luminal pH in the ileum, colon, and rectum has been compared with that of two other laxative agents. Lactulose produced marked acidification of proximal colonic contents but this effect was not consistently maintained into the distal colon.
