[Clinical presentation and diagnostic procedures in canine hypoadrenocorticism].

ABSTRACT: Hypoadrenocorticism (HA) is a rare endocrinopathy in dogs that may occur in a primary form, characterized by a combined deficiency of glucocorticoids and mineralocorticoids or isolated glucocorticoid deficiency, or in a secondary form with an isolated glucocorticoid deficiency. In addition, iatrogenic HA may also occur. The primary form represents the most frequent type of the disease. Clinical signs are variable and often nonspecific, ranging from lethargy, inappetence, vomiting, and weight loss to polyuria/polydipsia or acute, life-threatening adrenal crises associated with hypovol

[Treatment strategies for canine hypoadrenocorticism - a current overview].

ABSTRACT: Hypoadrenocorticism (HA) in dogs requires lifelong substitution therapy with glucocorticoids and, in most cases, also mineralocorticoids. For glucocorticoid replacement, prednisolone is administered at a maintenance dose of approximately 0.1 mg/kg/day. Alternative drugs such as hydrocortisone, dexamethasone, or methylprednisolone are used less frequently. ABSTRACT: In veterinary medicine, desoxycorticosterone pivalate (DOCP) is the only parenteral preparation approved for the replacement of mineralocorticoid hormones in Europe. ABSTRACT: The approved dosage is 2.2 mg/kg every 25-30

Glucocorticoid Receptor Signaling: Multilevel Organization, Roles in Fetal Development, and Postnatal Outcomes.

The hypothalamic-pituitary-adrenal (HPA) axis coordinates metabolic, immune, and behavioral responses to a changing environment. Its molecular effectors are the nuclear receptors for glucocorticoids and mineralocorticoids (the GRs/MRs), encoded by nr3c1/nr3c2. The MR serves as the high-affinity sensor of basal hormone concentrations, whereas the GR amplifies the stress response and mediates negative feedback. Despite their shared domain architecture, the receptors have diverged functionally: isoform composition, post-translational modifications, and the complement of co-regulators together det

Clinical and molecular characteristics of a series of Chinese children with pseudohypoaldosteronism: a case series.

BACKGROUND: Pseudohypoaldosteronism (PHA) is a rare disorder characterized by renal resistance to mineralocorticoids, leading to hyperkalemia, hyponatremia, and metabolic acidosis. It is primarily classified into type I (PHAI), with subtypes including renal (caused by NR3C2 mutation), systemic (caused by SCNN1A/B/G mutations), and secondary forms, and type II (PHAII), which is commonly associated with mutations in genes involved in the WNK signaling pathway (WNK1, WNK4, KLHL3, CUL3). However, comprehensive cohorts delineating the clinical and genetic spectrum of PHA in Chinese children are lac

Comprehensive Quantitative Urinary Steroid Profiling of 29 Steroids Using Liquid Chromatography-Tandem Mass Spectrometry.

Steroids are critical for numerous physiological processes; disruption in their metabolism is associated with numerous endocrine disorders. Steroid quantification is essential to improve the understanding and diagnosis of these pathologies. Historically, urinary steroid profiling has been performed using low-throughput gas chromatography mass spectrometry (GC-MS), providing holistic coverage of steroid classes with low cross-reactivity. Here, we translate our previous GC-MS urinary steroid profile to a liquid chromatography tandem-MS (LC-MS/MS) platform, offering a validated, comprehensive ove

Contemporary management of adrenocortical carcinoma: A narrative review.

Adrenocortical carcinoma (ACC) is a rare and aggressive malignancy arising from the adrenal cortex, characterized by local invasion and frequent distant metastases. Tumor size is a key predictor of malignancy, with lesions larger than 4 cm carrying a high likelihood of being malignant. Subsets of ACCs are functional, secreting excess glucocorticoids, sex steroids, or mineralocorticoids, leading to characteristic clinical syndromes. Diagnosis is often delayed due to nonspecific presentations, and management remains challenging because of limited effective systemic therapies, high recurrence rat

Neonatal reference intervals for salivary steroids: focus on prematurity, stressful events and perinatal betamethasone.

CONTEXT: Painlessly obtained samples can improve diagnostics and care in paediatric conditions with affected steroidogenesis. OBJECTIVE: To establish neonatal reference intervals (RIs) and identify sources of biological variation for steroid hormones in saliva. METHODS: Steroid concentrations were determined using LC-MS/MS in 412 saliva samples from 174 infants aged 0-4 weeks, born at gestational age 26-42 weeks, recruited from three neonatal intensive care units and one maternity ward. We used six approaches to construct RIs with partitioning based on results from linear mixed models. RESU

Adaptive changes in adrenal steroid metabolism under extreme physical and psychological stress: a narrative review.

BACKGROUND: Chronic exposure to sleep deprivation, insufficient recovery, inadequate energy availability, extreme environmental conditions, repetitive high-intensity exercise, and psychological threat can alter hypothalamic-pituitary-adrenal (HPA) axis function. Acute activation of the HPA axis is adaptive, whereas prolonged or repetitive activation may contribute to allostatic load, altered daily glucocorticoid exposure, and changes associated with psychological and metabolic maladaptation. METHODS: This narrative review synthesizes evidence on adrenal steroid metabolism under extreme physic

Maternal prenatal depression and anxiety and steroid hormones in amniotic fluid.

Maternal prenatal depression and anxiety (PDA) have been associated with increased risks of adverse birth and neurodevelopmental outcomes in children. While fetal exposure to too high or low levels of steroid hormones has been proposed as a potential biological mechanism underlying these effects, few studies have directly investigated this hypothesis using fetal tissue samples, and the existing studies have been limited to examining cortisol, cortisone or testosterone. We studied associations between PDA and steroid hormones in amniotic fluid by measuring a panel of 17 steroid hormones - inclu

Hydrocortisone versus dexamethasone in cerebral salt-wasting after aneurysmal subarachnoid hemorrhage.

INTRODUCTION: Cerebral salt wasting syndrome (CSW) is frequently observed in aneurysmal subarachnoid hemorrhage (SAH) patients and results in excessive natriuresis with decreased extracellular fluids, leading to hyponatremia and hypovolemia. Hyponatremia is associated with an increased complication rate and potential mortality. This study compares hydrocortisone and dexamethasone for CSW-associated hyponatremia prophylaxis after non-traumatic SAH. METHODS: This retrospective cohort study analyzed data from 510 consecutive patients with non-traumatic SAH who were admitted to the University Hosp

BACKGROUND: Endoscopic stapling to treat pharyngeal pouch is a relatively new technique with the potential to reduce the morbidity associated with the open approach for pharyngeal pouch. Despite enthusiasm for the endoscopic approach there have been no series reported in Australia, and descriptions of outcomes and benefits are currently anecdotal. The aim of the present study was to determine the outcome associated with endoscopic stapling of pharyngeal pouch in an Australian setting. METHODS: All patients admitted for endoscopic stapling for a pharyngeal pouch between 1998 and 2002 by surgeo

Abnormal activation of the mineralocorticoid receptor in the aldosterone-sensitive distal nephron contributes to fructose-induced salt-sensitive hypertension.

Fructose high-salt (FHS) diets increase blood pressure (BP). The mineralocorticoid receptor (MR) regulates Na+ reabsorption by the aldosterone-sensitive distal nephron (ASDN). The MR can be transactivated by glucocorticoids, including those locally produced by hydroxysteroid 11-beta dehydrogenase 1 (11β-HSD1). The epithelial sodium channel (ENaC) is a key transporter regulated by the MR. We hypothesized that fructose-induced salt-sensitive hypertension depends, in part, on abnormal activation of MRs in the ASDN, with consequent increases in ENaC expression. We found that aldosterone-upregulate

Mineralocorticoids and outcome improvement in heart failure patients with and without atrial fibrillation: a systematic review and a meta-analysis.

BACKGROUND: The presence of atrial fibrillation (AF) in patients with heart failure (HF) is common and increases the risk of adverse events. Whether the benefit of mineralocorticoid receptor antagonists (MRAs) in patients with HF varies according to AF remains to be understood. RESEARCH DESIGN AND METHODS: We systematically searched MEDLINE and EMBASE and included all studies reporting prevalence of AF and incidence of a composite primary outcome of cardiovascular death and HF hospitalization. Random-effects models were used to estimate pooled odds ratios (OR), 95% confidence intervals (CI),

An Infant With Hyponatremia, Hyperkalemia, and Faltering Growth: A Case Report.

Infants presenting with hyponatremia and hyperkalemia routinely engage clinician concern for congenital adrenal hyperplasia (CAH), a set of disorders resulting in difficulty producing glucocorticoids, mineralocorticoids, and sex steroids in the adrenal glands. However, we present the case of an infant with classic CAH-like symptoms, a negative newborn screen, and clinical features inconsistent with the diagnosis of CAH. Ultimately, the patient was diagnosed with pseudohypoaldosteronism (PHA), a condition in which aldosterone is unable to activate its receptor in the kidney. Notably, he also ex

Corticosteroids modulate bovine oocyte maturation through mineralocorticoid receptor.

The ovary is continuously exposed to circulating corticosteroids, yet their physiological roles in oocyte physiology remain unclear. This study investigated how mineralocorticoids and glucocorticoids regulate nuclear maturation in bovine cumulus-oocyte complexes (COCs). COCs collected from 2 to 5 mm small antral follicles were subjected to 21 h of in vitro maturation (IVM). Immunohistochemistry and RT-PCR showed that the mineralocorticoid receptor (MR) and 11β-hydroxysteroid dehydrogenase type 2 (HSD11B2) were predominantly expressed in oocytes, whereas the glucocorticoid receptor (GR) was res
