RNA-Based Therapies for Lipoprotein(a) Reduction: Antisense Oligonucleotides, Small Interfering RNAs, and Emerging Platforms.

PURPOSE OF REVIEW: Lipoprotein(a) [Lp(a)] is an independent, genetically determined, and causal risk factor for atherosclerotic cardiovascular disease (ASCVD) and calcific aortic valve stenosis. Elevated Lp(a) affects approximately 20% of the global population and is not meaningfully modified by lifestyle intervention or existing lipid-lowering pharmacotherapy. This review summarizes the biology of Lp(a) as a therapeutic target, evaluates the efficacy of currently available lipid-lowering agents against this target, and describes the mechanistic principles, clinical evidence, and unresolved qu

Lipoprotein(a) Inhibition: Current Evidence, Therapeutic Landscape, and Future Directions.

Lipoprotein(a) [Lp(a)] is a genetically determined, apolipoprotein B-containing lipoprotein that contributes to cardiovascular risk through atherogenic, proinflammatory, and potentially antifibrinolytic mechanisms. Genetic, epidemiological, and mechanistic evidence supports a causal role for elevated Lp(a) in atherosclerotic cardiovascular disease and calcific aortic valve disease. In contrast, associations with other cardiovascular phenotypes are less consistent. Since circulating concentrations are largely inherited and only modestly affected by lifestyle measures and conventional lipid-lowe

CMET renovates conventional glass ionomer cement into a multifunctional pulp-protective restorative material: An in vitro study.

It is essential to minimize dental pulp exposure to external stimuli to maintain pulp vitality. Therefore, pulp-protective materials require sealing ability, antibacterial activity, and biocompatibility. Conventional glass ionomer cement (GIC) possesses some of these properties, while calcium 4-methacryloxyethyl trimellitate (CMET) can induce calcification, suggesting potential utility in pulp protection. This study compared conventional GIC, CMET-modified GIC (C-GIC), and Ca-free 4-methacryloxyethyl trimellitate (4-MET)-modified GIC (4M-GIC). 4M-GIC showed decreased compressive strength and s

Dihydromyricetin enhances the anti-calcification performance of glutaraldehyde-cross-linked bovine pericardial valve materials by stabilizing elastin.

At present, most biological valves used for clinical application are created using glutaraldehyde (GLUT) cross-linked animal pericardia or aortic valve materials. Since GLUT cannot cross-link with elastin, which would be further degraded and absorbed by the body after being implanted into the body, structural damage and calcification of biological valves can occur. In this study, we proposed a new elastin cross-linking agent, dihydromyricetin (DMY). We evaluated the characteristics of DMY in different treatment groups and in rat animal models. Using circular dichroism, Fourier transform infrar

Lipoprotein(a) in the statin era: from risk marker to therapeutic target.

Lipoprotein(a) [Lp(a)] is a predominantly genetically determined lipoprotein whose concentration is generally stable over time, although clinically relevant within-person variation can occur. Elevated concentrations affect about one in five people. It has emerged as one of the most important causal contributors to the residual cardiovascular risk that persists despite effective LDL-cholesterol lowering in the statin era, and it is also causally implicated in calcific aortic valve stenosis. For decades Lp(a) was measurable yet effectively untreatable, because conventional lipid-lowering therapi

Effect of environmental parameters on growth and calcification in marine coccolithophore Gephyrocapsa huxleyi CCMP371.

The coccolithophore Gephyrocapsa huxleyi is a major producer of CaCO3 in the world's oceans due to its production of coccoliths. Its domesticated cultivation may serve as a sustainable source of biogenic CaCO3 for several industrial applications. Intraspecific variation within G. huxleyi is well recognized, leading to varied physiological responses to environmental drivers. This study optimized the net production of coccoliths in a calcifying strain of G. huxleyi, CCMP371, by serially exploring the impacts of salinity, media nutrient concentration, light intensity, and temperature on growth an

Nebivolol Protects the Thoracic and Abdominal Aorta and Their Perivascular Adipose Tissue From the Differential Detrimental Effects of Obesity.

The effects of obesity on the vasculature and perivascular adipose tissue (PVAT) function vary according to the vascular territory. Nebivolol is a third-generation β-blocker that exerts vasculoprotective effects in obesity. The thoracic and abdominal aorta show different responses under physiological conditions, and their PVATs have a distinctive composition and secretory profile. We hypothesized that obesity would affect thoracic and abdominal aortic responsiveness, as well as their PVAT function, differently and that nebivolol would restore obesity-induced vascular and PVAT changes. To test

Uremic Toxin-Driven Vascular Calcification in Chronic Kidney Disease: Molecular Pathways and Integrated Phenotypes.

BACKGROUND: Vascular calcification (VC) affects up to 90% of patients with end-stage renal disease and increases cardiovascular mortality 3- to 5-fold. Once considered passive mineral deposition, VC is now recognized as an active, toxin-driven process orchestrating vascular smooth muscle cell transdifferentiation, endothelial dysfunction, and matrix remodeling. However, current uremic toxin classifications remain biochemically oriented, providing limited clinical guidance for risk stratification and therapeutic selection. METHODS: This comprehensive review reframes uremic toxin-driven VC thro

Immediate DIEP flap breast reconstruction following neoadjuvant therapy: Histopathological assessment of vascular integrity in internal mammary and deep inferior epigastric arteries.

BACKGROUND: The impact of neoadjuvant chemotherapy (CT) and radiotherapy (RT) on the microvasculature critical for autologous breast reconstruction remains unclear. The internal mammary artery (IMA) and deep inferior epigastric artery (DIEA) are key conduits in deep inferior epigastric perforator (DIEP) free flap surgery. Prior studies suggest that radiation and chemotherapeutic agents may induce vascular injury; however, the clinical relevance of such alterations in flap viability is debated. We aimed to evaluate whether neoadjuvant CT or combined CT+RT induce histopathological changes in the

Beyond the pump: a narrative review of the acute molecular, metabolic, and neuropsychological responses to a bout of resistance exercise.

BACKGROUND: Resistance training is widely recognized for its chronic adaptations, including strength, muscle hypertrophy, and improvements in musculoskeletal health. However, a single bout of resistance exercise also elicits a range of acute physiological and psychological responses across multiple systems. These immediate responses have generally been examined within individual physiological domains, with less attention given to their coexistence and temporal overlap following a single session. METHODS: This narrative review synthesizes evidence relating to the acute systemic effects of a si

Medical therapies to prevent the development and progression of calcific aortic valve stenosis: a contemporary review.

INTRODUCTION: Aortic stenosis (AS), the leading cause of valvular heart disease related mortality, affects 12% of individuals over 75 years and is set to expand as the population ages. Once symptoms develop, severe symptomatic AS carries an average survival of approximately two years. The only management currently available is aortic valve replacement (AVR), either surgically or via transcatheter aortic valve implantation, but this addresses only the end stage of the disease process which is often associated with irreversible myocardial remodeling. There are currently no pharmacotherapies prov

Lipoprotein(a)-lowering therapies: a promising future.

Lipoprotein(a) [Lp(a)] is a significant, genetically determined contributor to the risk of atherosclerotic cardiovascular disease (ASCVD), which remains the leading cause of mortality worldwide despite successes in the management of LDL cholesterol. Lipoprotein(a) possesses increased atherogenicity, contributing to residual cardiovascular risk. Elevated Lp(a) levels affect a substantial proportion of the population, rendering this a potentially high-impact therapeutic target, but currently available lipid-lowering agents and lifestyle interventions have minimal impact on lowering Lp(a), and li

Lipoprotein(a): Cardiovascular Risk and Emerging Targeted Therapies.

Lipoprotein(a) [Lp(a)] is a genetically determined, low-density lipoprotein-like particle that has emerged as an important and independent contributor to atherosclerotic cardiovascular disease and calcific aortic valve stenosis. Elevated Lp(a) concentrations, defined as approximately 125 nmol/L (≥ 50 mg/dL) by most consensus thresholds, affect an estimated 20% of the general population and confer lifelong inherited cardiovascular risk that is not adequately captured by standard lipid panels. Epidemiological, observational and genetic studies consistently show that elevated Lp(a) concentrations

Endocrine disorders linked to chronic kidney disease: Mechanisms and clinical implications.

Chronic kidney disease (CKD) is a progressive condition that disrupts multiple endocrine axes, contributing to a broad range of hormonal imbalances, including mineral and bone disorder, gonadal dysfunction, thyroid abnormalities, insulin resistance, and elevated prolactin levels. These disturbances not only exacerbate comorbidities such as cardiovascular disease, anemia, diabetes, and sarcopenia, but may also accelerate the decline of renal function. This article aims to provide a comprehensive overview of the pathophysiology, clinical impact, and management strategies for endocrine disorders

Mineral and Bone Disease in CKD and Kidney Transplantation: Controversies, Gaps, and a Path Forward.

The pathophysiology of chronic kidney disease (CKD)-related mineral and bone disorder (CKD-MBD) extends far beyond simple mineral imbalances. Evolving from the traditional understanding of secondary hyperparathyroidism, the conceptual framework has recently been updated to recognize 2 distinct but overlapping clinical syndromes: CKD-associated osteoporosis, which encompasses the significantly increased fracture risk and microarchitectural deterioration in this population; and CKD-associated cardiovascular disease, which accounts for vascular and structural cardiac abnormalities, including medi

Aortic valve stenosis in 2026: from emerging mechanisms to clinical drug development.

Calcific aortic valve stenosis (AS) is the most prevalent valvular heart disease in high-income countries, affecting 9.4 million persons globally. While surgical and transcatheter aortic valve replacement have transformed outcomes, they address end-stage disease rather than the underlying pathobiology. The development of pharmacological therapies to slow or halt AS progression would represent a paradigm shift in disease management. Recent advances have substantially expanded our understanding of AS pathogenesis. Large-scale genome-wide association studies have identified over 260 risk loci, wi

Hydranencephaly and porencephaly predominated among neuropathologic findings in stillborn and neonatal piglets during the first major outbreak of Japanese encephalitis in Australia.

In 2022, piggeries in southeastern Australia experienced an increase in reproductive losses and occasional neurologic disease in neonates with up to 100% mortality in some litters. Molecular testing identified a genotype IV Japanese encephalitis virus (JEV) as the etiological agent, detected for the first time in Australia. Necropsy of 235 JEV-positive domestic piglets, mostly stillbirths with 30% mummified, revealed hydranencephaly-porencephaly impacting the dorsal cerebrum as the most common lesion. Microscopically, this was characterized by varying degrees of parenchymal collapse as a resul

Novel Lipoprotein (a) Therapies: A Comprehensive Review.

PURPOSE OF REVIEW: Lipoprotein(a) [Lp(a)] is a genetically determined, independent risk factor for atherosclerotic cardiovascular disease and calcific aortic valve stenosis. Plasma Lp(a) levels are 70-90% heritable, largely unresponsive to lifestyle changes, and poorly controlled with typical lipid-lowering drugs such as statins and PCSK9 inhibitors. The development of powerful RNA-based therapies offers a new chance to specifically target and significantly lower Lp(a). We examined available phase 1-3 trial data, regulatory updates, and trial registries up to mid-2026 to provide an overview of

Retrospective epidemiological surveillance of bovine tuberculosis in a slaughterhouse in Northeastern Brazil.

Bovine tuberculosis (BTB), mainly caused by Mycobacterium bovis, remains an endemic zoonosis with significant economic and public health impact, particularly in major beef-producing countries such as Brazil. This study aimed to evaluate the occurrence, epidemiological profile, and macroscopic features of BTB-like lesions identified during routine post-mortem inspection at an officially inspected slaughterhouse in Northeastern Brazil over a six-year period (2020–2025). A retrospective observational study was conducted based on official inspection records from 79,318 slaughtered cattle. Thirty-s

Sirtuin 1 modulation unlocks the therapeutic potential of sodium-glucose co-transporter 2 inhibitors (SGLT2i) in calcific aortic valve stenosis.

BACKGROUND: Calcific aortic valve stenosis (AS) affects 3% of older adults and lacks medical treatment. The deacetylase Sirtuin 1 (SIRT1) could be involved in many pathways linked to AS progression. Sodium-glucose co-transporter 2 inhibitors (SGLT2i), glucose-lowering agents, have been shown to reduce cardiovascular events (likely via SIRT1), but their possible benefits in AS are unknown. Our study aims to uncover the role of SIRT1 in AS progression and assess the benefit of SGLT2i to slow down the aortic valve fibro-calcification processes. METHODS: RNA-seq data of human aortic valve specime
