Synthesis, Biological Evaluation, and Molecular Docking of Pyridine-Containing Squaramides as DNase I Inhibitors.

BACKGROUND: DNase I inhibitors may be useful as biochemical tools and as potential leads for investigating pathological processes associated with excessive DNA degradation. Squaramides represent attractive scaffolds for drug discovery owing to their rigid, highly polarized structure and versatile hydrogen-bonding properties. METHODS: Four novel pyridine-containing squaramides (5a-5d), comprising two symmetric and two asymmetric derivatives, were synthesized and structurally characterized by spectroscopic methods and, for 5a and 5b, single-crystal X-ray diffraction. Their DNase I inhibitory ac

Multiple Recurarization Episodes After Sugammadex Reversal in Hyperthermic Intraperitoneal Chemotherapy: A Case Report.

Sugammadex reverses rocuronium-induced neuromuscular blockade via 1:1 molar encapsulation. Recurarization is rare when sugammadex is dosed as recommended. We report a 53-year-old woman undergoing hyperthermic intraperitoneal chemotherapy (HIPEC) who developed three sequential recurarization episodes despite an initial sugammadex dose (5.4 mg/kg), ultimately requiring 1400 mg cumulatively. HIPEC-associated hyperthermia and a prolonged rocuronium infusion likely promoted peripheral-to-central redistribution of rocuronium and overwhelmed free sugammadex in plasma. In patients with large cumulativ

Concurrent TMS-EEG to characterise interhemispheric signal propagation over the motor and prefrontal regions: An independent replication of GABAB receptor mechanisms.

Callosal abnormalities disrupt interhemispheric communications that impair the recovery of a series of neurological and psychiatric disorders. Understanding interhemispheric communication could help identify treatments for disorders with dysfunctional callosal transmission. This study aimed to validate the sensitivity of a TMS-EEG-based measure of interhemispheric signal propagation (ISP) to GABAB receptor modulation, while independently replicating the pharmacological mechanisms of interhemispheric inhibition. A group of 29 participants received an oral dose of either baclofen, a GABAB recept

Quantitative Neuromuscular Monitoring in General Anesthesia: A Systematic Review of Recovery and Safety Outcomes.

Quantitative neuromuscular monitoring is increasingly used during general anesthesia to confirm recovery after neuromuscular blocking agents (NMBAs) and reduce postoperative respiratory risk. Residual neuromuscular blockade remains a preventable concern, yet monitoring practices, device performance, reversal use, and recovery thresholds remain variable across perioperative settings. This systematic review evaluated evidence on quantitative neuromuscular monitoring in general anesthesia, focusing on recovery assessment, residual blockade detection, device agreement, reversal practice, and posto

Immediate Perioperative Hypersensitivity and Anaphylaxis in Neuroanesthesia Practice: A Focused Review.

Perioperative hypersensitivity reactions are uncommon but potentially catastrophic. Neurosurgical and spine patients are especially vulnerable, as the injured brain and spinal cord tolerate anaphylaxis-induced hypotension poorly, cerebral and spinal perfusion may fall precipitously, and draping of the head and torso may obscure cutaneous signs. These procedures also rely heavily on topical hemostatic agents. National audits estimate the incidence of perioperative anaphylaxis at roughly 1 in 5000 to 10,000 anesthetics, but estimates vary geographically. No study has reported a denominator-based

Protistan grazer-mediated regulation of domoic acid: interactions between Noctiluca scintillans and Pseudo-nitzschia reshape toxin fate and risk in coastal waters.

Domoic acid (DA)-producing Pseudo-nitzschia and the heterotrophic dinoflagellate Noctiluca scintillans frequently co-occur during coastal harmful algal blooms, yet their interactions and consequences for toxin cycling remain poorly understood. Here, controlled co-cultures were conducted to examine grazer-induced defense, grazing dynamics, and DA accumulation-depuration. Sustained 3-d exposure stimulated a 3-4-fold increase in cellular DA production, without detectable changes in cell size or frustule silicification, indicating a primarily chemical defense response. Cellular DA induction exceed

Anesthetic Management of a Patient With Myasthenia Gravis Undergoing Thoracoscopic Thymectomy: A Case Report and Literature Review.

Myasthenia gravis is an autoimmune neuromuscular disorder characterized by fluctuating skeletal muscle weakness and fatigability. Perioperative management is challenging because patients may have bulbar or respiratory muscle involvement and altered sensitivity to neuromuscular blocking agents, increasing the risk of postoperative respiratory failure and myasthenic crisis. Thymectomy is an established therapeutic option in selected patients with myasthenia gravis and may be performed using minimally invasive thoracoscopic techniques. We report the anesthetic management of a 48-year-old man with

Disease Control in Generalized Myasthenia Gravis: Consensus Definition, Evolution, and Patient Characterization.

BACKGROUND AND OBJECTIVES: Generalized myasthenia gravis (gMG) is a chronic fluctuating neuromuscular disease. Some patients keep experiencing symptoms or adverse events (AEs) despite tailored therapeutic schemes. The aim of this study was to characterize adequately controlled gMG and describe patients' evolution and outcomes, including estimated direct health costs in a universal health care system. METHODS: Spanish expert neurologists developed a definition of adequately controlled gMG using Delphi methodology. This was applied to an existing cohort of patients recently diagnosed with gMG d

Combining Pharmacological Therapy and Ultrasound-Guided Botulinum Toxin Injection Resulted in Clinical Improvement in Bilateral Arterial-Neurogenic Thoracic Outlet Syndrome: A Case Report.

BACKGROUND: Thoracic outlet syndrome (TOS) results from compression of the brachial plexus and/or the subclavian vessels. Botulinum toxin (BTX) has been shown to be useful in TOS both as a predictor of surgical outcome and as a therapeutic option. CASE REPORT: We present the case of a 21-year-old woman with a rare form of bilateral TOS with combined arterial and neurogenic involvement. After surgical treatment on the right side, the patient developed symptoms on the left side. We adopted a multimodal therapeutic approach that effectively reduced her symptoms and improved her quality of life.

Oculopharyngeal Muscular Dystrophy: From Molecular Pathogenesis to Gene Therapy.

Oculopharyngeal muscular dystrophy (OPMD) is a rare, late-onset, autosomal dominant muscular dystrophy caused by a trinucleotide GCN repeat expansion in the PABPN1 gene on chromosome 14q11.2. The disease is clinically characterised by progressive ptosis, dysphagia, and proximal limb weakness, with onset typically between the fifth and sixth decades. OPMD affects approximately 1:100,000 individuals in Europe, with substantially higher prevalence in specific founder populations, reaching 1:600 among Bukharan Jews and 1:1,000 in French Canadians. The expanded polyalanine tract in PABPN1 promotes

Perioperative Anesthetic Considerations for Temporal Bone Tumor Resection in Fibroblast Growth Factor 23 (FGF23)-Related Tumor-Induced Osteomalacia: A Case Report.

Phosphaturic mesenchymal tumors (PMTs) are uncommon, predominantly benign neoplasms frequently associated with dysregulated phosphate metabolism through the overproduction of fibroblast growth factor 23 (FGF23). Elevated levels of FGF23 lead to phosphate depletion, which may impair tissue oxygen delivery, central respiratory drive, and respiratory muscle function. Systemic manifestations may include osteomalacia and neuromuscular dysfunction, with an increased risk of respiratory and cardiovascular complications. PMTs often present with nonspecific clinical findings and may remain unrecognized

Beyond reconstruction: minimally invasive aesthetic and regenerative medicine in postoperative oral and maxillofacial surgery.

PURPOSE: Oral and maxillofacial reconstruction restores anatomical continuity and function after tumor resection, trauma and congenital deformity, yet patients frequently remain affected by fibrosis, scar tethering, impaired oral competence, asymmetry and altered dynamic facial function. These deficits can impair quality of life, social interaction and body image. Conventional revision surgery carries additional morbidity, and minimally invasive options have emerged as a postoperative adjunct. This article reviews how minimally invasive aesthetic and regenerative medicine can extend reconstruc

The Expanding Therapeutic Landscape in Myasthenia Gravis.

Myasthenia gravis (MG) is a rare autoimmune disorder caused by autoantibodies targeting postsynaptic proteins of the neuromuscular junction. Although conventional therapies can achieve remission or sufficient symptom control in most patients with generalized MG (gMG), approximately 10%-30% of patients remain treatment refractory with an increased risk of myasthenic crises and mortality. In addition, prolonged immunosuppression can cause undesirable side effects. Over the past decade, research has focused on identifying new therapeutic targets, which paved the way to advance the development of

AAVcc47-μDys Gene Therapy Ameliorates Respiratory Pathology in mdx Mouse Model of Duchenne Muscular Dystrophy.

Duchenne muscular dystrophy (DMD) is a progressive X-linked neuromuscular disorder caused by deficiency of dystrophin - a protein essential for muscle integrity. Loss of dystrophin results in global muscle weakness, including in the muscles of respiration. As a result, respiratory failure is a leading cause of morbidity and mortality in patients with DMD. Gene therapy using adeno-associated virus (AAV) is ideal for monogenetic diseases. However, the dystrophin gene is too large to be packaged into AAV; therefore, microdystrophin (μDys) containing important elements of the dystrophin gene is us
