Infections and the neuromuscular junction: From direct neurotoxicity to immune-mediated dysfunction.

Infectious diseases and disorders of the neuromuscular junction (NMJ) are linked by complex and heterogeneous mechanisms that extend beyond direct pathogen-mediated injury. While some infectious agents can directly impair neuromuscular transmission, others may act as triggers of autoimmunity or contribute to the exacerbation of pre-existing NMJ disorders. However, the mechanisms underlying these interactions remain incompletely understood. In this narrative review, we summarize the current evidence regarding infections associated with NMJ dysfunction. Available evidence suggests that infection

Applying pharmacokinetic and pharmacodynamic concepts from human clinical anesthesia to laboratory animal anesthesia.

Human clinical anesthesia, veterinary anesthesia, and laboratory animal anesthesia have advanced through the continued exchange of knowledge and techniques. Propofol is an example of this relationship: it was first evaluated in several animal species during its development and later became a major intravenous anesthetic in human clinical practice. Using propofol in human clinical anesthesia as an example, this review discusses how concepts for assessing anesthetic depth and applying pharmacokinetic and pharmacodynamic principles in human anesthesia may be applied to laboratory animal anesthesi

Omitting face-mask ventilation before tracheal intubation in low-risk adults: a randomized non-inferiority trial.

BACKGROUND: Face-mask ventilation (FMV) is routinely performed between the induction of general anesthesia and tracheal intubation, but it can cause gastric insufflation, and its necessity in low-risk patients is uncertain. We examined whether omitting FMV before intubation is non-inferior to routine FMV for oxygen desaturation, and whether the two approaches differ in gastric insufflation. METHODS: In this single-center, randomized, group-sequential non-inferiority trial, low-risk adults undergoing elective thyroid surgery were assigned to tracheal intubation with FMV (rocuronium 0.6 mg/kg)

Human iPSC-Derived Spinal Neurons Carrying the ALS FUS (P525L) Mutation Exhibit Lower Response to Inhibitory Neurotransmitters.

Amyotrophic lateral sclerosis (ALS) is a progressive neuromuscular disorder characterized by motoneurons degeneration. Functional studies have linked ALS to hyperexcitability and excitotoxicity, but the cause of the disease is unknown, though familial ALS cases are linked to pathogenic variants in several genes, including SOD1, TARDBP and FUS. Here we focused on the effect of the severe FUS (P525L) mutation on the functional properties of human spinal neurons derived from induced pluripotent stem cells (hiPSCs). This mutation delayed functional maturation, as revealed by the observation that m

CRISPR-responsive hybrid MOF assembling assay for rapid electrochemical detection of domoic acid.

Domoic acid (DA), a potent neurotoxin, poses a serious threat to food safety and public health. Traditional detection methods often struggle to balance sensitivity with practical applicability. To address this, we developed an electrochemical biosensor based on a CRISPR-responsive hybrid metal-organic framework (MOF) assembling assay. This platform employs a novel negative feedback mechanism where the DA aptamer itself serves as the activator for CRISPR/Cas12a. Target binding inhibits Cas12a's trans-cleavage activity, thereby protecting the signal reporter from cleavage and enabling significan

Comparison of Methylprednisolone and Dexamethasone in Their Interaction With Sugammadex in Pediatric Patients: A Randomized Controlled Trial.

BACKGROUND: Sugammadex may interact with steroidal compounds; however, comparative clinical data on different peri-operative corticosteroids in children are limited. This randomized controlled trial primarily aimed to compare the effect of methylprednisolone and dexamethasone, administered at clinically equivalent glucocorticoid doses, on sugammadex-mediated reversal time of rocuronium-induced neuromuscular block in children. Secondary aims were to compare extubation time, postoperative nausea, pain, and adverse events related to upper airway oedema between groups. METHODS: Sixty children und

A distributed event-triggered framework for coordinating athlete autonomic states via wearable sensor networks.

BACKGROUND: Interpersonal autonomic physiology (IAP) research has established that physiological synchrony (PS), the temporal interdependence of autonomic nervous system (ANS) activity across individuals, is an established phenomenon in team-based settings. However, PS in athlete cohorts has been studied exclusively as a passive observational variable; no framework actively regulates autonomic coherence under the communication constraints of wireless body area networks. This study integrates IAP and distributed control theory by formulating team-level autonomic coherence as a leaderless multi-

Structural identification of the RY12 domain of RyR1 as an ADP sensor and the target of the malignant hyperthermia therapeutic dantrolene.

Malignant hyperthermia (MH) is a pharmacogenetic condition triggered by volatile anesthetics, which activate MH susceptible mutants of ryanodine receptor 1 (RyR1). The precise binding sites and mechanisms of the therapeutic dantrolene and MH triggers have remained unclear. Here, we present structures of RyR1 bound to dantrolene and the MH trigger 4-chloro-m-cresol (4CmC), revealing the dantrolene and 4CmC binding sites in atomic detail. Dantrolene binds stacked with ATP or ADP in the RY12 domain at the corner of the receptor, inducing a conformational change. Functional analyses confirm ATP or

Psychotropic drugs in drinking water? A scoping review of evidence on diazepam and fluoxetine.

The global prevalence of mental disorders has increased considerably in recent decades. This evidence, associated with population growth and the expansion of access to medications, favors the growing global consumption of psychotropic drugs. Consequently, an increasing load of these substances is introduced into the environment, especially into water bodies, characterizing them as emerging contaminants. This scoping review study aimed to integrate data and provide a global overview of the occurrence of widely used psychotropic drugs (diazepam, fluoxetine, and haloperidol) in drinking water. Th

Investigating the effects of intrathecal baclofen and ventral-dorsal rhizotomy on dystonia in children with cerebral palsy (PREDICT-Dystonia): protocol for a prospective observational clinical trial.

INTRODUCTION: Dystonia, a form of elevated muscle tone in CP, significantly impairs function and quality of life. Multiple neurosurgical interventions are used to reduce dystonia, including intrathecal baclofen (ITB) and ventral dorsal rhizotomy (VDR). ITB is used most often for moderate to severe generalised dystonia in CP; however, the evidence supporting its effectiveness is weak due to patient variability and study limitations. Further, VDR is becoming more prevalent but there is minimal data available on its outcomes. This proposal aims to evaluate ITB and VDR's ability to reduce quantita

Serotonergic Toxicity and Hyponatremia Associated with Antidepressant Therapy: The Pivotal Role of SIADH.

Serotonin syndrome (SS) and the syndrome of inappropriate antidiuretic hormone secretion (SIADH) are well-recognized adverse drug reactions (ADRs) associated with serotonergic agents, particularly antidepressants. Although traditionally regarded as distinct clinical entities, accumulating evidence suggests that SIADH may represent a key pathophysiological link between serotonergic toxicity and acute hyponatremia, thereby explaining their frequent clinical overlap and, in some cases, severe or fatal outcomes. Excessive serotonergic activity can stimulate hypothalamic pathways regulating arginin

Neuromuscular Blockade for Intracranial Hypertension: Quantifying Real-World Effectiveness on Intracranial Pressure.

BackgroundContemporary intracranial hypertension guidelines, including SIBICC (2019) and the 2024 ACS Best Practices, list neuromuscular blockade as a Tier Two, short-duration option for refractory ICP elevation. Despite its physiologic rationale, the real-world effect of initiating continuous paralysis on ICP has not been quantified using large-scale intra-patient physiologic data.MethodsWe performed a retrospective intra-patient before-after study at a Level 1 trauma and neurocritical care center (2016-2021). A Review of Minimally Invasive Therapies for Men with Overactive Bladder.

PURPOSE OF REVIEW: This manuscript aims to provide an overview of minimally invasive treatment for overactive bladder (OAB) in men, highlighting how concomitant bladder outlet obstruction in men can complicate management and lead to underutilization of these therapies in this patient population. RECENT FINDINGS: The 2024 AUA/SUFU guidelines now allow practitioners to offer minimally invasive therapy as an upfront treatment for OAB. Recent studies have closely examined the efficacy of several key procedures, including intradetrusor onabotulinumtoxin-A (BTX-A), sacral neuromodulation (SNM), and

A clinical algorithm for painful TMD associated with sleep bruxism: the role of botulinum toxins.

INTRODUCTION: Temporomandibular disorders (TMD) and sleep bruxism (SB) are commonly seen in both general and specialty dental practices. The relationship between them is still elusive. Some studies have found an association between TMD and SB, while others failed to confirm this relationship. The complexity extends to management strategies, particularly when patients present with symptoms of painful TMD as well as SB. Several treatments have been proposed for this presentation, including botulinum toxin injections. Despite the presence of evidence, clear guidance on management is lacking. AIM
