Pre-hospital time and risk of perforation in patients with acute appendicitis in 112 countries (AlliGatOr): an international, prospective, observational cohort study.

BACKGROUND: Acute appendicitis is the most common surgical emergency worldwide. Concern that delayed treatment might increase perforation can lead to unnecessary surgery and high rates of negative appendicectomy, where a normal, non-inflamed appendix is removed. However, whether perforation is related to pre-hospital time (symptom onset to first surgical assessment) has not been explored in contemporary global data. METHODS: We conducted an international, multicentre, prospective, observational cohort study (AlliGatOr) of patients undergoing emergency appendicectomy for suspected acute append

Association of group prenatal care with postpartum visit attendance: a secondary analysis of the Centering and Racial Disparities randomized clinical trial.

OBJECTIVE: The American College of Obstetricians and Gynecologists recommends a comprehensive postpartum visit (PPV) within 12 weeks after birth.1 Nationally, an estimated 60% to 65% of patients attend this visit, and attendance is lower among patients with public insurance.2 Group prenatal care (GPNC) has been associated with higher PPV attendance in observational studies that could not account for self-selection.3 We evaluated whether GPNC, compared with individual prenatal care (IPNC), was associated with PPV attendance in a planned secondary analysis of the Centering and Racial Disparities

Perforated appendicitis secondary to a ruptured pyosalpinx managed by laparoscopic appendectomy and salpingectomy: a case report.

Pyosalpinx, the accumulation of pus within the fallopian tube, rarely causes acute appendicitis. We report a 38-year-old woman who presented with a 10-day history of right lower quadrant pain and low-grade fever and was diagnosed radiologically with a perforated appendix. At laparoscopy, intraperitoneal pus, an appendix perforated at its tip, and a perforated right fallopian tube near the utero-tubal junction were found. Laparoscopic appendectomy with right salpingectomy was performed. Histopathology confirmed acute appendicitis and acute salpingitis; cultures showed no growth and GeneXpert wa

Adult intestinal malrotation: A rare incidental finding in trauma imaging.

Intestinal malrotation is a congenital anomaly resulting from abnormal rotation and fixation of the midgut during embryonic development. It is typically diagnosed in infancy, and its detection in adults-particularly in asymptomatic individuals-is rare. In adults, clinical presentation ranges from chronic, nonspecific symptoms to acute intestinal obstruction and midgut volvulus. A 36-year-old woman presented to the emergency department following a motor vehicle collision. She reported no gastrointestinal complaints and denied any history of recurrent abdominal pain, bilious vomiting, abdominal

Interobserver Variability in Intraoperative Classification of Acute Appendicitis and Postoperative Antibiotic Management.

BACKGROUND: During laparoscopy for suspected acute appendicitis (AA), surgeons classify the appendix as normal, phlegmonous, gangrenous, or perforated, with or without abscess. The surgeon's assessment directly influences postoperative antibiotic treatment and length of hospitalization. There is currently no objective gold standard for this classification. This study aimed to evaluate interobserver variability in the laparoscopic classification of AA. METHODS: Surgeons and surgical trainees viewed video recordings of laparoscopies from 30 patients suspected of AA. Each participant anonymously

Histological Stages of Appendicitis Associated With Imaging Characteristics: A Computed Tomography and Ultrasound-Based Comparative Study.

INTRODUCTION: Appendicitis diagnosis relies heavily on medical imaging procedure studies; however, a definite diagnosis is obtained through histological assessment. Understanding the imaging characteristics and their relation to various histological phases enhance the quality of treatment recommendations. This study analyzed imaging characteristics of acute appendicitis in relation to its histological severity. METHODS: A cross-sectional study was conducted using data from a third-level hospital. Appendectomy specimens from patients with prior computed tomography (CT) scans or ultrasound (US)

Duplicated Appendix With Mucocele Mimicking an Appendiceal Neoplasm in a 45-Year-Old Woman: A Case Report.

BACKGROUND Appendiceal duplication is an extremely rare congenital anomaly, with an incidence of 0.004% to 0.009%. An association with an appendiceal mucocele is even rarer. It can complicate the diagnosis and management of right iliac fossa pathology and remains difficult to identify preoperatively. Failure to recognize it can result in recurrent symptoms, missed pathology, intraoperative challenges, and medicolegal consequences. This report describes the case of a 45-year-old woman with history of a loculated peri-appendiceal abscess managed with percutaneous drainage and intravenous antibio

Differences in microbiota profiles of simple and complicated pediatric appendicitis cases.

This study aimed to identify the differences in microbial composition between simple appendicitis (SA) and complicated appendicitis (CA) in a limited sample size using culture and microbiota analysis. Appendix tissues and rectal swabs for microbiota analysis and conventional cultures were collected from patients. 16S rRNA regions were amplified using a DNA preparation kit with primers designed to target the V3-V4 region. Sixteen (41%) of the patients were classified as CA based on the histopathological analysis, and the rest as SA. The effect size was slightly greater in rectal swab samples th

Delayed diagnosis of familial glucocorticoid deficiency type 4 in a child with epilepsy: a case report and literature review.

OBJECTIVES: Familial glucocorticoid deficiency type 4 (FGD4) is a rare autosomal recessive form of primary adrenal insufficiency caused by biallelic variants in the NNT gene and characterized predominantly by isolated glucocorticoid deficiency. Neurological presentations may obscure the underlying adrenal disorder. We describe a pediatric case with a novel NNT variant not previously associated with FGD4 and review the reported clinical and genetic spectrum. CASE PRESENTATION: A 10-year-old girl presented with a 4-year history of epilepsy and longstanding diffuse hyperpigmentation. A homozygou

Intraoperative cangrelor during mechanical thrombectomy: is the systematic addition of aspirin needed?

OBJECTIVES: In the hyperacute setting of mechanical thrombectomy (MT) for large-vessel-occlusion stroke, periprocedural platelet inhibition may be required in cases of refractory occlusions or underlying cervical or intracranial arterial disease at risk of reocclusion. Cangrelor is increasingly used in this context, although its use remains largely supported by observational evidence, and the impact of concomitant aspirin remains uncertain. This study investigated the safety and efficacy of the addition of intravenous aspirin to cangrelor during MT. METHODS: We conducted a retrospective analy

Night-Time Presentation as an Independent Predictor of Unnecessary Appendectomy in Children: A Prospective Cohort Study.

BACKGROUND: Unnecessary appendectomy (UA) remains an important quality indicator in pediatric emergency surgery, reflecting persistent diagnostic uncertainty despite advances in clinical assessment and imaging. The aim of this study was to identify predictors of UA in children. METHOD: This investigation was designed as a prospective cohort study involving consecutive pediatric patients who underwent appendectomy for suspected acute appendicitis during January 2022 through December 2025. Unnecessary appendectomy was defined as the removal of a macroscopically and histologically normal appendi

Modifying quality and equity of care: emerging roles of genetic disease modifiers in β-hemoglobinopathies.

Genetic disease modifiers (GDMs) strongly affect disease and therapy outcomes, rendering their characterization essential for prognosis, treatment decisions, and therapy development. A paradigm for the importance of GDMs are β-hemoglobinopathies, for which a growing pipeline of emerging curative and disease-modifying therapies are based on the manipulation of GDMs. However, beyond their role as therapeutic targets, GDMs are also gaining clinical importance in the choice and planning of disease-modifying therapies, the selection and customization of curative therapies, and the equitable provisi

Leveraging spatial structure to design spatially-targeted malaria interventions at the community-scale.

Progress in malaria control has stagnated since the early 21st century in many countries, requiring new approaches such as the use of spatially-targeted interventions. However, evidence on the effectiveness of spatially-targeted interventions is mixed, depending on both the setting and intervention. In addition, the potential role of community health programs in implementing spatial targeting is underexplored, with models focusing on coarser spatial scales than those needed to guide programmatic activities. We developed a two-age-class, spatially-explicit model of malaria at the community-scal
