Short-term linaclotide plus polyethylene glycol in patients at increased risk of inadequate bowel preparation: A randomized, double-blind, multicenter trial.

BACKGROUND AND AIM: The benefit of combining linaclotide with polyethylene glycol (PEG) for improving bowel preparation quality and adenoma detection in patients at risk for inadequate bowel preparation remains uncertain. This study aimed to evaluate its efficacy and safety. METHODS: From August 2022 to July 2023, a multicenter, randomized trial assigned participants to a 2-day linaclotide or placebo plus PEG regimen. The primary endpoints included adequate bowel preparation rate and mean number of adenomas detected per colonoscopy. RESULTS: The modified intention-to-treat (mITT) (n=672) and

Submucosal metastatic leiomyosarcoma of the colon presenting with intussusception and lower gastrointestinal bleeding: a case report.

Colonic leiomyosarcoma (LMS) is a highly aggressive tumor arising from smooth muscle cells and is generally associated with a poor prognosis. This report presents a case of colonic LMS originating from the submucosal layer of the ascending colon. A 76-year-old woman was admitted with intermittent hematochezia persisting for over a month. Imaging studies revealed a tumor in the ascending colon accompanied by intussusception. The patient underwent a right hemicolectomy with systematic lymphadenectomy involving the middle colic root and right branch regions. Postoperative histopathological analys

Colonic Malakoplakia With an Adenomatous Appearance on Magnification Endoscopy: A Case Report.

Malakoplakia is a rare chronic granulomatous disease associated with impaired macrophage phagocytosis of bacteria. Because colonic malakoplakia presents with varied endoscopic appearances, its characteristic features have not been clearly defined, and differentiation from colorectal neoplasms during endoscopy may be difficult. We report a case of colonic malakoplakia with adenomatous features evaluated using magnifying narrow-band imaging (NBI). A patient receiving hemodialysis and corticosteroid therapy for immunoglobulin A nephropathy underwent total colonoscopy after a positive fecal immuno

Poorly cohesive carcinoma diffusely involving the whole gastrointestinal tract: a case report.

BACKGROUND: Gastric cancer is the fourth leading cause of cancer death worldwide. We reported a 64-year-old male patient with paroxysmal dull pain around the periumbilical region after meals. After admission, the patient underwent contrast-enhanced computed tomography (CT) scans of the upper and lower abdomen, which revealed marked thickening of the gastric antrum and multiple enlarged lymph nodes around the gastric antrum and in the retroperitoneum. Esophagogastroduodenoscopy revealed possible tumor lesions at the cardia, gastric body, duodenal bulb, and descending part of the duodenum. Colon

[Lower gastrointestinal bleeding secondary to Chronic Nonspecific Ulcerative Colitis: A clinical case].

BACKGROUND: Gastrointestinal bleeding (GIB) has a low incidence in pediatrics. It could be of the upper or lower track, the first one being the most common. Etiology varies according to the age group. Chronic nonspecific ulcerative colitis (UC) is heterogeneous in nature, with a varying severity among patients. The purpose of the present manuscript is to exhibit the unusual case in pediatrics of an adolescent with a lower tract GIB due to UC. CLINICAL CASE: A 16-year-old female was brought to the pediatric emergency department for a lower tract GIB. Ten days prior receiving medical attention,

Robot-Assisted Ileocecal Resection Using the Saroa Surgical System.

Saroa is a novel robotic surgical system that can estimate the gripping forces of robotic forceps and convey this information to the console surgeon via a remote-control interface. A patient with a polyp located adjacent to the Bauhin valve was diagnosed with adenocarcinoma with submucosal and vascular invasion after endoscopic resection and subsequently underwent additional surgical resection. Port placement was modified twice intraoperatively due to the limited range of motion of the robotic arms. During the first setting, the ileocolic artery and vein were resected, and the ascending colon

Jejunocecal fistula combined with gastro-ascending colonic fistula in a child with chronic diarrhea and weight loss: a case report.

BACKGROUND: Gastrointestinal fistulas in children are rare, and the coexistence of a jejunocecal and gastric ascending colon fistula is exceptional. To our knowledge, no instance of this fistula combination has been reported previously. These patients present with nonspecific symptoms, which may lead to delayed or missed diagnoses. CASE PRESENTATION: We evaluated a 12-year-old boy with a 3-year history of intermittent nausea and vomiting, and a weight loss of 30 kg. Initially, he was managed as for gastroenteritis; however, his response was minimal. Further investigation included endoscopy and

Dynamic Gastrointestinal Simulation Reveals that Short-Term Mango Purée Feeding Modulates (Poly)phenol Metabolism, Gut Microbiota and Metabolic Activity.

This study evaluated the impact of gastrointestinal digestion and colonic fermentation on mango (poly)phenols and their influence on gut microbiota using the dynamic gastrointestinal simulator simgi®. The system was fed with 10 g/day of mango purée for five days, providing 89.5 μmol of phenolic compounds, and inoculated with fecal microbiota from three healthy donors. Samples from the stomach, small intestine, and colon (ascending, transverse and descending) were analysed by UHPLC-HRMS to identify parent compounds and metabolites. Microbial communities were profiled by 16S rRNA gene sequencing

Integrated Assessment of Intestinal Barrier Function and Microscopic Inflammation Using Confocal Laser Endomicroscopy for Relapse Prediction in Ulcerative Colitis.

BACKGROUND: Although the current strategy in ulcerative colitis (UC) focuses on achieving endoscopic healing to improve long-term outcomes, patients with persistent microscopic inflammation or intestinal barrier dysfunction remain at increased risk of relapse. OBJECTIVE: We evaluated whether structural and functional abnormalities of the ileal and colonic mucosa assessed by probe-based Confocal laser Endomicroscopy (pCLE) could predict the loss of therapeutic targets. METHODS: The prospective single-center study included 81 UC patients in clinical and endoscopic remission monitored for 24 mo

A case of intestinal mucosa-associated lymphoid tissue (MALT) lymphoma characterized by diffuse capillary hyperplasia nodules with hemorrhage.

A 52-year-old man presented to our department, complaining of recurrent episodes of hematochezia for half a month. Colonoscopy showed diffuse nodular mucosal eminences of approximately 2-3 mm in size in the colorectum and terminal ileum, presenting a "cobblestone" appearance, with visible capillary hyperplasia that are prone to rupture and bleeding upon light touch. Narrow-band imaging (NBI) showed that the hyperplastic capillaries presented "root-like" branching. We performed deep biopsies in areas with dense capillary networks or prone-to-bleeding regions at the nodules of the rectum, sigmoi

Acute cecal volvulus and large bowel obstruction in a patient with persistent Ladd bands and chronic malrotation: case report and review of literature.

Cecal volvulus occurs when the ascending colon and terminal ileum twist around the mesenteric pedicle, compromising bowel perfusion and leading to obstruction. This condition may result from congenital or acquired abnormalities affecting cecal fixation. Intestinal malrotation is rare in adults and is usually asymptomatic. We report the case of a 50-year-old male who presented with a large bowel obstruction due to cecal volvulus associated with chronic malrotation and Ladd bands. He underwent successful surgical treatment and was discharged home. This report discusses the atypical anatomy, clin

Colon Cancer Presenting as an Incarcerated Incisional Hernia: A Case Report.

Colorectal cancer within a hernia sac is rare. Although cases involving inguinal hernias have been reported, colorectal cancer in an incisional hernia has not been documented. A 91-year-old woman with a history of cholecystectomy 30 years earlier presented with melena, hematemesis, and anorexia. CT showed an incisional hernia at the right subcostal region containing the ascending colon with wall thickening and fat stranding, raising concern for incarceration and possible malignancy. Emergency laparotomy revealed an incarcerated ascending colon with a type 2 adenocarcinoma. Right hemicolectomy

Lymphoglandular Complex-Like Colorectal Carcinoma Arising in Tubular Adenoma: A Case Report.

Lymphoglandular complex-like colorectal carcinoma (LGCC) is characterized by invasive tumors confined entirely by lymphoid stroma and is very rare. Histologically, it can closely mimic adenomatous polyps involving lymphoglandular complexes (LGCs) or pseudoinvasion/misplacement of adenomatous epithelium. Here, we report a 57-year-old patient who underwent routine surveillance colonoscopy and was found to have a 30 mm sessile polyp in the ascending colon. The polyp was removed by endoscopic mucosal resection. Initial histologic sections show fragments of tubular adenoma. Due to the large size of

Inter-rater Reliability and Accuracy of NBI Score for Activity Evaluation in Ulcerative Colitis.

INTRODUCTION: Colonoscopy is a critical tool for the management of Ulcerative Colitis (UC). In this study, we aim to explore the accuracy of Virtual Chromoendoscopy (VCE) using Narrow Banding Imaging (NBI), and magnification using Near Focus (NF) for a better definition of endoscopic inflammation grade in UC patients compared to standard white light (WL) endoscopy alone. METHODS: This is a non-randomized prospective study including all the patients who underwent a colonoscopy (for any reason) between April and September 2023 (with protocol number n. 60/2019.20). During the endoscopic evaluati

Impact of Computer-Aided Detection on Endoscopist's Gaze-Shift Distance During Colonoscopy: a Randomized Controlled Trial (With Video).

BACKGROUND AND AIM: Computer-aided detection (CADe) facilitates colorectal lesion detection, but it remains unclear whether CADe affects endoscopist's eye strain by altering gaze patterns. This study aimed to determine whether CADe-assisted colonoscopy is superior to conventional colonoscopy in reducing endoscopists' gaze-shift distance. METHODS: This single-center randomized controlled trial enrolled participants and randomly assigned them to two groups: first observation with CADe, then second observation without CADe (CADe-first group) in the ascending colon, or observations in the reverse

Perforated Meckel's Diverticulum With Dual Gastric and Pancreatic Heterotopia: A Pediatric Case Report.

Meckel's diverticulum is a common congenital gastrointestinal anomaly that can lead to life-threatening complications such as perforation. While rare in children, a perforated Meckel's diverticulum often mimics acute appendicitis, posing a significant diagnostic challenge. We report the case of a five-year-old male who presented with acute right lower quadrant pain, hematochezia, vomiting, and signs of peritonitis. Emergency laparotomy was performed, which confirmed a perforated Meckel's diverticulum densely adherent to the ascending colon. Histopathological examination of the resected specime
