Medullary Carcinoma of the Colon: A Case Report.

Medullary colon carcinoma (MCC) is a rare neoplasm that differs from conventional adenocarcinoma in clinical, histological, and molecular aspects. We present the case of a 68-year-old man with no significant medical history. Physical examination revealed a distended abdomen, painful on superficial and deep palpation. A pelvic and abdominal computed axial tomography (CT) scan with S/C IV contrast reported a solid tumor lesion with lobulated contours, located intraluminally within the cecum and part of the ascending colon, measuring 80 mm. The patient underwent exploratory laparotomy and right h

Combined use of EUS, ESD, and laparoscopic techniques in the diagnosis and treatment of a mucinous adenocarcinoma of the ascending colon originating from the submucosal layer: a case report.

This report describes a 53-year-old woman admitted for evaluation of a space-occupying lesion in the ascending colon detected one year earlier and recent right lower quadrant pain. Initial colonoscopy revealed a 2.0 × 2.0 cm submucosal elevated lesion with surface ulceration near the ileocecal valve. Biopsy showed inflammatory mucosa with abundant mucin but no malignancy, and CT revealed no definite abnormalities. One year later, the lesion enlarged to 3.5 × 3.0 cm with increased ulceration. Endoscopic ultrasound (EUS) demonstrated a hypoechoic submucosal mass, and endoscopic submucosal dissec

Adult ileocolic intussusception caused by a terminal ileal lipoma: A case report.

BACKGROUND: Adult intussusception is a rare clinical condition. Unlike in pediatric cases, adult intussusception often has a definable lead point, with benign or malignant tumors being the most common etiology. Small intestinal lipoma is a rare cause of adult intussusception. It typically presents with non-specific symptoms and an insidious onset, frequently leading to misdiagnosis or discovery only during emergency surgery. Abdominal computed tomography (CT) plays a pivotal role in preoperative diagnosis, yet its specific findings in lipoma-induced intussusception warrant further emphasis. C

Idiopathic mesenteric phlebosclerosis: A case report.

RATIONALE: Idiopathic mesenteric phlebosclerosis (IMP) is a rare disease with unclear etiology and pathogenesis. Although most IMP patients have a long-term history of traditional Chinese herbal medicine use, this article reports a case without such a special medical history, which may provide new insights into the disease's etiology. PATIENT CONCERNS: A 59-year-old middle-aged man was admitted to the gastroenterology department due to recurrent upper abdominal pain for 1 month. The fecal occult blood test was weakly positive. DIAGNOSES: No significant abnormalities were found in tumor marke

Durable complete response to pembrolizumab after BRAF/MEK inhibition in recurrent MSI-H/dMMR, BRAF V600E-mutant colon cancer: a case report.

Patients with metastatic colorectal cancer (mCRC) harboring both microsatellite instability-high/deficient mismatch repair (MSI-H/dMMR) and the BRAF V600E mutation represent a distinct clinicomolecular subgroup characterized by aggressive biology and distinct therapeutic challenges. While MSI-H/dMMR predicts sensitivity to immune checkpoint inhibitors (ICIs), the concurrent BRAF V600E mutation may contribute to secondary resistance or shortened durability of response in a subset of patients treated with ICI monotherapy. Here, we present the case of an 85-year-old female with locally advanced a

A microscopically assisted limited colectomy mouse model of early colon cancer.

BACKGROUND: Currently, there is a lack of an adequate rodent model for investigating surgical techniques and comprehensive treatment options for patients with colorectal cancer (CRC). This study presents a mouse model that involves the orthotopic implantation of colon cancer, followed by a limited colectomy, with the aim of addressing this issue. METHODS: To establish an early colon tumor model, luciferase-tagged CT-26 cells were implanted into the ascending colon wall of BALB/c mice. After a 1-week observation period, a limited colectomy procedure was performed. The progression of cancer and

Minimally invasive management of colonic lipoma causing intussusception with endoloop in an adult.

Intussusception is a condition in which one segment of the intestine (intussusceptum) telescopes into the lumen of an adjacent segment (intussuscipiens), potentially leading to obstruction. Giant colonic lipomas are rare and often asymptomatic, but when large enough, can serve as a lead point for intussusception. We present the case of a 62-year-old male patient who presented with nausea and vomiting. A computed tomography (CT) abdomen/pelvis without contrast showed intussusception of the ascending colon into the transverse colon without bowel obstruction, along with a lobulated fatty mass at

Case Report: Synchronous colorectal adenocarcinomas with discordant mismatch repair status: a case of lynch-like syndrome and serrated pathway association.

OBJECTIVE: Synchronous colorectal cancers (SCRCs) with discordant mismatch repair (MMR) status present unique clinical and therapeutic challenges. This case report describes a rare instance of synchronous ascending colon and rectal adenocarcinomas arising from distinct tumorigenic pathways (Lynch-like syndrome and serrated pathway). We aim to explore the mechanism underlying MMR status heterogeneity, and emphasize the clinical value of lesion-specific molecular profiling combined with regional metastatic lymph node MMR phenotyping for individualized treatment of this condition. METHODS: A 67-

Acupuncture for Pain Management in a Geriatric Patient with Colorectal Cancer: A Case Report.

INTRODUCTION: Colorectal cancer (CC) causes pain, thereby affecting a patient's quality of life and functional status. Pain management with analgesics can cause adverse effects. Acupuncture is a nonpharmacological treatment that effectively manages cancer-related pain and provides a safer alternative. In this case report, battlefield ear acupuncture (BFA) and body acupuncture were used. CASE PRESENTATION: A 76-year-old female patient with right lower abdominal pain for the past 3 months prior to hospitalization. The pain was stabbing, intermittent, and worsened in the seated position, also bl

Puncture and catheterization of the ascending colon for succus entericus reinfusion in a patient with a high-output single-lumen stoma: a case report.

BACKGROUND: High-output enterostomy is a common complication following ostomy procedures. In patients with loop ileostomies, the incidence of dehydration can reach 30%, and it represents the most frequent cause of readmission after ileostomy surgery, with rates as high as 16.9%. In severe cases, this condition can progress to renal failure (Dis Colon Rectum 55: 175-80, 2021, J Gastrointest Surg 17: 298-303, 2013, Nurs Stand 37: 71-6, 2022). High-output ileostomy (HOI) often imposes dietary restrictions on patients, increasing the risk of malnutrition. Previous studies have demonstrated that il

Coexistence of Primary Colorectal Cancer and Multiple Colonic Metastases from Pancreatic Tail Cancer: A Case Report.

INTRODUCTION: Colonic metastasis from pancreatic cancer is extremely rare. Distinguishing metastatic pancreatic cancer from primary colorectal cancer is clinically important because the therapeutic strategies differ substantially. We report an exceptionally rare case of pancreatic tail cancer with multiple colonic metastases coexisting with primary colorectal cancer. CASE PRESENTATION: An 83-year-old man presented with right lower abdominal pain, constipation, and weight loss. Four years earlier, MRI and magnetic resonance cholangiopancreatography (MRCP) had demonstrated a cystic lesion in th

Robot-Assisted Right Hemicolectomy for Ascending Colon Cancer in a Kidney Transplant Recipient: A Case Report.

During colorectal surgery, the renal allograft and transplanted ureter may constrain port placement and instrument movement; therefore, kidney transplant recipients require careful planning. Here, we report a case of robot-assisted right hemicolectomy for ascending colon cancer adjacent to an ipsilateral renal allograft. A 62-year-old woman underwent deceased-donor kidney transplantation 16 years prior, with a renal allograft placed in the right iliac fossa. Colonoscopy revealed a near-circumferential lesion in the ascending colon, and biopsy revealed a well- to moderately differentiated tubul

[A Case of Unresectable Colon Cancer Treated with Pembrolizumab That Underwent Conversion Surgery].

The patient is a man in his 60s referred with a diagnosis of ascending colon cancer with right ureteral invasion, hydronephrosis, and right psoas muscle and nerve invasion. Complete resection in a single stage was deemed difficult during surgery, and chemotherapy was selected as the treatment plan. Given the MSI-H status, pembrolizumab was selected as the primary therapy. Twelve courses were administered. Following the normalization of tumor markers and imaging-based tumor shrinkage, improvement in ureteral and psoas muscle invasion was observed, enabling conversion surgery (right hemicolectom

Malignancy-mimicking Kayexalate-associated extraintestinal mass and literature review.

The gastrointestinal side effects of Kayexalate administration are well-established, and despite its discontinued use in the United States, it is still important to consider in the clinical and pathologic differential, given the vast time range between administration and clinical presentation. One less common, though significant, complication of Kayexalate use is the formation of a malignancy-mimicking mass or pseudotumor. This presentation has been reported several times both within and outside of the gastrointestinal tract. Here we describe the case of a 65-year-old female presenting with a

Complete pathologic response after neoadjuvant immunotherapy for mismatch repair-deficient colon cancer with invading adjacent organs.

Mismatch repair-deficient (dMMR) colorectal cancer exhibits poor responsiveness to fluoropyrimidine-based chemotherapy but remarkable sensitivity to immune checkpoint inhibitors (ICIs). Neoadjuvant dual ICI therapy results in high pathologic response rates with minimal toxicity. However, the immunotherapy-induced rapid regression of locally advanced tumors with suspected invasion into adjacent organs may result in fistula formation. We describe here the case of an 80-year-old woman with locally advanced dMMR ascending colon cancer (cT4bN2aM0, stage IIIC), who underwent diverting ileostomy foll

Relationship between ileocolic arterial stump length and lymph node yield - a retrospective study using machine learning.

PURPOSE: In right-sided colon cancer surgery, ileocolic artery stump length may reflect the extent of mesenteric resection and lymph node harvest. This study evaluated the association between postoperative stump length measured on CT, lymph node yield, and recurrence or progression within 24 months. METHODS: This single-center retrospective study included 64 patients who underwent right or extended right hemicolectomy for right-sided colon adenocarcinoma between 2016 and 2024. Right-sided tumors were defined as those located in the cecum, ascending colon, hepatic flexure, or proximal transver

Aggressive, Widely Metastatic Inflammatory Myofibroblastic Tumor in an Adult Presenting as an Obstructing Endobronchial Mass.

Inflammatory myofibroblastic tumor (IMT) is a rare mesenchymal neoplasm, most commonly occurring in children and young adults. It is typically locally aggressive and rarely metastasizes. Adult-onset IMTs with extensive metastasis are exceedingly uncommon, particularly with large pulmonary involvement, and can mimic sarcoma or sarcomatoid carcinoma on imaging and histology. Anaplastic lymphoma kinase (ALK) positivity aids diagnosis and provides a potential therapeutic target. A 56-year-old male presented with progressively worsening dyspnea, nonproductive cough, and right-sided chest pain. Comp
