Enfortumab Vedotin-Induced Cutaneous Toxic Effects and Survival in Urothelial Carcinoma.

IMPORTANCE: Enfortumab vedotin (EV) is an antibody-drug conjugate approved for the treatment of locally advanced or metastatic urothelial cancer. Cutaneous adverse events (cAEs) are common during EV therapy, with prior studies suggesting an association between EV-related cAEs and improved survival; however, there are insufficient data to delineate the survival benefit of EV-induced cAEs from those associated with concurrent immune checkpoint inhibitors (ICIs). OBJECTIVE: To evaluate the association of EV-induced cAEs with survival and to characterize the timing and morphologic mechanisms of E

Structural basis for Mg²⁺-glycine synergy in the tandem glycine riboswitch revealed by cryo-EM.

Part I of this article discussed the potential functional importance of genetic mutations and alleles of the human cytochrome P450 2D6 (CYP2D6) gene. The impact of CYP2D6 polymorphisms on the clearance of and response to a series of cardiovascular drugs was addressed. Since CYP2D6 plays a major role in the metabolism of a large number of other drugs, Part II of the article highlights the impact of CYP2D6 polymorphisms on the response to other groups of clinically used drugs. Although clinical studies have observed a gene-dose effect for some tricyclic antidepressants, it is difficult to establ

Vulvovaginal Candidiasis Among American Indian and Alaska Native People, Indian Health Service, 2016-2022.

Vulvovaginal candidiasis (VVC) is an infection caused by the yeast Candida that affects more than 50% of women in their lifetime. We aimed to describe VVC among American Indian/Alaska Native (AI/AN) girls and women who were receiving care in the Indian Health Service (IHS) system during 2016-2022. We calculated the annual VVC prevalence per 1000 IHS user population and examined underlying medical conditions, previous diagnoses, and antifungal treatment. Among 6 million female patient-years, 70 766 patients had ≥1 VVC diagnosis code (2022 prevalence: 14.2 per 1000 IHS user population). Frequent

Clinical Features and Current Therapeutic Approaches to Monilethrix: A Systematic Review.

Monilethrix is a rare congenital hair disorder characterized by fragile, beaded hair shafts due to mutations in various keratin or desmoglein 4 genes. A systematic review of 24 studies (16 pediatric only, 3 mixed adult and pediatric, 3 adult only, and 2 unspecified) evaluating various treatment modalities identified topical and oral minoxidil as the most effective therapies, while oral retinoids and other treatments showed variable efficacy. Further pediatric research and clinical studies are necessary to assess long-term treatment outcomes and explore novel therapeutic strategies for this cha

A rare dermatologic condition: Insights from three cases of circumscribed acral hypokeratosis.

Circumscribed acral hypokeratosis (CAH) is a rare dermatologic condition primarily affecting acral surfaces, presenting as sharply demarcated, asymptomatic lesions with characteristic histopathological features, notably abrupt hypokeratosis, and variable thinning of the granular cell layer. Typically observed in middle-aged to elderly women as solitary lesions, CAH may occasionally present as multiple lesions, complicating diagnosis and management. We report four illustrative cases emphasizing the clinical and histologic variability of CAH. Case presentations have included lesions with clinica

Antibiotic prophylaxis for corneal abrasion.

RATIONALE: Corneal abrasion is a condition frequently treated by eye care professionals, emergency physicians, and primary care physicians. Topical ophthalmic antibiotics are the most common therapy for corneal abrasion. However, there has been no comprehensive summary and synthesis of the evidence regarding antibiotic prophylaxis in traumatic corneal abrasion. In this review update, we evaluated the current evidence regarding the benefits and harms of antibiotic treatment for this relatively common emergency condition. This is an update of a review published in 2022. OBJECTIVES: To assess th

Importance of a multi-tiered treatment approach for intellectually and developmentally disabled patients with hidradenitis suppurativa.

Hidradenitis suppurativa (HS) is a chronic, debilitating skin condition that is characterized by painful pustules, nodules, abscesses, and sinus tracts. The complicated and fast-evolving treatment of HS consists of a multi-tiered approach that includes, antibacterial, antihormonal, anti-inflammatory, and surgical options. Studies have demonstrated an earlier age of onset and increased prevalence of HS in patients with intellectual and developmental disability (IDD) compared to patients without IDD. To explore the use of an intensive multi-tiered HS management algorithm that requires monthly of
